"apellidos" => "Maseda" ] ] "afiliaciones" => array:1 [ 0 => array:2 [ "entidad" => "Servicio de Anestesiología y Reanimación, Hospital Universitario La Paz, Madrid, Spain" "identificador" => "aff0005" ] ] "correspondencia" => array:1 [ 0 => array:3 [ "identificador" => "cor0005" "etiqueta" => "⁎" "correspondencia" => "Corresponding author." ] ] ] ] "titulosAlternativos" => array:1 [ "es" => array:1 [ "titulo" => "Pronóstico y predictores de mortalidad de los pacientes ancianos críticos" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Increased life expectancy has led to a greater number of elderly patients admitted to hospitals and intensive care units (ICU).<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">1–3</span></a> Elderly patients, particularly those with severe comorbidities or resident in long-term care facilities, present a high risk of mortality in the ICU.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">1–6</span></a> Recent studies suggest that ICU care may not improve survival in elderly patients, and may even worsen their quality of life,<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">7,8</span></a> and for many elderly patients, preserving quality of life is more important than prolonging survival.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">9</span></a> The majority of seriously ill hospitalised elderly patients prefer to avoid unnecessary prolongation of life through life-sustaining treatment.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">10</span></a> Despite this, more than 70% of patients do not discuss their preferences with their doctors, and life support techniques may be used that are inconsistent with the preference of most patients that prioritise comfort and quality of life.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">11–13</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Determining the prognosis of elderly patients in ICUs is the first essential step in improving communication and decision-making regarding admission to these units and the use of life-sustaining treatment. It is important to identify the patients that could benefit from life-sustaining treatment in order to avoid therapeutic futility.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of our study was to evaluate mortality in patients aged ≥80 years admitted to the surgical ICU (SICU), overall hospital mortality and associated factors, and the use of life-sustaining treatment in this group of patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and method</span><p id="par0020" class="elsevierStylePara elsevierViewall">We carried out a retrospective observational study in patients aged ≥80 years admitted to the SICU of the University Hospital La Paz between June 2012 and June 2015. A cut-off point of 80 years was chosen because this is the criterion usually used to evaluate the prognosis of critically ill elderly patients,<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">1,2,4,14</span></a> and is also used in the Simplified Acute Physiology Score (SAPS <span class="elsevierStyleSmallCaps">II</span>).<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">15</span></a> Our unit treats critically ill patients with perioperative medical complications and those who have undergone both scheduled and urgent abdominal, vascular, head and neck and neurosurgery. The study was approved by the Institutional Review Board of the University Hospital La Paz (code HULP PI-2692).</p><p id="par0025" class="elsevierStylePara elsevierViewall">The admission clinical and demographic variables of all patients were collected: age, sex, age-adjusted Charlson comorbidity index<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">16</span></a> (which estimates the risk of mortality at 10 years [<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>]), type of admission (scheduled surgery, unscheduled surgery or medical), length of stay in the ICU (days), length of hospital stay (days), and use of life-sustaining treatment during their admission to the SICU ordered by their attending doctor (use of vasopressors, invasive mechanical ventilation for more than 24<span class="elsevierStyleHsp" style=""></span>h, and renal replacement therapies). Severity of disease was determined by the SAPS II<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">15</span></a> scale and the Sequential Organ Failure Assessment (SOFA)<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">17</span></a> score at the time of admission. Admission was classified as scheduled surgery, unscheduled surgery or medical, as is the classification used in the SAPS <span class="elsevierStyleSmallCaps">II</span> scale, which determines prognosis according to the type of admission.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">The factors associated with SICU and in-hospital mortality were analysed. Fisher's exact test or the Chi-square test were used, as appropriate. A univariate Cox regression model was used to analyse quantitative variables. A predictive model for each objective variable was then calculated using logistic regression in order to control confounding factors and identify variables associated with ICU and in-hospital mortality (dependent variables). A number of independent variables were selected based on their clinical-epidemiological relevance or their association in the univariate analysis, and were introduced into a multivariate Cox regression model. Survival curves were estimated using the semi-parametric Kaplan–Meier method. All tests were bilateral, and statistical significance was set at <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05. The analysis was performed with SPSS for Windows, version 20.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">A total of 2492 patients were admitted to the SICU during the study period, of which 141 died in the unit (mortality rate of 5.6%).