They represent about 1–2% of all invasive breast malignancies.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">1,2</span></a> Apocrine cells are characterized by abundant eosinophilic and granular cytoplasm, large centrally located nuclei, and distinctive cell borders with the characteristic androgen receptor (AR) positive, generally estrogen receptor (ER) and progesterone receptor (PR) negative steroidal profile. Overexpression of human epidermal growth factor receptor 2 (HER2) and epidermal growth factor receptor (EGFR) are also frequently documented.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">1</span></a> Morphological differentiation in PBNECs is more difficult. PBNECs are more likely to have ER/PR+, HER2− profile. PBNEC diagnosis is based mainly on the expression of the neuroendocrine markers, including synaptophysin or chromogranin.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">3,4</span></a> According to the classification by the World Health Organization (WHO) in 2012, PBNECs are divided into 3 groups: well-differentiated, poorly differentiated, and invasive breast carcinomas with neuroendocrine differentiation.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">3</span></a> The available knowledge about the diagnosis and management of these aggressive tumors is limited in published data.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">3,5</span></a> Histopathological examinations are widely accepted as the gold standard for ABC and PBNEC, due to the diagnostic difficulties by conventional imaging methods.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">1,3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In recent years, <span class="elsevierStyleSup">18</span>F-FDG PET/CT has been documented and widely used as a useful imaging modality for in vivo glucose hypermetabolism particularly in pretherapeutic staging and in detecting metastases of breast carcinomas.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">6</span></a> The maximum standardized uptake value (SUVmax) levels were significantly associated with the tumor diameter, histological type, higher histological grades, Ki-67 index, hormone receptor status, relapse, and prognosis.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">7–9</span></a> The relationship between <span class="elsevierStyleSup">18</span>F-FDG uptake and histopathological tumor types has been demonstrated in breast cancers. The highest uptake values were determined in apocrine and invasive ductal carcinomas, while the lowest <span class="elsevierStyleSup">18</span>F-FDG uptake was obtained in lobular type breast cancer. In addition, the association among <span class="elsevierStyleSup">18</span>F-FDG uptake and ER and PR receptor negativity, HER2 receptor positivity and high Ki-67 expression was reported.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">10</span></a> In the present study, we aimed to analyze <span class="elsevierStyleSup">18</span>F-FDG uptake patterns of patients with ABC and PBNEC, and to compare them with invasive ductal carcinoma (IDC). In addition, we aimed to evaluate the role of <span class="elsevierStyleSup">18</span>F-FDG PET/CT in ABC and PBNEC staging.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0015" class="elsevierStylePara elsevierViewall">A total of 570 patients’ (585 lesions) files who were operated on, or had undergone true-cut or fine needle aspiration biopsies in our institution, between March 2009 and May 2018, were included in this study. Five hundred and sixty six (99.3%) of them were female and 4 (0.7%) were male, with a mean age of 54.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>13.3 (range<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>23–88) years. Fifteen patients were diagnosed with bilateral tumors. Histopathological analysis and <span class="elsevierStyleSup">18</span>F-FDG PET/CT imaging were performed preoperatively for all 585 lesions. Our retrospective study was approved by the local ethics committee (2015/598), and all patients were asked for their verbal or written consent for the use of their individual clinical findings for research purposes.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Histological analysis</span><p id="par0020" class="elsevierStylePara elsevierViewall">Primary breast cancer diagnosis and histopathological analysis were based on tissue samples derived by cut-cut biopsy or fine-needle aspiration biopsy before PET/CT imaging, or by excisional biopsy, breast-sparing surgery, or radical mastectomy after PET/CT imaging. For histological staging, the Scarff Bloom Richardson (SBR) classification system was used. The equal and more than 10% positivity of ER and PR receptors were accepted as positive by immunohistochemical (IHC) staining in tumor tissues. HER2 scoring was performed as follows:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0025" class="elsevierStylePara elsevierViewall">Score 0 (negative): Lesions observed with negative immunostaining.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0030" class="elsevierStylePara elsevierViewall">Score 1: Weak staining, or staining in less than 30% of the tumor cells.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">-</span><p id="par0035" class="elsevierStylePara elsevierViewall">Score 2: Complete membranous staining (at least 10%) even if it has uniform or weak staining.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">-</span><p id="par0040" class="elsevierStylePara elsevierViewall">Score 3: At least 30% uniform and strong membranous staining in tumor cells.</p></li></ul></p><p id="par0045" class="elsevierStylePara elsevierViewall">During the routine procedure, the cut-off value of Ki-67 for distinguishing between Luminal A and B types has been set at 15% by immunohistochemical and gene profiling methods; values <15% are accepted as negative and ≥15% accepted as positive.