Despite the undeniable advances in the possibilities of assessing and treating paediatric acute pain, it remains a challenge for anaesthesiologists due to the special physiological, behavioural, and even anatomical characteristics of paediatric patients.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> This is particularly true in Spain, because a lack of university training in pain-related topics has been reported,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> and specific paediatric pain management programmes are poorly implemented.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">As this study is only exploratory, the authors of the paper do not present any hypothesis to be validated, the aim of the study is just to improve knowledge about the usual clinical practices of Spanish paediatric anaesthesiologists, and the organisational and training limitations they face, in order to suggest potential measures for improvement to the competent bodies and institutions.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Procedure</span><p id="par0035" class="elsevierStylePara elsevierViewall">A first version of the survey based on the methodology used in previous studies was used in this study.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> This first version of the survey included about 40 questions and was discussed telematically by the Pain Group within the Paediatric Anaesthesia Section of the Spanish Society of Anaesthesiology-Resuscitation (SEDAR). This group included fifteen professionals with extensive experience in paediatric pain management. Finally, the authors of the article decided on the final version of the survey with 28 questions that were considered adequate to assess three aspects: professional data of the participants, usual clinical practice in the assessment and treatment of paediatric acute pain including patients with intellectual disabilities, and aspects of training/organisation of such treatment. Most of the questions were closed-ended (YES/NO), some were multiple choice with mutually exclusive answers, and the fewest were multiple choice with non-exclusive answers (so that several answers could be selected at the same time). The respondents completed the survey on an unpaid basis throughout March 2021, using an internet platform; it took approximately 10−15 min to complete. The full version of the questionnaire is available from the corresponding author.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The Clinical Studies Commission of the Faculty of Medicine of the Autonomous University of Madrid authorised the procedure used for the present study.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Respondents</span><p id="par0045" class="elsevierStylePara elsevierViewall">The respondents were recruited from among the 6541 SEDAR members with extensive experience in acute pain management. Only anaesthesiologists who regularly work with paediatric patients with acute pain were considered as experts in this study. Finally, the 264 members who regularly participate in the activities of the Paediatric Section of SEDAR were invited to participate in the study. Of these, 150 agreed to complete the survey (57% of potential respondents).</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">The 150 physicians who agreed to participate in the survey answered the 28 questions included. Most were women (68.9%) with extensive clinical experience in paediatric acute pain (mean years of experience: 14.3; SD: 7.7). The results of the survey are shown in <a class="elsevierStyleCrossRefs" href="#tbl0005">Tables 1–3</a>. They are reflected as percentages of the positive responses of the 150 respondents (responses that did not receive any votes are excluded).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">General data</span><p id="par0055" class="elsevierStylePara elsevierViewall">Most of the respondents worked in tertiary paediatric specialist centres in the autonomous communities of Madrid and Catalonia, with experience predominantly in the field of paediatric acute postoperative pain (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Clinical practice</span><p id="par0060" class="elsevierStylePara elsevierViewall">Although most used prescribed systemic analgesia based on paracetamol, and assessed the intensity of acute pain, only slightly more than half (56.3%) of the respondents did so in a protocolised manner. The respondents also had significant experience with patients with cerebral palsy. Detailed results of these responses are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Training/organisational aspects</span><p id="par0065" class="elsevierStylePara elsevierViewall">A significant proportion of the Spanish paediatric anaesthesiologists represented in the survey (37.1%) acknowledged significant shortcomings in their specific training in paediatric acute pain, which they solved in a self-taught manner using the available scientific literature and their usual clinical practice (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><p id="par0070" class="elsevierStylePara elsevierViewall">Likewise, although most recognised the importance of a specific organisational structure for paediatric acute pain management in hospital, they admitted that this only occurred on 25% of occasions, mainly due to the lack of specific personnel for this task, such that only 27% participated in specialised paediatric acute pain care programmes. <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the more detailed aspects of these responses.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">Although paediatric acute pain is multidisciplinary and we should ideally have included paediatricians, surgeons, emergency physicians, and nurses, the present article is the first study to use a national survey in Spain to address the involvement of anaesthesiologists caring for paediatric patients in acute pain management.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Similar surveys have been conducted among Italian paediatricians,<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> and among healthcare personnel in Sweden (including physicians and nurses).