id="sect0025">What is known</span><p id="par0005" class="elsevierStylePara elsevierViewall">The theory of humanised care advocates holistic care that considers the individual as a whole, and this involves recognising the interconnection of the physical, mental, emotional and spiritual dimensions of health. These are important aspects in the professional care of people with cancer and their families, but there are few case reports exemplifying the use of this theory in the clinical practice of oncology nursing.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">What it contributes</span><p id="par0010" class="elsevierStylePara elsevierViewall">This case report presents a significant and novel contribution to nursing science by presenting the practical application of the theory of humanised care in a nursing process that integrates scientific knowledge in a virtual care environment, designed to support a patient facing the challenge of gastric cancer.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Introduction</span><p id="par0015" class="elsevierStylePara elsevierViewall">Gastric cancer is the fifth most prevalent and deadliest cancer in the world.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Both the disease and the treatment side effects have an impact on physical, psychosocial and spiritual health.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> People with gastric cancer therefore require complex and specialised oncological care, where quality and humanisation are sought through scientific knowledge.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The aim of humanised care practice is to combine scientific knowledge with human, spiritual and transpersonal values, with the goal that the person achieves physical, psychosocial and spiritual well-being.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> However, there is little evidence in the literature that exemplifies the humanised nursing process in the field of oncology nursing, specifically in people with gastric cancer.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The purpose of this article is to present a clinical case, approached through the nursing process using the NANDA, NIC and NOC taxonomies and based on the principles of the theory of humanised care. The methodology was based on health education using tele-nursing with interactive and individualised sessions via the zoom platform.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Gordon’s functional patterns and the Watson Caritas Patient Score (WCPS)<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> scale was assessed. This measure humanised nursing care according to the caritas processes. The scale was assessed again at the end of the process. For the diagnosis, the NANDA taxonomy was used and for prioritisation the reasoning network according to the Current State Results Analysis Model.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> For planning, the nursing taxonomy of outcomes (NOC) and interventions (NIC) was used. Registration was made on the NNN consult platform. To develop the “moments of care” the Caritas Care Processes (PCC) and the “transpersonal relationship” <a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> were used.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Case description</span><p id="par0035" class="elsevierStylePara elsevierViewall">The patient was a 45-year-old, Costa Rican woman, in a partnership, who was a farmer and street vendor. She had a personal history of asthma and a family history of cancer. She stated that in her youth she was a homeless person and used psychoactive substances, including tobacco, alcohol and marijuana, but at the time of the assessment she had a home and did not actively consume any substance. She was diagnosed with gastric cancer. Treatment consisted of: 6 cycles of the Folfox protocol. She was scheduled for a gastrectomy, which was not performed since she had metastases in the peritoneum, mesentery, small intestine and colon. She was finally offered palliative chemotherapies (she was unaware of the protocol). At the time of evaluation, she had undergone an exploratory laparotomy and was recovering at home.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">General and diagnostic assessment</span><p id="par0040" class="elsevierStylePara elsevierViewall">The findings of the assessment and diagnoses (Dx) of the altered patterns are detailed below. This stage was carried out in two tele-nursing sessions.</p><p id="par0045" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Health perception-management pattern:</span> she does not consume prescribed medications due to her history of addiction, but when she experiences pain or discomfort, she consumes Tramal and Cannabidiol (CBD), without medical prescription she has a Port-a-Cath. Dx: 00004 risk of infection, 00213 risk of vascular trauma, 00276 ineffective self-management of health, and 00217 risk of allergic reaction.</p><p id="par0050" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Nutritional-metabolic pattern:</span> her BMI is 32.9 kg/m<span class="elsevierStyleSup">2</span>. She eats 5–6 times per day of varied food in small portions, and also eats other foods due to anxiety. Dx: 00232 obesity.</p><p id="par0055" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Elimination pattern:</span> no problems defecating or urinating.</p><p id="par0060" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Activity-exercise pattern:</span> walks regularly, but cannot perform high-impact exercises due to recent laparoscopic surgery.</p><p id="par0065" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Sleep-rest pattern:</span> there are no problems with sleep or rest.</p><p id="par0070" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Cognitive-perceptive pattern:</span> she understands that the treatment for gastric cancer developed from being curative to palliative. They told her that she had a limited life expectancy, so by not performing the gastrectomy, she decided not to continue with the chemotherapy treatment to have a “dignified death.” She then reconsidered and continued the treatment. She is constantly afraid of having alopecia and questions the decision she made. This conflict manifests itself with mood changes and fragility crying. Dx: 00083 conflict of decisions</p><p id="par0075" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Self-perception-self-concept pattern:</span> she accepts weight loss, but now has anticipatory concern about possible changes in her body and femininity that the cytotoxic drugs may cause. Specifically, she is concerned about the feelings of rejection from her children and her partner (younger than her) due to alopecia. Dx: 00153 Risk of low situational self-esteem.