RA, right atrium; LA, left atrium; RV, right ventricle; LV, left ventricle.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">With longer life expectancy the global population increasingly consists of more elderly people and according to the World Health Organisation (WHO) the number of people over 60 years of age will increase from 900 million to 2 billion between 2015 and 2050, from 12% to 22% of the total population worldwide<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. This is linked to an increase in surgical procedures in this population group, which translates into a global challenge, not just for achieving total coverage but also in terms of adjusting the medical-surgical field to this population group to fit in with its specific needs<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Subarachnoid regional anesthesia is a frequently used technique, especially in older patients, due to its efficacy, rapid onset of action, minimal effects on mental status, reduced blood loss, shorter hospital stay, lower incidence of postoperative nausea and vomiting, and protection from thromboembolic events<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3–6</span></a>. However, subarachnoid anesthesia is not without risks, the main ones being haemodynamic changes such as decreased blood pressure due to vasodilatation and bradycardia, secondary to sympathetic blockade<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–9</span></a>. The magnitude of hypotension will largely depend on the number of spinal segments blocked<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10</span></a>. The effects on cardiac function have mostly been explained as a compensatory response to the decrease in blood pressure due to relative hypovolaemia secondary to vasodilatation<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,11</span></a>. However, there is as yet no study evaluating the direct effects of subarachnoid anesthesia on left ventricular systolic function in fragile hearts given the structural changes associated with age.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Structural changes in the cardiovascular system associated with ageing are an increase in vascular wall stiffness with subsequent increase in afterload, fibrosis of the cardiac musculature leading to impaired relaxation during ventricular diastole, increased aortic root diameter and left ventricular hypertrophy<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,13</span></a>. Resting systolic function is preserved, with normal left ventricular ejection fraction maintained during ageing in patients without previous heart disease<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>. However, a decrease in longitudinal left ventricular contractility has been demonstrated by measuring mitral annulus velocity with tissue Doppler<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>. The addition of echocardiography to anaesthetic monitoring allows a more sensitive assessment of transient changes in left ventricular function that may be associated with subarachnoid anesthesia in older patients.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The aim of this study was to use transthoracic echocardiography to assess the changes caused by subarachnoid anesthesia in left ventricular systolic function in patients over 60 years of age, without previous cardiovascular disease, based on mitral annular plane systolic excursion (MAPSE) as a surrogate measure of left ventricular function<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a>. In addition, the inferior vena cava collapsibility index (IVCVI), left ventricular outflow tract velocity time integral (LVOTVTIVTI) and cardiac output (CO) are assessed, as these measures are related to cardiac preload and afterload, determinants of cardiac contractility<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,17</span></a>.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Patients and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">An observational, longitudinal, prospective study was conducted at the Hospital Universitario San Ignacio in the city of Bogotá, Colombia. From February to August 2018 we included patients over 60 years of age, with no known previous cardiovascular disease (no history of heart failure, acute myocardial infarction, unstable angina, cardiac arrhythmia or history of structural heart disease such as: congenital heart disease, valvular heart disease, dilated heart chambers or other findings described in previous echocardiogram), undergoing elective surgical procedures, under regional subarachnoid anesthesia, in supine decubitus and requiring sensory block greater than or equal to T10 and who gave prior consent to participate in the study. Patients who did not agree to participate in the study or with any of the following characteristics were excluded: haemoglobin less than 8 mg/dl, arterial hypotension (MAP less than 60 mmHg), bradycardia (heart rate less than 60 lpm), tachycardia (heart rate greater than 100 lpm) prior to the start of subarachnoid anesthesia. Patients in whom the technique had to be converted to general anesthesia were excluded.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The anaesthetic protocol was the same for all patients: monitoring with electrocardiogram, oximetry and non-invasive blood pressure, nasal prongs with oxygen at 2 l/min, mild iv sedation with midazolam (maximum dose 0.2 mg/kg) and fentanyl (maximum dose 1 µg/kg), administration of Ringer's lactate at maintenance dose (3 ml/kg/h) without loading bolus. Subsequently, subarachnoid regional anesthesia was administered as the only anaesthetic technique, with 0.5% hyperbaric bupivacaine between 10 and 15 mg, depending on the type of surgical procedure and the anthropometric characteristics of the patient, seeking to obtain a sensitive level equal to or greater than T10.</p><p id="par0035" class="elsevierStylePara elsevierViewall">In case of hypotension with MAP of 60 mmHg or lower, administration of the alpha 1 agonist etilefrin in boluses was protocolised.