Mexico" "etiqueta" => "f" "identificador" => "aff0030" ] ] "correspondencia" => array:1 [ 0 => array:3 [ "identificador" => "cor0005" "etiqueta" => "⁎" "correspondencia" => "Corresponding author." ] ] ] ] "titulosAlternativos" => array:1 [ "es" => array:1 [ "titulo" => "Factores de riesgo clínicos en pacientes con enfermedad pulmonar intersticial con anticuerpos anti-MDA5" ] ] "resumenGrafico" => array:2 [ "original" => 0 "multimedia" => array:7 [ "identificador" => "fig0015" "etiqueta" => "Fig. 3" "tipo" => "MULTIMEDIAFIGURA" "mostrarFloat" => true "mostrarDisplay" => false "figura" => array:1 [ 0 => array:4 [ "imagen" => "gr3.jpeg" "Alto" => 1862 "Ancho" => 2509 "Tamanyo" => 147280 ] ] "descripcion" => array:1 [ "en" => "<p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">Comparison of the titers of anti-MDA5 autoantibodies between patients with and without dermatological signs.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Anti-MDA5 (melanoma differentiation-associated gene 5) is a myositis-specific autoantibody (MSA) that has been associated with rapidly progressive interstitial lung disease (RP-ILD) and clinically amyopathic dermatomyositis (CADM), characterized by skin ulcers, rash, Gottrons papules, Gottron's sign, palmar papules, arthritis and often without muscular weakness.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">1–3</span></a> Moreover, some patients with interstitial lung disease (ILD) positive to anti-MDA5 may be classified as interstitial pneumonia with autoimmune features (IPAF),<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a> a classification recently proposed to identify individuals with interstitial pneumonia and features suggestive of a connective tissue disease (CTD) that do not meet established classification criteria for a CTD.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">5</span></a> RP-ILD associated with anti-MDA5 is a severe condition, often refractory to treatment, with high morbidity and mortality.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">6,7</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Clinical manifestations associated with anti-MDA5 autoantibodies have been very recently described. There is still a long way to fully explore and clarify this autoantibody's clinical manifestations and prognostic factors. In 2020, Allenbach et al.,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a> described that the disease associated with anti-MDA5 autoantibodies has at least three clinical phenotypes. These may differ in prognosis and survival, highlighting the group that presented as “very severe lung disease” with a mortality rate of up to 80% at three months of follow-up, markedly higher compared to the other two phenotypes defined as “pure dermato-rheumatologic pattern” and “severe skin vasculopathy” with less than 5% mortality.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a> Although it seems clear that pulmonary manifestations related to anti-MDA5 autoantibodies are severe, the clinical phenotype of ILD patients positive to anti-MDA5 autoantibodies may also be heterogeneous, and it may be that some clinical manifestations have a relevant prognostic relevance. Nowadays, factors associated with poor prognosis in the lung phenotype are still unclear. The present study aims to describe a cohort of ILD patients who are positive for anti-MDA5 and identify clinical risk factors associated with its survival.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients and study design</span><p id="par0015" class="elsevierStylePara elsevierViewall">This single-center cohort study included ILD patients positive for anti-MDA5 autoantibodies, enrolled between January 2017 and February 2020. Patients were evaluated and managed in the Interstitial Lung Disease and Rheumatology Unit (ILD&RU) at the Instituto Nacional de Enfermedades Respiratorias, Ismael Cosío Villegas (INER), a national referral center for respiratory diseases located in Mexico City. To be included, patients must have had a confirmed diagnosis of ILD by high-resolution chest tomography (HRCT) and be positive for anti-MDA5 autoantibody.</p><p id="par0020" class="elsevierStylePara elsevierViewall">At baseline, all patients evaluated at the ILD&RU have a complete evaluation for autoantibodies associated with ILD; this evaluation included antinuclear autoantibodies and autoantibodies specific and associated with myositis and systemic sclerosis. Pulmonologists and a rheumatologist evaluated patients; relevant clinical signs and symptoms related to anti-MDA5 were registered, such as duration of pulmonary symptoms (dyspnea and cough), arthritis, muscle weakness and its’ severity, skin manifestation associated with dermatomyositis (dermatomyositis rash, Holster sign, and Gottron sign, Gottron papules, palm papules, skin ulcers over dermatomyositis rash), Raynaud's phenomenon, sclerodactyly, and proximal dysphagia. Patients were evaluated if they met the Bohan and Peter or Sontheimer criteria to be classified as an inflammatory myopathy (IIM) or if they were classified as interstitial pneumonia with autoimmune features (IPAF) according to a joint proposal by the American Thoracic Society/European Respiratory Society (ATS/ERS) in 2015. Also, laboratory baseline values were registered, including leukocytes and lymphocytes and C reactive protein (CRP).</p><p id="par0025" class="elsevierStylePara elsevierViewall">Patients were managed according to the criteria of attending physicians, and all treatments were registered. Pulmonologists or rheumatologists periodically evaluated patients according to the clinical evolution; medical management was modified at the physicians’ discretion. Patients were classified with RP-ILD if they had rapid deterioration of ILD within three months.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> The local institutional review board approved the study protocol (approval code number: C03-20). Informed consent was given to all patients to participate in the study.