(B) Measurement of diaphragmatic excursion using a 4<span class="elsevierStyleHsp" style=""></span>MHz convex transducer in the subcostal space.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par1005" class="elsevierStylePara elsevierViewall"><elsevierMultimedia ident="tb1005"></elsevierMultimedia></p><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Postoperative respiratory muscle function is key to determining whether a patient will develop postoperative respiratory failure (PRF) or not.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">1</span></a> PRF occurs when gas exchange does not meet metabolic needs, leading to hypoxaemia with or without hypercapnia. PRF is defined by impaired blood gas exchange occurring after surgery as a result of anaesthesia- and surgery-induced changes.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">2</span></a> It has an incidence of between 0.2% and 3.4%,<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">2</span></a> and occurs more frequently in patients at increased risk of PRF,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">3</span></a> such as those with cardiopulmonary disease, morbid obesity, or the elderly. PRF only occurs in these patients when they cannot generate sufficient inspiratory flow and gas exchange does not meet their metabolic needs.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Interscalene brachial plexus block (IBPB) causes iatrogenic phrenic nerve block and ipsilateral hemidiaphragm paralysis, reducing inspiratory flow by approximately 30%.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">4</span></a> This reduction does not produce PRF when the minute volume generated by the contralateral hemidiaphragm and accessory muscles is sufficient to compensate for demand.</p><p id="par0015" class="elsevierStylePara elsevierViewall">We used point of care diaphragmatic ultrasound (D-POCUS) as a diagnostic tool to evaluate the status and changes of the ventilatory pump during the perioperative period. We focussed on the contralateral hemidiaphragm as an innovative predictive marker of risk for PRF in the event of hemidiaphragmatic paralysis.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">We used perioperative D-POCUS in patients scheduled for shoulder surgery under general anaesthesia with IBPB for perioperative analgesia. Study patients were selected at the General University Hospital of Valencia (Spain). Written informed consent was obtained from all patients. We also obtained permission to publish this article from the Research Ethics Committee of the General University Hospital of Valencia.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The procedure was performed in the same way in all patients. First, ultrasound-guided IBPB was performed by administering 20<span class="elsevierStyleHsp" style=""></span>ml of 0.5% levo-bupivacaine. After successful blockade, anaesthesia was induced with propofol (2<span class="elsevierStyleHsp" style=""></span>mg/kg<span class="elsevierStyleSup">−1</span>), fentanyl (2<span class="elsevierStyleHsp" style=""></span>μg/kg<span class="elsevierStyleSup">−1</span>) and rocuronium (0.6<span class="elsevierStyleHsp" style=""></span>mg/kg<span class="elsevierStyleSup">−1</span>), and maintained with sevoflurane (1 MAC). After surgery, dexketoprofen (50<span class="elsevierStyleHsp" style=""></span>mg) and sugammadex (2<span class="elsevierStyleHsp" style=""></span>mg/kg<span class="elsevierStyleSup">−1</span>) were administered at the time of emergence to reverse neuromuscular blockade.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Ultrasound evaluation of both hemidiaphragms (ipsilateral and contralateral to the locoregional brachial plexus block) was performed twice in all cases, before surgery (performed before IBPB) and in the PACU after surgery (20<span class="elsevierStyleHsp" style=""></span>min after emergence from anaesthesia). The variables studied were diaphragmatic shortening fraction (SF) and excursion (DE) in the semi-recumbent position (30 degree elevation) and the existence of signs of PRF (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">Perioperative D-POCUS includes SF and DE measurements (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The technique and clinical indications for ultrasound evaluation of the diaphragm were recently summarised in a review article.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">5</span></a></p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">Signs of PRF were low oxygen saturation (SaO<span class="elsevierStyleInf">2</span> <90% or SaO<span class="elsevierStyleInf">2</span>/FiO<span class="elsevierStyleInf">2</span> <300), respiratory rate increase > 30<span class="elsevierStyleHsp" style=""></span>rpm, dyspnoea, need for non-invasive mechanical ventilation or reintubation during the postoperative period.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Data were analysed with SPSS® v. 24, performing the Wilcoxon signed rank test for paired data from both hemidiaphragms recorded both before and after surgery.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">Six patients were included in this case series. Patients characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. All patients were high-risk for PRF due to cardiopulmonary disease, morbid obesity or advanced age.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">2</span></a> The perioperative ultrasound evaluation of the diaphragm (with SF and DE values from both hemidiaphragms) is shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. All 6 patients studied showed a significant reduction in SF and DE of the ipsilateral hemidiaphragm: SF<span class="elsevierStyleInf">pre-post</span> 38%<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>18 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.031) and DE<span class="elsevierStyleInf">pre-post</span> 40.1<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>18<span class="elsevierStyleHsp" style=""></span>mm (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.031). However, despite their high-risk status, only 16% (1/6) showed signs of PRF. When we studied changes in the contralateral hemidiaphragm during the postoperative period, we observed that no significant changes in the SF or DE of the contralateral hemidiaphragm occurred in the 5 patients that did not exhibit signs of PRF: SF<span class="elsevierStyleInf">pre-post</span> 0.06%<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>33 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.997) and DE<span class="elsevierStyleInf">pre-post</span> 8.64<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.4<span class="elsevierStyleHsp" style=""></span>mm (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.378).