Its prevalence in larger series is 19–29%, which is higher in older people (<a class="elsevierStyleCrossRef" href="#bib0070">Golub, Chen, Otto, Hapner, & Johns, 2006</a>) Dysphonia has an important effect on quality of life (QOL). In <a class="elsevierStyleCrossRef" href="#bib0090">1997, Jacobson et al.</a> (Jacobson, Johnson, Grywalski, Silbergleit, Jacobson, Benninger, et al., 1997) developed the Voice Handicap Index (VHI) to measure the effects of dysphonia in functional, physical and emotional fields, which contained 30 items with good reliability (Cronbach's alpha<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.96). Later, Rosen et al. (<a class="elsevierStyleCrossRef" href="#bib0125">Rosen, Lee, Osborne, Zullo, & Murry, 2004</a>) in 2004, developed and validated a short version (VHI-10) that made the wide use of the survey easy. It has been applied to many conditions with good reliability and reproducibility (<a class="elsevierStyleCrossRefs" href="#bib0030">Cohen, Dupont, & Courey, 2006; Taito et al., 2019</a>) and has been validated in many languages, including Spanish (<a class="elsevierStyleCrossRef" href="#bib0110">Nunez-Batalla et al., 2007</a>). Although the VHI-10 is widely used in the medical literature, only five references to its use in the Latin-American population were found in PubMed (<a class="elsevierStyleCrossRefs" href="#bib0025">Bonilla-Velez, Small, Bonilla-Escobar, Sharum, & Tulunay-Ugur, 2018; Godoy López, Godall Castell, & Gassull Bustamante, 2017; Gomez-Puerta et al., 2014; Guzman et al., 2013, 2017; Ramirez, Jimenez, Lopez, & Ysunza, 2018</a>) all of them using the European Spanish version of the VHI-10. There is evidence that the European Spanish language uses different linguistic expressions, and therefore, that the psychometric properties can be different from those of Latin-American Spanish (<a class="elsevierStyleCrossRef" href="#bib0045">de Molina Ortés y Hernández-Campoy, 2018</a>).</p><p id="par0010" class="elsevierStylePara elsevierViewall">The aim of this study was to generate a cross-cultural adaptation of the VHI-10 in the Latin-American Spanish language and to assess its psychometric properties in a Colombian population.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">This study was approved by the Ethics in Research Committee of the Universidad de Antioquia and was performed in accordance with ethical standards. Written informed consent was obtained from all patients.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Design</span><p id="par0020" class="elsevierStylePara elsevierViewall">This study utilized a prospective cross-sectional design to validate the VHI-10 in patients with dysphonia.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">VHI-10 scale</span><p id="par0025" class="elsevierStylePara elsevierViewall">The original version of the VHI-10 was obtained from <a class="elsevierStyleCrossRef" href="#bib0125">Rosen et al. (2004)</a>. The VHI-10 was developed in 2004 using a sample of 269 patients and was validated using a sample of 819 persons. It has 10 questions (5 for functional impact, 3 for physical and 2 for emotional impairment) that are rated from never to always on a scale ranging from 0 to 4. The maximum score is 40, and a higher score represents a worse result. The questionnaire can be self-administered.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">We followed the ISPOR guidelines for the translation and cross-validation of health-related scales (<a class="elsevierStyleCrossRefs" href="#bib0115">Patrick et al., 2011; Rothman et al., 2009; Wild et al., 2005</a>). The first translation was made by two independent native English-speaking translators. A consensus document was back-translated by two independent native Spanish-speaking translators. The final document was compared with the original instrument and was then evaluated by two native Spanish-speaking surgeons (one otorhinolaryngologist and one head and neck surgeon) to produce a final document. A pilot test was performed with twelve healthy volunteers to assess readability, but their results were not included in the final analysis. We compared the new version with the validated version in the Spanish language.</p><p id="par0035" class="elsevierStylePara elsevierViewall">We asked all candidates (dysphonic and non-dysphonic) to complete the written version of the Latin-American Spanish versions of the VHI-10 (LASVHI-10), a 4-point severity scale (0, normal; 1, mild; 2, moderate; and 3, severe) as described by <a class="elsevierStyleCrossRef" href="#bib0110">Nunez-Batalla et al. (2007)</a> and two questions from the Spanish validated University of Washington Quality of Life (UWQOL) scale (one question about general QOL and 1 about speech) (<a class="elsevierStyleCrossRef" href="#bib0105">Nazar et al., 2010</a>). For all cases, one of the researchers acted as assistant to explain the study objectives and to solve questions.