At present, their classification remains that which was updated in 2013, when a group of rare entities was introduced, including idiopathic pleuroparenchymal fibroelastosis and other rare histological patterns.</p><p id="par0015" class="elsevierStylePara elsevierViewall">High-resolution computed tomography (HRCT) is essential in diagnosing patients with suspected diffuse interstitial lung disease (DILD). Radiologists should be aware of their different radiological manifestations and the role of HRCT in studying a patient with known or suspected interstitial disease. A multidisciplinary team (pulmonologists, rheumatologists, radiologists and pathologists) is essential for managing and diagnosing DILD.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Nowadays, special attention should be paid to preventing the development of diffuse interstitial diseases in people with a hereditary predisposition to developing them or in those with systemic diseases with frequent pulmonary involvement. The identification in HRCT of interstitial lung abnormalities that may represent initial (preclinical or subclinical) manifestations of interstitial disease is a controversial topic and is currently under discussion.</p><p id="par0025" class="elsevierStylePara elsevierViewall">In the last decade, there have been significant advances in the diagnosis of DILD, including the availability of effective treatments. Current antifibrotic treatment may influence the course of many interstitial diseases, mainly fibrosing ones, by slowing their progression. In this context, artificial intelligence and the use of specific algorithms will be useful tools in quantifying the post-treatment fibrotic component, assessing the progression or stability of the disease.</p><p id="par0030" class="elsevierStylePara elsevierViewall">This monograph highlights the diagnostic complexity of imaging methods, especially HRCT, in characterising interstitial pneumonias.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Dr Giménez et al. describe in detail the different patterns observed in the HRCT of patients with interstitial lung disease. The clinical, radiological and pathological characteristics in the diagnosis of idiopathic pulmonary fibrosis are discussed by Dr Marcelo Sánchez et al.</p><p id="par0040" class="elsevierStylePara elsevierViewall">In another article, Dr Juan Arenas et al. review organising pneumonia as a pattern of lung response to diverse aetiologies. Dr Susana Hernández et al. discuss the diagnostic role of HRCT in assessing interstitial manifestations associated with connective tissue diseases. The complexity of pulmonary cystic lesions with their various clinical-radiological manifestations has been reviewed and updated by Dr Beatriz Cabeza et al. Dr Silvia Navarro et al. discuss not only the role of smoking in the development of pulmonary fibrosis but also the different parenchymal manifestations associated with tobacco use and the importance of HRCT in its diagnosis. Dr Amador Prieto et al. have conducted an excellent review of HRCT manifestations in a broad spectrum of occupational respiratory diseases (pneumoconiosis, hypersensitivity pneumonitis, drugs, and poisons). The diagnosis of hypersensitivity pneumonitis requires a high level of suspicion, especially in patients with a history of occupational exposure. In the last chapter of the monograph, I wanted, along with Dr Ana Giménez, to include an updated review of pleuroparenchymal fibroelastosis, one of the rare fibrosing interstitial types of pneumonia included in the latest classification of interstitial lung diseases.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Finally, I would like to thank all the authors who have participated in this monograph for their work and dedication to this project.
