The absence of proteinuria or hypertension are atypical variants of preeclampsia<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Although hypertension and proteinuria are prerequisites for the diagnosis of preeclampsia, this multisystemic disease can also present with atypical manifestations that occur without hypertension or proteinuria, such as HELLP syndrome.</p><p id="par0015" class="elsevierStylePara elsevierViewall">HELLP syndrome, a severe form of preeclampsia that occurs in 10%-15% of women with this disease<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>, is characterised by thrombocytopaenia (<150,000 platelets/mcL), haemolysis, and liver dysfunction with elevated transaminases and LDH<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–6</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Retinal microvascular changes occur in 25%-100% of preeclampsias, and are associated with the development of hypertensive retinopathy due to vasoconstriction of the retinal arterioles<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,6</span></a>. However, visual symptoms are only reported in 25% of women with severe pre-eclampsia, and in 50% of women with eclampsia, and are due to retinal and choroidal circulation dysfunction<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3,5,7</span></a>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Serous retinal detachment (SRD) is a rare complication that affects 1% of women with preeclampsia<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–9</span></a>, a rate that can increase 7-fold when preeclampsia is associated with HELLP syndrome<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,5</span></a>. Although most cases of SRD are secondary to hypertensive retinopathy<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>, the complication has also been reported in normotensive pregnant women<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,4</span></a>, suggesting that hypertension does not fully explain the parthenogenesis of this phenomenon.</p><p id="par0030" class="elsevierStylePara elsevierViewall">We report a case of SRD after caesarean section in a pregnant woman with atypical preeclampsia due to HELLP syndrome without hypertension. This case is particularly interesting because SRD preceded HELLP syndrome.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Case report</span><p id="par0035" class="elsevierStylePara elsevierViewall">The patient was a 37-year-old primigravida who had undergone in vitro fertilization. She presented in the 34th week due to premature rupture of membranes. On admission, her BP was 118/85 mmHg and labs showed proteinuria with creatinine 1.46 mg/dl (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">Despite her normal BP, the presence of proteinuria inclined the diagnosis towards an atypical variant of preeclampsia. Treatment started with corticosteroids to accelerate foetal maturation, perfusion of magnesium sulphate (S0<span class="elsevierStyleInf">4</span>Mg), and strict control of BP and urine output. We then proceeded to induce labour, during which the patient required epidural analgesia. Eight hours after the start of induction, an urgent caesarean section was indicated due to foetal distress. The procedure was performed after updosing anaesthesia delivered through the epidural catheter. Bleeding was insignificant, and the patient remained stable throughout the procedure.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Upon admission to the recovery room, the patient reported loss of visual acuity (VA) and blurred vision. Over the following hours, labs showed progressive thrombocytopaenia and elevation of LDH and AST/ALT (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), all of which are compatible with the development of HELLP syndrome.</p><p id="par0050" class="elsevierStylePara elsevierViewall">After consulting with ophthalmology, a brain CT scan was performed to rule out signs of cortical blindness. Then fundoscopy was performed, which showed bilateral temporo-inferior bullous serous RD with macular involvement (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">S0<span class="elsevierStyleInf">4</span>Mg perfusion was maintained in the recovery room. BP remained normal, but renal function deteriorated, so the patient was given fluid replacement and diuretics.</p><p id="par0060" class="elsevierStylePara elsevierViewall">At 72 h postpartum, both renal function and analytical parameters had improved (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The patient was transferred to the ward after 5 days in the recovery room, stable, with VA of 0.4/1 in both eyes.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Fifteen days after the SRD, VA was 0.7/1 in both eyes, with partial resolution of the SRD.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Four weeks later, the ophthalmology follow-up showed full recovery of vision with bilateral 1/1 VA and no sequelae.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">Preeclampsia is a hypertensive disorder of pregnancy that presents with hypertension (≥140/90 mmHg) and proteinuria (≥ 0.3 g/24 h) after the 20th week of pregnancy and continues through to the postpartum period<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,3</span></a>. Between 10% and 15% of patients with preeclampsias develop HELLP syndrome (thrombocytopaenia, haemolysis and elevated liver enzymes), generally in the 3rd trimester or postpartum<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>. HELLP syndrome is a variant of severe preeclampsia that does not always meet the conventional diagnostic criteria of hypertension or proteinuria.