Nonetheless, the amount of fluids administered to patients during surgery varies widely.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> During surgery, clinician are required to administer fluids as maintenance fluid therapy, to replace blood loss, or to improve tissue perfusion.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Both excessively liberal and restrictive fluid therapy have been associated with poor postoperative outcomes, including acute kidney injury, anastomotic dehiscence, or paralytic ileus.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Given the complexity of determining the patient's volume status, goal-directed fluid therapy was introduced to guide the administration of fluids and drugs based on monitoring their hemodynamic impact on tissue perfusion.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> The fluid challenge (FC) method was initially used, a technique in which clinicians administer fluids and then test the preload reserve of the patient. The FC technique, therefore, balances the benefit of increasing tissue oxygen delivery against the risk of oedema.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> Although the FC is a cornerstone of haemodynamic management, it is underutilized and often incorrectly used,<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,6</span></a> and new techniques have been developed to monitor and predict fluid responsiveness, such as dynamic preload dependence indicators. However, some authors point out that the use of static markers of preload, such as central venous pressure (CVP), are inadequate.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The literature is silent on the indications for FC and how it is performed in patients undergoing surgery. We performed a sub-study of the multicentre observational Fluid Day study<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> to assess the indications, current practice, and opinions on the benefit of FC in the perioperative setting, based on the hypothesis that the use of intraoperative FC varies greatly and is poorly understood.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">This was a sub-study of the Fluid Day study, an observational study conducted by the Haemostasis, Transfusion Medicine and Fluid Therapy Section of the Spanish Society of Anaesthesia and Critical Care. The study was approved by the Spanish Agency of Medicines (SED-HEA-2018-01) and the ethics committee of the University Hospital of de Bellvitge (Barcelona, Spain) on 15 March 2018 (HTF-FLU-2018-01) and prospectively registered on ClinicalTrials.Gov (<a href="ctgov:NCT03630744">NCT03630744</a>). Internal Review Board approval was obtained in each centre, and the local investigators were responsible for ensuring that local requirements were met.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Briefly, the Fluid Day was a 48-h prospective, observational, cross-sectional, prospective study primarily aimed at evaluating how anaesthesiologists administer fluid therapy in surgical patients. Patients over 18 years of age undergoing elective or emergent surgery were included. Patients undergoing out-of-operating-room procedures, ophthalmologic surgery, and surgery performed under local anaesthesia were excluded. The study was conducted in 131 Spanish hospitals. The hospitals were asked to register the surgeries performed between 18 and 19 February 2019. The on line case report form was closed on 5 May.</p><p id="par0030" class="elsevierStylePara elsevierViewall">We included patients who received at least 1 FC, defined as administration of any bolus of fluid (crystalloid or colloid) in a short time to evaluate and/or treat a specific hemodynamic impairment - not specified in Fluid Day. Administration of red blood cells or fresh frozen plasma was not considered a FC. Although patients were followed up for 24<span class="elsevierStyleHsp" style=""></span>h after surgery in the Fluid Day study, only FCs performed in the operating room were included in this sub-study.</p><p id="par0035" class="elsevierStylePara elsevierViewall">We collected prospective data on patient demographics, indications for FC, the type, amount, and rate of fluids administered, the haemodynamic variable or clinical situation that triggered the FC, and the haemodynamic variables used to assess response to FC. Data were anonymized using a secure web application (REDCap (Research Electronic Data Capture version 8.11.9.) and securely hosted in the REDCap systems of the Bellvitge Biomedical Research Institute - IDIBELL. The case report form is available on the study website (<a href="https://www.fluidday.com/utilidades.html">https://www.fluidday.com/utilidades.html</a>). Data management complied with the EU directive on data protection (2016/679)</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study aims</span><p id="par0040" class="elsevierStylePara elsevierViewall">The main objective of this study was determine the variables used by anaesthesiologists to trigger/indicate FC in the operating room. The secondary aims were to evaluate the type and volume of fluid administered during an FC and to compare the proportion of patients receiving further fluid administration after FC at the discretion of the bedside physician.