It commonly involves an inability to relax, apathy towards work, occupational isolation, and most importantly, lower levels of quality and satisfaction at work.

Medical professionals work in emotionally stressful environments and the nature of their work entails high levels of responsibility. When these conditions persist over an extended period of time or are exacerbated by other circumstances, they can lead to psychological issues among healthcare staff. The IT and technological boom has brought about new and increasingly complex diagnostic and therapeutic radiology tests which have led to a notable increase in radiologists’ workloads and the attendant responsibility, which, in turn, raises the risk of this pathology among this group of professionals.6

Several studies in various countries have studied the prevalence and nature of occupational burnout among medical professionals.7–10 A recent study in the United States ranked the prevalence of burnout among healthcare staff, placing radiologists in seventh position out of a total of 20 specialities. Secondary aims of this article are to identify factors which could trigger or accentuate the problem so as to guide the design and implementation of preventive and corrective measures. This, in turn, could lower the stress associated with the issue and potentially improve work-based performance and job satisfaction among radiologists.
Material and methodology
A cross-sectional, observational, descriptive and analytical study was carried out with an anonymous online questionnaire, designed using Google Forms ® (Google LLC, Alphabet Inc., Mountain View, California, USA) It was sent out to practising and trainee radiologists in Spain and completion was entirely voluntary. The questionnaire was disseminated as widely as possible through several different channels of communication and was available for seven months (from January to June 2019). To be more precise, a direct link to the questionnaire was sent by email, WhatsApp, Twitter and Facebook along with a request for collaboration and to share the survey as widely as possible among the branches and affiliates of Spain’s radiology societies. It is not known how many radiologists and radiology trainees received the request.

Prior to filling out the questionnaire, participants were informed that their involvement was completely voluntary, any data provided would only be used for research purposes, and personal data would remain anonymous at all times. The only inclusion criterion to determine the validity of a response was completion of the MBI-HSS questionnaire. Any questions left completely or partially unanswered were excluded.

The questionnaire consisted of three main parts (Appendix B, Annex 1). The MBI-HSS questionnaire comprised 22 items divided into three main sections: emotional exhaustion was assessed by nine items, depersonalisation by five and feelings of low personal accomplishment by eight. Participants had to grade each item according to a scale that indicated how often they experienced each situation at work. The scale ranged from zero (‘never’) to six (‘every day’). The final scores from each section were obtained by adding together the scores of all items in the section. The three sections were then classified into three grades (low, medium and high) according to cut-off points which were taken from those previously established in the literature,4,5,14 as shown in Table 1. This section focused on the working conditions of radiologists, recognising that they have changed significantly over the past decade, primarily due to the IT and technological boom which has completely transformed the radiological working environment. The questions in the third part focus on radiologists’ perceptions and opinions regarding possible causes of stress and factors that could improve their work situation (Appendix B Annex 1).

A database generated from the responses to the questionnaire was used to regroup the variables and levels, homogenise the data, and analyse the coherence of the responses. It was then used for the numerical and visual descriptive analysis of the variable. Later, the database was also used to carry out different statistical analyses. The chi-square test of independence was used to analyse the association of different nominal categorical variables with both the burnout and non-burnout groups. The following correlation coefficients were used to analyse the correlation with the presence of burnout: the phi coefficient for two binary nominal categorical variables; Cramér’s V for nominal categorical variables that have more than two levels; Kendall’s tau for ordinal categorical variables, and the point-biserial correlation coefficient for continuous variables. The two sample Z test of proportions was used to analyse differences between the different groups within a variable (such as speciality, autonomous community, and gender), as well as differences in responses given with regard to causes of stress and improvements to morale among those suffering from burnout. In all the aforementioned statistical tests, a p-value of less than 0.5 was considered statistically significant. Lastly, the Odds Ratio (OR) was used to analyse risk and protective factors. In this case, any factor whose OR did not include 1 within the 95% confidence interval (CI)—meaning that prevalence for the condition or factor analysed is the same for the high risk group as for the low risk group—was considered statistically significant.

The following software was used for data processing and statistical analysis: Spyder (v4.1.4) and Jupyter Notebook (v6.0.3) with Python version 3.8.3 and the Pandas (v1.0.5), NumPy (v1.18.5), SciPy (v1.5.0) and Matplotlib (v3.2.2) packages.
Results
A total of 226 radiologists completed the online form in the seven-month period between January and July 2019. It should be noted that those who work in a centre involved in a training programme are more protected against the phenomenon. Morale among radiologists can be improved by improving the service organisation, reducing workloads, and assigning fewer on-call shifts. The most prevalent causes of stress are chaotic working conditions, on-call shifts, heavy workloads, and a lack of time to spend with one’s family.

The Ganeshan study in the United States15 obtained similar results and revealed that 29% of radiologists matched the criteria of occupational burnout. The researchers identified several stress factors in the workplace such as heavy workloads, the inability to strike a work-life balance, concerns over not attaining a higher academic rank, lack of autonomy at work, lack of recognition by other specialists or patients, and worries over litigation or allegations of negligence. In Canada, 72% of radiologists and radiology trainees experienced at least one symptom of occupational burnout.16 Other recent studies among musculoskeletal radiologists and neuroradiologists demonstrated that up to 80% of respondents presented high levels in one or more dimensions of occupational burnout.17,18

Likewise, our study revealed that in the burnt-out group, most participants highlighted on-call shifts, chaotic working conditions, the high volume of studies assigned to them each day, and the fact that their job did not leave them enough time to spend with their families as the main stress factors.