<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Surveys on organisational and care aspects have also been published aimed at anaesthesiology departments in Germany.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">In the present study, anaesthesiologists who regularly participate in the activities of the Paediatric Section of SEDAR were directly surveyed, because they are considered particularly representative in the care provided to children with acute paediatric pain in Spain in 2021 (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The sample size is relatively small, although it represents almost half of the usual attendees at national paediatric anaesthesia congresses in Spain and, therefore, this limited representation must be considered. In this sense, the sample yields data that coincide with the Spanish demographic situation, with a majority of female anaesthesiologists, with several years of experience and attending an average of two patients per day with acute postoperative pain, similar to the adult population.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> Most activity is carried out in tertiary paediatric hospitals located in the large capitals of autonomous communities with larger populations and, therefore, with a higher proportion of the paediatric population requiring care in these hospitals, which does not prevent a certain bias in excluding regional hospitals where the situation could be even worse.</p><p id="par0090" class="elsevierStylePara elsevierViewall">All these national surveys highlight the current major deficiencies in the treatment of paediatric acute pain. This would coincide with the data obtained in the present study, which shows that it is possible to address important objectives to improve the situation of paediatric acute pain management in Spain.</p><p id="par0095" class="elsevierStylePara elsevierViewall">In terms of routine clinical practice, the survey showed that, even today, 1:5 of Spanish paediatric anaesthesiologists (20%) do not routinely assess the intensity of acute pain in their patients (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). However, good knowledge of the scales to measure pain intensity in older children (of language acquisition age) was observed, following the recommendations of the international expert group and using tools such as the VAS scale or the numerical rating scale.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> The problem seems to lie in the less frequent use of pain intensity measurement scales in preverbal children. The creation of a specific Spanish scale, such as the LLANTO scale,<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> seems to have facilitated this task compared to English-language scales such as the FLACC<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> or CHEOPS,<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> but even so, it seems advisable to insist on the dissemination of these simple, effective, and validated scales for the paediatric population under 3 years of age.</p><p id="par0100" class="elsevierStylePara elsevierViewall">In terms of analgesic treatment (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), the extensive use of non-opioid analgesics such as paracetamol or metamizole was positively observed as the safest option (93%),<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22,23</span></a> which allows for rescue analgesia with opioid medication such as fentanyl or morphine, following a pattern of prescribed systemic analgesia (92%). Similarly, the practice seems to be widespread of multimodal analgesia using local anaesthetics infiltrated together into the wound,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> and, to a lesser extent, locoregional blocks. Anti-inflammatory drugs such as steroids (70%), probably due to their antiemetic potential<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> and non-steroids such as ibuprofen (67.5%),<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> which appears to have replaced ketorolac, probably because it does not have a technical datasheet for use in paediatrics and is potentially more dangerous.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> Finally, the widespread use of an analgesic drug such as tramadol, which has been questioned by the Food and Drug Administration for safety reasons, especially in paediatric patients, is surprising.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Unfortunately, the study highlights major shortcomings in the use of regional techniques for postoperative analgesia, which can provide prolonged analgesia in such difficult cases as pectus excavatum surgery,<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> or safe and simple non-pharmacological techniques (which have been shown to be particularly beneficial in acute paediatric pain), or analgesic adjuvants such as ketamine or dexmedetomidine.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Paediatric patients with intellectual disabilities deserve special mention. It is striking that most paediatric anaesthesiologists report treating this type of patients (primarily cerebral palsy and autistic spectrum patients, since genetic syndromes are identified prenatally and are usually the cause of pregnancy termination). In any case, very few of these patients, only 7.9%, benefit from an appropriate assessment, such as the modified FLACC scale.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a> Again, measures should be taken to address these shortcomings in coming years, as many of these children do not receive adequate pain management, due to failures in the assessment and knowledge of pain, especially if they do not have the verbal capacity to express it.