Resumen
Objetivo

Determinar la utilidad de la biopsia con aguja gruesa guiada mediante estereotaxia en el estudio de calcificaciones mamarias.

Material y métodos

Estudiamos retrospectivamente los casos de 71 mujeres a las que se realizó biopsia estereotáxica y biopsia quirúrgi-ca por presencia de calcificaciones en el estudio mamográfico. Todos los casos se revisaron mediante doble lectura radiológica y las calcificaciones se clasificaron siguiendo los criterios de la terminología BI-RADS en categorías 3, 4 y 5.

Resultados

De los 71 casos, 19 fueron clasificados dentro del gru-po 3, 30 en el grupo 4 y 22 en el grupo 5. La biopsia con aguja gruesa reveló carcinoma en 30 casos, hiperplasia ductal atípica en 8, lesión esclerosante compleja en 1 y en 32 lesiones benignas de las cuales 14 fueron falsos negativos tras la biopsia quirúrgica. Para las calcificaciones de categoría 3, la biopsia con aguja gruesa presentó un VPP del 100% y un VPN del 75%, y en las calcificaciones categorías 4 y 5 tuvo un VPP alto (90,62%), y sin embargo, un VPN bajo (42,85%).

Conclusiones

En las calcificaciones del grupo 3, la biopsia con aguja gruesa es un método útil para decidir el seguimiento radiológico o la realización de biopsia quirúrgica. En los grupos 4 y 5, la técnica presenta limitaciones, siendo útil para planificar el procedimiento qui-rúrgico. En los casos en que se obtiene diagnóstico de hiperplasia duc-tal atípica y carcinoma in situen la biopsia con aguja gruesa, se debe indicar realización de biopsia quirúrgica.

Objective

To determine the utility of stereotactic core needle biopsy in the study of breast calcifications.

Material and methods

We performed a retrospective study of 71 women who had undergone stereotactic biopsy and surgical biopsy after mammography had revealed the presence of calcifications. All radiologic images were subjected to double reading and the calcifications were classified as categories 3, 4 or 5 according to the Breast Imaging Reporting and Data System (BI-RADS).

Results

Of the 71 patients, in 19 the lesions were classified as category 3, in 30 as category 4 and in 22 as category 5. Core needle biopsy revealed carcinoma in 30 cases, atypical ductal hyperplasia in 8, a complex sclerosing lesion in 1 and benign lesions in 32, 14 of which were shown by surgical biopsy to be false negatives. Core needle biopsy had a positive predictive value (PPV) of 100% and a negative predictive value (NPV) of 75% in category 3 calcifications, while in categories 4 and 5, the PPV was 90.62% and the NPV only 42.85%.

Conclusions

Core needle biopsy is a useful tool to indicate the suitability of radiologic follow-up or the need for surgical biopsy in category 3 lesions. In lesions corresponding to categories 4 and 5, the utility of this technique is more limited, although it aids in planning the surgical procedure. Surgical biopsy should be carried out in all cases in which core needle biopsy reveals the presence of atypical ductal hyper-plasia or carcinoma in situ.