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Pulmonary function test</span><p id="par0030" class="elsevierStylePara elsevierViewall">PFTs were performed at baseline in the Department of Respiratory Physiology of the INER, a specialized respiratory physiology laboratory. Spirometry (to obtain forced vital capacity) and DL<span class="elsevierStyleInf">CO</span> were performed using Easy One Pro and Easy One Pro Lab (Ndd® Zurich, Switzerland). According to sex, age, height, and weight, the predicted values for each subject were obtained from the PLATINO study<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a> and NHANES Tables.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">11</span></a> All spirometry and DL<span class="elsevierStyleInf">CO</span> tests fulfilled the acceptability and reproducibility criteria (ATS/ERS 2005).<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">12,13</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">HRCT evaluation</span><p id="par0035" class="elsevierStylePara elsevierViewall">HRCT was performed with a 1.0 or 1.5<span class="elsevierStyleHsp" style=""></span>mm thick axial section taken at 1<span class="elsevierStyleHsp" style=""></span>cm intervals and reconstructed using a high spatial frequency algorithm. Between 20 and 25 CT scan images were acquired for each patient. HRCT was blind-evaluated by two experts (MM and HN M-T). Experts assessed the HRCT and classified the images according to the official ATS/ERS Statement of the International Multidisciplinary Classification of the Idiopathic Interstitial Pneumonia<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> in nonspecific interstitial pneumonia (NSIP) pattern (fibrotic or cellular), organizing pneumonia (OP) pattern, usual interstitial pneumonia pattern (UIP), lymphocytic pneumonia pattern (LP). Any discrepancy in the interpretation was solved by consensus. The fibrotic component, defined by reticular opacities and inflammation by ground-glass opacities, was graded according to the Goh scores.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">13</span></a> We evaluated the agreement to determine the extent of pulmonary disease with the Goh between the two experts (Intraclass correlation coefficient 0.68 (95% CI: 0.54–0.82)). The assessment of MM was applied in the data analysis; MM has a high intra-observer agreement (Intraclass correlation coefficient 0.90 (95% CI: 0.84–0.94)).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Autoantibodies anti-MDA5 determination</span><p id="par0040" class="elsevierStylePara elsevierViewall">At baseline, all patients with ILD evaluated at the ILD&RU had a complete evaluation for autoantibodies, including antinuclear autoantibodies and autoantibodies specific and associated with myositis. According to the manufacturer's instructions, the IgG anti-MDA5 was measured using EUROIMMUN immunoblot strips (Autoimmune Inflammatory Myopathies 16 Ag). This commercial line blot assay for myositis diagnosis was assessed on its diagnostic accuracy for IIM and showed high specificity.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">14</span></a></p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">Numerical variables are described with mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD or median and interquartile range (25%–75% IQR) according to the distribution of the variables. In the case of categorical variables, the frequency and percentage were used. We used the t-test or Wilcoxon rank-sum test as appropriate to compare numerical variables and the exact Fisher test to compare categorical variables. Kaplan Meier method was used to estimate survival function. A univariate Cox regression analysis was performed to evaluate the strength of association with mortality of the different risk factors considered, calculating a Hazard Ratio (HR) with a 95% confidence index (95% CI). Due to the small sample size, no multivariate Cox regression analysis was done. All analyses were two-sided, <span class="elsevierStyleItalic">α</span> was set at 5%. The statistical software Stata v. 14.2 was used in all analyses.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">Fifty-three ILD patients positive for anti-MDA5 autoantibodies were included. Patients had a median time from the onset of symptoms to the baseline evaluation of 12 months (10–24 months). The main characteristics of the cohort are described in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Most patients were women (56.6%), the median age was 64 years old (56–73 years old (25%–75% IQR)), and almost all patients had cough and dyspnea (96% and 98%, respectively). Most patients did not meet the Bohan and Peter (B&P) or Sontheimer criteria; however, 74% met the ATS/ERS 2015 criteria for IPAF.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a> Nine patients (17%) had dermatomyositis rash such as Gottron papules in 5 patients, palmar papules, Gottron sign, and v neck sign in four patients, facial dermatomyositis rash in three patients, and three had skin ulcers in the area affected by the dermatomyositis rashes. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows representative images of these skin lesions. Overlapping of skin rashes was frequent, and eight patients with dermatomyositis rashes had a history of photosensitivity. Only one of the patients with dermatomyositis rashes could be classified as inflammatory myositis according to the B&P or the ACR/EULAR criteria. The rest (eight patients) could be diagnosed with amyopathic dermatomyositis. Mechanic's hand sign was found in 26.4% of the patients. Patients had a great extent of lung disease, and the most frequent HRCT patterns were fibrotic NSIP and cellular NSIP with or without OP. Patients were treated accordingly to the criteria of attending physicians; all patients received high corticosteroids doses as an initial treatment along with mycophenolate mofetil (38%), methotrexate (23%), or azathioprine (19%). At follow-up, 50% of the patients had a modification of the initial drug treatment due to no response or worsening of pulmonary disease.