Glucose 6-phosphate dehydrogenase (G6PDH) is involved in this metabolic pathway and is present in all cells. This enzyme oxidises glucose-6-phosphate to 6-phosphogluconate acid in a proton donor reaction, where nicotinamide adenine dinucleotide phosphate (NADPH) is reduced, maintaining the erythrocyte’s glutathione in a reduced state (GSH). This glutathione protects the haemocyte from oxidative stress by stabilising its membrane.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In Spain, the incidence in adults is estimated to be between 0.1% and .5%. It has a prevalence of 400 million people worldwide, being higher in Africa, Southern Europe, Middle East, Asia, Middle East and the Pacific Islands. These countries often have neonatal screening for the disease.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">When a mutation in the enzyme occurs, adequate levels of reduced NADPH are not generated, leading to a deficit of GSH, which exposes the cell to oxidative stress.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> In addition, due to the lack of reduced NADPH, methaemoglobin (MetHb) reductase cannot act by reducing ferric ion to ferrous ion, favouring the formation of MetHb, which precipitates to form Heinz corpuscles. This process produces haemolysis, especially of extravascular origin, and is triggered by situations in which oxygen free radicals are generated, such as acidosis, fever, infections, hypothermia, hypoxia, perioperative ischaemia, favism, or administration of oxidant drugs.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–3</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">6PDH deficiency is classified into 5 groups, with group I being the most severe, associated with an enzyme activity of less than 10%, leading to severe and chronic haemolytic anaemia, and groups IV and V being the mildest, with enzyme activity sometimes higher than normal.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,4,5</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">The anesthetic management of G6PDH deficiency is proposed in different literature reviews and clinical cases; however, there is no general consensus on the use of drugs and other medications during the perioperative period. The literature, although scarce, recommends avoiding agents that cause oxidative stress, as well as those that induce the formation of MetHb, which can trigger haemolysis, anaemia and jaundice 24−72 h after administration.</p><p id="par0030" class="elsevierStylePara elsevierViewall">We describe the case of a 45-year-old man diagnosed with G6PDH deficiency, in an asymptomatic state, who underwent surgery for low anterior rectal resection due to cancer.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Clinical case description</span><p id="par0035" class="elsevierStylePara elsevierViewall">A 45-year-old man was admitted on a scheduled admission for laparoscopic low anterior rectal resection. He had no known drug allergies or toxic habits. Background: G6PDH deficiency, diagnosed by enzyme dosage through a peripheral blood sample at birth, due to the existence of relatives with this deficiency, hitherto asymptomatic, and stage IV adenocarcinoma of the middle rectum with pulmonary metastases, subjected to radiotherapy and adjuvant chemotherapy. Currently being treated with omeprazole 20 mg and dexamethasone 4 mg as antiemetic prophylaxis during chemotherapy treatment. No previous surgical-anesthetic history.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The pre-anaesthesia assessment showed a body mass index of 24.68 kg/m<span class="elsevierStyleSup">2</span>, with no predictive parameters for a difficult airway or significant pathological findings in the pre-operative analyses, and she was classified as a grade III perioperative risk according to ASA.</p><p id="par0045" class="elsevierStylePara elsevierViewall">On arrival in the operating theatre, he was monitored with a 5-lead ECG, serial measurement of non-invasive blood pressure, pulse oximeter, anesthetic depth (BIS), neuromuscular blockade with a 4 (TOF) train and non-invasive temperature. Anxiolytic treatment was started with midazolam 2 mg and an epidural catheter was placed at lumbar L1-L2 level. Subsequently, general anaesthesia was induced after adequate preoxygenation, administering fentanyl 150 g, propofol in TCI Schnider model with effect concentration at 4 g/mL for 3 min and rocuronium 50 mg, and orotracheal intubation was performed by direct laryngoscopy, with TET with internal diameter of 8 mm, without incidents, classified according to Cormack-Lehane level 1 scale. Anesthetic maintenance was performed with propofol in Schnider model TCI with an effect concentration between 2.5–3.5 g/ml, fentanyl depending on haemodynamics at a total cumulative dose of 350 g and bupivacaine 0.125 % via epidural catheter 8 ml. No further neuromuscular relaxant was administered during the surgical procedure. In the presence of Apfel scale equal to 2 points, antiemetic prophylaxis was decided with dexamethasone 4 mg in induction and then ondasetron 4 mg prior to education. Fluid therapy consisted of 700 ml of Plasma-lyte® BIEFFE MEDITAL, S. A. and 100 ml of 0.9% physiological saline. The patient remained haemodynamically stable throughout the surgical procedure, with a constant temperature, around 36.5 C. The procedure lasted 150 min. At the end, metamizole 2 g and dexketoprofen 50 mg were administered as analgesic rescue and residual neuromuscular blockade (TOF 0.7) was reversed with neostigmine 2 mg and atropine 1 mg, achieving a TOF ratio above 90%. Finally, the patient was extubated and transferred to the Postanaesthesia Recovery Unit (PARU), without incident.