</p><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Patient sample</span><p id="par0040" class="elsevierStylePara elsevierViewall">The inclusion criteria were as follows: adult patients with clinical findings of dysphonia made by a physician, who had undergone a diagnostic confirmatory test such as stroboscopy, video laryngoscopy or electromyography and for whom Spanish was the native language. Patients with physical impairments that prevented reading, hearing or understanding the scale, those with psychiatric illness and those who did not consent to participate were excluded. We also obtained demographic characteristics (age, sex, scholar level, type of work). For the test–retest analysis, randomly selected patients completed the VHI-10 instrument for a second time two weeks after the first evaluation.</p><p id="par0045" class="elsevierStylePara elsevierViewall">A sample size of 97 patients was calculated using the Bonnett formula (<a class="elsevierStyleCrossRef" href="#bib0020">Bonett, 2002</a>) with the following parameters: a type I error of .05, a power of 80%, a known Cronbach's alpha estimate of .86, an expected Cronbach's alpha estimate of .91 and a scale of 10 items. A test–retest analysis was planned in a random subsample of 71 patients. We also included 22 non-dysphonic controls who assisted with general ENT visits.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0050" class="elsevierStylePara elsevierViewall">The categorical variables are presented as percentages and ranges, and the continuous variables are shown as the mean and standard deviation. For the analyses of paired samples, we used the Kruskal–Wallis test and Mann–Whitney <span class="elsevierStyleItalic">U</span> test and set the significance level to <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05. We used Stata statistical software (StataCorp, TX, USA, Version 9.1) for the psychometric validation, and we used a principal components exploratory factor analysis with varimax rotation to identify the underlying dimensions as measures of construct validity and to provide evidence regarding whether the instrument reproduced the same factor-loading pattern as that observed for the original English VHI-10 (<a class="elsevierStyleCrossRef" href="#bib0050">Deary, Webb, Mackenzie, Wilson, & Carding, 2004</a>). A reliability test involving Cronbach's alpha, the item-total correlation and the Guttman split-half reliability coefficient was performed. Values higher than .70 for Cronbach's alpha and .8 for split-half reliability were considered acceptable. For the convergent/concurrent validity, we selected a visual analog scale performed by the patient (a 4-point severity scale for voice quality -0, normal; 1, mild; 2, moderate; and 3, severe-) and two relevant questions from the Spanish version of the University of Washington Quality of Life scale (UWQOL) scale, (Item 7, that asses difficulties for speech and the third general question about global quality of life in the last seven days) (<a class="elsevierStyleCrossRef" href="#bib0105">Nazar et al., 2010</a>) and we used non-dysphonic controls for the known-group validity. The scores were compared using Spearman's correlation coefficients. For the test–retest reliability, we used the Lin coefficient. Multiple linear regression was used to evaluate the impact of age, sex, scholarity, occupation and dysphonia on the LASVHI-10 score.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Characteristics of the population</span><p id="par0055" class="elsevierStylePara elsevierViewall">We interviewed 119 patients. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the sociodemographic and clinical characteristics of the patients. The mean age was 47.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>14.5 years (median 48, range 20-76). Most of the patients were married females, and 42.9% had professions not related to vocal effort. The most common diagnosis was functional dysphonia. There was no difference in sex (female 59.1% vs 65.9, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.54) or age (46.3 vs 47.6, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.70) between the controls and the cases.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Cross-cultural adaptation</span><p id="par0060" class="elsevierStylePara elsevierViewall">The translations produced a draft with some changes made to the syntaxis of the questions. Item F9 was changed due to difficulties in understanding the European Spanish version. Item P5 was changed because the word “tensar” from the European version was not understood in Latin America; this word was changed to “esforzar”. Item E6 was also changed because the word “minusvalia” from the European version has a different meaning in Latin America; thus, this word was changed to “discapacitado”.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The pilot test was conducted among 10 people (two of them, healthy 12-year-old children in order to guarantee a level of readability), but their results were not included in the final analysis. No changes in words were made after this phase. A change in the format of the scale was suggested, which related to increasing the letter size and widening the spaces. In general, there was good agreement between the translated and original English versions of the questionnaire. The final version of the LASVHI-10 scale can be found in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Item description</span><p id="par0070" class="elsevierStylePara elsevierViewall">The mean score for the LASVHI-10 scale was 18.