Due to their legal implications, these incidents are rarely reported. We present the case of a 70-year-old man who underwent remnant gastrectomy that required urgent reoperation due to the presence of 3 surgical sponges in the splenic space, one suprahepatic and one in the lower margin of the liver (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Despite the precautions currently taken in all surgeries, this complication still occurs.