</p><p id="par0040" class="elsevierStylePara elsevierViewall">Two hundred and ninety-nine patients aged ≥80 years admitted to the SICU during the study period were included (12% of the total). Fifty-four patients aged ≥80 years died in the SICU (18.1%), and 80 patients (26.8%) died during hospitalisation (overall in-hospital mortality).</p><p id="par0045" class="elsevierStylePara elsevierViewall">The clinical and demographic characteristics of the patients are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. Mean age was 84.43<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.55 years. Mean SAPS <span class="elsevierStyleSmallCaps">II</span> score was 45.48<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>14.59.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">Among study patients, 128 (46.8%) were admitted after scheduled surgery, 144 after unscheduled surgery (48.2%) and 27 (9%) for medical reasons. Mean ICU stay was 5.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.6 days, and mean hospital stay was 18.1<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>15.2 days.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Among the patients who died in the SICU, the median time from admission to death was 17 days (interquartile range [IQR] 10–42 days). Among the patients who died during their hospital stay (both in the SICU and on the ward), the median hospital stay was 48 days (IQR 19–86 days).</p><p id="par0060" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRefs" href="#tbl0015">Tables 3 and 4</a> show the univariate and multivariate analysis of the SICU- and in-hospital-related mortality factors.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">In the multivariate analysis, SICU mortality was independently correlated with age, SAPS <span class="elsevierStyleSmallCaps">II,</span> the need for renal replacement therapy (RRT), and the need for invasive mechanical ventilation lasting over 24<span class="elsevierStyleHsp" style=""></span>h. Age (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.125; 95% CI: 1.054–1.192; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), SOFA score (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.154, 95% CI: 1.079–1.235; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), the need for RRT (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.924, 95% CI: 1.121–3.302; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.018) and the need for invasive mechanical ventilation lasting over 24<span class="elsevierStyleHsp" style=""></span>h (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3.144; 95% CI: 1.771–5.584; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001) were independently correlated with in-hospital mortality.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The type of admission, although not independently correlated with mortality, correlated with SICU and in-hospital mortality in the univariate analysis. An intercomparison of types of admission using adjusted residuals showed that both SICU and in-hospital mortality were significantly higher after unscheduled surgery vs scheduled surgery (25% vs 10.9%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01, and 36.1% vs 18%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002, respectively) but not after admission for medical reasons (SICU mortality of 14.8% and in-hospital mortality of 18.5%).</p><p id="par0075" class="elsevierStylePara elsevierViewall">Among patients who received vasopressors, 46.0% died (69 patients) during their hospital stay. Among patients who received invasive mechanical ventilation lasting over 24<span class="elsevierStyleHsp" style=""></span>h, 58.7% (61 patients) died. Among patients who received RRT on admission, 80% (24 patients) died during their hospital stay.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">We observed that patients aged over 80 years who die in the SICU have a prolonged stay in the unit (17 days). Similarly, patients aged over 80 years who die after admission to the SICU have a prolonged stay on the ward (median of 48 days). The duration ICU and hospital stay in our series is longer than that reported by Heyland et al.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">14</span></a> (median of 13 and 23 days, respectively). Strand et al.,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">18</span></a> in a study performed in Scandinavian ICUs, found a median duration of ICU stay among non-survivors of between 1.3 and 1.9 days, with the shortest length of stay being observed in non-survivors aged over 80 years. Bofelli et al.,<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">19</span></a> in a study in Italian ICUs, reported a median stay of 3 days among ICU non-survivors. In our study, the duration of ICU and hospital stay in non-survivors was probably high due to poor prior planning with patients and families.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Mortality in the SICU and on the ward was similar to that described by Oeyen et al. In the same study, the authors followed-up critically ill patients aged over 80 years for 7 years, and found that although their quality of life declined progressively over time, most octogenarians considered it acceptable. Among survivors at 1 and 7 years after discharge from the ICU, 81% and 72%, respectively, preferred to be re-admitted to an ICU if necessary.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">20</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Mortality among our patients was higher than that described by Paz Martín et al.