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.7 (median 18, range 0–40, skewness (.71) and kurtosis (.01)) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The items with poorer quality (i.e., experienced ‘almost always’ and “always” responses) were “P5: I feel as though I have to strain to produce voice”, “P6: The clarity of my voice is unpredictable” and “P3: People ask, ‘What's wrong with your voice?”’. The items with better quality (i.e., ‘never’, ‘almost never’ and “sometimes” responses) were “F9: I feel left out of conversations because of my voice”, “F10: My voice problem causes me to lose income” and “E6: My voice makes me feel handicapped”.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">The results of the 4-point severity scale was as follows: normal (20.3%), mild (20.3%), moderate (37.4%) and severe (22.0%). The distribution of the WU-QOL question on speech was as follows: “My speech is the same as always” (82.6%), “I have difficulty saying some words but I can be understood over the phone” (13.9%) and “Only my family and friends can understand me” (3.5%). The distribution of the WU-QOL question on global QOL was as follows: outstanding (18.6%), very good (26.3%), good (33.9%), fair (16.1%) and poor (5.1%).</p><p id="par0080" class="elsevierStylePara elsevierViewall">The absence of dysphonia was the only statistically significant factor for the LASVHI-10 score in multivariate analysis (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.04)</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Construct validity</span><p id="par0085" class="elsevierStylePara elsevierViewall">Factor analysis was performed by replicating the methods of the study by <a class="elsevierStyleCrossRef" href="#bib0050">Deary et al. (2004)</a>. A principal components factor analysis showed a two-factor solution using the Kaiser criterion (eigenvalue<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>1). The two factors explained 69% of the variability (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The diagnostic test results were good (Kaiser–Meyer–Olkin value was .90, and a Bartlett sphericity test yielded <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Internal consistency reliability</span><p id="par0095" class="elsevierStylePara elsevierViewall">The Cronbach's alpha value for the global scale was .92. The results of the individual items used to assess convergent and divergent validity are shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>. All the Cronbach's alpha values for the individual items were >.7 and had item-test correlations >.4. The Guttman split-half reliability coefficient lambda 2 was .92.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Concurrent validity</span><p id="par0100" class="elsevierStylePara elsevierViewall">The comparison between the LASVHI-10 and the 4-point severity visual analog scale demonstrated a high correlation. The LASVHI-10 score increased in relation to the perceived severity of voice quality (4.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.0 for normal, 12.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.7 for mild, 22.1<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.6 for moderate and 29.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.3 for severe; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.01). However, the correlation with the speech item of the UW-QOL questionnaire and global QOL was low (rho<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.56 and .38, respectively). For the known group validity, patients with dysphonia showed a higher LASVHI-10 score compared to those without dysphonia. <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> shows that items with the largest mean differences were “F8: My voice difficulties restrict my personal and social life”, “P5: I feel as though I have to strain to produce voice”, “E4: My voice problem upsets me” and “P3: People ask, ‘What's wrong with your voice?”’</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Test–retest reliability</span><p id="par0105" class="elsevierStylePara elsevierViewall">In a sample of 71 patients, the means of the initial and the final LASVHI-10 were 22.0<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.0 and 19.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.07, respectively. The concordant correlation coefficient for the global scale was .80.</p></span></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">Dysphonia is a common complaint in general otorhinolaryngological practice, with a prevalence of approximately 20% (<a class="elsevierStyleCrossRef" href="#bib0070">Golub et al., 2006</a>). To date, the most widely used scale to assess the effects of dysphonia is the VHI-10, whose main objective is to identify modifications in voice quality and grade severity in daily social and professional life. <a class="elsevierStyleCrossRef" href="#bib0110">Nunez-Batalla et al. (2007)</a> adapted it to the European Spanish language. However, differences in linguistic expressions between European Spanish and Latin-American Spanish suppose a difference in the reliability of the tool (<a class="elsevierStyleCrossRef" href="#bib0045">de Molina Ortés y Hernández-Campoy, 2018</a>). Approximately three hundred million people speak Spanish in Latin America, and almost 40 million people speak Spanish in the USA; therefore, it is expected that there is a large number of patients who could benefit from the use of a language-specific validated instrument.