</p><p id="par0050" class="elsevierStylePara elsevierViewall">There were no incidents in the immediate postoperative period in the PARU or later on the hospital ward. The patient was discharged from general surgery 4 days after surgery.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Discussion</span><p id="par0055" class="elsevierStylePara elsevierViewall">G6PDH deficiency is a rare disease and there are few publications describing the perioperative evolution of patients with this disease. The anesthetic management of these patients can be complex, with the priority being to prevent the development of complications. The recommendations described are aimed at avoiding any drug (<a class="elsevierStyleCrossRefs" href="#tbl0005">Tables 1 and 2</a>)<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2,5</span></a> or circumstance that predisposes the cell to oxidative stress, with the ultimate aim of reducing the incidence of haemolytic crisis.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">Regarding the drugs used in anaesthesia (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>),<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,5</span></a> isoflurane, sevoflurane, fentanyl and rocuronium are agents that are considered safe according to the literature. However, an in vitro study by Altikat et al.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> shows that isoflurane, sevoflurane, diazepam and midazolam reduce G6PDH activity, while halothane, ketamine and prilocaine do not affect G6PDH activity. There are now reports of safe use of sevoflurane and midazolam.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a> According to an animal study by Bezerra et al.,<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> caution should be exercised when using sevoflurane together with enzyme inducing agents such as isoniazid, as they increase the activity of the CYP2E1 enzyme, which is responsible for the biotransformation of sevoflurane and increased production of oxygen free radicals. There are no data in the literature on rocuronium influencing cellular oxidative stress and it is a safe drug to use in these patients. Propofol is safe due to the presence of a phenol ring with antioxidant activity. Remifentanil directly increases levels of the oxidative stress-inhibiting enzyme superoxide dismutase, although its use, like that of fentanyl, is controversial due to insufficient data on its safety in the most severe deficits. Dexmedetomidine is considered a safe antioxidant agent in patients with this deficit.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">In the case of local anesthetics, prilocaine and lidocaine can cause MetHb, as they are similar in structure. However, prilocaine has a greater tendency to cause MetHb due to its primary metabolite, <span class="elsevierStyleItalic">O</span>-toludin, which oxidises haemoglobin directly. Benzocaine and nitroglycerin, as well as nitroprusside or sodium nitrate and silver nitrate, also cause MetHb and should be avoided in these patients.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> The rest of the local anesthetics have not been studied in depth, although most clinical cases concerning this disease refer to locoregional management with epidural/intradural anaesthesia with bupivacaine, which in this case was the drug of choice, and there are no data on the use of ropivacaine.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Regarding paracetamol and non-steroidal anti-inflammatory drugs, there is controversy, with studies advising against high doses as they may produce MetHb. Other studies indicate that, given at short intervals at normal established doses, they are safe.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a> There is scant literature on the safety of antiemetics such as ondansetron in anaesthesia; however, this drug has been used without incident in these patients.</p><p id="par0075" class="elsevierStylePara elsevierViewall">As for MetHb, in these patients, in the absence of the enzyme, methylene blue does not show a complete response, because NADPH is also necessary for the action of this drug as a reducing agent, otherwise this agent is oxidising, and may aggravate the cuadro.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> In case of methaemoglobinaemia, supportive treatment would consist of red blood cell transfusions, hyperbaric therapy with supplementary oxygen and urine alkalinisation.</p><p id="par0080" class="elsevierStylePara elsevierViewall">If a haemolytic crisis occurs, it should be identified as soon as possible. Warning signs and symptoms indicating the onset of haemolytic crisis may include jaundice, cyanosis, headache, fatigue, tachycardia, dyspnoea and haemoglobinuria. The picture ends when all G6PDH-deficient erythrocytes have been haemolytised and new ones with enzymatic activity have been synthesised by stimulation of erythropoiesis, after 4–7 days.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2,8</span></a> During general anaesthesia, these may be masked and the patient should be closely monitored. Hypotension itself is not a specific indicator of haemolysis and may not be evident until a more advanced stage where haematuria appears.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,5</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">If a haemolytic crisis develops in the perioperative period, once detected, the first step would be to remove possible triggering agents. To avoid acute renal failure due to haemoglobinuria, it is necessary to maintain diuresis by means of crystalloids together with diuretics such as mannitol or furosemide, as well as alkalinising the urine.