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">The cause of death in anti-MDA5-positive patients was respiratory failure associated with RP-ILD</span><p id="par0055" class="elsevierStylePara elsevierViewall">During the clinical follow-up of the enrolled patients, 12 non-survivors were recorded (22.64%). The cause of death of these patients was respiratory failure associated with RP-ILD. A comparison of non-survivors with survivors is in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. Dermatological signs of anti-MDA5 positive patients were strongly associated with death secondary to RP-ILD (HR: 3.7, 95% CI: 1.02–13.35) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). No other variables were associated with mortality. Due to the small sample size, no multivariable Cox regression model was performed to adjust for confounding.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Comparison of patients with and without dermatological signs associated with anti-MDA5</span><p id="par0060" class="elsevierStylePara elsevierViewall">Patients with dermatological signs were younger, had a more frequent OP/NSIP HRCT pattern, had a lower fibrotic extent in HRCT but no difference in the ground glass extent in HRCT, and had a higher frequency of Raynaud's phenomenon. Also, a tendency toward a higher incidence of pneumomediastinum was found (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). Moreover, when we analyzed the results of the immunoblots, patients with dermatological signs had higher relative units/mL (RU/mL), reflecting a higher anti-MDA5 autoantibodies concentration (30 (13–136) median (IQR) vs. 9 (8–12) (IQR), <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0006) (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">This study was designed to describe a cohort of ILD patients positive for anti-MDA5 autoantibodies and identify clinical risk factors associated with worse survival, the skin manifestations (skin ulcers, dermatomyositis rash, Gottron's papules, Gottron's sign, and V dermatomyositis rash) associated with anti-MDA5 were strongly associated with mortality. Moreover, patients with skin manifestations had significant clinical and radiological differences from those without dermatological manifestations. Patients with dermatological signs were younger, had more frequent OP/NSIP (78% vs. 29.5%), and had a lower extent of fibrosis but a greater ground glass extent in the HRCT scan evaluation. Finally, patients with dermatological signs had higher RU/mL of anti-MDA5 autoantibodies in the immunoblot testing, reflecting higher anti-MDA5 autoantibodies concentration.</p><p id="par0070" class="elsevierStylePara elsevierViewall">In 2005, Sato et al., identified, for the first time in a cohort of Japanese patients with clinically amyopathic dermatomyositis and rapidly progressive interstitial lung disease, autoantibodies against a protein called gene 5 associated with melanoma differentiation (anti-MDA5).<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">13</span></a> The clinical features of idiopathic inflammatory myopathies complicated with ILD vary widely; however, RP-ILD is one of the most severe complications. Within this broad clinical spectrum, myositis-specific serum autoantibodies are helpful markers for diagnosing polymyositis or dermatomyositis and are associated with different clinical phenotypes of the disease.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">7</span></a> It has been previously reported that among the autoantibodies associated with ILD, anti-MDA5 is the autoantibody associated with the worst prognosis once the diagnosis is confirmed.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">15</span></a> However, the group of anti-MDA5 positive patients is very heterogeneous, so it is necessary to look for risk and prognostic factors to focus on clinical management.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Allenbach et al. recently proposed a clinical classification of dermatomyositis associated with anti-MDA5 autoantibodies. The authors distinguish three clinical phenotypes: (a) the first one is made up mostly of women with rapidly progressive ILD (RP-ILD) associated with mechanic’ hands, with a high mortality rate (80%); (b) a second rheumatological group also composed mainly of women, with development of arthralgia or arthritis, with less frequency of RP-ILD and a better survival prognosis; and (c) the third phenotype described is made up of men with predominant symptoms of skin ulcers, digital necrosis, calcinosis, associated with proximal muscle weakness and relatively infrequent RP-ILD.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a> The cohort of patients analyzed in this work is challenging to place in any of the three proposed clinical phenotypes since it predominantly comprises women with respiratory symptoms. Although the findings related to cutaneous signs were not predominant in the group, these patients presented poor clinical prognoses.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Dermatological signs, especially skin ulcers, have been previously reported in patients with RP-ILD positive to anti-MDA5 autoantibodies.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">9,16,17</span></a> Our study confirms this previous observation. Patients with dermatological signs had higher titers of anti-MDA5 autoantibodies, reflecting a more inflammatory anti-MDA5 ILD group of patients with a worse prognosis and, perhaps, a different pathophysiology pathway. Mechanics’ hand sign was frequent in the patients of this cohort; this clinical sign was initially described in polymyositis positive to antisynthetase or PM-scl autoantibodies. Mechanics’ hand sign is characterized by fissuring and cracking of the palms, the radial side of the fingers, and the ulnar aspect of the thumbs. In the same way as Allenbach et al.,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a> we decided to report and analyze the mechanic's hand sign separately from the classic dermatomyositis rashes. Our study confirms the high prevalence of mechanic's hand sign in anti-MDA5 positive patients. Mechanics’ hand sign was not associated with mortality in this cohort (HR: 1.19 (95% CI: 0.31–4.48)).