Abstract

Transcatheter aortic valve implantation (TAVI) is the alternative to surgical valve replacement, expanding its indications in the latest guidelines. Multimodal CT (MDCT) is essential in patient selection and detection of complications. Vascular complications are frequent, so it is important to analyse the anatomy of the vessels before the procedure. Regarding annular ruptures and ventricular perforations, the volume and distribution of calcium and the ventricular diameter play an important role. Finally, valve migration is a rare complication that can occur both during and after TAVI. Proper planning of the MDCT procedure reduces the risk of complications and gives the interventional cardiologist security both before and during the procedure.

El implante valvular aórtico transcatéter (TAVI) es la alternativa al reemplazo valvular quirúrgico. En las últimas guías se han ampliado sus indicaciones. La TC multimodal (TCMD) es fundamental, tanto en la selección de pacientes como para valorar las complicaciones. Las complicaciones vasculares son frecuentes, por lo que es importante analizar la anatomía de los vasos previo al procedimiento. En cuanto a las roturas anulares y perforaciones ventriculares, tanto el volumen y la distribución del calcio, como el diámetro ventricular juegan un papel importante. Finalmente, la migración valvular es una complicación rara que puede ocurrir tanto durante como después del TAVI. La planificación adecuada del procedimiento mediante la TCMD reduce el riesgo de complicaciones y da seguridad al intervencionista antes y durante el procedimiento.
