When BMI is higher than 40<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> we call it morbid obesity (MO), which is associated to different degrees of a metabolic syndrome that consists on cardiovascular and respiratory functional overload as well as an Obstructive Sleep Apnoea Syndrome (OSAS).</p><p id="par0010" class="elsevierStylePara elsevierViewall">Residual neuromuscular blockade is observed in up to 41% of patients in the general surgical population.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Residual paralysis affects genioglossus muscle activity<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> and changes also occur in swallowing as well as in ventilatory response to hypoxia.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> These effects are particularly dangerous in morbidly obese patients mostly affected by OSAS.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> Therefore, neuromuscular blockade in these patients should be fully reversed before tracheal extubation is attempted.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The cyclodextrin sugammadex reverses a blockade by selectively binding to plasma circulating rocuronium molecules. Then a concentration gradient results favouring the release of the NMBA from acetylcholine postsynaptic receptors.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> Sugammadex ensures complete recovery of the pharyngeal muscles.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> We hypothesized that, reversing the neuromuscular blockade with sugammadex could improve postoperative respiratory outcomes in morbidly obese patients under bariatric surgery.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patient selection</span><p id="par0020" class="elsevierStylePara elsevierViewall">After obtaining approval from the ethics committee of the Hospital Universitari de Bellvitge, Barcelona, and the Spanish Ministry of Health and Science (approval reference SAB-SUG-2011-01) for a prospective observational study, consecutive patients scheduled for laparoscopic bariatric surgery from January 2010 to June 2011 were recruited. Patients were enrolled if they gave signed consent. Exclusion criteria were a contraindication for aminosteroid neuromuscular blocking agents and chronic renal failure. No exclusion criteria regarding weight were established. These patients form the sugammadex study group (SG).</p><p id="par0025" class="elsevierStylePara elsevierViewall">From our database of laparoscopic bariatric surgical patients, a cohort of cases corresponding to those introduced from January 2008 to December 2009 was created, matched to the study group by age and associated OSAS. These form the historical group (HG).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study procedures</span><p id="par0030" class="elsevierStylePara elsevierViewall">Intraoperative standard non-invasive monitoring was used including the bispectral index (BIS). Oxygen by face mask was given for 5<span class="elsevierStyleHsp" style=""></span>min before anaesthesia induction, which was accomplished with 2–3<span class="elsevierStyleHsp" style=""></span>μg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> real body weight (RBW) of fentanyl followed by 2<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> RBW of propofol so as to obtain a BIS value lower than 60. In the SG, neuromuscular transmission was monitored by acceleromyography at the adductor pollicis (TOF-Watch SX; Organon Ltd., Dublin. Ireland) in accordance with the Good Clinical Research Practice in Studies of Neuromuscular Blocking Agents<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>; in the HG neuromuscular monitoring was not used. To facilitate tracheal intubation: succinylcholine 1<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> RBW or rocuronium 1<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> ideal body weight (IBW) was used in the SG, while succinylcholine 1<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> RBW or rocuronium 1<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> IBW or <span class="elsevierStyleItalic">cis</span>-atracurium 0.2<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> IBW was administered in the HG. Sevoflurane and fentanyl were used to maintain anaesthesia at BIS values <60; rocuronium was administered (0.15<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> IBW) at reappearance of T2 (second response of TOF) in the SG, whereas a bolus of 0.15<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> of rocuronium IBW or a bolus of 0.03<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> of cis-atracurium IBW was administered when needed at the anaesthesiologist criteria in the HG.