</p><p id="par0085" class="elsevierStylePara elsevierViewall">MDA5 was initially identified in 2002 as an IFN-1 inducer in human melanoma cells; the primary function of this sensor is to activate lytic mechanisms against human cancer cells.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">18</span></a> Later, the function of MDA5 was described most thoroughly as an inducer of IFN-1 production and other genes that collectively neutralize viral infections, particularly those caused by RNA viruses.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">15,19</span></a> On the other hand, due to this essential function for the identification of viruses, the expression of MDA5 has been reported in multiple tissues, although in low quantity in physiological contexts<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a>; however, in the development of dermatomyositis, overexpression has been found in lung, skin, and muscle, which is associated with the symptoms described in patients with anti-MDA5.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">21</span></a> However, in patients with dermatomyositis, overexpression of MDA5 in the skin and some other tissues, even on the cell surface, has been reported.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">21</span></a> On the other hand, the relationship between the levels of anti-MDA5 autoantibodies and the development of the disease has been previously documented<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">3</span></a>; which is confirmed by the findings made in this work since the highest levels of autoantibodies were present in those patients with greater respiratory manifestations and even pulmonary failures were the causes of death.</p><p id="par0090" class="elsevierStylePara elsevierViewall">On the other hand, the most significant number of cutaneous signs could corroborate a previously proposed hypothesis regarding pathophysiology; large titles of circulating autoantibodies could bind to MDA5 ectopically overexpressed in several tissues such as the skin, which would cause the formation of immune complexes capable of inducing complement fixation or antibody-dependent cytotoxicity, initiating epithelial lesions or further worsening existing ones.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a> In addition, the quantification of anti-MDA5 autoantibodies made in this protocol has shown to have a prognostic value since the highest levels were strongly associated with death secondary to RP-ILD observed at follow-up, which had a median of 376 days. Contrary to previous observations,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> lymphopenia and CRP were not associated with survival.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Our study confirmed the difficulty of treating ILD patients positive for anti-MDA5. At follow-up, 50% of the patients had a modification of the initial medical treatment due to no response to initial medical treatment or worsening of pulmonary disease. Several treatment schemes have been described, and none have been clearly demonstrated as safe and effective. Recently, a triple combination of plasmapheresis, cyclophosphamide, and tacrolimus has been described as effective in treating RP-ILD, although with an increased risk of infections.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">22</span></a> Moreover, several case reports of successful outcomes in patients positive for anti-MDA5 autoantibody with tyrosine kinase inhibitors have been reported.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">23–25</span></a> Our findings may add information from prognostic factors that may be useful in designing clinical trials for treating ILD patients positive to anti-MDA5 autoantibodies, allowing us to adequately stratify patients with dermatological signs as a high-risk group for a worse prognosis.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Our study has several limitations; one is the heterogeneity of medical treatments that may confound the evaluation of associations. The sample size did not allow us to perform multivariable analysis to adjust for confounding, especially to adjust for the presence of dermatological signs with the titers of anti-MDA5 autoantibodies. We recognize that further studies must confirm the results of this study. Moreover, no ferritin or other biomarkers, such as KL6 baseline values, were available. Both biomarkers have previously been reported to have worse survival.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">26</span></a> Nevertheless, we consider that the data provided may be helpful in the risk stratification of patients with ILD positive to anti-MDA5 and confirms a simple clinical observation, as are dermatological signs that are clearly associated with worse survival in this clinical condition.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Finally, taking all our results together, we can conclude that cutaneous manifestations such as erythema and cutaneous ulcerations may be associated with short-term fatal outcomes in patients with ILD associated with anti-MDA5. Further studies must confirm the results of this study. These results open the possibility of future explorations and guide better clinical management of patients with this autoimmune entity.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Ethical considerations</span><p id="par0110" class="elsevierStylePara elsevierViewall">The local institutional review board approved the study protocol (approval code number: C03-20).
