G—Ex vivo detection in the surgical specimen.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">We present the case of a 39-year-old man who began a diagnostic study for presenting symptoms of diarrhea, epigastric pain, nausea and vomiting. The upper gastrointestinal endoscopy revealed ulcers in the duodenum and the colonoscopy was normal. CT and MRI showed hypervascular liver lesions suggestive of metastases without showing the origin of the primary tumor.</p><p id="par0010" class="elsevierStylePara elsevierViewall">A liver biopsy allowed the diagnosis of metastatic well differentiated neuroendocrine tumor (NET), grade 1, Ki-67: 10% (positive for chromogranin and synaptophysin). The chromogranin value was 265 ng/mL. The Somatostatin receptor scintigraphy showed liver lesions with moderate-high uptake, and a focus of radiotracer uptake in the small intestine localized by SPECT/CT, suggestive to correspond to the primary tumor (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">In order to localize the primary tumor, we proposed a radioguided abdominal surgery, using the newer, less invasive laparoscopic approach. The preparation consisted in a residue-free diet 48 h before, fasting and colonic preparation 24 h before, and diuretic administration right before the intervention. The radiotracer <span class="elsevierStyleSup">111</span>In-Pentetreotide (141 MBq) was injected the day before surgery.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The small intestine was inspected by laparoscopy, without identifying any injury. Then, a laparoscopic gamma probe review was done, from the terminal ileum, at its junction with the cecum, proximally to reach the angle of Treitz and the duodenum-jejunal junction. A significant increase of uptake was identified, corresponding to a small ulcer at the level of the serosa of a proximal jejunum loop (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Resection of a 15 cm jejunal segment (which included the lesion) was performed, with jejunal-jejunal anastomosis. Afterwards, the radioactivity in the surgical specimen was checked “ex-vivo” (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). The pathological analysis confirmed the diagnosis of NET. The postoperative course was without complications, discharging the patient 48 h after surgery. The patient remains asymptomatic 36 months after the intervention and is being treated with octreotide.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">This type of scenario would correspond to patients with unresectable liver disease and localized primary tumor, in which cytoreductive surgery is not possible, but resection of the tumor is feasible. In this situation, in addition to the clinical benefit obtained in case of functioning tumors, there are retrospective studies that suggest an impact in terms of survival. In such circumstances, primary tumor resection is recommended. Patients may even become hepatic transplant candidates. The surgical approach to small size gastroenteropancreatic primary NETs represents a challenge for surgeons, accounting for up to 30% negative laparotomies.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">There are three key points to successful tumor detection by radioguided surgery. First, an adequate radiotracer accumulation in the tumor, in order to identify it. Second, to achieve an adequate tumor-background ratio. And third, a rigorous intraoperative intestinal check.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Finally, it is necessary to emphasize the prominent role of Nuclear Medicine in the management of NETs, not only in the field of diagnosis, but also in therapy (both in the promising field of theragnosis and radioguided surgery), where it is confirmed as the first line of treatment in certain cases of NETs. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