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">8</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">Tumors were divided into 4 molecular subgroups based on hormone receptor status, according to the results of histopathological analysis:<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">1.</span><p id="par0055" class="elsevierStylePara elsevierViewall">Luminal A: ER(+) and/or PR(+), HER2(−) and low Ki-67 (<15%).</p></li><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">2.</span><p id="par0060" class="elsevierStylePara elsevierViewall">Luminal B: ER(+) and/or PR(+), HER2(+) or HER2(−) and high Ki-67 (≥15%).</p></li><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">3.</span><p id="par0065" class="elsevierStylePara elsevierViewall">Triple negative and basal-like (TN): ER(−), PR(−) and HER2(−).</p></li><li class="elsevierStyleListItem" id="lsti0040"><span class="elsevierStyleLabel">4.</span><p id="par0070" class="elsevierStylePara elsevierViewall">HER2 type: ER(−), PR(−) and HER2(+).</p></li></ul></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085"><span class="elsevierStyleSup">18</span>F-FDG PET/CT imaging</span><p id="par0075" class="elsevierStylePara elsevierViewall">In our study, primary breast masses were reported in terms of their dimension, location, and quantity (multicentricity, multifocality); SUVmax; and the distant nodal and organ metastases status according to PET/CT results. Patients with blood glucose levels lower than 150<span class="elsevierStyleHsp" style=""></span>mg/dL after at least 6<span class="elsevierStyleHsp" style=""></span>h of fasting were admitted for the procedure. Standard 3.7–5.2<span class="elsevierStyleHsp" style=""></span>MBq/kg <span class="elsevierStyleSup">18</span>F-FDG intravenous injection was administered to the patients. Whole-body PET/CT imaging was obtained, including the area from vertex to upper femur while the patients were in supine position 60<span class="elsevierStyleHsp" style=""></span>min after IV injection (first 108 imaging performed by Siemens Biograph 6 HD LSO, and later 462 imaging performed by Siemens mCT 20 ultra HD LSO PET/CT [Siemens Molecular Imaging; Hoffman Estates, IL, USA]). For SUVmax calculation, regions of interest (ROI), which included the location of highest uptake, were drawn on PET cross-sections. SUVmax was calculated according to the following formula: maximum activity inside the ROI (MBq/g)/injected FDG dosage/body weight (MBq/kg).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0080" class="elsevierStylePara elsevierViewall">All data were analyzed with SPSS (Statistical Package for the Social Sciences) software for Windows (v21.0; IBM, Armonk, NY, USA). Individual and aggregate data were summarized using descriptive statistics, including mean, standard deviations, medians (min–max), frequency distributions, and percentages. Normality of data distribution was verified by Kolmogorov–Smirnov test. Comparison of the variables with normal distribution was made with the Student's <span class="elsevierStyleItalic">t</span>-test. Evaluation of categorical variables was performed by Chi-square test. The kappa statistic was calculated to evaluate the agreement. <span class="elsevierStyleItalic">p</span>-Values of <0.05 were considered statistically significant.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0085" class="elsevierStylePara elsevierViewall">Invasive ductal carcinoma was the most frequent breast carcinoma, with a rate of 77.7% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>446) (mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD; 54.0<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>13.1 years) in our study group. Apocrine type breast cancer at 4.1% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>24), and 2.4% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>14) PBNEC were diagnosed. The remaining 101 lesions had other histopathological types of breast cancer (invasive lobular, mucinous, mixed, etc.). The mean age of ABC patients was 55.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11.2 (range<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>38–81) and 67.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>12.1 (range<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>47–85) years in PBNEC patients. The mean age in the ABC group was similar the IDC, and there was no significant difference between the two groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.29). However, the mean age in the PBNEC group was significantly higher when compared to IDC (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002).</p><p id="par0090" class="elsevierStylePara elsevierViewall">Histopathologic and PET/CT findings of patients with ABC are presented in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD primary tumor SUVmax of the ABC group was similar to the IDC group, and no statistically significant difference was observed between the two groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.46) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Comparison of histopathologic and PET/CT findings of patients with IDCs and ABCs are presented in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. The rate of positive ER and PR receptors was found to be statistically higher in the IDC group than in the ABC group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.012 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.016, respectively) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). Ki-67 expression was found to be high (≥15%) in 84.3% of IDC cases (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>376), but it was high in the entire ABC group. A significant difference was found between two groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.035). The rate of positive HER2 was found to be statistically higher in the ABC group than in the IDC (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.021) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">Axillary lymph node involvement was observed in 68.2% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>304) of IDC patients and in 91.7% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>22) of ABC patients by PET/CT imaging. The rate of ALN involvement in ABC patients was statistically higher (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.015) than IDC patients. As a result of histopathological analysis, the sensitivity and specificity of PET/CT for ALN involvement were 100% and 80%, respectively, in the ABC group. There were no statistically significant differences between the ABC and IDC groups according to distant lymph nodes and organ metastases (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.05) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><p id="par0100" class="elsevierStylePara elsevierViewall">Primary tumors mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD of SUVmax in the histopathologic subtypes of ABC patients was presented in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>. HER2 subtype has the highest mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD SUVmax of primary tumor <span class="elsevierStyleSup">18</span>F-FDG uptake and this subtype uptake ratio was found significantly higher than Luminal B subtype in ABC patients (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.038). Additionally, there was statistically significant difference between TN and Luminal B subtype in patients with ABC, according to the <span class="elsevierStyleSup">18</span>F-FDG uptake (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.019) (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). The number of TN subtype patients was 48 (10.8%) in the IDC group. The primary tumors mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD SUVmax was 19.8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>12.7 in this group. Primary tumors mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD SUVmax was found 16.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.0 in 6 (25%) TN ABC patients. There was no significant difference between these two groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.272), and there was not a significant difference between HER2 subtype of IDC and ABC groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.29) for primary tumor uptake of FDG. However, the mean SUVmax in IDC patients with TN subtype was significantly higher than in Luminal A (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>237) and Luminal B (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>130) subtypes IDC patients (10.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.1 and 12.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.7, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001 respectively) in our study. The mean SUVmax in HER2 subtype was found 15.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.8 (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>31) in IDC patients and there was significant difference between HER2 and Luminal A subtypes (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001) (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1–3</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">The lowest mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD SUVmax was calculated in the PBNEC group, but there was not statistically significant difference among PBNEC, IDC and ABC groups (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Histopathologic and PET/CT findings of patients with PBNEC are presented in <a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>. Comparison of histopathologic and PET/CT findings of patients with PBNEC and IDC are presented in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. The rates of positive ER and PR receptors was found to be higher in the PBNEC group than in the IDC group, but only PR receptor rate was significantly different (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.036). However, the rate of positive HER2 receptors was similar in the PBNEC compared to IDC (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.373). Ki-67 expression was found to be lower than 15% in 71.4% of patients with PBNEC, and significantly higher than IDC group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">Axillary lymph node involvement and distant organ metastasis rate were similar in IDC and PBNEC patients by PET/CT (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.796 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.993, respectively). The sensitivity and specificity of PET/CT for ALN involvement were 90% and 75%, respectively, in our study group. However, there was a statistically significant differences between the PBNEC and IDC groups according to distant lymph nodes metastases (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><p id="par0115" class="elsevierStylePara elsevierViewall">When the relation between <span class="elsevierStyleSup">18</span>F-FDG uptake and the molecular subtype classification were assessed in PBNEC patients, the mean SUVmax in the Luminal A subtype was lower than the Luminal B subtype, but no statistically significant difference was found between two subtypes (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.1). Additionally, there was no statistically significant difference between IDC and PBNEC groups comparison for Luminal A and Luminal B subtypes (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.259 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.488, respectively).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">Apocrine type tumors of breast and PBNEC are rare tumors, and a limited number of articles have been published in the literature and most of them consist of case reports or brief reports.