class="elsevierStylePara elsevierViewall">Chronic rhinosinusitis (CRS) is defined as an inflammation of the nose and paranasal sinuses which persists more than 3 months characterized by nasal discharge and/or obstruction, facial pain and/or pressure, and decreased sense of smell.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">1,2</span></a> Chronic rhinosinusitis is an inflammatory condition that significantly impairs quality of life (QoL) and requires proper management.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">3</span></a> Assessing patients for medical treatment, their response to medical therapy and selecting patients for endoscopic sinus surgery (ESS) depends on symptoms, nasoendoscopy and sinus computed tomography scan (CT) findings. Sinus CT imaging is useful to assess the sinonasal anatomy for preoperative planning and aids in determining the extent and burden of disease. Moreover, sinus CT has the advantage to evaluate the outcome of both medical and surgical treatments for CRS. Lund-Mackay (LM) staging system is one of the most acceptable and popular scoring systems to localize and quantify chronic inflammation used in sinus CT imaging.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">4</span></a> However, correlation of CRS clinical manifestation with the sinus CT imaging remains unsettled.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">5</span></a> Thus, assessment of response towards medical therapy and timing of ESS remains challenging. Although, the severity of disease on sinus CT imaging was assumed to correspond to severity of symptoms and endoscopic findings, previous studies failed to prove their correlations.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Depending on the presence or absence of nasal polyps, CRS is classified as CRS with nasal polyps (CRSwNP) and CRS without nasal polyps (CRSsNP).<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">1</span></a> Eosinophilic infiltration seen on histological studies is the hallmark of CRSwNP. Further categorization of CRS can be divided into 2 groups, eosinophilic chronic rhinosinusitis (ECRS) and non-eosinophilic CRS (NECRS) based on the predominance of eosinophils in the tissue.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">7</span></a> When the predominance inflammatory cells are eosinophilic, it is termed as eosinophilic, otherwise it is classified as non-eosinophilic or neutrophilic. By categorizing CRSwNP into ECRS and NECRS, assessment of the subjective and objective clinical manifestations may have a different outcome than previous studies of CRSwNP. The aim of this study was to correlate the sinus CT imaging to the clinical symptoms and endoscopic findings in patients having eosinophilic and non-eosinophilic types of CRSwNP.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design</span><p id="par0015" class="elsevierStylePara elsevierViewall">This was a prospective study conducted in a tertiary hospital from 1st April 2018 until 1st April 2019. The study was approved by the Human Research Ethics Committee (USM/JEPem/18020107). The study was performed according to Helsinki Declaration and written consent was obtained from each subject. Patients with CRSwNP that met the criteria of the ‘European position paper on rhinosinusitis and nasal polyps’ guidelines (EPOS)<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">1</span></a> and availability of a sinus CT scan within 1 month of the time of evaluation were included in this study. Patients younger than 18 years old, presence of congenital nasal lesion or paranasal sinus malformation (such as choanal atresia), pregnant women, history of chronic rhinosinusitis with complications, history of operation to the sinus, history of systemic disease (such as cystic fibrosis) and patients on both immunotherapy and biologics were excluded from the study.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Patients were evaluated for clinical symptoms using Sinonasal Outcome Test-22 (SNOT-22) questionnaires and rigid nasoendoscopic examination was performed using 0-degree telescope (Karl Storz, Tuttlingen, Germany). Sinus CT scan was performed with Siemens SOMATOM Definition AS+, which has the capability to produce 128-slices of images per rotation. Plain sinus CT scan with 1<span class="elsevierStyleHsp" style=""></span>mm cut was done with axial, coronal and sagittal reconstruction. The sinus CT images were collected from computers at the workstation with 2 Mega Pixel monitor (Barco MPG 2121 monitor-resolution 2048<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>1536) via Picture Archive Communication System (PACS) in PACS Universal Viewer Version 5.0 SP6. Tissue biopsy of the nasal polyps was taken during endoscopic sinus surgery, performed under general anaesthesia. None of the patients experienced any complications following the procedure.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Symptoms</span><p id="par0025" class="elsevierStylePara elsevierViewall">The symptom scores were graded according to the SNOT-22 questionnaires.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">8</span></a> SNOT-22 is a quality of life-related measure of sinonasal function consisting of 22 questions. The first section is rhinology symptoms and contains seven questions: sneezing, runny nose, need to blow nose, postnasal discharge, thick nasal discharge, blockage/congestion of nose and sense of taste/smell. The second section is facial and ear symptoms and contains four questions: facial pain/pressure, ear fullness, ear pain and dizziness. The third section is sleep function and contains three questions: waking up at night, difficulty falling asleep and lack of a good night's sleep. The fourth section is psychological and contains six questions: reduced concentration, fatigue, reduced productivity, frustration/restlessness/irritability, embarrassment, and sadness. Two questions are unclassified. Symptom severity was graded from 0 to 5 as follows: grade 0 (no problem), grade 1 (very mild problem), grade 2 (mild or slight problem), grade 3 (moderate problem), grade 4 (severe problem), and grade 5 (problem as bad as it can be).