The patient is an 83-year-old male with a history of arterial hypertension and hiatal hernia, who came to our emergency department with fever and abdominal pain. Examination detected a painful mass in the right hypochondrium. Magnetic resonance cholangiography revealed 2 hydatid cysts with multiple vesicles in their interior, located in segment VIII and between segments V and VI,which measured 12×11cm and 19×12cm, respectively (Fig. 1). An interruption in the pericyst as well as the formation of adjacent vesicles were identified, which extended towards segment IV.
Diagnosis: giant hydatid cyst.
