A 78-year-old male with a history of low anterior resection of the rectum due to adenocarcinoma of the middle third and sigmoidectomy with terminal colostomy for metachronous adenocarcinoma. During follow-up, metastasis in liver segment III was diagnosed by abdominal computed tomography. The scan also demonstrated a celiacomesenteric arterial trunk (Fig. 1, images A and B), which is a rare vascular variant (about 0.7% of the population in radiological studies). There were no other alterations of the hepatic vascular anatomy. The patient underwent laparoscopic liver resection of segment III, with no postoperative complications.
This finding illustrates the importance of an adequate vascular study in all patients who are candidates for liver and biliopancreatic surgery, as there are anatomical variations that require modifying the surgical technique.
