Anaesthesia departments (including the Intensive Care Unit [ICU]) are involved in complex care environments with a multitude of potential risks that can affect or harm hospitalized patients, and that can even be life-threatening. Although anaesthesiology is the only medical specialty that has succeeded in significantly reducing mortality rates, as reported in the 1999 “To err is human” study (possibly the “<span class="elsevierStyleItalic">big bang”</span> of patient safety) published by the Institute of Medicine,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a> the incidence of adverse events is still as high as 29%, according to the 2012 SYREC<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">4</span></a> study performed in 79 Spanish ICUs. This evidence compels us to analyse the risks in ICUs and implement measures to reduce or totally eliminate this burden.</p><p id="par0010" class="elsevierStylePara elsevierViewall">One method of reducing risks is to approach them in a structured way from different perspectives (structural, functional and clinical). This systematic approach focuses on reducing risk and efficiently increasing patient safety levels. An example of this is the procedure designed to obtain UNE 179003:2013 certification, which requires compliance with standards and procedures designed to reduce the risk of incidents that may affect patients during the care process.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The recent review of UNE 179003:2010 led to the publication of the UNE179003 standard <span class="elsevierStyleItalic">Clinical risk management for patient safety</span> in 2013<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">5</span></a> by the Spanish Association for Standardization and Certification. The new standard helps healthcare organisations implement a risk management system that consolidates a safety culture aimed at reducing the number of adverse events and their impact on patients.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The new 2013 standard introduces important changes that emphasise strategic dimension of organisation-wide risk management that includes facilities, equipment, materials and health technologies: identification, assessment (analysis<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>evaluation) and treatment plans. The document emphasizes the need for process monitoring and review, and reinforces aspects such as organisation-wide communication and information. According to the standard, risk management is the key to improving patient safety in the context of a non-punitive safety culture in which mistakes are learning tools, and all groups are committed to best practices and standards.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">7</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">The main aim of this study is to describe the procedure developed to obtain 179003:2013 Standard certification in the Anaesthesia Department of Povisa Hospital. The secondary objectives have been to describe the risks identified and the results of the adverse event reporting programme.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">The implementation of the safety programme for obtaining UNE 179003:2013 Standard certification started in 2007, when patient safety was established as a strategic goal in POVISA Hospital.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The hospital serves a catchment area of 139,000 patients, and is subsidised by the Galician Regional Health Service (SERGAS). It has 14 operating rooms with morning and afternoon schedules, a 23-bed post-anaesthesia care unit (PACU), and a polyvalent ICU run entirely to the 19-bed Anaesthesia Department. Each year, the Department cares for approximately 700 medical, coronary, postoperative, multiple trauma and burn patients with a mean APACHE score over the last 8 years of 16.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.5. The Anaesthesia Department has 31 anaesthesiologists and 2 residents.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The Department sought to gradually introduce a safety culture<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a> as a key element of its strategic plan. For this purpose, it adopted a multisystemic, multifactorial approach that considers adverse events to be system failures caused by latent conditions and not professional negligence. It also created effective prevention and rapid detection systems for investigating the underlying cause of the adverse event.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Basic risk management guidelines were also introduced in order to identify the risk, systematically and rigorously evaluate its frequency and severity, reduce the incidence, and perform long-term assessment of the improvement measures implemented.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The aim was to gradually establish a safety culture by raising awareness, reviewing evidence-backed procedures to promote safer practices, redesigning processes to minimise latent conditions, and finally, deploying quantitative and qualitative methodological tools. In other words, to develop an indicator-based monitoring system to periodically evaluate the implementation of processes and their results, and to implement a system that enables clinicians to learn from their mistakes (the adverse incident reporting and registry system developed by SENSAR,<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">8,9</span></a> to which Povisa hospital has subscribed since 2009).