Abstract

Castleman disease, a rare lymphoproliferative disorder characterized by non-neoplastic lymph node enlargement, typically presents as nodal involvement, while extranodal manifestations are uncommon. We present a unique case of unicentric Castleman disease localized to the gluteal region in a 27-year-old female. Initially mistaken for a neurogenic or vascular tumour, fine needle aspiration cytology (FNAC) revealed a polymorphous population of lymphoid cells with numerous traversing capillaries suggestive of reactive hyperplasia, possibly Castleman disease, which was later confirmed by histopathology. This case highlights the diagnostic challenges posed by rare extranodal presentations of Castleman disease and underscores the importance of interdisciplinary collaboration in accurate diagnosis and management.

La enfermedad de Castleman, un trastorno linfoproliferativo poco común caracterizado por un agrandamiento de los ganglios linfáticos no neoplásicos, se presenta típicamente como afectación ganglionar, siendo poco frecuentes las manifestaciones extraganglionares. Presentamos un caso único de enfermedad de Castleman unicéntrica localizada en la región glútea en una mujer de 27 años. Inicialmente confundida con un tumor neurogénico o vascular, la citología por aspiración con aguja fina mostró una población polimorfa de células linfoides con numerosos capilares transversales sugestivos de hiperplasia reactiva, posiblemente enfermedad de Castleman, que luego fue confirmada por histopatología. Este caso destaca los desafíos diagnósticos que plantean las raras presentaciones extraganglionares de la enfermedad de Castleman y subraya la importancia de la colaboración interdisciplinaria en un diagnóstico y tratamiento precisos.