</p><p id="par0035" class="elsevierStylePara elsevierViewall">At the end of surgery, the neuromuscular block was reversed. In patients sustaining a deep blockade, the dose of sugammadex was divided into two fractions as follows: 4<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> IBW was first administered and the TOFr was recorded every 20<span class="elsevierStyleHsp" style=""></span>s. If the TOFr was less than 0.9 until 3<span class="elsevierStyleHsp" style=""></span>min apart, another sugammadex dose of 2<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> IBW was administered. To reverse a moderate blockade, sugammadex was administered as follows: 2<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> IBW was first administered and the TOFr was recorded every 20<span class="elsevierStyleHsp" style=""></span>s. If the TOFr was less than 0.9 until 2<span class="elsevierStyleHsp" style=""></span>min apart, another sugammadex dose of 2<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> IBW was administered; more details about methodology can be found in reference 9. In the HG a dose of neostigmine of 0.04<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> IBW was first administered and another additional neostigmine dose of 0.02<span class="elsevierStyleHsp" style=""></span>mg<span class="elsevierStyleHsp" style=""></span>kg<span class="elsevierStyleSup">−1</span> IBW was administered ten minutes afterwards if deemed necessary. For both groups, tracheal extubation was carried out when the patient was fully awake and with adequate tidal volume and inspiratory force.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The main objective was the percentage of patients who required mechanical ventilation in the immediate postoperative period, that included those patients maintained with tracheal intubation after surgery and patients in whom tracheal re-intubation was performed during the first two hours after surgery, not related to surgical re-operation. The secondary objective was the percentage of patients in whom pathological changes were noticed when comparing preoperative and postoperative chest X-ray. This comparison was accomplished by two different observers. Preoperative chest X-ray was carried out in the standing position whereas postoperative chest X-ray was performed in the sitting position in a special bed at admission in the surgical Intensive Care Unit (ICU). The comparison was done taking into account the possible radiological differences that might exist between the two positions (different pulmonary volume or cardiac size). Samples of arterial blood gases were obtained prior to anaesthesia induction, after tracheal extubation at ICU admission and 24<span class="elsevierStyleHsp" style=""></span>h after the procedure at the ward. Other variables recorded were patient characteristics, surgical and postoperative events.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">To contrast the hypothesis, we estimated that 5% of patients would need postoperative mechanical ventilation after surgery in the HG whereas, for significant clinical difference, it was estimated that 1% of patients treated with sugammadex would require mechanical ventilation. At a level of statistical significance of 0.05 for comparisons and power of 0.80, the calculated sample size was 320 patients. A non-parametric Mann–Whitney <span class="elsevierStyleItalic">U</span> test was used to compare continuous variables and a chi-square test was used to compare discontinuous variables. Statistical significance was set at <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05. Data are presented as absolute number (%) or median (range).</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">We enrolled 160 patients in the SG that were matched by age and associated OSAS against 160 patients of the HG. The patients of the HG were selected as consecutive patients who were introduced from January 2008 to December 2009 in our database of laparoscopic bariatric surgeries. Patients’ characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. An abnormal preoperative chest X-ray was found in 10% in the SG and 12% in the HG, respectively. The main findings were heart enlargement in 14 patients and parenchyma lung reductions in 2 patients in the SG and the heart enlargement in 14, parenchyma lung reduction in 4 and pleural effusion in 2 patients in the HG. Rocuronium was used in all patients of the SG, while in the HG rocuronium and cis-atracurium were administered in 55 (34%) and 105 (66%) patients respectively. No adverse effects of sugammadex or neostigmine were recorded.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">Two patients in the SG (one because of slow arousal and the other because of neck haematoma related to central venous puncture) and four in the HG (all of them because of hypoventilation after surgery) could not be extubated in the operating room. One patient in the HG required reintubation because of hypoventilation. The percentage of patients requiring postoperative mechanical ventilation in the SG was (percentage, 95% CI), 1.25% (0.34–4.4) vs. 3.13% (1.34–7.11) in the HG. This was not statically different. On the contrary, significantly less chest X-ray postoperative pathological changes were observed in the SG: 11 patients (6.9%) vs 26 (16.3%) in the HG (odds ratio, 95% CI), 0.36 (0.18–0.8), <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.015 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). The main new findings were: atelectasis in 9 patients and atelectasis plus pleural effusion in 2 patients in the SG; 22 patients showed parenchyma lung reduction and 4 parenchyma plus pleural effusion in the HG. There were no differences versus pre and postoperative arterial blood gases.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0060" class="elsevierStylePara elsevierViewall">The main new findings of this study were that no statistical difference was found in the requirement of postoperative mechanical ventilation depending on the reversal agent used. On the contrary, significative differences in postoperative pathological X-ray changes were found.