</p><p id="par0055" class="elsevierStylePara elsevierViewall">A Core Safety Unit was created to implement these projects. The Unit is made up of representatives from the hospital management and various medical and nursing departments, including the Anaesthesia Department, in representation of the Surgical Block and the ICU, and the General Surgery, Nursing, Oncology, Internal Medicine, Pharmacy, Emergency, Orthopaedic Surgery and Preventive Medicine departments. A Core Safety Unit made up of 6 anaesthesiologists and the head nurse was created in the Anaesthesia Department, under the direction of the departmental head. The Unit was in charge of mapping out the risks identified in different functional and organisational areas and performing a quantitative and qualitative analysis of these incidents using SENSAR methodology. On the basis of the results of this analysis, the Unit planned and monitored improvement measures and risk reduction programmes, submitted improvement measure to the hospital management when their approval was needed, and took part in patient safety training sessions.</p><p id="par0060" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Working teams:</span> specific working groups formed of anaesthesiologists and nurses were created in each of the units applying for certification (Surgical Block, PACU, ICU). Our approach to clinical risk management included proactive actions (clinicians seek to anticipate and prevent risks to patient safety) and reactive actions (notifying and analysing incidents that have already occurred) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Proactive approach:</span> potential risks were identified after studying mandatory legal regulations, scientific evidence,<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">10,11</span></a> recommendations from accreditation bodies<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">12</span></a> and recommendations from in-hospital improvement groups, and after a series of brainstorming sessions involving the working teams described above. Once the risks were identified, each one was weighted using a risk matrix, and intermediate, high or extreme risks were prioritised (although all risks were included). The risks thus identified were analysed using the Failure Mode and Effect Analysis (FMEA) method,<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">13</span></a> which is a qualitative technique in which a list of potential failings is systematically linked to their consequences. The method can be used to analyse design changes or modifications in a particular process. FMEA evaluates the severity (<span class="elsevierStyleItalic">S</span>) of the effects of the failure on the patient; the probability (<span class="elsevierStyleItalic">P</span>) or likelihood of the failure occurring, and the detectability (<span class="elsevierStyleItalic">D</span>), or the ease or difficulty of detecting the failure. These 3 factors make up the risk priority number (RPN<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleItalic">D</span><span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleItalic">S</span>). The greater the RPN, the more important the risk. We used the risk matrix recommended by the UNE 179003 standard. Risks were weighted according to the opinion of the hospital staff and experts in the field of clinical safety, always taking into account the above-mentioned sources.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The Quality Unit developed a risk management computer programme that defines the risk, the causes, the effects, the creation and implementation over time of improvement measures, and the specific indicators used, as shown in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Reactive approach:</span> used after the event has occurred. Risks were reported and analysed, and corrective measures were implemented using the systems specifically recommended in the Helsinki Declaration on Patient Safety in Anaesthesiology (mainly SENSAR).<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">14</span></a> The root cause analysis (RCA)<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">15</span></a> model provided in the SENSAR database was used to analyse reported events.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistics</span><p id="par0080" class="elsevierStylePara elsevierViewall">Quantitative variables, which were normally distributed according to the Kolmogorov–Smirnov test, are shown as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD. Qualitative variables are shown as frequency followed by percentage in parentheses. The chi-square test with Yates correction or Fisher's exact test, as appropriate, were used to compare proportions between qualitative variables. A value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 was considered significant.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The statistical analysis was performed using SPSS v.15 for Windows (SPSS Inc., Chicago, IL, USA). or R v.3.0.1 (R Development Core Team, 2013, Vienna, Austria).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0090" class="elsevierStylePara elsevierViewall">The Anaesthesia Department, the ICU and the PACU obtained UNE 179003-2010 certification on 5 July 2012. They were recertified according to version 197003:2013 on July 5, 2015, and again on 7 July 2018. Periodic evaluations have been carried out each year since certification was first obtained. The units certified were Anaesthesia, Surgical Block and sterilisation area, the PACU and the ICU (under the responsibility of the Anaesthesia Department). The entire hospitalisation was also certified, although this is beyond the scope of this study.