"textoCompleto" => "
The SARS-CoV-2 pandemic has a dramatic impact on millions of people around the globe. Medical doctors, nurses, radiographers and many other healthcare personnel are on the forefront of battle against the new coronavirus. Radiology as a specialty is mainly involved in the diagnosis and follow-up of typical lung changes1 and, to a lesser extent, extrapulmonary manifestations.2 Considering the unparalleled challenges with which the health services of many countries are confronted with, the effect of the pandemic on scientific radiological congresses scheduled 2020/2021 (e.g. ECR, SERAM, ESGAR) looks just like a footnote. This footnote of transition from face-to-face congresses to virtual gatherings, to some extent, is a reflection of how the serious global emergency of the SARS-CoV-2 pandemic has generally forces the society explore new and creative ways of interaction and communication.

This editorial looks at the emerging advantages with the new online format and also some key technical points in order to make it a success. Then, the undisputed benefits of traditional live meetings are briefly reviewed. The pros and cons of both formats will have major implications for future meetings of national and international radiological societies. Regardless of the future format, however, the success will still be fundamentally related to an appealing programme and engaging speakers who are interesting to the audience.3

When travel restrictions and lockdowns were put in place by a rapidly increasing number of countries in early 2020, all planned major radiological congresses were canceled. Some of them such as ECR and SERAM were first postponed to dates later during the year before decisions were taken to either run a totally virtual meeting (ECR 2020) or to reschedule the congress in 2021 (35 Congreso SERAM, May 19–22, 2021, Zaragoza). It is a sign of high-level professionalism and well established digital infrastructure how many national and international radiological societies successfully converted their traditional annual congresses to virtual meetings at very short notice. Likewise, the willingness of speakers to prerecord talks or deliver live online lectures and to be available for live chats during sessions as well as the high number of registrations indicate that our specialty is both flexible enough and well prepared for the modern electronic age.4

It is remarkable, if not surprising, to see the number of positive aspects which have already been identified with virtual meetings (economy, communication, energy, etc). Some very recent publications have reviewed potential advantages of the new era. Statements include that virtual conferences can offer more than an alternative to conventional face-to-face events and that the shift for some may even be permanent.5–7 Others take it even further in that it is well past time for scientists to revise conference paradigms and embrace virtual participation to decrease travel and enrich conference experiences and that scientific leaders should embrace creative models for virtual participation as a routine element of academic conferences.17

Seemingly, there is a lot to like about virtual conferencing. Completing the logistics and administrative work for an online conference takes a significantly shorter time frame and financial resources compared to live meetings.8,9 It may be premature to give solid data but the American Astronomical Society estimated costs for their virtual meeting in June 2020 to be around 60% less compared to an equivalent live gathering in January 2020.7 Virtual conferences potentially attract a greater number of attendees,7,10 and many of the booming online video conferencing platforms are capable of dealing with the larger audience accordingly.4 Virtual meetings are more inclusive and facilitate geographic diversity.8 Considering that most of the costs of a live meeting are usually related to travel and lodging,3 online access to cutting-edge science and education is provided to radiologists around the world who would otherwise not be able to attend a far-off conference in person.8 Cost-savings of online meetings can be passed on to registrants,9 and lower registration fees may be an additional incentive for audience participants including students and radiographers of which many were deprived from in-person conferences in the past because of cost barriers. In addition, there is considerable time saving as traveling back and forth to join a congress is not necessary.11 This not only applies to the general attendees but also to busy speakers who sometimes cannot afford to spend more than just the day of their presentation(s) onsite. An increasingly appreciated significant by-product is the reduction of greenhouse gas emissions resulting from far less traveling to conferences.5,6 Estimates of the carbon cost of conferences vary, but range from 0.5 to 2 or more tones of CO2 per participant in travel alone.7 The sum total of travel associated with attendance at one large academic conference can release as much CO2 as an entire city in a week.10

Most virtual congresses offer online access by archiving pre-recorded lectures for registered attendees and scientific society members over a couple of weeks, if not months. So people can watch all presentations from the comfort and safety of his/her home and at the time of their choosing (“asynchronous learning”).9,12 The European radiology flagship meeting ECR 2020 has put together an additional programme of ten “Online Highlight Weeks” from July to December 2020 (2–4 lectures per day). With this approach, attendees have the freedom to follow a dedicated topic every week without missing lectures when sessions run in parallel at a live meeting. Other conferences open access to recorded talks ahead of time to give participants the opportunity to better prepare for online conversations on the day and time of the session.13 Another feature which suits online gatherings more than traditional face-to-face congresses is the presentation of last minute topics and talks shortly before or during a meeting (granted that appropriate speakers are prepared to deliver).14