</p><p id="par0065" class="elsevierStylePara elsevierViewall">One patient of the HG managed with neostigmine, exhibited signs of residual neuromuscular blockade that required reintubation. It has been reported that most patients whose neuromuscular blockade was reverted with neostigmine are extubated with some residual effect of neuromuscular blockade,<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> which might result in inaccurate protective reflexes.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> This fact is particularly dangerous in morbidly obese patients with OSAS, which have structurally narrower and more collapsible pharyngeal airways. When sugammadex was compared with neostigmine for moderate neuromuscular blockade in laparoscopic bariatric surgery, both drugs were able to reverse it, although median time for neostigmine was 9.5<span class="elsevierStyleHsp" style=""></span>min as opposed to 2.7<span class="elsevierStyleHsp" style=""></span>min in sugammadex patients.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> In contrast to anticholinesterasic agents, sugammadex is able to reverse deep and intense neuromuscular blockade<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,9</span></a> and also was able to reverse neuromuscular blockade in a morbid obese patient affected by liver dysfunction.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> However, in a recently reported case of a morbidly obese patient, five minutes after the sugammadex administration, the patient needed to be reintubated because of reoccurrence of the blockade.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> Moreover, both reversal of neuromuscular blockade and consciousness are necessary for a safe tracheal extubation at the end of anaesthesia.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Although in our study it was found that more patients in absolute number needed mechanical ventilation in the HG than in the SG (5 vs 2), this finding was not statistically significant. So, in our cohorts, the use of sugammadex did not reduce requirements of postoperative mechanical ventilation. Nevertheless, the need of postoperative reintubation or mechanical ventilation is not a usual event in any population, nor in high risk ones. So, maybe if we had tried to find other more common postoperative respiratory critical events (episodes of SpO<span class="elsevierStyleInf">2</span> <90% upon ICU arrival, need for airway manoeuvres and/or stimulation to maintain SpO2 >90% in the ICU, or for ventilatory support, etc.) more statistical significance could have been found.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,14</span></a> Anyway, basing on the fact that the need for reintubation is an important critical event, the clinical difference, although not statistical, should be taken into account. Moreover, comparison with a retrospective cohort limited the postoperative respiratory variables to analyze only to that which had been recorded in the historical database.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Anaesthesia induced atelectasis may occur in nearly all anesthetized patients, as a consequence of abnormalities in regional ventilation produced during anaesthesia and surgery,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> and because a clinically relevant fade in forced vital capacity is caused as a consequence of residual neuromuscular blockade.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> Significantly more atelectasis and atelectasis-pleural effusion were observed in the HG in comparison with the SG in our study. Although these chest X-ray changes were detected, the arterial blood gases were similar for both groups, which could be explained by two facts: first of all, most of the patients were diagnosed of OSAS and were on Non-Invasive Ventilation Support treatment preoperatively that was re-started after tracheal extubation in the ICU; moreover, patients were placed in the sitting position in a special bed and received precocious active physical therapy in ICU.</p><p id="par0080" class="elsevierStylePara elsevierViewall">We did not randomize our patients to sugammadex or neostigmine so differences in neuromuscular blockade reversal times cannot be compared. Even though our cohorts were comparable, lack of randomization becomes a formal limitation of our study. Also, the administration of repeated unprotocolized bolus of neuromuscular blocking agent in the HG, could partially influence the results and limit its implications. Nevertheless, it is important to note that all patients received a reversal agent, so residual curarization could mostly be avoided.</p><p id="par0085" class="elsevierStylePara elsevierViewall">We conclude that in our study, the need for postoperative mechanical ventilation was not associated to the reversal agent chosen. However, significantly less atelectasis was observed depending on the reversal agent used. Therefore, prospective randomized studies with larger number of patients and performance of adequate tests are needed in order to find differences in postoperative respiratory outcomes of patients under laparoscopic bariatric surgery.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Funding</span><p id="par0090" class="elsevierStylePara elsevierViewall">This study received support from the department.