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">3,11</span></a> Kitajima et al. reported a ratio of 0.64% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2) ABC in 306 patients.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">12</span></a> These ratio remained similar in a study conducted with 89 cases by Miyake et al.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">13</span></a> On the other hand, at the University of Helsinki, reported 0.1% of cases of PBNEC in 1367 cases.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">2</span></a> However, compared to published articles, the rate of patients with ABC and PBNEC is remarkably higher in our study. These ratios were found as 4.1% for ABC and 2.4% for PBNEC.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Apocrine type breast cancer should be distinguished from IDC because it might not respond to classic hormone agents due to different hormone receptor profiles and aggressive clinical behaviors.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">14</span></a> However, the similarities in clinical and prognostic characteristics of ABC and IDC may cause diagnostic difficulties.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">15</span></a> In our study, primary tumoral <span class="elsevierStyleSup">18</span>F-FDG uptakes of patients with ABC and IDC were similar. It is well known that ER and PR expressions in breast carcinoma is associated with good prognosis, negative ER, PR and positive HER2 expressions are associated with poor prognosis.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">16</span></a> The characteristic hormone profile (ER−, PR−, and AR+ of the ABC subtype has a prominent role in distinguishing from IDC.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">2</span></a> ER positivity in ABC was documented in the literature as 3.8–60%, PR positivity as 4.8–40%, and HER2 positivity as 50%.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">17</span></a> Zhang et al. compared clinicopathological features of 260.596 cases consisting of 840 ABC (0.32%) and 259.756 IDC (99.68%) patients. The researchers provided the patients’ data from the Surveillance, Epidemiology and End Result (SEER) database in 2017. It was reported that 29.9% of ABC lesions and 77.2% of IDC lesions were identified as ER positive; 23.2% of ABC lesions and 67.1% of IDC lesions were identified as PR positive. Statistically significant differences were observed between ABC and IDC groups according to the ER and PR expressions (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001).<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">18</span></a> In accordance with this data, the positive PR and ER receptor ratios found in our study were statistically lower in the ABC group than the IDC group. However, HER2 receptor status was significantly higher in the ABC group compared to IDC group in our study results.</p><p id="par0130" class="elsevierStylePara elsevierViewall">PBNEC most frequently occurs in older women. Yang et al. reported a mean age of 59.2 years in PBNEC patients.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">3</span></a> Similarly in our study, the mean age of patients with PBNEC was 67.9 years, which is higher than the mean ages of both ABC and IDC patients (55.5 and 54.0 years, respectively).</p><p id="par0135" class="elsevierStylePara elsevierViewall">The disadvantages of poorly differentiated PBNECs in <span class="elsevierStyleSup">18</span>F-FDG PET/CT imaging were highlighted in some of studies.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">19</span></a> The sensitivity of <span class="elsevierStyleSup">18</span>F-FDG PET/CT depends on lesion size, metabolic activity, the surrounding tissue, and the serum glucose level. False-negative results can occur in small-sized lesions or those with low metabolic activity.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">20</span></a> Kamaleshwaran et al. detected an intense <span class="elsevierStyleSup">18</span>F-FDG uptake in the inner quadrant of the left breast and showed a metastasis in the T4 vertebra of a 45-year-old female patient by whole-body PET/CT scan, and researchers noted that <span class="elsevierStyleSup">18</span>F-FDG PET/CT imaging is beneficial to detect primary NEC and bone metastasis.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">21</span></a> In our study, although <span class="elsevierStyleSup">18</span>F-FDG uptake in patients with the PBNEC was found to be lower compared to ABC and IDC patients, differences among groups were not statistically significant.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Axillary lymph node involvement in breast cancer has been documented as the most important prognostic factor affecting survival in numerous studies.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">22</span></a> Although the prevalence of ALN involvement was reported between 1% and 4% in ABC, Dreyer et al. Reported positive ALN involvement in 6 of 14 triple negative ABC patients.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">17,23</span></a> In a study Olewniczak et al. Detected ALN involvement in 128 (50.9%) of 251 IDC patients.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">24</span></a> In our study, ALN involvement was observed in 91.7% of ABC patients and this ratio was significantly higher than IDC and PBNEC patients by PET/CT (68.2% and 71.4%, respectively).</p><p id="par0145" class="elsevierStylePara elsevierViewall">In a meta-analysis consisting of 58 articles and 108 patients diagnosed with PBNEC, the mean tumor diameter was 3.7<span class="elsevierStyleHsp" style=""></span>cm, ALN involvement was 51%, and distant metastases was 9%.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">25</span></a> In addition, Roininen et al. reported mean primary tumor size of 2.2<span class="elsevierStyleHsp" style=""></span>cm, ALN involvement of 39.6%, and distant metastases 9.3% in their study.