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">The variables were analysed according to type and presented in tables and figures to evaluate the type, volume, and ratio of fluids used during FC. Categorical variables were described as the number of cases and the percentage of the total per category. Continuous variables that follow a normal distribution were described as the number of cases, the mean, and the standard deviation. Continuous variables that did not follow a normal distribution were described as the number of cases and the median (Q1; Q3). The type of FC (crystalloid/colloid) was analysed by logistic regression. The volume was analysed by linear regression. The conditions of application of the model were tested.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The Risk Stratification Before Elective Surgery classification (<a href="https://www.uclahealth.org/anes/risk-stratification">https://www.uclahealth.org/anes/risk-stratification</a>) was used in the Fluid Day study to combine risk scores according to patient comorbidity and complexity of the surgery, stratifying patients into low and high risk and procedures into low, intermediate, high and very high-risk surgeries. The following groups were formed: low-intermediate surgery-low risk patient, low-intermediate surgery-high risk patient, high-very high-risk surgery-low risk patient, high-very high surgery- high-risk patient. However, for exploratory purposes in this sub-study, we divided the patients who received an intraoperative FC into 2 groups: a low-moderate risk group, which included the low-intermediate surgery-low risk patient, low-intermediate surgery-high risk patient; and a high-very high-risk group, which included the high-very high-risk surgery-low risk patient, high-very high surgery- high-risk patient groups.</p><p id="par0055" class="elsevierStylePara elsevierViewall">A <span class="elsevierStyleItalic">p</span> value of less than 0.05 was considered statistically significant.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">The Fluid Day study included 6314 patients in 131 hospitals. Of these, 396 patients received at least 1 FC during the intraoperative period (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Data on demographics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Most patients were ASA II (51.5%) undergoing elective, low-intermediate risk surgery (38.1%), mainly general surgery. The amount of fluid (crystalloids and colloids) used for FC was 250<span class="elsevierStyleHsp" style=""></span>ml (200–400<span class="elsevierStyleHsp" style=""></span>ml). Crystalloids were used in 306 (77.3%) patients, with a median of 250<span class="elsevierStyleHsp" style=""></span>ml (200–300<span class="elsevierStyleHsp" style=""></span>ml). Colloids were used in 100 (25.3%) patients, with a median of 250<span class="elsevierStyleHsp" style=""></span>ml (200–500<span class="elsevierStyleHsp" style=""></span>ml). Crystalloids alone were used in 292 (73.7%) patients, colloids alone in 86 (21.7%), and both in 14 (3.54%) patients. In 4 (1%) patients, the type of fluid administered was not specified.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">The main indication for FC was arterial hypotension, defined as a decrease in systolic arterial pressure (SBP) (62.1% of FCs), and/or in mean arterial blood pressure (MAP) (54.5% of FCs). The main static marker of preload used was cardiac output (CO) (7.58% of all FCs), while CVP was the least used static preload parameter (2.27%). Some (7.58%) FCs were indicated for a decrease in urine output, while 17.9% were indicated for bleeding, without using a specific haemodynamic variable. Other variables used to administer FC, such as stroke volume variation (SVV), are listed in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Although FC was performed in 396 cases, response was only recorded in 385 patients. The most frequently used variable to evaluate response to FC was arterial pressure: increase in SBP in 171 (44.4%) patients, and increase in MAP in 171 (44.4%) patients. FC-induced increase in stroke volume (SV) or cardiac output (CO) was evaluated in only 36 (9.35%) and 29 (7.53%) cases, respectively, and the decrease in stroke volume variation (SVV) in 36 (9.35%) patients (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Of the 396 patients, 354 (89.3%) presented a positive FC, and of these, 20.9% received an additional FC. Of the patients with a negative FC, 35.1% received a further FC (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Demographics and surgery performed in patients included in the low-moderate risk surgery group are shown in Table 1 of the Supplementary material. The majority of these patients were ASA II (54.4%) undergoing elective (82.7%) gastrointestinal (34.7%) surgery. Of the 239 patients in this group in whom FC was performed, 178 (74.5%) received crystalloids alone and 50 (20.9%) colloids alone. The volume of fluid (crystalloids and colloids) used for FC was 250<span class="elsevierStyleHsp" style=""></span>ml (200–400). Crystalloids (with a median of 250<span class="elsevierStyleHsp" style=""></span>ml [200–400<span class="elsevierStyleHsp" style=""></span>ml]) were used for FC in 187 (78.2%) of the 239 patients, and in 59 (24.7%) colloids (with a median of 250<span class="elsevierStyleHsp" style=""></span>ml [200–300<span class="elsevierStyleHsp" style=""></span>ml]) alone were used for FC.