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Echocardiographic measurements and variables of interest were taken at baseline, prior to the start of subarachnoid anesthesia and 5 min after administration. The study variables were as follows:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">•</span><p id="par0045" class="elsevierStylePara elsevierViewall">MAPSE: Apical 4-chamber window in M-mode perpendicular to the lateral border of the mitral annulus was used in supine position (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">•</span><p id="par0050" class="elsevierStylePara elsevierViewall">LVOTVTI: Apical 5-chamber window with pulsed Doppler at the level of the left ventricular outflow tract just before the aortic valve was used in the supine position.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">•</span><p id="par0055" class="elsevierStylePara elsevierViewall">IVCCI: In the supine position, a subcostal M-mode window was used 2 cm distal to the inferior cava approach to the right atrium. Measurement of the major (A) and minor (B) diameter during inspiration and expiration. The collapsibility index was calculated using the following mathematical formula: A-B/A.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">•</span><p id="par0060" class="elsevierStylePara elsevierViewall">CO: The calculation was performed taking into account the LVOTVTI value, the heart rate (HR) and standardising the left ventricular outflow diameter (LVOD) to 2 cm which is the population mean value. This standardisation of LVOD was decided since this is a measure with poor reproducibility and consequently when interpreting the evolution of CO it will not be known whether its modifications are a consequence of haemodynamic changes or of differences in the measurement of LVOD. Thus, by converting it into a constant and taking the value of the population mean, it will be possible to confirm that changes in this value are the result of haemodynamic changes. The calculation was performed with the following mathematical formula: CO = 3.14 × (D/2)2 × LVOTVTI × HR</p></li></ul></p><p id="par0065" class="elsevierStylePara elsevierViewall">A portable ultrasound machine with the following features was used to measure these variables: SonoSite Edge® (FUJIFILM SonoSite, Inc. Boothell, WA, USA) 5-1 MHz sector transducer model P21n. All measurements were performed by the same anaesthesiologist with special training in transthoracic echocardiography.</p><p id="par0070" class="elsevierStylePara elsevierViewall">For sample size calculation for comparison of paired quantitative variables with a target difference greater than 30% (having MAPSE as the main variable) was estimated, statistical power of 80% and an alpha error of .05, expecting 54 patients, with expected losses between 10%–15% given by the probability of bad echocardiographic window for a total of 60–65 patients. For statistical analysis, normal distribution was assessed for quantitative variables using the Shapiro-Wilk test, followed by the description of variables by means of averages and standard deviation. Counting, proportions and percentages were used for categorical variables. Hypothesis testing was carried out with Student's <span class="elsevierStyleItalic">t</span>-test for paired variables, with a probability of less than .05 being considered significant. Data processing was performed in Stata Corp. 2017. Stata® Statistical Software: Release 15. College Station, TX: StataCorp LLC. The study was approved by the institutional ethics committee with registration number FM-CIE-0442-17.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0075" class="elsevierStylePara elsevierViewall">A total of 66 patients were included, of whom 12 were excluded due to poor echocardiographic window. No patient required conversion to general anesthesia. Thus, the final number of patients with echocardiographic evaluation was 54, with a mostly male population of 79.6%. Most surgeries were urological, with the most frequent being transurethral resection of the prostate. The most prevalent comorbidity was hypertension, and 9.3% of the population had no associated disease (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Regarding anaesthetic management, the anaesthetic protocol described was followed for all patients. In all patients recruited, a sensory level equal to or greater than T10 was achieved. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> contains the anaesthetic management information.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Regarding the haemodynamic response, a statistically significant decrease in systolic, diastolic and mean arterial blood pressure was observed 5 and 10 min after subarachnoid anesthesia was administered (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). Decrease in MAP below 60 mmHg occurred in 14.8% of the total patients, who were treated with single doses of ethylephrine (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Heart rate after administration of subarachnoid anesthesia increased at 5 min by 4%, returning to baseline values at 10 min, with no statistically significant differences (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">Of the total number of patients, 15% presented a sensory block greater than T5, none of whom presented haemodynamic repercussions or symptoms requiring management or conversion to general anesthesia.</p><p id="par0095" class="elsevierStylePara elsevierViewall">The analysis of the echocardiographic measures studied (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>) showed a decrease in MAPSE of only 3.3%, 5 min after subarachnoid anesthesia was administered, which was not statistically significant. With regards to the other measures, it was found that 5 min after the anesthesia was administered, there was a decrease in LVOTVTI of 2.5% and an increase in CO of 0.8%, which were not statistically significant. IVCCI was the only value that showed a decrease of 13.3% with respect to the baseline measurement, which was statistically significant.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">Of the patients who presented arterial hypotension defined as MAP less than 60 mmHg during the first 5 min after anesthesia was administered and before performing echocardiographic measurements, it is difficult to analyse the data obtained as there were very few, only 11%. However, there was a decrease in MAPSE at 5 min of 4.1%, LVOTVTI remained the same, IVCCI showed a decrease of 16% and CO showed an increase of 7%, none of these data being statistically or clinically significant.