Questions and answers are an integral part of scientific meetings. In the case of virtual conferences, an almost unlimited number of questions can be posted in chat rooms during and after an online presentation. At the same time, moderators have the opportunity to screen questions and prioritize the most insightful inquiries. So lengthy statements or pontifications which can sometimes be seen during the limited time for discussion at live meetings are avoided.4,6,14 If a live question and answer-format is preferred, this can also be implemented with state-of-the-art communication platforms. As a virtual equivalent of personally approaching a speaker after a talk, follow-up e-mails can be sent. Some attendees might even find digital communication with eminent speakers to be less intimidating than a face-to-face encounter.15 Alternatively, a post-talk session in a break out room may be offered where speakers can chat with members of the audience who are interested in further details.

There are technical items and challenges which are important to consider in order to have a successful online conference. The platforms currently available for virtual conferencing with hundreds, if not thousands, of attendees logged-in for the same session generally run smoothly albeit not perfectly yet. There are still moments of less-than-optimal sound and occasional technical “glitches”.15 Video links can utilize significant bandwidth, Therefore, a nimble PCO (professional conference organizer) together with a savvy IT provider who can quickly get on top of any technical issues as they arise are critical.17

There are requests of attendees to permit to turn on their cameras to make the conference more interactive and to provide a better sense of community. However, too many registrants turning on their cameras carry the risk to overload the system and negatively affect the overall audio and video quality. It may also make meetings more vulnerable to cyber-attacks.8 In general, online formats are exposed to all known online safety issues, such as network or hardware failures, hackers’ attacks now known as “zoombombing”, and other IT safety concerns.4 Data confidentiality and copyright are additional risks since it is not know who is listening or possibly recording the talks. Nevertheless, with the ubiquitous smartphone, even live closed meetings are not today immune to such unethical behaviour.8 But, no doubt, with increasing use and experience these bugs, shortcomings and safety issues will be overcome, and attendees can enjoy the new virtual reality of scientific gatherings.

Some societies such as ESGAR had very limited time for the online transition of an entire meeting. According to the overwhelmingly positive evaluation results, the virtual ESGAR 2020 and also other online radiological congresses (e.g. ECR 2020) went extremely well. The great success, to a considerable extend, was related to the technical expertise that staff members in the offices of the societies already had before the pandemic. For example, they successfully assisted speakers in getting them comfortable with virtual platforms for either prerecording talks or live presentations. One of many lessons learned is that any platform is no better than the staff who operates it.

Not surprisingly, there are significant downsides of virtual meetings or, to put it differently, particular benefits of live conferences. Despite the fact that much of what we can do in-person can occur with almost equal quality through virtual formats our humanistic sense notes the importance of face-to-face meetings, i.e. virtual conferences can’t entirely replicate the conference experience.6 In fact, there are key features of face-to-face meetings which may not be achieved on the same level in the virtual format. For example, serendipitous encounters between sessions and small talk at the posters or the hallway of a conference center, which is where a lot of collaboration normally happens, live exchange with scientists working in the same field, meetings with potential future collaborators, creation of proposals and projects through random interactions with people and live social networking, either intended or randomly, one-on-one time spent with potential employers, residency programs, and fellowship directors11 and fresh perspectives from simply leaving one’s home ground.15 People also travel to meetings to take advantage of opportunities for tourism and reconnect with old friends and colleagues3 which highlights the strong social but also cultural component to conferences.11 Interestingly, these issue not only seem to apply to the more senior colleagues who grew up mostly with traditional meeting formats (“digital immigrants”) but also to young radiologists (“digital natives”).

Face-to-face meetings offer a unique opportunity to see world-known experts live. Being in a lecture hall packed with people eager to listen to a charismatic and highly skilled speaker in the field lecturing about a compelling topic is a great live experience that goes beyond the same lecture from the same speaker on a computer at home. Live speakers can engage an audience also by nonverbal cues and body language that a remote speaker cannot. It is a bit like watching a champions league match in Estadio Santiago Bernabéu or Camp Nou either live or in front of the TV at home. Presenters, for their part, could find it challenging to deliver a talk to a monitor instead of a live audience. Without seeing people’s faces it is almost impossible to know what kind of reaction the speaker gets and to tell if people understood what she/he said.15 It is like an actor on stage in front of empty seats and lack of response from the audience, including applause.