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The most frequent indication for FC was arterial hypotension (85.8%), while static indicators of preload were rarely used (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). The most frequently used variable to evaluate FC response was increased blood pressure (89.3%). Seven (29.2%) patients that responded positively to FC received an additional FC after haemodynamic assessment (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><p id="par0095" class="elsevierStylePara elsevierViewall">Most patients in the high-very high surgical risk group were ASA II (47.1%) undergoing elective (95.4%) orthopaedic (34.7%) surgery (Table 2 of the Supplementary material). Crystalloids were used in 119 (75.8%) of the 157 patients, with a median of 250<span class="elsevierStyleHsp" style=""></span>ml (200–300<span class="elsevierStyleHsp" style=""></span>ml). Colloids were used in 41 (26.1%) of the 157 patients with FC, with a median of 250<span class="elsevierStyleHsp" style=""></span>ml (238–500<span class="elsevierStyleHsp" style=""></span>ml). The most frequent indications for FC were decreased SBP and decreased MAP, in 121 (77.1%) and 94 (59.9%) patients, respectively. Decrease in CO was the most frequently used static variable in these patients (14.6%), while the most frequently used dynamic variable was SVV (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Haemodynamic response to FC was evaluated on the basis of SBP and MAP in 130 (86.1%), 76 (50.3%), and 65 (43.0%) patients, respectively (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). Of the 157 FCs, 141 (91.6%) presented a positive response. Of these, 38 (27%) received a new FC after the first was deemed positive, while 103 (73%) patients did not receive further fluids after a postive FC (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">The three main findings of this multicentre observational study are, firstly, that FC is underused; second, that there is significant variability in the indication and evaluation of response to FC; finally, response to initial FC did not affect the decision to administer more fluids.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Administration of an FC is not a standardized technique, and varying volumes and types of fluid are used. In our study, the volume of fluid administered for FC varied slightly, although it was less than previously reported by Toscani et al.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> The fluid of choice was crystalloid in patients undergoing low-risk and high-risk surgery. Although FC with colloids can be expected to elicit a greater haemodynamic response than crystalloids,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> recent controversies and misunderstandings surrounding the administration of colloids, particularly hydroxyethyl starches,<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,13</span></a> may have led to crystalloids being the first-choice fluid.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Like other authors of observational studies analysing the haemodynamic alterations and variables leading to FC in critically ill patients,<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a> and in line with a survey of US and European anaesthesiologists,<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> we found hypotension to be the most frequent indication for FC in both low-risk and high-risk surgeries, while CO, stroke volume (SV), and dynamic preload-dependency variables are rarely used, despite being recommended by experts.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">Although use of hypotension might be expected in low-risk patient or patients undergoing low-risk surgery, it was also frequently used in patients undergoing high-risk surgery. Several studies have shown that changes in pulse pressure, SBP or MAP are poor surrogates for CO,<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,19</span></a> even though normalising CO is, hypothetically, the goal of fluid therapy.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> Despite this, changes in SBP and MAP were most frequently used to estimate response to FC in our study, suggesting that for many anaesthesiologists the main purpose of performing FC is to increase blood pressure rather than ventricular outflow. If this is indeed the case, it shows that the use and interpretation of haemodynamic response to FC is misunderstood by many anaesthesiologists (FC is a test that assesses preload-dependency and the patient’s response to fluid therapy), and that the assumption that arterial pressure increases in parallel with SV is not always valid, since the pressure-volume relationship is not easily predictable and also depends on arterial tone.