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">2</span></a> In our study, the mean diameter of primary tumors was 3<span class="elsevierStyleHsp" style=""></span>cm, ALN involvement and distant metastases rates were found 28.6% and 21.4%, respectively. In contrast to literature in our PBNEC group, a high rate distant metastases was observed, although a lower rate ALN involvement was detected.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The high <span class="elsevierStyleSup">18</span>F-FDG uptake ratio in patients with the TN subtype breast cancer has been highlighted in published data.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">22</span></a> In a study conducted by Keam et al. in 78 cases, significantly increased SUVmax were documented in the TN group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.008).<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">26</span></a> The TN subtype is an heterogeneous group morphologically and in terms of treatment response. The researchers underlined the aggressive clinical behavior and poor prognosis of the TN subtype in the IDC, but the rarity of ABC prevalence with the TN subtype considerably limits the available knowledge in literature.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">14</span></a> Choi et al. detected that 9.8% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>12) of the patients had the ABC subtype in 122 patients with the TN subtype.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">27</span></a> Tsutsumi has detected TN subtype in 48% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>21/44) of ABCs among 429 cases. In the same study, the TN subtype in the IDC subtype was reported as 7.07% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>23/325).<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">28</span></a> However, Guo et al. detected 48.9% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>44/90) of HER2 negative in ABCs with ER and PR negative subtypes.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">16</span></a> In our study, the TN and HER2 subtype rates in the ABC group were found 25% and 16.7%, and in IDC group these ratios were found 10.8% and 6.9%, respectively.</p><p id="par0155" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">In conclusion</span>, there are no studies evaluating the clinical characteristics of subtypes of ABC with <span class="elsevierStyleSup">18</span>F-FDG PET/CT, and the SUVmax of the TN ABC subtype was compared with both non-TN ABC and TN IDC patients for the first time in our study. The highest <span class="elsevierStyleSup">18</span>F-FDG uptakes were detected in the TN subtype IDC and in the HER2 subtype ABC patients. Further analysis with <span class="elsevierStyleSup">18</span>F-FDG PET/CT in TN and HER2 subtypes in the ABC and IDC subtypes may contribute to better understanding of tumor behaviors and management of ABC patients particularly with TN subtype. On the other hand, metabolic characteristics of PBNEC were analyzed in detail by <span class="elsevierStyleSup">18</span>F-FDG PET/CT for the first time in this study. Although <span class="elsevierStyleSup">18</span>F-FDG uptake in the Luminal B subtype of PBNEC was higher than Luminal A subtype, the difference was not statistically significant. In addition, we aimed to compare <span class="elsevierStyleSup">18</span>F-FDG PET/CT findings between ABC, PBNEC and invasive ductal carcinoma (IDC).</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">A total of 570 patients and 585 breast lesions were retrospectively included in this study. After patients were classified into molecular subtypes according to the histopathological analysis, <span class="elsevierStyleSup">18</span>F-FDG PET/CT imaging was performed. The SUVmax findings of primary tumors obtained from <span class="elsevierStyleSup">18</span>F-FDG PET/CT were compared between the groups.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">IDC was the most prevalent breast carcinoma (77.7%, <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>446), with a low proportion of ABC (4.1%, <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>24) and PBNEC (2.4%; <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>14) diagnosed. Después de clasificar a los pacientes en subtipos moleculares de acuerdo con el análisis histopatológico, se realizaron imágenes de PET/TC con <span class="elsevierStyleSup">18</span>F-FDG y se compararon los hallazgos del análisis visual y SUVmax de los tumores primarios entre los distintos grupos.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">El IDC fue el carcinoma de mayor prevalencia, con una tasa de detección del 77.7% (n= 446) en nuestro estudio. Sin embargo, se diagnosticaron 4.1% (n= 24) ABC y 2.4% (n= 14) PBNEC. El SUVmáx medio más alto se registró en el subtipo HER2 del ABC y se encontró una diferencia estadísticamente significativa entre la ratio de captación de <span class="elsevierStyleSup">18</span>F-FDG de los subtipos HER2 y TN comparados con el tipo luminal B del ABC (<span class="elsevierStyleItalic">p</span>= 0.038, <span class="elsevierStyleItalic">p</span>= 0.019, respectivamente). Aunque la captación de <span class="elsevierStyleSup">18</span>F-FDG en el subtipo Luminal B de PBNEC fue mayor que el subtipo Luminal A, la diferencia no fue estadísticamente significativa.Además, la tasa de metástasis axilar fue significativamente mayor en el grupo ABC que en los otros grupos (<span class="elsevierStyleItalic">p</span>= 0.015).</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusiones</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Los subtipos histopatológicos del grupo ABC mostraron diferentes grados de captación de <span class="elsevierStyleSup">18</span>F-FDG con respecto a los subtipos del grupo IDC, a pesar de que en general la captación de <span class="elsevierStyleSup">18</span>F-FDG fue menor en el grupo de PBNEC.
