In most cases, it is caused by a variable-size terminal deletion of chromosome 22. The SHANK3 gene encodes a postsynaptic scaffold protein. Haploinsufficiency of the SHANK3 gene is the cause of the major neurological features associated with deletion 22q13, although involvement of other proximal genes may also contribute to the development of this syndrome.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">PMS affects men and women in the same proportion, and is characterized by neonatal hypotonia, global developmental delay, normal or accelerated growth, delayed or absent language development, and minor dysmorphic feature (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Though considered a rare disease, its incidence is not known with certainty. PMS is thought to be under-diagnosed, and could be one of the leading causes of idiopathic mental retardation.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">As this syndrome has no specific treatment, patients benefit from continuous multidisciplinary treatment and follow-up, which maximizes developmental outcomes and can diagnose and treat or palliate the complications that develop during growth and adult life.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Recent research has identified 6 mitochondrial genes proximal to the SHANK3 gene in the 22q13 region. Since many individuals with PMS have a deletion that includes at least one of these genes, it is very possible that abnormalities in mitochondrial function could account for some of the symptom variations associated with PMS.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Case report</span><p id="par0030" class="elsevierStylePara elsevierViewall">We present the case of a 12-year-old girl diagnosed with PMS treated with clobazam, melatonin, and aripiprazole. She was born by scheduled caesarean section at 41 weeks of gestation due to severe polyhydramnios in the third trimester. Perinatal history includes: hypotonia and severe neonatal depression, jaundice, and transient digestive intolerance. She was diagnosed with PMS at a few months of life through genetic analysis due to psychomotor retardation. Her clinical manifestations include: psychomotor retardation with severe intellectual deficit and no verbal communication or purposeful hand movements, stereotypic movements in the upper limbs, and epilepsy manifesting as complex partial seizures from the early years of life. She had been seizure-free for the past 7 years, and was not currently taking anticonvulsants. Lack of sphincter control and difficulty swallowing, with risk of choking. She presented unsteady gait, and therefore used a wheelchair, lymphoedema in the lower limbs, limited extension of both elbows, and cavo-varus foot, and was dependent for all activities of daily living. She had been admitted to hospital due to intractable vomiting at the age of 10 years.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The airway examination found SAHS after sleep study and slight tongue protrusion, which did not raise suspicion of difficult intubation.</p><p id="par0040" class="elsevierStylePara elsevierViewall">She had previously undergone 3 surgical interventions - 1 urgent and 2 scheduled, under general anaesthesia using different hypnotics. A different anaesthesiologist was present at each surgery, and chose the anaesthesia induction and maintenance agents according to their criteria and the characteristics of the patient.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The patient underwent the following surgeries:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0050" class="elsevierStylePara elsevierViewall">Adenoidectomy at 4 years of age. Induction and maintenance with propofol. Surgery time: 45 min.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0055" class="elsevierStylePara elsevierViewall">Laparoscopic appendectomy at 7 years of age. Induction with propofol and maintenance with sevoflurane and remifentanil. Surgery time: 90 min.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">-</span><p id="par0060" class="elsevierStylePara elsevierViewall">Exodontia for supernumerary canines at 12 years of age. Induction with propofol and maintenance with desflurane and remifentanil. Surgery time: 35 min.</p></li></ul></p><p id="par0065" class="elsevierStylePara elsevierViewall">Each surgery report noted easy facemask ventilation and orotracheal intubation (Cormack-Lehane I), with no delay in awakening reported. Rescue analgesia was not required in any of the interventions following induction with fentanyl (2 mcg/kg) and conventional intravenous analgesia before eduction. Dexamethasone and ondansetron were administered in all procedures, and no postoperative nausea or vomiting was reported. No postoperative anaesthesia-related complications were reported in any of the interventions.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Discussion</span><p id="par0070" class="elsevierStylePara elsevierViewall">Some authors have suggested that the SHANK3 mutation could be responsible for greater anaesthetic sensitivity and delayed waking after anaesthesia in these patients. The only study in PMS and general anaesthesia was performed in mouse models, and showed that the SHANK 3 mutation was associated with increased sensitivity to anaesthesia with isoflurane.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">There is still insufficient evidence to show that functional loss of the SHANK3 protein heightens sensitivity to the drugs most widely used in general anaesthesia. This question takes on added clinical relevance in the context of concerns associated with neurotoxicity of anaesthetic agents, all of which suggest that reduced anaesthetic exposure is likely to be generally advisable, particularly in patients who may be in vulnerable states associated with extremes of age or neurologic disease, possibly including autism.