The patent is a 69-year-old male with melena, constitutional syndrome and severe anemia (hemoglobin=6.4g/dL; prior to hospitalization 14.1g/dL). Physical examination showed evidence of a large palpable mass in the left hemiabdomen, which computed tomography defined as a tumor measuring 18.5×20cm that infiltrated several jejunal loops (Fig. 1A). During surgery, a giant intraperitoneal mass was found in contact with the omentum and jejunal loops. We performed en bloc resection of the tumor and omentum as well as double bowel resection (Fig. 1B). The pathology study reported a high-grade gastrointestinal stromal tumor that affected 2 segments of the small intestine; diameter=21cm; mitosis=13.3/50 HPF; C-Kit/DOG-1: intensely positive; Ki-67: 25%–30%, some areas 50%, and stage: pT4N0.
