Comparación de los resultados mediante técnica segmentaria con barra única y barra de apoyo a única en el tratamiento quirúrgico de la escoliosis idiopática

Se compara el resultado del tratamiento quirúrgico de la escoliosis idiopática en dos grupos homogéneos de pacientes, unos tratados mediante la técnica de artrodesis y fijación con barra única y otros en quienes fue asociada una barra corta apoyada en la barra única. Se realiza un estudio retrospectivo de 36 pacientes (18 de cada grupo) con un seguimiento mínimo de 24 meses. El objetivo de este estudio fue analizar la corrección obtenida, determinando y comparando los resultados finales de estas técnicas quirúrgicas. Las correcciones obtenidas tanto en el plano coronal como en el sagital fueron semejantes con ambas técnicas. La oblicuidad de la vértebra límite y la traslación de la vértebra apical fueron corregidas de forma adecuada con las dos configuraciones. La desrotación obtenida fue semejante en ambos grupos. La corrección del desequilibrio raquídeo fue mejor en el grupo de barra única (52,5%) que en el de apoyo a única (25,3%). Las complicaciones fueron más frecuentes en el grupo de barra única derivadas de fallos del implante (30% de luxaciones de uñas y 11% de roturas de barra), precisándose reintervención en tres enfermos. Con la barra de apoyo a única no se ha requerido ninguna reintervención. La asociación de un tornillo distal y gancho en pinza con técnica de apoyo a única puede ser la solución definitiva de estas complicaciones.

The result of surgical treatment of idiopathic scoliosis was compared in two homogeneous groups of patients, one group of patients treated with fusion and single-rod fixation and the other group with the addition of a short rod to support the single rod. A retrospective study was made of 36 patients (18 in each group) with a minimum follow-up of 24 months. The aim of this study was to analyze the correction achieved to determine and compare the outcome of these surgical techniques. The corrections achieved in the coronal and sagittal planes were similar with both techniques. The oblique angle of the limiting vertebra and translation of the apical vertebra were corrected adequately with both fixator configurations. The derotation achieved was similar in both groups. The correction of spinal imbalance was better in the single-rod group (52.5%) than in the single-rod with supporting rod group (25.5%). Complications were more frequent with the single-rod technique and derived from implant failure (30% hook dislocation and 11% rod rupture). Three patients had to be reoperated. With the single rod and supporting rod, no reoperation was required. The association of a distal screw and clip hook with the support of a single rod could be a definitive solution for these complications.