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Various visual disturbances have been associated with preeclampsia, including: blurred vision, photopsia, retinal oedema, retinal or vitreous haemorrhage, Purtscher-like retinopathy, central retinal vein occlusion, SRD or cortical blindness<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,9</span></a>. Blurred vision and loss of VA are the most common symptoms found. The presence of SDR is a severe form of preeclampsia<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Acute vision loss due to SDR is a rare complication that occurs in 1% of women with severe preeclampsias<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–9</span></a> and in 10% of women with eclampsia<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,6,9</span></a>. The likelihood of SDR in preeclampsia can increase 7-fold when it is associated with HELLP syndrome, which increases the risk of SDR in pregnant women with preeclampsia<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Due to the teratogenic effects of fluorescein on foetal development, few studies have attempted to explain the pathophysiology of SRD in preeclampsia. However, choroidal ischaemia due to arteriolar vasoconstriction has been suggested as the main pathophysiological mechanism. Ischemia involving the retinal pigment epithelium causes degradation of the retinal barrier and formation of a serous exudate from the choroid, through the RPE, into the subretinal space, producing serous retinal detachment<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–8</span></a>.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Another pathogenic mechanism described is haemolysis, present in HELLP syndrome, leading to obstruction of the choroidal capillaries and choroidal ischaemia<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,5</span></a>. Other authors<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,8</span></a>, however, have suggested that placental thromboplastin may activate the extrinsic coagulation system, and the resultant disseminated intravascular coagulation causes choroidal ischaemia and ultimately SRD.</p><p id="par0100" class="elsevierStylePara elsevierViewall">In their review of 43 documented cases of retinal detachment, Hussain et al.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>, found that only 1 of the 97 women diagnosed with preeclampsia presented normal BP. Similarly, Singh et al.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> and Tranos et al.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> each reported a case of SRD in pregnant women with HELLP syndrome without hypertensive retinopathy. Our patient, like those described by the foregoing authors, did not develop hypertension or hypertensive retinopathy.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Although most patients with SRD develop signs of hypertensive retinopathy<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>, it is not present in all cases. Other mechanisms, such as concurrent microangiopathic haemolysis or hypercoagulability, have been put forward as mechanisms of SRD in patients with normal BP. These would be responsible for choroidal ischaemia, and would explain why hypertensive retinopathy in itself is not the only determining factor of SRD in preeclampsia, and why SRD is more common in pregnant women with HELLP syndrome.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The true incidence of SRD in preeclampsia and HELLP syndrome is uncertain, and current estimates derive from case reports. It is described more frequently in primiparous women (60%), is usually bilateral (89%), more frequent in the postpartum period (69%), and when a caesarean section was performed (76%)<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>. In general, SRD is associated with a higher number of caesarean sections<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,6</span></a>, although its appearance is not a contraindication for vaginal delivery<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Prepartum SRD has been associated with greater severity, since alterations in the retinal microcirculation correlate with those of the placenta, and are a useful marker of foetal well-being<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>. The presence of SRD or papilledema is an indication for termination of pregnancy to save the patient’s sight<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Our case is unusual because the appearance of SRD in the immediate puerperium preceded other clinical manifestations of an atypical HELLP syndrome that progressed without high BP. In our patient, caesarean section was performed due to foetal distress. The patient did not present high BP in either the pre- nor postpartum period, but she did report loss of VA prior to the onset of thrombocytopaenia and elevated transaminases and LDH.</p><p id="par0125" class="elsevierStylePara elsevierViewall">In the differential diagnosis, thrombotic thrombocytopenic purpura and pregnancy-associated atypical haemolytic uremic syndrome were ruled out in favour of HELLP syndrome due to compatible laboratory abnormalities and improved renal function after delivery<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall">SRD is treated conservatively, without the need for surgery. It generally has a favourable prognosis with full recovery of vision between 2-12 weeks after delivery<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,6</span></a>, Termination of pregnancy will reverse the ocular manifestations and prevent visual sequelae.</p><p id="par0135" class="elsevierStylePara elsevierViewall">The benign course of SRD means that it is underdiagnosed, possible because the symptoms are mild, and the condition resolves after delivery. However, despite the good prognosis, some authors have observed changes in the retinal pigment epithelium, with areas of infarction of the choriocapillaris, or "Elschnig spots". These changes can mimic macular dystrophy or retinal degeneration and, though rare, can cause permanent visual impairment<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>.