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Nasal endoscopy</span><p id="par0030" class="elsevierStylePara elsevierViewall">Nasal endoscopy was performed for all patients using rigid endoscope 0-degree telescope, 2.7<span class="elsevierStyleHsp" style=""></span>mm diameter (Karl Storz, Tuttlingen, Germany). The examination was performed by an otorhinolaryngologist blinded to the symptoms and sinus CT findings. Endoscopic findings of the inferior and middle turbinates, nasal mucosa, nasal septum and nasal obstruction were recorded. Endoscopic grading was performed using the endoscopic Lund Kennedy (LK) grading system<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">9</span></a> as follows: grade 0 (no polyp), grade 1 (polyp confined in the middle meatus) and grade 2 (polyp beyond the middle meatus). The presence of discharge was graded 0 (no discharge), 1 (clear, thin discharge) and 2 (thick, purulent discharge). For mucosal oedema, scarring and crusting were graded 0 (absent), 1 (mild) and 2 (severe) respectively for each finding.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Sinus CT imaging</span><p id="par0035" class="elsevierStylePara elsevierViewall">Sinus CT scans were reviewed in the coronal, axial, and sagittal planes. The reviewer was blinded to the symptoms and endoscopic findings. Sinus CT scans were graded according to the sinus CT LM staging system.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">4</span></a> The following scores were allocated: maxillary sinus score (M score), anterior ethmoid sinus score (AE score), posterior ethmoid sinus score (PE score), sphenoid sinus score (<span class="elsevierStyleItalic">S</span> score), frontal sinus score (<span class="elsevierStyleItalic">F</span> score), and the ostiomeatal complex score (OMS score). Each side was graded separately. For all sinus systems, a score of 0 was given when there was no opacification, a score of 1 for partial opacification and a score of 2 for total opacification. For the ostiomeatal complex, a score of 0 was given for no obstruction and a score of 2 for obstruction.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Tissue specimen</span><p id="par0040" class="elsevierStylePara elsevierViewall">Tissue specimen was fixed with formaldehyde solution and later with haematoxylin and eosin staining. It was seen under high powered microscope (400× magnification). The number of eosinophils was counted and ECRS was defined when tissue eosinophil was more than 70/high powered field (hpf).<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">10,11</span></a></p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">Descriptive statistics were analyzed using mean and standard deviation (SD) for numerical data or frequency and percentage for categorical data unless otherwise stated. For the comparison of the mean difference between groups, independent <span class="elsevierStyleItalic">t</span>-test was used if they were normally distributed, while Mann–Whitney <span class="elsevierStyleItalic">U</span> test was used if they were not. Pearson correlation test was used to determine the correlation between each numerical data which was normally distributed or Spearman correlation for data which was not normally distributed. IBM SPSS Statistics for Windows (Version 21.0. Armonk, NY: IBM Corp.) was used. A value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 was considered statistically significant.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Results</span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Patient characteristics</span><p id="par0050" class="elsevierStylePara elsevierViewall">A total of 44 patients were recruited for this study (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The mean age of the participants was 45.05(SD 13.53). <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the overall symptoms of both ECRS and NECRS. More than half of the participants were men (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>31, 70.51%). A total of 18 (40.9%) patients had history of smoking while 17 (38.6%) of patients had positive allergic history. Positive allergy history was defined by clinical symptoms of patient following exposure to allergens, either aeroallergens or food allergens. The symptoms described were nasal itchiness and skin rashes or itchiness of other parts of the body. The allergens involved were mainly seafood. There was no difference in the number of eosinophils between those with and without history of allergy. Only 12 (27.3%) patients had underlying history of asthma.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Correlation of symptoms, nasoendoscopy and sinus CT scores</span><p id="par0055" class="elsevierStylePara elsevierViewall">There was no significant difference (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.05) in the symptoms score, nasoendoscopy score and CT score between patients having ECRS and NECRS (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The correlation of symptom, nasendoscopy and sinus CT scores between ECRS and NECRS is shown in <a class="elsevierStyleCrossRefs" href="#tbl0020">Tables 4 and 5</a> respectively. Assessments of the subjective symptom's severity is combined with the objective evaluation in the forms of nasoendoscopic examination and sinus CT imaging. The role of endoscopic examination in reaching the correct diagnosis is widely accepted.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">12</span></a> Lohiya et al.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">13</span></a> performed a study comparing nasal endoscopy against sinus CT as the gold standard, found no significant difference of both modalities in diagnosing CRS. Hence, nasal endoscopy is useful to diagnose CRS and avoid the use of sinus CT. Its use as a diagnostic tool for CRS reduces cost and risk of radiation exposure. However, its role in influencing the treatment choice of medical versus surgical in the management of CRS patients is unclear. Rathor et al.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">14</span></a> reported poor correlation between nasoendoscopic examination and sinus CT imaging. A study by Stankiewicz et al.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">15</span></a> reported poor correlation between subjective symptom evaluation and nasoendoscopic assessment. Another study by Ferguson et al.