</p><p id="par0115" class="elsevierStylePara elsevierViewall">We validated a cross-cultural adaptation of the VHI-10 to Latin-American Spanish. The results showed some differences from the European Spanish version, which forced us to make changes to some items.</p><p id="par0120" class="elsevierStylePara elsevierViewall">All the methodological steps recommended to guarantee the performance of the instrument were performed. The internal validity, reliability and reproducibility reached similar results as the original validation developed by <a class="elsevierStyleCrossRef" href="#bib0125">Rosen et al. (2004)</a>. The factor analysis showed a solution with two factors, which was different from the results of the study by <a class="elsevierStyleCrossRef" href="#bib0050">Deary et al. (2004)</a> and from the Chinese validation study (<a class="elsevierStyleCrossRef" href="#bib0100">Lam et al., 2006</a>) that found a one-factor solution, and it resembled the results of the original VHI with a main effect of physical and functional factors. These outcomes can be explained by the differences in value given to each symptom by populations with different cultures and by the proportion of dysphonic patients in the sample (<a class="elsevierStyleCrossRef" href="#bib0100">Lam et al., 2006</a>). However, the LASVHI-10 had a high percentage of variability explained by the instrument (69%). The reliability of the individual items was high, similar to those originally reported (<a class="elsevierStyleCrossRefs" href="#bib0005">Amir et al., 2006; Costa, Oliveira, & Behlau, 2013; Forti et al., 2014; Lam et al., 2006</a>) and the item-rest coefficients were higher than .4. A concurrent validity assessment showed a high Spearman's coefficient with the autoreported severity scale. However, the correlation with the WU-QOL scale items of speech and global QOL was low. Most likely, the speech items of the WU-QOL evaluate more complex constructs, including the oral phase of speech, that are not relevant in dysphonic patients. The known criteria evaluation showed that the instrument had the ability to discriminate between clinical conditions such as the presence or absence of dysphonia and its perceived severity. The instrument was also stable in time, with a test–retest evaluation showing high correlation. All these measures guarantee that the LASVHI-10 can be used confidently in routine practice. Some studies have used the VHI-10 as a screening tool for stroboscopy (<a class="elsevierStyleCrossRef" href="#bib0055">DeVore y Carroll, 2020</a>) or electromyography (<a class="elsevierStyleCrossRef" href="#bib0040">Daggumati, Panossian, & Sataloff, 2019</a>) or to evaluate the results of larynx (<a class="elsevierStyleCrossRef" href="#bib0015">Ballard, Abramowitz, Sukato, Bentsianov, & Rosenfeld, 2018</a>) and thyroid surgery (<a class="elsevierStyleCrossRef" href="#bib0095">Kovatch et al., 2019</a>) in benign and malignant conditions. A reliable scale allows the development of research with reproducible results.</p><p id="par0125" class="elsevierStylePara elsevierViewall">It must be realized that the cross-sectional design impedes the evaluation of LASVHI-10 as a tool to evaluate changes over time, and this impediment must be evaluated in a specific planned study. Our sample size was small, but the subject-to-item ratio was 11, which is similar to that in other reported studies (<a class="elsevierStyleCrossRef" href="#bib0010">Anthoine, Moret, Regnault, Sebille, & Hardouin, 2014</a>).</p><p id="par0130" class="elsevierStylePara elsevierViewall">In conclusion, the results of this study provide a reliable and objective instrument to be used in clinical practice and for research objectives. The LASVHI-10 is a brief, concise and easy-to-respond self-report instrument that is very useful in clinical practice to evaluate vocal quality and helps in screening for more complex procedures such as video laryngoscopy or stroboscopy. The Voice Handicap Index-10 (VHI-10) was translated and back-translated and then adapted to the Latin-American Spanish language (LASVHI-10). It was then applied to adult patients with clinical findings of dysphonia made by a physician. A psychometric validation consisting of a principal components exploratory factor analysis with varimax rotation, concurrent validation comparing the visual analog scale performed by the patient and two relevant questions of the Spanish version of the University of Washington Quality of Life (UWQOL) scale, known group validity assessment and test–retest reliability assessment was performed.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">A total of 119 patients were included. The mean score for the LASVHI-10 scale was 18.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.7 (median 18, range 0–40). A principal component factor analysis showed a two-factor solution with good diagnostic test results. The Cronbach's alpha value for the global scale was .92.