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">21</span></a> in postoperative critically ill patients. The greater need for life-sustaining treatment in our group suggests that this difference was probably due to the greater severity of disease in our patients. The need for invasive mechanical ventilation was independently associated with in-hospital mortality in both studies (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>7.146 in Martin Paz et al. and OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3.144 in our study). Furthermore, in our series we observed that need for RRT, the SOFA score and age were independently correlated to in-hospital mortality. Although the indication for RRT and the time to start of the treatment varies greatly in critically ill patients, the criterion common to all cases is a diagnosis of stage 3 acute renal failure according to both AKIN<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">22</span></a> and KDIGO<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">23</span></a> guidelines. These patients, therefore, present severe acute renal failure, which is probably a determining factor in our high mortality figures. Although the need for vasopressors was associated with mortality in the univariate analysis, it was no longer an independent predictor of mortality in the multivariate analysis. Paz Martín et al. reported similar results.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">21</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">In these patients, the poor prognosis associated with the need for RRT and invasive mechanical ventilation lasting more than 24<span class="elsevierStyleHsp" style=""></span>h should be taken into account when planning care together with patients and families, ideally before the onset of a life-threatening situation. This means that each patient must be assessed individually, but the results should never lead to life-sustaining treatment being routinely ruled out in these patients.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Although mortality was higher among patients undergoing unscheduled surgery in the univariate analysis, unscheduled surgery was no longer an independent predictor of mortality in the multivariate analysis. Similar results have been reported in previous studies.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">2,21,24</span></a> Although medical admissions were uncommon in our unit, in contrast to other studies they were not associated with increased mortality.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">2</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">A striking finding in our study is that, in contrast to other reports, age itself was an independent predictor of both ICU and in-hospital mortality, irrespective of comorbidity determined by the Charlson index.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">20</span></a> We observed only minor differences in the mean age of survivors and non-survivors of hospitalisation, because all patients were at least 80 years of age. In the multivariate analysis, however, the OR is 1.121, meaning that mortality increases by 12.1% for each additional year of age. This is a very significant increase.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Our findings raise important questions about end-of-life care in postoperative critically ill patients. A Canadian survey on patient preferences reported that elderly patients rated the avoidance of life support machines to unnecessarily prolong life as one of the most important aspects of end-of-life care.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">10</span></a> In another study, less than 12% of elderly patients admitted to hospital wards preferred to receive complete medical treatment, including life-support measures if necessary.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">25</span></a> Wright et al.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">26</span></a> showed that aggressive medical treatment in patients with advanced cancer was associated with worse quality of life and a greater risk of episodes of major depression in caregivers. The authors concluded that discussing end-of-life care with the patient was associated with fewer ICU admissions and less use of life-sustaining treatment, but did not increase the incidence of episodes of depression. in patients. Caring for elderly patients in the ICU can also be extremely costly considering the poor survival outcomes.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">27</span></a> Discussions between the patient, their carers, and their attending doctors to decide in advance the care to be provided in life-threatening situations ensures a better quality of end-of-life care, greater satisfaction on the part of family members, and fewer unwanted admissions to the ICU.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">28</span></a> An accurate determination of the prognosis of these patients is a fundamental factor in such meetings, and discussions should include the aims of aggressive treatment.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The main limitation of our study is its retrospective nature and single-centre design. More importantly, we did not analyse quality of life, and no long-term follow-up of patients was performed. In addition, we did not have access to medical orders to maintain or withdraw life-sustaining treatment on admission or during ICU stay. Because of this, overall hospital mortality rates are more important than intra-ICU mortality in our assessment of prognosis, since some patients may have moved to the hospital ward after the withdrawal of life-sustaining treatment.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The strength of our investigation lies in the relatively high number of critically ill, mainly surgical, patients aged over 80 years included in our series.</p><p id="par0125" class="elsevierStylePara elsevierViewall">In conclusion, age, SAPS <span class="elsevierStyleSmallCaps">II,</span> the need for RRT, and the need for invasive mechanical ventilation lasting over 24<span class="elsevierStyleHsp" style=""></span>h are all independent prognosis factors for mortality in critically ill patients aged ≥80 years admitted to the ICU, and the SOFA score, the need for RRT and the need for invasive mechanical ventilation lasting over 24<span class="elsevierStyleHsp" style=""></span>h are all independent prognosis factors for in-hospital mortality.</p><p id="par0130" class="elsevierStylePara elsevierViewall">The average stay in the ICU of elderly non-survivors is prolonged, as is the average length of hospital stay in patients who die on the ward.