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The subgroup analysis between normotensive and hypotensive patients did not show any statistically significant values for the echocardiographic variables studied (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">From the analysis of the data obtained in this study, it can be concluded that subarachnoid regional anesthesia with a sensory level equal to or higher than T10 is a technique that does not affect cardiac function in the population over 60 years of age, with no previous cardiovascular disease. Therefore, in addition to the previously described benefits such as efficacy, rapid onset of action, minimal effects on mental status, reduced blood loss and protection from thromboembolic events<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3–6</span></a>, there is also the absence of transient left ventricular dysfunction. Although in older patients or those with heart disease these results may not be extrapolated, they do rule out a deleterious effect of subarachnoid anesthesia per se on cardiac function and therefore reinforce its safety.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Ruling out the presence of left ventricular dysfunction during this anaesthetic technique allows us to confirm that the haemodynamic changes presented in these patients are secondary only to the vasodilatation caused by the sympathetic blockade generated, leading to relative hypovolaemia. Therefore, treatment of hypotension, without bradycardia, during subarachnoid anesthesia will require support of venous return only, and not of the pump function of the heart. This is relevant because ephedrine, an alpha- and beta-agonist, is often used to treat hypotension in subarachnoid anesthesia and beta-stimulation, which in some patients may have harmful cardiac effects, is unnecessary. In our study we used the available alpha agonist, etilefrin, while phenylephrine is currently recommended for the management of hypotension not associated with bradycardia in patients undergoing subarachnoid anesthesia<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,19</span></a>.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The addition of echocardiography to the anaesthesiologist's monitoring and screening arsenal allows a more direct assessment of the patient's condition and the effects of interventions on the patient<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a>. MAPSE should be considered as a useful tool in the assessment of left ventricular function, as it is easy and quick to perform, at the patient's bedside and without major technical requirements for its acquisition and measurement. It requires only a 4-chamber echocardiographic window. MAPSE measurement can be useful in all patients undergoing surgery who, either because of their age or medical history, are suspected of having a left ventricular dysfunction that needs to be evaluated more objectively.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Of the patients initially recruited, 18% had to be excluded from the study because they did not have a good 4-chamber window. The literature reports that a satisfactory transthoracic echocardiogram is achieved in up to 98% of cases<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a>. However, it should be borne in mind that this refers to performing the examination in its entirety (all echocardiographic windows), which means that even if one window is poor, the others are available to assess cardiac function. Similarly, this examination is usually performed routinely with the patient in the left lateral decubitus position (except for the subcostal window). Our study was limited to the supine position, in order to compare values before and after anesthesia, and this justifies the proportion of patients in whom a good window was not found. One of the limitations of echocardiographic examination during anesthesia is the different positions required for different interventions and the difficulty in making changes in position to improve the possibility of finding adequate windows in all patients.</p><p id="par0130" class="elsevierStylePara elsevierViewall">In this study, only 14.8% of patients presented hypotension secondary to subarachnoid anesthesia and none presented clinically relevant bradycardia requiring pharmacological treatment. This contrasts with that reported in large observational studies in non-obstetric patients, where the incidence of hypotension with this technique is around 33% and of bradycardia around 13%, with age over 40 years being identified as a risk factor for hypotension<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8,22</span></a> in which the population of this study is situated. The limited doses of bupivacaine used and its hyperbaricity related to greater haemodynamic stability<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> and the elective nature of the surgery explain the lower incidence of haemodynamic effects<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a>. The doses used achieved an adequate sensory level for the surgical procedures performed and no conversion to general anesthesia was required in any case.