Given its considerable benefit in cardiac and haemodynamic monitoring, it is now a quality standard in cardiovascular surgery. TOE, however, is not limited to monitoring—in more than 7% of patients, according to some series, the technique can reveal new findings not present in the preoperative study, and leads to changes in the surgical plan in up to 43.8% of these cases. TOE is considered a semi-invasive technique that is only absolutely contraindicated in patients with upper gastrointestinal tract lesions<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Intraoperative echocardiography has gradually expanded beyond cardiac surgery to become a fundamental perioperative diagnostic tool in other areas, such as vascular, thoracic, or liver surgery. It is also gaining ground in many other fields, due to both its role in haemodynamic monitoring (evaluation of myocardial contractility, estimation of cardiac output and blood volume status) and its capacity to provide real-time visualization of the heart and surrounding anatomical structures (identifying valvular dysfunctions, intracardiac shunts or the presence of cardiac thrombosis). It is therefore very useful in patients with high cardiovascular risk and in surgeries with a high risk of bleeding or haemodynamic instability<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. The use of echocardiography is becoming increasingly common in out-of-operating room settings, such as the critical care area. Its use as a semi-invasive and real-time diagnostic tool make echocardiography the technique of choice for determining the aetiology of shock and evaluating the patient's response to the treatment administered. No other point-of-care tool has the same diagnostic capability as echocardiography, which can evaluate items such as cardiac function, blood volume, and cardiac response to vasoplegia. It can also rapidly differentiate between the 4 main types of shock (cardiogenic, obstructive, hypovolaemic and distributive) and, in this context, guide fluid therapy, the administration of inotropics, vasopressors and other treatments during the patient’s evolution<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The use of echocardiography to evaluate lung anatomy and function is an emerging field. Although its use in this context has not yet been validated, it can provide data of great interest both during the perioperative period and in critically ill patients. It can visualise the pleural spaces (thus identifying potential occult bleeding), diagnose consolidations, atelectasis or pulmonary oedema, and assess muscle function. It therefore provides information that can help diagnose the aetiology of hypoxaemia and can help optimize mechanical ventilation and alveolar recruitment manoeuvres<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Retroperitoneal tumour surgery, which is associated with a high risk of haemodynamic instability and bleeding, is one example of the many new fields in which intraoperative echocardiography is being introduced. TOE can also be used for intraoperative evaluation of the inferior vena cava, and is therefore very useful for diagnosing potential thrombosis at this level (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Vena caval thrombosis has been associated with different types of retroperitoneal tumours, and some authors have described the use of TOE to guide the surgical removal of these masses. Vena caval thrombosis is also a widely described complication in renal (5%–15% of patients) and adrenal (around 3% of patients) neoplasms. The presence of inferior vena caval thrombosis increases the risk of intraoperative bleeding, incomplete tumour resection, and pulmonary embolism. Echocardiography can provide an intraoperative diagnosis, and can describe the size and location of the thrombus, detect embolic events during surgical manipulation, guide vascular clamping, and confirm total excision of the thrombus, and can therefore change the surgical plan according to the findings described<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">However, although TOE is now widely used during the perioperative period, particularly by anaesthesiologists (70% of TOE examinations in cardiac surgery are performed by anaesthesiologists), training in this technique is still absent from the official anaesthesia curriculum in Spain. Many anaesthesiologists acquire TOE skills through specific short-term rotations, usually lasting less than 2 months, or through European accreditation programmes<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>.</p><p id="par0030" class="elsevierStylePara elsevierViewall">In conclusion, transoesophageal echocardiography is a minimally invasive technique that provides haemodynamic monitoring and real-time diagnosis of possible complications at all stages of surgery. It is now used in a growing number of fields beyond its traditional application in cardiovascular surgery. New uses for TOE are becoming increasingly frequent, both during intraoperative management and in the field of critical care or emergency medicine. This is why training in TOE is currently recommended for all professionals involved in perioperative cardiac surgery and critical patient care, and usually starts during anaesthesiology residency<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a>. Given the widespread use of this technique in recent years, we believe it is highly likely that the recommendation for TOE training will extend to other specialists involved in non-cardiovascular surgery.
