In fact, estimates suggest that if the current trend continues, the population of individuals aged 64 years and over will reach 11 million people (3 million more than at present) in 15 years. This figure will increase to 14 million in 50 years.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In this context, we believe it is important to study the epidemiology of TBI in the elderly, a population that, although biologically more frail, is increasingly active.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">8</span></a> Indeed, it is reasonable to assume that understanding TBI in this age group will help establish prevention and treatment strategies for these patients.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The aim of this study has been to describe the changes in the epidemiology of TBI in a cohort of patients over 65 years of age admitted to the intensive care unit (ICU) of a tertiary hospital over a period of 25 years.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and method</span><p id="par0025" class="elsevierStylePara elsevierViewall">This was a retrospective cohort study of patients aged over 65 years admitted to the general ICU of a tertiary university hospital with a diagnosis of TBI. Patients were included regardless of the presence or absence of other concomitant diagnoses. Patients presenting an episode of cardiac arrest prior to their arrival in the intensive care unit were excluded. The study was conducted from 1 January 1991 to 31 December 2015. Data were collected from the patient's clinical history, both in paper and electronic format.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Our hospital is a reference centre for traumatic injury, particularly in patients who require admission to an ICU, and serves a region of approximately 600,000 inhabitants.</p><p id="par0035" class="elsevierStylePara elsevierViewall">We collected demographic variables (age, sex, date of admission, date of discharge from the ICU, from the hospital, and exitus, if applicable, reason for admission, and mechanism of injury), clinical variables at the time of admission (heart rate, blood pressure, respiratory rate, and Glasgow coma scale), laboratory variables at the time of admission (haemoglobin, leukocytes, platelets, blood sugar, creatinine, urea, prothrombin activity and blood gases), and comorbidities. We also recorded the Acute Physiology and Chronic Health Evaluation II scale <span class="elsevierStyleSmallCaps">(</span>APACHE II) score, and calculated the Injury Severity Score (ISS). We then grouped patients by 5-year periods (1991–1995, 1996–2000, 2001–2005, 2006–2010 and 2011–2015) to facilitate a comparative analysis of the variables collected.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">Continuous variables are described as mean and standard deviation and categorical variables as percentages. Comparison of means between periods was performed by analysis of variance (ANOVA), applying a post hoc Turkey test in the case of statistical significance on ANOVA. The <span class="elsevierStyleItalic">χ</span><span class="elsevierStyleSup">2</span> test was used to compare proportions. A linear trend <span class="elsevierStyleItalic">p</span> value was calculated to evaluate the time trend of the variables analysed over the study period. All analyzes were performed using the statistical package Stata 14/SE (StataCorp, College Station, TX, USA).</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">TBI was diagnosed in 2712 admissions to our ICU over the study period. Of these, 446 (16% of the total) occurred in people aged over 65 years.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The mean age at admission was 74.82<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.52 years, with a range of between 65 and 95 years. Over half (60.54%) were men (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>270) and 39.46% were women (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>176).</p><p id="par0055" class="elsevierStylePara elsevierViewall">The comorbidities, the mechanism of injury, the reason for admission, and the injuries associated with the TBI are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">The clinical and analytical parameters collected at the time of admission and the severity scale scores are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">The lesions caused by the TBI are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Comparison between 5-year periods</span><p id="par0070" class="elsevierStylePara elsevierViewall">The comparative analysis of the periods 1991–1995, 1996–2000, 2001–2005, 2006–2010 and 2011–2015 (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>) showed a trend towards ageing among the patients admitted, but this was not significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.235). It is interesting to note the increase in the number of patients presenting comorbidities, specifically, hypertension, heart disease, cancer, diabetes mellitus, coagulopathy (including patients on anticoagulants and/or antiplatelet treatment) and disability (physical or mental) (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). Significant differences were between periods were found in the 2 severity scales used (APACHE II and ISS).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Regarding TBI-induced lesions, in the latter years of the study we observed a progressive increase in the appearance of traumatic subarachnoid haemorrhage, together with cerebral oedema and subdural haematoma; the incidence of subdural haematoma, for example, doubled in the last 5 years analysed (2010–2015 period). We also observed an increase in the proportion of patients with epidural haematoma (1.39% in the 1990–1995 period vs. 9.46% in 2010–2015), with a significant linear trend (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0187).