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">Signs of PRF were only observed in the patient with reduced contralateral hemidiaphragm function. SF<span class="elsevierStyleInf">pre</span> vs. SF<span class="elsevierStyleInf">post</span> 95.2% vs. 28.12% (SF<span class="elsevierStyleInf">pre-post</span> showed a 70% reduction), and DE<span class="elsevierStyleInf">pre</span> vs. DE<span class="elsevierStyleInf">post</span> 16.4<span class="elsevierStyleHsp" style=""></span>mm vs. 14.7<span class="elsevierStyleHsp" style=""></span>mm (DE<span class="elsevierStyleInf">pre-post</span> showed a 12% reduction).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0060" class="elsevierStylePara elsevierViewall">The use of perioperative diaphragmatic ultrasound to detect paralysis of the hemidiaphragm after IBPB is not a new approach.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">6</span></a> However, we measured the function of the contralateral hemidiaphragm in order to predict the risk of PRF, and this is a novel technique. Classic IBPB causes a high rate of acute ipsilateral hemidiaphragmatic paralysis,<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">4</span></a> and in general this technique should be avoided in patients at high risk of respiratory failure (usually defined as patients with cardiorespiratory disease, severe COPD, morbid obesity, advanced age, etc.).</p><p id="par0065" class="elsevierStylePara elsevierViewall">In this case series we observed that despite acute hemidiaphragmatic paralysis, function of the contralateral hemidiaphragm was sufficiently preserved to avoid signs of PRF, even in these high-risk patients. Similarly, the only patient in the case series who showed signs of PRF also showed reduced contralateral hemidiaphragm function associated with ipsilateral hemidiaphragm paralysis postoperatively.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The preoperative D-POCUS of this patient showed predominantly right hemidiaphragm paralysis (absolute values of SF and DE twice those of the left hemidiaphragm). The postoperative D-POCUS showed complete paralysis of the right hemidiaphragm, with reduced contralateral hemidiaphragm values (SF<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>28% and DE<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>14.5<span class="elsevierStyleHsp" style=""></span>mm). The patient also developed signs of PRF with dyspnoea and respiratory rate increase from 20 to 34<span class="elsevierStyleHsp" style=""></span>rpm. Dyspnoea was accompanied by desaturation (from 94% to 89%) in the PACU while breathing ambient air (50% decrease in SaO<span class="elsevierStyleInf">2</span>/FiO<span class="elsevierStyleInf">2</span> from 466 to 247 postoperatively).</p><p id="par0075" class="elsevierStylePara elsevierViewall">The patient's respiratory symptoms improved, and he was discharged from the PACU 4 hours later. These symptoms could be explained by the complete ipsilateral blockade of his dominant hemidiaphragm (ipsilateral SF and DE were double the contralateral values), together with a 70% reduction in postoperative contralateral SF (from 95% to 28%). In addition to hemidiaphragmatic paralysis, only pain (VAS<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>7/10) and postoperative changes could explain this reduction in diaphragmatic muscle function, since this patient did not present residual neuromuscular block (TOF ratio<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.91%) and his clinical symptoms also improved with analgesics and an increase in inspired fraction of oxygen.</p><p id="par0080" class="elsevierStylePara elsevierViewall">In this case series we have observed that most patients considered to be at high risk maintain a healthy diaphragm, and despite acute paralysis of one hemidiaphragm the other hemidiaphragm continues to function sufficiently to prevent the appearance of PRF.</p><p id="par0085" class="elsevierStylePara elsevierViewall">This study opens a new field of research into the perioperative function of the hemidiaphragm. We do not know if there are absolute SF and DE values that would allow the function of the contralateral hemidiaphragm to be labelled “normal”, in other words, that would allow us to rule out the appearance of PRF after acute hemidiaphragmatic paralysis. We have been unable to find any articles addressing this issue in the literature; the closest match would be the absolute values of SF and DE needed to predict the success of weaning in critically ill patients on mechanical ventilation.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">7,8</span></a> In these studies, patients with SF <30% or DE <25<span class="elsevierStyleHsp" style=""></span>mm had a high risk of weaning failure leading to respiratory failure.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">7,8</span></a> Coincidentally, the only patient in our series who showed signs of PRF also presented absolute values below the cut-off point established in these weaning studies. Logically, these values are not suitable for our purpose. In the postoperative period, many situations could influence hemidiaphragmatic function, such as postoperative changes, pain, and postoperative residual neuromuscular block, among others.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">2</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">We suspect that after ipsilateral hemidiaphragmatic paralysis, the ability to maintain or increase the function of the contralateral hemidiaphragm postoperatively is probably more important than any absolute preoperative baseline value. Ongoing prospective studies will clarify these issues.</p><p id="par0095" class="elsevierStylePara elsevierViewall">In this high-risk patient series, only patients with acute ipsilateral hemidiaphragm paralysis and reduced contralateral function (compared to baseline) developed PRF. Assessing the function of the contralateral hemidiaphragm using D-POCUS during the perioperative period appears to be the best way to label patients as high-risk for developing postoperative respiratory failure. Obviously, IBPB should be avoided in patients with complete contralateral hemidiaphragmatic paralysis. Nevertheless, our study has shown for the first time that postoperative observation of a reduction in contralateral hemidiaphragm function compared to baseline is probably a warning of impending PRF, and we must be prepared to treat it.</p><p id="par0100" class="elsevierStylePara elsevierViewall">In conclusion, perioperative D-POCUS allows early detection of acute hemidiaphragm paralysis. Both hemidiaphragms were evaluated by ultrasound, before and after the procedure, measuring Thickening Fraction (TF) and Diaphragmatic Excursion (DE).