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Since certification, 101,032 patients have been treated in the Surgical Block and 5955 patients with a mean APACHE II score of 16.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.5 and a mean age of 65.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>14.7 have been admitted to the ICU for a mean stay of 4.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.2 days. Patient were distributed as follows: medical 38.7%, surgical 38.3%, coronary 16.8%, trauma 4.09% and burn 2%.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Proactive action identified 26 specific anaesthesia- or PACU-related risks, 21 in the ICU and 19 in the Surgical Block (total of 66 potential risks) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">In each case, as shown in <a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>, the risk, its causes, its effects, the risk mitigation plan, and the corresponding indicators used for ongoing evaluation of the risk reduction process were identified. In each case, a person was appointed to implement and follow up the improvement measure using the corresponding indicators.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">The RPN of all risks included in the risk map was calculated, and the mean RPN was seen to decrease each year following implementation of the corresponding improvement measures. For example, in the Surgical Block the initial mean RPN of 124.5 decreased to 67.23 this year. In the ICU, the RPN has gone from 84.4 to 42.2 (in 2018).</p><p id="par0115" class="elsevierStylePara elsevierViewall">In 2018, a total of 1114 events were reported and submitted to the UNE evaluation team. All (100%) were analysed, and 1096 (98.4%) were resolved with all the corresponding measures implemented. The mean time (days) between reporting and analysis was 5.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>16.8, and the mean time in days from reporting to complete resolution was 84.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>64.2.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Each event was analysed by a member of the department's Core Safety Unit and presented at the Unit's weekly meeting. In some cases, the corrective measures established required the event to be presented at the monthly safety meeting held by Anaesthesia Department personnel (the Department's clinical patient safety meeting is held on the first Friday of each month).</p><p id="par0125" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the distribution of events by type and setting, and by severity (harm caused to the patient).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> list the measures implemented following analysis of the event.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0135" class="elsevierStylePara elsevierViewall">Each quarter, a report is sent to all Anaesthesia personnel listing the events reported and the measures implemented (the most urgent ones are dealt with immediately by alerting the anaesthesiologists involved on their mobile phone).</p><p id="par0140" class="elsevierStylePara elsevierViewall">All the members of all units have access to the reports of the events analysed, and are informed monthly in a bulletin describing the outcomes of the events reported and of the risk-reduction measures implemented.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0145" class="elsevierStylePara elsevierViewall">We present the procedure followed to obtain our first UNE 179003:2010 standard certification for the ICU, Surgical Block and PACU, and to renew our certification according to the 179003:2013 standard.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Applying the methodology for obtaining our first UNE certification, which coincided with our programme for obtaining Joint Commission standard certification, has enabled us to focus our efforts on preventing health risks to patients and also learn from our mistakes and prevent errors from re-occurring.</p><p id="par0155" class="elsevierStylePara elsevierViewall">The involvement of the hospital's management board is essential in any risk management programme. In our hospital, patient safety was included among our 2007 strategic goals, and led to the progressive deployment of an organisational safety culture as one of the cornerstones of our strategic plan. In our case, the involvement of the manager of the hospital's nursing department (who is also in charge of quality assurance) in implementing the safety policy ensured the availability of the material resources required, and facilitated staff training.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Although we applied FMEA<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">13</span></a> methodology, an approach that was also used in the subsequent certification of other hospital units,<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a> other simpler risk assessment methods can also be chosen, depending on the targets set in each case.</p><p id="par0165" class="elsevierStylePara elsevierViewall">Periodic reports on patient safety and the outcomes of corrective measures should be created using appropriate computer software.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">16,17</span></a> In our case, this has allowed us to periodically monitor both the status of the measures introduced and the results obtained using the indicators developed to monitor the practical implementation of the measures and their impact on the risk in question.