He was taken to the operating room intubated, under conscious sedation, and with antihypertensive drugs for blood pressure control. Intraoperative TEE revealed a dilated aortic root and ascending aorta, with effacement of the sinotubular junction. An intimal flap was observed protruding from the sinotubular junction up to the level of the left ventricular outflow tract in diastole, causing free aortic regurgitation due to probable lesion of a valve leaflet, corresponding to type A dissection. Acute lung oedema. Right axillary line for selective cerebral perfusion. Ascending aorta replacement and aortic valve resuspension.