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Subarachnoid anesthesia causes vasodilatation of capacitance vessels, which decreases venous return and thus the effective circulating volume at the level of the inferior vena cava, thereby increasing the collapsibility of the inferior vena cava during inspiration. Multiple studies show that spontaneously breathing hypovolaemic patients have a higher IVCCI index than nomovolaemic patients<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24,25</span></a>. It was expected that with the vasodilatation of the sympathetic blockade in our patients the IVCCI would increase, but this was not the case. During spontaneous inspiration, intrathoracic pressure drops, right heart chambers expand and central venous pressure falls; at the same time intra-abdominal pressure increases due to the descent of the diaphragm and the contraction of abdominal muscles. The combination of these "forces" determines IVCCI<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24,25</span></a>. Although the influence of intra-abdominal pressure increases in obstetric patients on the level of blockade has been extensively studied, there is no clear information on intra-abdominal pressure changes in subarachnoid anesthesia. The 11% of patients in our study with hypotension showed a lower IVCCI than normotensive patients, but without reaching statistical significance probably due to the low number of patients. The low frequency of hypotension and the elective nature of the surgery suggest an adequate volumatic state and the level of sensory blockade suggests a low abdominal motor blockade which may have made the changes in IVCCI non-significant. Another possible explanation for the findings associated with IVCCI could be found in a detailed analysis of the physiology leading to collapse of the inferior vena cava, where after subarachnoid anesthesia there is paralysis of the abdominal muscles due to motor blockade causing them to fall unopposed on the abdomen, thus increasing intra-abdominal pressure, which would limit the descent of the diaphragm during inspiration during spontaneous ventilation, associated with paralysis of accessory muscles for inspiration, all leading to a lower decrease in intrathoracic pressure and thus lower IVCCI. Thus, motor blockade would partially compensate for the effects of sympathetic blockade on venous return.</p><p id="par0140" class="elsevierStylePara elsevierViewall">An important limitation in interpreting the results of this study is the consideration of "hypotension" after the administration of anesthesia as an absolute value of 60 mmHg, and not in relation to the percentage decrease in mean blood pressure with respect to baseline. We believe that this value was not the appropriate cut-off point, as the study population, over 60 years of age, is in itself a sample of "hypertensive" patients. The chosen cut-off point may have underestimated the number of patients with "hypotension" when analysing differences in echocardiographic measurements between "hypotensive" and "normotensive" patients.</p><p id="par0145" class="elsevierStylePara elsevierViewall">On the other hand, it is important to analyse the fact that 11% of patients were considered "hypotensive" during the first 5 min after anesthesia administration, so they received doses of etilefrin before echocardiographic measurements were performed. Despite the fact that this drug has not only an alpha but also a beta effect, we consider that the low percentage of patients requiring its administration and the low doses used (average single dose of 1.8 mg) may not have significantly affected ventricular contractility. For this assessment, we based ourselves on the fact that the increase in HR would manifest the beta effect and was only 4%, without statistical significance, nor do we believe that it altered the echocardiographic data obtained and analysed.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Other limitations of the study include the sample size and the loss of information in 18% of the patients, who could not enter the study due to the poor echocardiographic window. The study was conducted in elective patients, most of whom were male, and therefore we cannot extrapolate the results to one of the most frequent and fragile situations in the usual indication for subarachnoid anesthesia, such as emergency hip fracture surgery, which is also more frequent in women<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">26,27</span></a>. Another limitation of the study is that echocardiographic measurements were not taken more than 5 min after subarachnoid anesthesia had been administered, due to the logistics and time required to rotate the operating theatres, but we consider that, with the sensory block established, the sympathetic block is already established. In any case, we cannot rule out that the changes continued at least during the initial 20 min of anesthesia, the time considered for its full establishment<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a>, and that changes in left ventricular function detected could have been detected by echocardiography. Finally, it should be remembered that cardiac function is the result of many variables such as preload, afterload, systolic and diastolic biventricular function, and that MAPSE measurement is only a small link in this chain. In addition, CI-IVC, LVOT-VTI and CO were measured. Sin embargo, no se han estudiado los efectos que ésta pueda tener sobre la función sistólica ventricular izquierda.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Objetivos</span><p id="spar0120" class="elsevierStyleSimplePara elsevierViewall">Evaluar la función sistólica ventricular izquierda por ecocardiograma transtorácico en pacientes mayores de 60 años, sin enfermedad cardiovascular, bajo anestesia subaracnoidea instaurada.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Pacientes y métodos</span><p id="spar0125" class="elsevierStyleSimplePara elsevierViewall">Estudio observacional prospectivo con un total de 54 pacientes mayores de 60 años sin enfermedad cardiovascular, a quienes, tras la instauración de una anestesia subaracnoidea con bupivacaina hiperbárica al 0,5% con bloqueo sensitivoT10 o mayor, se les midió la función sistólica ventricular izquierda a partir del MAPSE (Mitral anular plane systolic excursion). Además, se midió el índice de colapsabilidad de vena cava inferior (ICVCI), la integral velocidad tiempo del tracto de salida del ventrículo izquierdo (ITTSVI) y gasto cardiaco (GC). Los valores a los 5 minutos del bloqueo se compararon con los obtenidos previamente al mismo.</p></span> <span id="abst0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Resultados</span><p id="spar0130" class="elsevierStyleSimplePara elsevierViewall">Se evidenció disminución del 3,3% del MAPSE y ligera disminución del IVTTSVI y GC, en ningún caso con significancia estadística ni clínica. El 14.8% de los pacientes presentó presión arterial media (PAM) igual o inferior a 60 mmHg.
