Objetivos

Analizar las discrepancias diagnósticas encontradas entre la resonancia magnética (RM) y la artroscopia y determinar las causas de las mismas.

Material y métodos

Se revisaron retrospectivamente las explora-ciones de RM de 248 rodillas. Cuarenta mostraron discrepancias diag-nósticas entre los resultados de RM y la artroscopia. Dos radiólogos reanalizaron independientemente las imágenes de 29 de los 40 estudios sin conocer el diagnóstico que previamente se había realizado en la RM y la artroscopia. Se correlacionaron sus interpretaciones con el diagnóstico de RM inicial, las imágenes de RM y los resultados de la artroscopia. Los errores diagnósticos originales de RM fueron clasifi-cados como errores inevitables, errores de interpretación y errores se-cundarios a hallazgos equívocos. No se pudieron revisar 11 exámenes de RM ya que no se pudieron recuperar las imágenes correspondientes.

Resultados

De 34 errores encontrados en los diagnósticos origina-les, 12 (35,5%) fueron clasificados como inevitables, 14 (41,2%) como errores de interpretación y 8 (23,5%) como secundarios a hallazgos equívocos.

Conclusión

Se evitaron el 41,2% de los errores retrospectivamen-te, probablemente debido a una experiencia mayor en la interpretación de RM en nuestro departamento. El 35,5% fueron inevitables incluso en la revisión retrospectiva. Un porcentaje pequeño de errores diagnós-ticos fue debido a la presencia de hallazgos sutiles equívocos.

Abstract
Objective

To analyze diagnostic discrepancies found between magnetic resonance (MR) and arthroscopy, and to determine the reasons that they occur.

Materials and Methods

Two-hundred and forty-eight MR knee explorations were retrospectively checked. Forty of these showed diagnostic discrepancies between MR and arthroscopy. Two radiologists independently re-analyzed the images from 29 of the 40 studies without knowing which diagnosis had resulted from which of the two techniques. Their interpretations were correlated with the initial MR diagnosis, MR images and arthroscopic results. Initial errors in MR imaging were classified as either unavoidable, interpretive, or secondary to equivocal findings. Eleven MR examinations could not be checked since their corresponding imaging results could not be located.

Results

Of 34 errors found in the original diagnoses, 12 (35.5%) were classified as unavoidable, 14 (41.2%) as interpretative and 8 (23.5%) as secondary to equivocal findings.

Conclusion

41.2% of the errors were avoided in the retrospective study probably due to our department’s having greater experience in interpreting MR images. 25.5% were unavoidable even in the retrospective study. A small percentage of diagnostic errors were due to the presence of subtle equivocal findings.