However, not only experienced speakers may find a virtual format challenging. The young and ambitious radiologist who’s abstract has been accepted may also miss the unique albeit stressful atmosphere of a live presentation — rehearsing in the slide preview room side by side with other young speakers, introduction to the senior moderator(s) in the lecture room, presentation of the talk within the allotted time, answering of questions from the moderator(s) and/or the floor. This exciting, very motivating and potentially career-building experience is hard to replicate on a home computer although some may prefer the virtual format as this setting is less stressful than in front of a live audience.8 One should also consider that sending a young radiologist to attend a conference is an excellent way to reward a successful project. A virtual conference cannot provide motivation in this way.

Attending from home comes along with other potential distractions such as incoming private e-mails, messages and phone calls as well as simultaneous home obligations.14 All of this does not typically interfere with face-to-face schedules. Also time zone planning of live sessions which is a relevant issue with international virtual meetings3,8 does not apply to in-person congresses. Attending an in-person congress transfers the attendee from the work and home environment to the stimulating learning atmosphere of the congress venue with many colleagues, interesting educational talks, scientific sessions and viewing of posters throughout the day. In case that a virtual meeting is followed from the workplace in hospital it is all too easy to divert one's attention to other work-related matters or the internet.16 As well, logging into the meeting may be problematic, especially in work settings where security measures may hinder the ability to log-in easily and reliably.8 Attending from home comes along with other potential distractions such as incoming private e-mails, SERAM) and international radiological societies (e.g. ESR) play a pivotal role in the promotion and support of science and education in radiology. Accordingly, the organization of annual meetings and regular workshops is one of their essential duties. These events are often a major revenue stream for societies and generate a substantial fraction of the operating budget.7 Therefore, cancellation of meetings, technical exhibitions and industry-sponsored satellite symposia appears to be a considerable economic damage, and the issue of industry support needs to be addressed carefully. The transition from live to virtual meetings instead of total cancellation is also financially challenging as economics of online meetings are different from conventional conferences. Although costs are lower, some societies lost money on their online meeting because the shift was done so quickly and attendance was granted at no charge (e.g. refund offered to paid attendees) or lower registration fees.7 Preparing and running a totally virtual meeting is a different task compared to in-person conferences. It requires investments in people with special skills either from IT companies and/or trained staff in the society’s office. In this case, the loss of revenues from the technical exhibit may partly be compensated by commercial spots placed on the online meeting platform12 or by a virtual exhibit hall with time built into the schedule for registrants to interact with the companies.9 Preparing and running a hybrid meetings (i.e.live and online conference) will potentially drive up costs because it would require more staff, and both a venue and an online platform.7

The sustained uncertainty about the further course of the pandemic has already forced some radiological societies to cancel also the 2021 live annual congresses (e.g. ESGAR, ECR) and to hold virtual meetings instead. As the operating budget may shrink accordingly the future of some scientific societies could be seriously jeopardized.12 Many of them have a long-standing tradition and provide extensive services to their members. To ensure that financial ramifications are not deleterious to the societies the return to in-person meetings as soon as the pandemic allows almost looks like a must. Alternatively, a virtual meeting with much more registrations at a lower fee but also little added costs may safeguard their budgets.9

What will be the future of radiological conferences once the SARS-CoV-2 pandemic fades and/or safe and efficient vaccines are broadly available? It can be assumed that virtual meetings lose some of their appeal once stay-at-home requirements and travel restriction loosen.6 A fair number, if not many, audience participants will eagerly return to traditional live congresses which will still remain indispensable for sharing culture and knowledge. As the pandemic teaches us, however, changes to deeply embedded and seemingly intractable practices like live conferences can happen in a global emergency with remarkable speed.10 Accordingly, aspects of today’s virtual “footprint” may become part of regular future in-person conferences with both physical and virtual attendees.3,4,9,11,14 Whether we like it or not, the scientific community probably expects this transition to hybrid meetings although costs will increase.6,7 The pressure to change may even increase the longer the community is forced to stay with virtual formats and the more accessible, intuitive, and widely available online platforms are. Therefore, meeting organizers and radiological societies may be well advised to further expand the existing digital products and services.
Declaration of interest
None.
"
