Objetivo

Valorar la utilidad del enema de bario en pacientes con colonoscopia incompleta.

Material y métodos

Durante un período de 10 meses se realizó un estudio prospectivo de 45 pacientes con colonoscopia incompleta (27 varones y 18 mujeres), que fueron examinados posteriormente mediante enema de bario (33 con técnica convencional y 12 con técnica modificada de doble contraste) con el fin de determinar la existencia de afección adicional en las porciones de colon no visualizadas en la colonoscopia.

Resultados

El enema de bario diagnosticó seis posibles lesiones adicionales (13,3%) en las porciones de colon no visualizadas en la co-lonoscopia incompleta (cuatro neoplásicas y dos no neoplásicas). Respecto a las lesiones neoplásicas, dos fueron verdaderos positivos y dos falsos positivos. Los dos verdaderos positivos correspondieron a adenocarcinomas (uno en ciego sincrónico y otro en ángulo esplénico). Los dos falsos positivos correspondieron a restos fecales adheridos a la pared intestinal. Respecto a las lesiones no neoplásicas, se detectaron múltiples estenosis en un paciente afecto de enfermedad de Crohn, y una fístula enterocólica en un paciente con una neoplasia de sigma.

El rendimiento diagnóstico del enema de bario en la detección de afección adicional en la porción de colon no visualizada mediante co-lonoscopia fue del 9%.

Conclusión

El enema de bario tras la colonoscopia incompleta permite la valoración completa del colon en la mayoría de los casos y ofrece información diagnóstica adicional.

To evaluate the usefulness of barium enema in patients with incomplete colonoscopy.

Materials and methods

There was carried out a prospective 10-month study of 45 patients with incomplete colonoscopy (27 men and 18 women), who were later examined by means of barium enema (33 conventional, 12 double-contrast) in order to check for additional pathology in portions of the colon not visualized by colonoscopy.

Results

Barium enema diagnosed six possible additional lesions (13.3%) in portions of the colon not visualized by incomplete colonos-copy (four neoplasias and two non-neoplasias). Regarding the neopla-sias, two were true positives and two false positives. Both true positives were adenocarcinomas (one synchronous caecum, and another in splenic angle). The two false positives corresponded to fecal matter stuck to the intestinal wall. Regarding the nonneoplasias, multiple stenosis was detected in a patient with Crohn’s disease, and an enteroco-lic fistula was found in a patient with sigmoid colon neoplasia.

The diagnostic yield of barium enema in the detection of additional pathology in colon portions not visualized by colonoscopy was of 9%.

Conclusion

Barium enema following incomplete colonoscopy permits a complete colon evaluation in most cases, and it offers additional diagnostic information.
