(D) No identifiable glottic structures.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">The use of supraglottic airway devices (SAD) is becoming increasingly common in patients requiring general anesthesia,<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">1</span></a> and these masks are particularly suitable for day surgery.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">2–4</span></a> Second generation SADs, with a gastric tube and more efficient airway seal, are recommended in patients and procedures requiring more advanced features than those available in first generation devices.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The Baska Mask<span class="elsevierStyleSup">®</span> (Logikal Health Products PTY Ltd., Morisset, NSW, Australia) is one of the latest second generation SADs to be released in Spain. It is made of transparent silicone, and is sold as either a single or multiple use mask (that can be reused up to 70 times). The most important feature of this mask is its self-sealing cuff that expands proportionally with increasing airway pressure during positive pressure ventilation. It has two lateral tubes, one on each side of the airway tube, which join in a large distal sump for continuous drainage and suction of laryngeal secretions. A tube can also be inserted through the mask to clear gastric fluid. The airway is elliptical in cross section, and has a reinforced bite block to prevent the tube from collapsing. The mask is available is 4 sizes, each with a different color connector.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The Baska Mask has been evaluated in 3 earlier studies with a limited number of patients.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">7–9</span></a> These studies reported an overall successful insertion rate of between 96% and 100%, and a sealing pressure greater than 35<span class="elsevierStyleHsp" style=""></span>mmHg.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The primary objective of our study was to evaluate the clinical performance of the single-use Baska Mask in terms of ease of insertion, airway seal and ventilation quality, facility of gastric access, anatomical position in relation to the glottis, and intra- and post-operative complications associated with use of the mask.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">The study was approved by the Independent Ethics Committee (2013), and was conducted from July 2013 to February 2014.</p><p id="par0030" class="elsevierStylePara elsevierViewall">We included 80 consecutive patients scheduled for day surgery under general anesthesia. All patients met the study requirements, agreed to take part, and signed an informed consent form. Patients diagnosed with a difficult airway in the preoperative workup, patients with a body mass index<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>40, and those that required prone positioning during surgery were excluded. The Baska Mask was inserted by the same anesthesiologist in all patients (AML), a practitioner with extensive experience in the use and evaluation of SADs. Prior to the study, the anesthesiologist had tested the device in 6 patients under the supervision of the inventor of the device.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The mask size was chosen following the inventor's indications. These are based on the usual criteria of weight and height (<50<span class="elsevierStyleHsp" style=""></span>kg and 150<span class="elsevierStyleHsp" style=""></span>cm: no. 3; 50–70<span class="elsevierStyleHsp" style=""></span>kg and 150–170<span class="elsevierStyleHsp" style=""></span>cm: no. 4; and >70<span class="elsevierStyleHsp" style=""></span>kg and 170<span class="elsevierStyleHsp" style=""></span>cm: no. 5) and also the characteristics of the patient's neck. The inventor recommended using a smaller size than that recommended for each weight and height group if the patient's neck is unusually shorter. Both sides of the cuff were lubricated with a water-soluble jelly.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Once the target depth of anesthesia has been reached (<50 on the bispectral index and/or no motor response to jaw thrust), the Baska Mast was inserted as recommended, namely, compressing the proximal end of the cuff between the thumb and index finger and pushing the tip along the midline of the hard palate. If resistance was encountered, we proceeded as follows: pulling the tab on the anterior side of the tube to increase the curvature of the device, or applying a lateral paramedian rotation and jaw thrust. If resistance persisted, the tip of the device was guided using the contralateral index finger to negotiate the palato-pharyngeal curve. If insertion was still not successful, the case was considered a failed attempt, the Baska Mask was removed and the patient was ventilated via a facemask. We evaluated the number of insertion attempts (no more than 3) and ease of insertion on the following scale: 1, easy, no resistance; 2, resistance, additional maneuvers required; 3, unsuccessful at the third attempt.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Once the device had been inserted and the ventilation system connected, ventilation quality was evaluated on a scale of 4: 1, optimal; 2, slight leak or elevated pressure corrected by position adjustments; 3, persistent leak that does not prevent ventilation; 4, insufficient ventilation. Ventilation was evaluated on the final successful attempt, or on the third and last attempt. If after 3 attempts (including insertion) adequate ventilation was not achieved, the evaluation was suspended and the device was replaced with another SAD, at the discretion of the anesthesiologist.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Airway sealing pressure was measured with a monometer (VMB, Medizintechnik GmbH™, Sulz, Germany) using the standard technique: blocking the expiratory phase and administering a continuous flow of oxygen until balance or the maximum pre-established level of 40<span class="elsevierStyleHsp" style=""></span>cm H<span class="elsevierStyleInf">2</span>O was reached.