The patient was referred to the Emergency Department for dyspnoea, where she was diagnosed with vocal cord paralysis (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) and an emergency tracheostomy was performed. In the operating room, intubation was performed under conscious sedation using fibrobronchoscopy and a number 6 endotracheal tube due to difficult airway. The procedure was uneventful. The surgical procedure was difficult, and the left internal carotid artery was sectioned due to the displacement of anatomical structures. Urgent massive transfusion was needed, the artery was ligated and the tracheostomy was performed. Non-invasive postoperative cerebral oximetry monitoring showed levels of between 60% and 75%, with no abnormalities (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).