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">16</span></a> investigated the relationship between symptom with both nasoendoscopy and sinus CT findings, revealed that irrespective of the endoscopic findings, sinus CT imaging is still required based on the patient's symptoms.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The present study demonstrated poor correlation of nasal endoscopy with both symptoms and sinus CT. Our results are consistent with previous studies<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">14–16</span></a> which had demonstrated poor correlation of nasoendoscopic findings with both symptoms and sinus CT evaluation. In clinical practice, all three modalities of assessment consisting of symptoms, nasoendoscopy and sinus CT are utilized in the evaluation of patients with ERCS and NECRS. Nasoendoscopic assessment is routinely used for diagnostic purpose and monitoring of response to medical therapy. There may be instances where nasoendoscopic evaluation showed improvements, but patients reported otherwise. Such conflicting circumstances may cause difficulty in deciding the appropriate treatment plan. Considering our findings, sinus CT may play a vital role in the decision-making process to guide clinicians on the proper management modalities for their patients. It is an objective assessment that correlates well with patients’ symptoms and thus, assist clinicians in choosing suitable patients that may benefit from surgery.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Sinus CT imaging provides surgeon information about the anatomy of the nasal sinuses before performing ESS, enabling a safe and adequate approach to the sinuses. In addition, by detailing the extent and severity of the disease, selection of an appropriate treatment option can be made in uncertain cases. In clinical practice, 2 radiographic staging systems were frequently employed. A study by Bhattachayya<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">17</span></a> compared 3 radiographic staging systems, Kennedy, Harvard, and LM systems, against the symptom score and found only LM correlated significantly with the nasal symptoms. In another study,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">18</span></a> the LM system was not only found to correlate significantly with symptoms but was also able to predict the outcome after treatment.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Several studies demonstrated no correlation between sinus CT and symptom scores using SNOT-20 or Chronic Sinusitis Survey.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">5,19</span></a> When SNOT-22 was used, similar findings were reported.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">20</span></a> The poor correlation between the sinus CT staging and the scores on the patient-based questionnaire could reflect the different orientation of the subjective and objective evaluation systems. Objective findings of severity such as degree of involvement of nose and sinuses, edema of sinus mucosa and range of nasal blockage may not necessarily correlate with CRS patients’ actual clinical symptoms and discomforts. Conversely, other studies showed significant correlation between sinus CT and symptom scores.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">21,22</span></a> Subjective and variable symptoms together with the heterogeneity of the study participants may contribute to the discrepancy in those studies.</p><p id="par0080" class="elsevierStylePara elsevierViewall">As most studies performed correlation of symptoms, nasoendoscopy and sinus CT imaging on patients suffering from CRS with or without polyps, not much is known on the correlation of the three modalities in the assessment of patients with ERCS and NECRS. ECRS behaves differently from NECRS due to its resistant to medical and surgical treatment with multiple recurrences.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">1</span></a> The classification of ECRS and NECR depends on the number of eosinophils found in the nasal mucosa with a wide range of cut-off values being used.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">7</span></a> Defining the primary CRS as type 2 and non-type 2 based on the endotype predominance, EPOS 2020 determined ECRS should be classified according to the histological number of eosinophils of 10/hpf or higher.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">1</span></a> On the other hand, recognizing the difference in the pathology of nasal polyps between Western and Asian populations, the ‘Japanese Epidemiological Survey of Refractory Eosinophilic Chronic Rhinosinusitis’ (JESREC) study group decided the cut-off value of 70/hpf is more representative of the significant difference between ECRS and NECRS in their management and outcome.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">11</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">The different immunological mechanism between ECRS and NECRS is apparent in their contrasting response towards corticosteroids. ECRS, a Th2-mediated inflammation, is strikingly very responsive when treated with corticosteroids as compared to NECRS, a non-Th2-mediated inflammation, which showed poor response.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">23</span></a> Although, the diagnosis of ECRS and NECRS is commonly made by mucosal biopsy, nasal cytology is another option. Nasal cytology allows a less traumatic alternative than biopsy in the assessment of nasal polyps.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">24</span></a> The present study showed significant correlation between symptom and sinus CT scores in both ECRS and NECRS. Furthermore, the significant correlation between clinical symptoms and sinus CT scoring in our study support the use of LM CT system as an objective measure to evaluate changes with medical therapy and determining the need for surgery. Additionally, CT imaging could identify each problematic sinus which is potentially beneficial as a guide in surgical planning. This highlights the value and advantage of sinus CT in the management of patients with CRSwNP.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Conclusions</span><p id="par0090" class="elsevierStylePara elsevierViewall">Sinus CT staging based on LM system correlates well with SNOT 22 symptoms score for both ECRS and NECRS.