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Insertion of the gastric tube (lubricated with a water soluble jelly) was evaluated as: 1, easy, no resistance; 2, moderate resistance; 3, impossible. This parameter was not measured in patients in whom ventilation was unsuccessful. Once insertion had been evaluated, the tube was removed and continuous suction was connected to one of the channels.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Finally, alignment with the anatomy of the glottis was evaluated with a flexible, 3.8<span class="elsevierStyleHsp" style=""></span>mm fiberbronchoscope (FBS) (LF-2, Shirakawa, Olympus Co., Japan) on the following scale: 1, complete vocal chords; 2, vocal chords centered and edge of the epiglottis visible through the tube; 3, incomplete vision, or glottis off-center; 4, no identifiable structures.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Complications observed during the procedure (coughing, hiccups, laryngospasm, O<span class="elsevierStyleInf">2</span> saturation <95%), on removal of the mask (blood on the cuff), and in the post-anesthesia care unit (throat pain, odynophagia, dysphonia, itching), were noted.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Insertion of the first 20 devices was compared with the rest of the series to evaluate the learning curve effect. Similarly, intra-group differences in terms of the size number of the device used.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0075" class="elsevierStylePara elsevierViewall">Data are presented as median and standard deviation or number of cases. Qualitative variables were analyzed using the Chi square test or Fisher's test, and Student's <span class="elsevierStyleItalic">t</span> test was used for quantitative variables. Significance was set at <0.05. Statistical analysis was performed with SPSS v. 18 (Chicago, IL, USA).</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0080" class="elsevierStylePara elsevierViewall">The anthropometric data of the 80 study patients are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, grouped according to Baska Mask size used. Insertion and ventilation variables are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. A learning curve effect was observed, showing a lower percentage of easy insertions among the first 20 cases (8/20) than in the remaining cases (41/60) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.02). Mask size 3 was easier to insert than the other two sizes (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002). Mask size 3 required less adjustment to achieve good ventilation compared with sizes 4 and 5 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.007). In 7 patients with a long neck in whom Baska Mask size 3 or 4 were used, the optimal ventilation position was achieved when the proximal connection remained within the dental arch.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Overall, the Baska Mask was successful in 96% of patients. In one case, the mask was replaced with a smaller size, which was successful, and in 2 patients it was replaced with another SAD.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Mean airway sealing pressure was 33<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7<span class="elsevierStyleHsp" style=""></span>cm H<span class="elsevierStyleInf">2</span>O, and was greater with mask size 3 vs sizes 4 and 5 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). In 65% of patients, a pressure of over 30<span class="elsevierStyleHsp" style=""></span>cm H<span class="elsevierStyleInf">2</span>O was noted.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Insertion of the gastric tube was easy in all patients in whom good ventilation was achieved.</p><p id="par0100" class="elsevierStylePara elsevierViewall">FBS exploration was performed in only 66 cases (82%) due to lack of resources. The structures visualized are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> and <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. In 3 cases, we could not identify any glottis structures due to distortion of the edge of the cuff, which appeared to be folded (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>D). In these cases, ventilation was unsuccessful, despite attempts to reposition the mask.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">Only mild intra- and post-operative complications were observed (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">The main findings of this observational study on the performance of the Baska Mask in adult patients have confirmed that this new SAD gives good ventilation and high airway sealing pressure when correctly positioned. Correct positioning frequently calls for repositioning of the mask. The differences found between the 3 available sizes shows that size selection criteria need to correspond more accurately to the patient population. The anthropometric and anatomical recommendations should be further refined, because although we used smaller sizes than those recommended, as shown by the mean weight and height data from each group, there were major differences in performance in each mask size.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Overall, the Baska Mask was inserted in all patients following the manufacturer's recommended technique, with a first-attempt success rate of 88%. Since the cuff of the Baska Mask is non-inflatable, it must be compressed between the fingers and certain maneuvers performed to facilitate insertion. The increases in cuff size would explain why the small size mask was easier to insert than the other two sizes. This is in line with the findings of earlier studies. In one of these, Alexiev et al.