Brain imaging tests showed no lesions and cerebrospinal fluid was normal. The complete study revealed the presence of a limited-stage small-cell lung cancer (SCLC) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The onconeural antibodies test was positive for anti-Zic4 antibodies. With the suspicion of a paraneoplastic <span class="elsevierStyleItalic">opsoclonus myoclonus</span> syndrome (OMS), a 5-day treatment with Cisplatin-Etoposide (standard in first-line) and immunosuppressive treatment with gamma globulin (0.5<span class="elsevierStyleHsp" style=""></span>mg/kg/day) was initiated. The patient showed improvement two weeks after the first cycle of chemotherapy, with complete resolution of the syndrome 3 weeks later. After 3 cycles of treatment, the patient showed a partial response.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">OMS is an extremely rare ocular motility disorder, with an incidence of 1/5,000,000<span class="elsevierStyleHsp" style=""></span>inhabitants/year. In adults, 20–40% are paraneoplastic, with SCLC being the most commonly associated neoplasm described.