<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">20,21</span></a> Surprisingly, very few anaesthesiologists use dynamic indicators of preload to assess the need for FC, even in high-risk surgery, even though it is easy to avoid their limitations in the operating room, and these variables have long been incorporated into bedside monitors and anaesthesia stations. It is particularly striking to note the scant use of tissue perfusion variables, such as lactate or ScvO<span class="elsevierStyleInf">2</span>. Unlike the FENICE study,<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> CVP was infrequently used in our study, arguably because most study patients were undergoing general surgery, and perioperative advances and improvements, including enhanced recovery after surgery protocols,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> have made central venous catheters all but redundant.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Interestingly, the decision to administer more fluids after assessing the initial FC response seems entirely arbitrary. Even though this was beyond the scope of our observational study, the practice does not appear to be evidence-based or harmful. Notably, fluids were administered in 29% of patients undergoing low-risk surgery and in 46% of patients undergoing high-risk surgery, even if the FC response was negative. This is in line with other studies in critically ill patients,<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> and raises concerns on patient safety, as fluid administration after a negative FC can be harmful. Following the FENICE study published in 2015,<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Perner, Viellard-Baron, and Bakker called on research networks to place fluid resuscitation at the top of their research agenda and to promote collaborative efforts to further fluid therapy research.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> Five years on, in a much more controllable intraoperative setting, clinicians are still administering fluids incorrectly. The fact that so few of the patients included in the Fluid Day study received at least 1 FC suggests that the concept of FC itself was not properly understood, and that it was actually performed in more patients. This is, therefore, a pressing need to educate anaesthesiologists on the variables routinely used in cardiovascular monitoring in patients undergoing surgery.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Our study has some strengths. It is a large prospective observational study investigating FC in patients undergoing surgery, and thus provides a reasonably accurate estimate of current practice with regard to FC. Second, the multicentre design limits cases from individual centres and increases the external validity of our findings. However, several limitations should also be acknowledged. We did not evaluate whether the FC was performed correctly. Although the volume of fluid administered was evaluated, we did not evaluate the rate of administration – a factor<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> that can influence outcomes – or whether patients were repositioned or other therapies started, and we did not determine when post-FC haemodynamic response was evaluated – which should ideally be 1<span class="elsevierStyleHsp" style=""></span>min after FC administration.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a></p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0130" class="elsevierStylePara elsevierViewall">Current practice and assessment of FC in patients undergoing surgery is highly variable and appears to be arbitrary. But inadequate fluid administration may lead to poor postoperative outcomes. Fluid challenges (FCs), in or outside the so-called goal-directed fluid therapy, allows testing the cardiovascular system and the need for further fluid administration. Our primary aim was to evaluate how anesthesiologists conduct FCs in the operating room in terms of type, volume, variables used to trigger a FC and to compare the proportion of patients receiving further fluid administration based on the response to the FC.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">This was a planned substudy of an observational study conducted in 131 centres in Spain in patients undergoing surgery.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">A total of 396 patients were enrolled and analysed in the study. The median [interquartile range] amount of fluid given during a FC was 250<span class="elsevierStyleHsp" style=""></span>ml (200–400). The main indication for FC was a decrease in systolic arterial pressure in 246 cases (62.2%). The second was a decrease in mean arterial pressure (54.4%). Cardiac output was used in 30 patients (7.58%), while stroke volume variation in 29 of 385 cases (7.32%). The response to the initial FC did not have an impact when prescribing further fluid administration.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">The current indication and evaluation of FC in surgical patients is highly variable. La principal indicación de la PV fue el descenso de la presión arterial sistólica en 246 casos (62,2%). La segunda indicación fue el descenso de la presión arterial media (54,4%).