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">7,8</span></a> reported a first-time success rate of 73% and 77% in 2 series of 30 and 71 female patients, respectively. In another, Van Zundert and Gatt<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">9</span></a> reported a first-time success rate of 88% in an observational study in 50 patients.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Ventilation was successful in 96% of patients, although in 39% the mask needed to be repositioned to resolve leaks or partial obstruction. The cuff opening of the Baska Mask, designed to improve sealing, is smaller than most in SADs. Therefore, it is essential to align it perfectly with the glottis in order to achieve good ventilation. In 5 cases (three size 3 masks, and two size 4 masks), in which the depth of insertion had to be adjusted, the tube was found to be too short, and the proximal connection remained within the dental arch. These findings, obtained during elective day surgery in patients that did not meet difficult airway criteria, did not in any way affect ventilation. However, the additional maneuvers needed to insert the mask and achieve good ventilation could lead to excessive delays in emergency situations calling for immediate airway rescue.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Mean sealing pressure was 33<span class="elsevierStyleHsp" style=""></span>cm H<span class="elsevierStyleInf">2</span>O, with pressures over 30<span class="elsevierStyleHsp" style=""></span>cm H<span class="elsevierStyleInf">2</span>O, reported in most study patients. This confirms the usefulness of the Baska Mask in situations, such as advanced indications, where higher ventilation pressures than those usually achieved with other devices are needed. In our study, leak pressure was significantly higher in the size 3 mask, while earlier studies reported a sealing pressure as high as 40<span class="elsevierStyleHsp" style=""></span>cm<span class="elsevierStyleHsp" style=""></span>H<span class="elsevierStyleInf">2</span>O in most patients using sizes 4 and 5.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">8,9</span></a> A gradual increase in sealing pressure up to 1<span class="elsevierStyleHsp" style=""></span>h after start of ventilation has been reported.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">7,9</span></a> In our case, pressure was measured immediately after insertion, which could in part explain the differences in these levels. One of the factors that contribute to sealing is correct alignment of the device's cuff opening with the glottis. Using endoscopy, we observed better alignment with size 3 than with the larger sizes, although the difference was not statistically significant. Another important factor is the pressure exerted by the device on the periglottic structures, which is obviously greater when the anteroposterior diameter of the device is larger. Although the gas administered through the non-inflatable cuff of the Baska Mask cannot over-inflate the cuff, the pressure exerted by the mask on the mucosa has not been measured.</p><p id="par0130" class="elsevierStylePara elsevierViewall">The anatomical position of the mask in relation to the glottic structures was correct in 90% of cases in which FBS evaluation was possible. In the 3 patients in whom ventilation was unsuccessful, a fold in the edge of the cuff or distortion of the cuff prevented us from identifying the structures. In 1 case, this was solved by inserting a smaller size mask.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Intraoperative complications were quickly resolved with no hameodynamic or venitlatory risk to the patient. On removal of the Baska Mask, a small amount of blood was observed on the mask in 15% of patients. This was more common in the larger sizes in which insertion was more difficult. Earlier studies reported an incidence of bleeding with the Baska Mask of 8%<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">9</span></a> and 18%.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">8</span></a> Postoperative complications were observed in 14% of patients, although these were minor and quickly resolved. This is consistent with the incidence reported with other SADs<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">10</span></a> and below that reported in inflatable masks when cuff inflation pressure is exceeded.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">11</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Our study has certain limitations. First, being an observational study we were unable to compare the mask with other devices currently available. Although the fact that all masks were inserted by the same anesthesiologist limits interpretation of the results, it prevents bias arising from difference in levels of expertise and learning time to be expected when different anaesthesiologists are involved. Furthermore, as the aim of the study was not to compare the different mask sizes, patients were not divided evenly into study groups and we were unable to calculate the size of the sample in order to detect differences. Finally, evaluation of alignment via FBS was not possible in all patients due to lack of resources in some cases, or workload in others. Despite this, we decided not to remove cases that could not be evaluated in this way, since it is not routine practice in verifying SAD position. Although the data did not allow us to establish size selection criteria for the Baska mask, they do suggest that smaller sizes are easier to insert and adapt better to the surrounding anatomical structures. Nevertheless, the data available so far are somewhat contradictory and prevent us from drawing meaningful conclusions with regard to mask size.</p><p id="par0145" class="elsevierStylePara elsevierViewall">In conclusion, the Baska Mask is a new approach to SAD, with good sealing pressure, effective ventilation in a high percentage of patients, and good evacuation of gastric fluid when needed. Due to its non-inflatable cuff, insertion maneuvers and readjustments are often required.