Similarly, radiology trainees experience greater levels of occupational burnout than their counterparts from other specialities. A recent meta-analysis, which included only the articles that used the MBI questionnaire, revealed that radiology trainees are in the top three disciplines to experience demotivation, together with those specialising in neurology and general surgery.8 It has been suggested that this is related to the isolated nature of radiology practice, the central role of imaging in diagnoses, and the need to be familiar with many different diseases as a factor of exhaustion, especially during on-call shifts at the beginning of the training programme.19 Another consideration is how the breadth of knowledge required in very little time at the start of the training programme may increase the perception of exhaustion among trainees due to their inexperience in managing and diagnosing unknown conditions.20 Trainee wages are relatively low with a current base salary of EUR 1,100 and this, when coupled with generally high costs of living adds another layer of stress for radiology trainees.

More than half of the respondents stated that they are strongly dissatisfied with current wage levels. Medscape carried out a questionnaire that revealed that all healthcare salaries in Spain are significantly lower than their European counterparts, even equating to 42% less than salaries in Germany or the UK.21 The Spanish Health Ministry acknowledged as much when it published a report setting out average wages.22 Yet, the literature suggests that one of the best ways to prevent and treat occupational burnout is to find the right combination of work-life balance and financial expectations without compromising family or the quality of one’s work.6,23

The only factor we have identified as safeguarding against occupational burnout is the centre’s involvement in a training programme. It may be that this reduces the sense of isolation6,24 or because the trainees might help with administrative tasks that can take up a lot of the radiologist’s time.25–27 However, it should also be noted that other studies have found that work-based teaching can decrease radiologists’ productivity by up to 30–50%.28

Research on predisposing factors also finds that radiologists who work a greater number of night shifts may be exposed to a higher risk of burnout,29,30 as sleep deprivation impairs connectivity and processing within brain structures, in turn leading to emotional dysregulation.31 Insufficient sleep also results in a reduced ability to sustain attention and can therefore affect image interpretation and report writing, with a consequent increase in medical errors.32,33 Improvements in the organisation of the service, a decrease in the number of on-call shifts, and the number of studies assigned to each professional have also been reported to help improve morale.

Several studies have reported that heavy workloads in radiology are one of the main sources of work-related stress.6,17,30,34,35 In our sample, the + OBG considers the high number of studies assigned each day to be a source of stress and emphasizes that a reduction in this number could improve morale. This factor may stem from the significant workload increase in radiology over the last 30 years due to the massification of digital studies stored in Picture Archiving and Communications Systems (PACS). Today, imaging often needs to be analysed in conjunction with large amounts of data, and with voice recognition and desktop publishing software. This is in addition to the general increase in the number of studies to be read by radiologists.29,34

Bhargavan observed a 70.3% increase in annual relative value units (RVUs) per FTE (full-time equivalent) radiologist from 1991–92 to 2006–07.34 Another study carried out in the UK between 2007 and 2010 which analysed the number of CT images evaluated, noted a 49–75% increase during those years.25 In our research, 80% of respondents stated that they have problems with their computers and IT systems and sense a lack of control over their own work. These results are worrying and this problem is likely to escalate in the future.

Other factors that contribute to the perception of a heavy workload include prolonged working hours with greater responsibilities outside normal hours, less time assigned to report writing,29 conflicting demands on time (clinical, academic, administrative) and insufficient staff to carry out routine tasks.35

Although there were no statistically significant differences between men and women in our study, our literature search revealed a strong association with gender and an increase in occupational burnout rates in radiology. Women are much more likely to experience burnout than men.17,36 Our results may be due to the emancipation of women in medical roles in recent years in a healthcare culture where workplace equality has been in place for many years and where women commonly hold positions of leadership and responsibility. Likewise, the dual role of mother-partner and doctor has changed in recent years and it is now much more common for the partner to share the household work.

While our study did not find a correlation between occupational burnout and the age of the radiologist or the years worked, it has been noted that the prevalence of burnout reduces with age and after more years worked as a radiologist.15 This discrepancy may be explained by the fact that after a certain number of years, a radiologist can obtain a position which allows them to better control their working hours and activities. Furthermore, their care-based responsibilities at home may now be reduced and they are likely to be in a better financial position or may have developed more skills for dealing with and preventing occupational burnout. It is also possible that the prevalence and severity of occupational burnout negatively affects the longest-serving professionals and so radiologists suffering from occupational burnout may have left the profession, leaving only those who have not burned out to the same degree.37

One of the main limitations of this research lies in the fact that there is no precise figure for the number of radiologists in Spain. In 2019, the Spanish Society of Radiologists (SERAM) had 6,000 members. A total of 226 radiologists answered our questionnaire and if we consider that they are likely members of SERAM, this still only accounts for 4%. Our small sample size should be considered a significant limitation as it does not guarantee that the results can be extrapolated. Another limitation of our questionnaire is that there may be differences in the response rate between radiologists in the +OBG and −OBG. It seems plausible that radiologists suffering from occupational burnout may have been more drawn to responding to the questionnaire, thus leading to bias. It is also possible that other professional, personal or family-related stress factors can contribute to the exhaustion felt by radiologists.

After reviewing the results of our survey, it strikes us that there may be other work-based and external factors that our study has not envisaged. Among the work-based factors that could contribute to exhaustion, but which were not taken into account are: the increase in the number of studies assigned to each radiologist, the complexity involved in each study, and the time spent on other administrative tasks and/or interruptions. The radiologist’s home and personal environment should also be taken into consideration as this could be an important protective or risk factor, depending on individual circumstances.
Conclusions
This article describes the impact of occupational burnout among radiologists in Spain.