</p><p id="par0080" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Role-relationship pattern:</span> she lives with her partner, and has social and economic support from her family.</p><p id="par0085" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Sexuality-reproduction pattern:</span> she does not manifest any problem with her sexuality or reproduction.</p><p id="par0090" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Coping pattern-stress tolerance:</span> she reports stress due to the guilt she feels, since she thinks that the cause of her gastric cancer is her history of street living and drug addiction. She has anxiety about the possible side effects of chemotherapies, and not necessarily about imminent death. Beck Anxiety Inventory: 41 severe anxiety. Dx: 00146.</p><p id="par0095" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Values-beliefs pattern:</span> Christian, her faith grew due to cancer.</p><p id="par0100" class="elsevierStylePara elsevierViewall">The WCPS had scores of 1 in all items, which means that she never perceived humanised nursing care during her previous experience in a public hospital.</p><p id="par0105" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> contains the summary of clinical reasoning and diagnostic prioritisation.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Care planning and execution</span><p id="par0110" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> details the main NANDA diagnoses, defining characteristics and related factors, and/or associated conditions and/or populations at risk, and their corresponding planning of outcomes (NOC and indicators) and interventions (NIC and activities), which were executed and evaluated. The activities were implemented in two oncology tele-nursing educational sessions and finally in an evaluation session. Each session was based on scientific evidence and caritas care processes.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Assessment of outcomes</span><p id="par0115" class="elsevierStylePara elsevierViewall">The results 40 days after the assessment indicated that the nursing interventions were effective. After the educational strategies, the patient discovered the power of information, so made the decision to seek a more effective therapeutic approach and increase their chances of prolonging life. These interventions also improved health self-management.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Self-image increased significantly and a decrease in anxiety levels was observed, from severe anxiety (41pts) to minimal anxiety (6pts) on the Beck Anxiety Inventory. These changes were attributed to the implementation of educational interventions for emotional support and promotion of self-esteem.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Care assessment with the WCPS scale revealed high satisfaction during the experience since the patient gave scores of 7 in all items. This indicated that she always perceived humanised nursing care during the humanised oncological nursing process, satisfying her basic human needs with dignity. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> contains a summary of how the caritas care processes were used.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Discussion</span><p id="par0130" class="elsevierStylePara elsevierViewall">Decision-making in the palliative cancer process is complex, full of uncertainty and associated with strong emotions.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Alterations in body image can contribute to psychosocial problems in people with cancer, including problems with sexuality; feelings of loss of femininity; fear or sorrow regarding their perception of the relationship with their partner; lack of communication, and the desire for normality.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Specifically in this case, the potential alopecia was causing a negative impact on self-esteem and even leading to the interruption of cancer treatment.</p><p id="par0135" class="elsevierStylePara elsevierViewall">This case also presented an additional challenge, because the patient had had a history of addictions and refused to follow the prescribed pharmacological treatment, instead adopting the consumption of other substances such as CBD. The success of cannabinoids with symptoms such as nausea and vomiting induced by chemotherapy, in addition to anxiety, neuropathic pain, and an appetite-stimulating effect has been demonstrated in multiple studies.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> However, before including CBD in their treatment regimen,<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> people with cancer should consider that inhaled cannabinoids can produce serious symptoms in people with cancer, such as hypotension, panic, anxiety, delirium, respiratory depression and ataxia, and may orally produce tachycardia, bradycardia, bradypnoea, hypotension and respiratory distress and hypotonia.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Likewise, it can interfere with specific chemotherapy treatments such as methotrexate, docetaxel, cisplatin, carboplatin, and doxorubicin.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> Self-care is a key concept for people with cancer, but it must be based on adequate and safe health information.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Conclusions</span><p id="par0140" class="elsevierStylePara elsevierViewall">The nursing process based on humanised care contributed to improving decision-making, and health self-management, and also reducing anxiety. The humanization of care is an essential element to achieve integral wellbeing of the individual. The aim of this article is to present a clinical case, using the nursing process with the NANDA, NOC and NIC taxonomies, and based on the principles of Watson’s theory of humanized care. The participant is a 45-year-old woman with gastric cancer in palliative stage. The assessment was performed using Gordon’s functional patterns and the Watson Caritas Patient Score scale to evaluate the care received previously in the health system. Eight nursing diagnoses were identified, prioritizing 3 diagnoses using the clinical reasoning web (decisional conflict, anxiety, and ineffective self-management of health). Expected outcomes and nursing interventions were planned and implemented through moments of care using health education through tele-nursing and the intentional use of Caritas processes of care in the transpersonal relationship. The results were evaluated with the scales of the indicators and anxiety was also evaluated with the Beck Anxiety Inventory. Health education in oncology nursing contributed to improve informed decision making, reducing anxiety and providing emotional support to facilitate self-management of health.