</p><p id="par0080" class="elsevierStylePara elsevierViewall">Regarding the mechanism of injury, falls from the same height have become more common in recent years, increasing from 8.33% in the 1991–1995 period to over 70% in the 2011–2015 period. In contrast, the percentage of traffic accidents (with the patient occupying a vehicle) decreased from 26.39% to 3.95% in the last period (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">We also observed a significant decrease in the association of TBI with skeletal trauma, but an increase in TBI associated with spinal trauma, both statistically significant.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0090" class="elsevierStylePara elsevierViewall">Our results show that the epidemiology of TBI in the over 65<span class="elsevierStyleHsp" style=""></span>s has changed in the last 25 years, with this type of injury occurring in older individuals, with more comorbidities.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Although no age cut-off point has been established for studies in the elderly population, most authors include individuals aged 65 or older<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">9</span></a>; therefore, this age was chosen as a cut-off point for the inclusion of patients in our study.</p><p id="par0100" class="elsevierStylePara elsevierViewall">The annual incidence of TBI in Europe is approximately 260 cases per 100,000 inhabitants.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">2</span></a> In Spain, the study by Gómez et al. on epidemiological trends in TBI from 1987 to 2012 showed changes in the epidemiology, with an increase in the average age and in the incidence of falls among elderly patients (>65 years).<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">10</span></a> In the Autonomous Region of Cantabria, important studies were performed in the 80<span class="elsevierStyleHsp" style=""></span>s by Vázquez-Barquero et al. This group found an incidence of TBI (in the entire age range of the population) of 91 cases per 100,000 inhabitants, 60% due to traffic accidents and 24% to accidental falls.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">11,12</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">One explanation for these changes is that improvements in health care has not only increased life expectancy, and hence the percentage of individuals aged over 65, but has also improved their quality of life and level of physical activity. The latter has been associated with an increased risk of injury in this population.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">13</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">This is why interest in the study of the epidemiology of severe injury and trends over time has increased in recent years, particularly in developed countries.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">14</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">Our results show a significant change in the epidemiology of the mechanism of injury over the study period, insofar as the most common cause of injury is now falls from the same height, with a significant decrease in the percentage of injuries caused by traffic accidents. These findings are in line with those of several studies performed in developed countries.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">6,15,16</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">It is reasonable to assume that the decrease in the number of admissions due to traffic accidents is due to road safety measures introduced in Spain in recent decades, such as the mandatory use of helmets, campaigns to educate and raise awareness of the dangers of drinking and driving, safety measures implemented on our roads and in the vehicles available on the market. However, proving or disproving these hypotheses is beyond the scope of this study.</p><p id="par0125" class="elsevierStylePara elsevierViewall">An interesting finding in our study is the significant increase in falls from the same height as a mechanism of injury in TBI, which is now the most common cause of this type of injury in the elderly population. Falls in the elderly population have become a public health problem of the first magnitude.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">17</span></a> In Spain, a total of 23,502 fatal falls were recorded in people over 65 years of age between 2000 and 2015 (76.1% of the total deaths due to falls in all age groups).<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">18</span></a> The steady increase in the incidence of falls observed over the study period could reflect the increased level of physical activity in this population group. Moreover, there are important differences between the elderly and the younger population: presence of certain comorbidities, need for medication, such as anticoagulants, and less capacity to adapt to stress situations,<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">19,20</span></a> which have been associated with an increased risk of falling.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Finally, the gradual decrease in the association of TBI with skeletal trauma could be explained by the decrease in traffic accidents and the increase in falls (a mechanism that generally produces a “pure” TBI, i.e., one that is not associated with injury elsewhere in the body).<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">21</span></a> This could also explain the recent increase in the incidence of spinal injury, which has coincided with the increase in the prevalence of TBI caused by falls.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0135" class="elsevierStylePara elsevierViewall">The prevalence of TBI in the over 65<span class="elsevierStyleHsp" style=""></span>s will steadily increase, and will occur in an increasingly elderly population with more comorbidities and thus more difficult to treat. The demographic, clinical variables were collected at the time of admission, including comorbidities, injury mechanisms, and injuries presented, and days of stay in ICU. A statistical analysis was carried out by five-year periods.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">A total of 446 TBI in patients over 65 years were included.