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The problem is of great magnitude, as in 2017 Hauer and Houtrow, using specific scales for pain assessment in people with cognitive disabilities (with individualised indicators, such as changes in conduct or behaviour), identified in this population an incidence of weekly pain in 44% of children with moderate cognitive disabilities, and almost daily pain in 42% of children with severe disabilities. The use of specific pain assessment scales for this population could prevent much unnecessary suffering.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">30</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">In fact, it is in the organisational and training aspects where the survey offers the greatest opportunities for improvement, as almost one in three of the anaesthesiologists surveyed acknowledges the lack of specific training in this field (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). Other specialties in Spain, such as paediatricians and primary care physicians, also present similar training gaps in paediatric pain.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> It is also striking that doctors have had to acquire their training in an almost self-taught manner, given the little formal training in pain in Spanish medicine, at both undergraduate and postgraduate level. Curiously, a more extensive pain training curriculum is implemented across veterinarians and dentists than that given to medical graduates.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">From an organisational perspective, although almost all the respondents (98%) recognised the need for an organised hospital structure specifically for the treatment of paediatric acute pain, only 25.2% (1/4) believed that this was being adequately achieved. The reasons given were more of an organisational nature, such as lack of assigned staff (62%) or time (51%), rather than a lack of material or scientific resources (33%) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><p id="par0130" class="elsevierStylePara elsevierViewall">This means that almost half of the physicians (43%) do not apply protocolised treatment for acute paediatric pain because it is not available to them (47%). Of the doctors who do apply specific protocols (57%), we find that they use them because they are provided by the hospital itself through the hierarchical organisation of the department (45%), and to a much lesser extent through the scientific societies. Actions taken to disseminate protocols by the respective national and international societies can ensure that they are encouraged by the heads of the relevant departments.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Finally, when asked about participation in specific structures for the treatment of paediatric acute pain, such as paediatric pain units, we found that only 28% of the doctors participate regularly in them, and of these units, only 18% attend uninterruptedly every hour of every day of the year. This corresponds to a recent study in which only 8 paediatric hospitals with established paediatric pain care programmes were identified.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Even fewer physicians (7%) work in a unit accredited by an external quality agency, which seems to apply to only one Spanish hospital. Another objective to consider would certainly be for tertiary hospitals specialising in paediatrics (84% of the physicians in the survey) to have these types of accredited units.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Among the limitations of the study would be the relatively small sample size, but given that the care of these patients is provided by highly specialised physicians of whom there are no more than three hundred in Spain, we consider the sample to be sufficiently representative and like that of other similar studies.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13–16</span></a> However, it must be borne in mind that the primarily the conditions in large specialist paediatric hospitals are described, therefore, the situation could be even worse in regional hospitals staffed by physicians with less experience in this type of patients. Likewise, the surveys were obtained from anaesthesiologists affiliated to SEDAR, so the answers seem to refer mostly to acute postoperative pain. Therefore, other healthcare groups involved, such as nurses, paediatricians, paediatric surgeons, psychologists need to be surveyed to draw a more complete picture of the situation.</p><p id="par0150" class="elsevierStylePara elsevierViewall">In any case, the results shown in this study faithfully represent the situation of acute pain from the perspective of the members of the Paediatric Section of SEDAR. The survey included 28 questions about their usual clinical practice in the assessment and treatment of acute pain, and also training and organisational aspects in paediatric acute pain.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">The survey was completed during March 2021 by 150 specialists in anaesthesiology. The respondents widely experienced in the management of acute paediatric pain (mean years of experience: 14.3: SD: 7.8), essentially in acute postoperative pain (97% of cases). Although 80% routinely used validated paediatric acute pain assessment scales, only 2.6% used specific scales adapted for patients with cognitive impairment. Most of the respondents routinely used analgesic drugs such as paracetamol (99%) or metamizole (92%), but only 84% complemented these drugs with a loco-regional blocking technique or other non-steroidal anti-inflammatory drugs (62%). Los encuestados presentaron una amplia experiencia en el tratamiento del dolor agudo pediátrico (media de años de experiencia = 14,3: DE = 7,8) y básicamente en dolor agudo postoperatorio (97% casos). Aunque el 80% de los mismos utilizaba de modo habitual escalas validadas de valoración de dolor agudo pediátrico, sólo el 2,6% utilizaba las específicas adaptadas para pacientes con discapacidad cognitiva. La mayoría de los encuestados empleaba habitualmente fármacos analgésicos como el paracetamol (99%) o el metamizol (92%), pero sólo el 84% los complementaba con alguna técnica de bloqueo loco-regional u otra medicación tipo Antinflamatorio No esteroideo (62%).