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">The abnormal behaviour of these patients can make their anaesthetic and perioperative management difficult. Some studies suggest that many patients with autism spectrum disorders (ASD) have altered response to pain and anaesthesia, and decreased perception of pain and delayed awakening have been observed.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">Pain tolerance is a very common feature of patients with deletion 22q13 syndrome.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> However, it is difficult to assess pain in these patients due to their deficits in communication, facial expression, and behavioural expression of pain. Caregivers may interpret this as insensitivity to pain, as occurs in patients with ASD<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>; therefore, it is essential to elicit the parents’ help in interpreting signs of pain and discomfort so that analgesics can be administered as soon as possible.</p><p id="par0090" class="elsevierStylePara elsevierViewall">PMS is associated with certain characteristics that could affect the choice of anaesthesia technique, such as frequent concomitant ASD or, less frequently, mitochondrial diseases.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Perioperative care of patients with ASD should be well-planned and managed by a multidisciplinary team that works with the parents to ensure a safe and stress-free perioperative period. These patients present a complex and incompletely understood pathogenesis, with changes caused by genetic, neurobiological, and possibly even environmental factors, so each patient must be individually evaluated to determine the most appropriate anaesthetic drug.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">The literature on anaesthesia in these patients currently focuses on premedication and non-pharmacological strategies. Premedication, at the discretion of the practitioner, is strongly recommended.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> Non-pharmacological strategies have been developed to help healthcare staff communicate with the patient and their family in order to minimise perioperative anxiety. There is no evidence that any specific intraoperative pharmacological strategy is more beneficial than any other in autistic children or children with ASD.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">Mitochondrial diseases cause a wide range of syndromes produced by defects in mitochondrial DNA or nuclear DNA. Mitochondrial dysfunction most commonly affects function of the central nervous system, the heart and the muscular system.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Six mitochondrial genes proximal to the SHANK3 gene in the 22q13 region (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) may have the potential to cause abnormal mitochondrial function and explain some phenotypic variations in individuals with PMS.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">Both halogenated anaesthetics and propofol depress mitochondrial function and can increase the risk of perioperative problems. All this, together with the fact that these diseases cause energy metabolism deficits, suggests that factors that can cause metabolic overload in these patients should be avoided, such as: prolonged fasting, hypoglycaemia, postoperative nausea and vomiting, hypothermia, prolonged tourniquets, acidosis and hypovolaemia. Surgery, like anaesthesia, is a risk factor for decompensation in these patients, causing cellular metabolism deficits that lead to metabolic acidosis and cell damage.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Choosing the best anaesthetic technique is difficult, since most anaesthetic agents are deleterious at the cellular level, although this does not cause clinically significant adverse events. No association between anaesthesia-related complications and any particular general anaesthesia technique has yet been established.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Conclusions</span><p id="par0120" class="elsevierStylePara elsevierViewall">There is currently no ideal anaesthesia strategy in PMS patients; however, several authors have noted that patients with ASD and mitochondrial disorders tolerate a wide range of anaesthetic agents. Nevertheless, there are no data on general anaesthesia in PMS - a rare disease that manifests with a wide range of signs and symptoms that vary in frequency and severity.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Regarding patients with ASD or mitochondrial disorders, no studies have so far proved the superiority of propofol over halogenated drugs or vice versa. Authors that have investigated this topic recommend choosing the anaesthetic agent on a case by case basis. Familiarity with the metabolism of each drug and its possible adverse effects and complications will allow clinicians to quickly detect and diagnose these complications.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Caution is advised when administering anaesthetics to patients with PMS: they must be closely monitored, anaesthesia should be induced slowly using appropriately titrated agents, and the effects of anaesthesia must have largely disappeared before assuming that the patient can breathe correctly and has recovered airway protective reflexes.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Anaesthesiologists should carefully review the patient’s history and choose an anaesthetic that is least likely to trigger complications in an already compromised patient. It is also important to bear in mind the characteristics of this syndrome and any concomitant diseases the patient may present in order to choose an anaesthesia technique that will minimise the appearance of potential surgery – and anaesthesia –related complications.</p><p id="par0140" class="elsevierStylePara elsevierViewall">These patients present dysmorphic characteristics that can make airway management difficult, so a detailed airway evaluation is essential. It is also important to administer adequate prophylaxis against anaesthesia-induced nausea and vomiting.
