Resumen
Introducción

La angioplastia y/o stento la cirugía de revascularización de la arteria renal puede disminuir la hipertensión arterial y la necesidad de fármacos antihipertensivos, pero esta circunstancia puede desempeñar, actualmente, un papel secundario en la estrategia terapéutica de la nefropatía isquémica, comparado con la posible prevención del fracaso renal. Existe un cambio sustancial en el objetivo principal del tratamiento de las estenosis arterioscle-róticas de las arterias renales que van encaminadas a la prevención del fracaso de la función renal.

Caso clínico

Presentamos un caso de cirugía de revascularización renal por reestenosis tras angioplastia-stentrenal en un riñón único normofuncionante y se revisa la literatura relacionada con la nefro-patía isquémica así como los resultados de los distintos procedimientos terapéuticos.

Conclusiones

La cirugía de revascularización renal se presenta como una buena opción, avalada por sus aceptables resultados de permeabilidad y supervivencia. Se necesita un estudio clínico aleatorizado y prospectivo que compare el tratamiento médico, la angioplastia con y sin stent,y la cirugía de la arteria renal para definir el método más óptimo de intervención de los pacientes con arteriosclerosis de la arteria renal.

Summary
Introduction

Angioplasty and/or stenting or revascularisation surgery of the renal artery can lower arterial hypertension and the need for antihypertensive medication. But at the present time this circumstance can play a secondary role in the therapeutic strategy of ischemic nephropathy when compared to the possible prevention of kidney failure. There is a substantial change in the main objective of the treatment of arteriosclerotic stenoses of the renal arteries that is oriented towards preventing the failure of kidney functioning.

Case report

We present a case of renal revascularisation surgery due to restenosis following a renal stent-angioplasty in a single kidney that functioned normally. We also review the literature concerning ischemic nephropathy and the results of the different therapeutic procedures used

Conclusions

Renal revascularisation surgery appears as a good option, backed by its acceptable results as regards patency and survival. There is a need for a random prospective clinical study that compares medical treatment, angioplasty with and without a stent and renal artery surgery, to define the optimal method for operating on patients with arteriosclerosis of the renal artery.

Introdução

A angioplastia e/ou prótese ou a cirurgia de revascularização da artéria renal pode diminuir a hipertensão arterial e a necessidade de fármacos anti-hipertensores, mas esta circunstância pode desempenhar, actualmente, um papel secundário na estratégia terapêutica da nefropatia isquémica, comparado com a possível prevenção da insuficiência renal. Existe uma alteração substancial no objectivo principal do tratamento das estenoses arterioscleróticas das artérias renais que vão encaminhadas para a prevenção da insuficiência da função renal.

Caso clínico

Apresentamos um caso de cirurgia de revascularização renal por re-estenose após angioplastia-prótese renal num rim único normofuncionante e revêse a literatura relacionada com a nefropatia isquémica assim como os resultados dos distintos procedimentos terapêuticos.

Conclusões

A cirurgia de revascularização renal apresenta-se como uma boa opção, avalizada pelos seus resultados aceitáveis de permeabilidade e sobrevivência. É necessário um estudo clínico aleatório e prospectivo que compare o tratamento médico, a angioplastia com e sem prótese e a cirurgia da artéria renal para definir o método óptimo para a intervenção em doentes com arteriosclerose da artéria renal.