</p><p id="par0170" class="elsevierStylePara elsevierViewall">A reliable measure of the effectiveness of the actions implemented has been the progressive decrease in the RPN of the risks studied, as shown in the results section. This shows the effectiveness of safety improvement measures implemented in all accredited units.</p><p id="par0175" class="elsevierStylePara elsevierViewall">The implementation of these measure was made possible mainly by staff training courses, which allowed them to adapt their routine clinical practice by using, for example, checklists, or by improving team work and communication.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">18</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">The analysis of adverse events has shown that poor communication lies behind many of the events reported, specifically 12.5% were due to communication errors, compared to 8% published by the SENSAR group.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a></p><p id="par0185" class="elsevierStylePara elsevierViewall">Clinical errors are the most common events in our series, reaching 42.1% compared to 26% published by SENSAR, followed by medication errors (18% vs. 24% in SENSAR) and communication errors.</p><p id="par0190" class="elsevierStylePara elsevierViewall">Approximately 6% of adverse events resulted in greater morbidity or caused the death of the patient, similar to the 4.28% described in the SYREC study.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">4</span></a></p><p id="par0195" class="elsevierStylePara elsevierViewall">With regard to significant differences between types of event, it is interesting to note that, as shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, events with the greatest impact for the patient are more frequent in the ICU, due to the underlying severity of patients admitted to that unit, while equipment-related events are more frequent in the Surgical Block, owing to the number of instruments needed to perform surgical interventions.</p><p id="par0200" class="elsevierStylePara elsevierViewall">The entire process must be dynamic, and the risks, corrective measures, and indicators established for the entire risk map must be continually re-evaluated. The head of each department and unit must encourage their staff to comply with the measures implemented in order to standardise clinical practice. This will facilitate inter- and intra-departmental communication, event analysis, and the introduction of measures to eliminate, or at least mitigate the consequences of the risk identified.</p><p id="par0205" class="elsevierStylePara elsevierViewall">It is essential to follow up results using a system of indicators.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">10</span></a></p><p id="par0210" class="elsevierStylePara elsevierViewall">Our hospital has a computerised risk management system (<a class="elsevierStyleCrossRefs" href="#fig0010">Figs. 2 and 3</a>), which ensures that results are always available for evaluation.</p><p id="par0215" class="elsevierStylePara elsevierViewall">For the past 8 years, our hospital has topped the list in the number of events reported on the SENSAR database. This is evidence that our staff has fully embraced the new safety culture and are directly involved in reducing clinical risks associated with patient care. The programme has, in turn, benefited our staff, insofar as medical professionals have been described as the “second victims” of care-related adverse events, and reducing the incidence of such errors reduces work-related stress.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">19</span></a></p><p id="par0220" class="elsevierStylePara elsevierViewall">UNE certification is valid for 3 years, with annual follow-up audits. After this period, recertification audit is required.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Limitations</span><p id="par0225" class="elsevierStylePara elsevierViewall">One of the most important limitations of our study is its single-centre design. However, we used a similar methodology to that used in similar Spanish studies,<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a> which enabled us to use the same approach when applying for UNE 179003 for other units attached to the Anaesthesiology and Intensive Care Department. However, factors inherent to the methodology used may interfere in the results; for example, estimates suggest that fewer than 10% of the number of real events that occur are reported, and this depends directly on the degree of staff involvement.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0230" class="elsevierStylePara elsevierViewall">This approach to risk management has allowed us to improve patient safety in our department by reducing the number of care-related risks. This was based on a risk management system, focusing on pro-active analysis using failure modes and effects analysis (FMEA) with the description of causes, consequences, risk weighting, and specific risk-minimising measures. A description is also presented of the analysis of reported adverse events (reactive analysis) in the Safety in Anesthesia and Resuscitation (SENSAR) notification system and the measures implemented over an eight-year period.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The UNE 179003:2010 certification was obtained in July 2012, and the re-certification was achieved in July 2015. A total of 66 potential risks were established, which were weighted using a risk probability index (RPI), and measures were implemented that reduced this RPI by half.
