It was formerly known as <span class="elsevierStyleItalic">Flavobacterium indologenes</span> or <span class="elsevierStyleItalic">Flavobacterium aureum</span> due to the formation of bright yellow colonies in microbiological cultures. The incidence of nosocomial infections caused by CBI has increased in recent years in intensive care units, probably due to the use of broad-spectrum antibiotics, particularly carbapenems.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">1–3</span></a> The entities most commonly associated with CBI colonisation are bacteraemia and ventilator-associated pneumonia (VAP).<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">1</span></a> These nosocomial infections result in increased duration hospital and intensive care unit (ICU) length of stay, health care costs, and mortality.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">3</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Case reports</span><p id="par0010" class="elsevierStylePara elsevierViewall">We performed a descriptive, retrospective study analysing demographic, clinical, evolutionary and prognosis variables in patients diagnosed with ventilator-associated tracheobronchitis (VAT) and VAP due to CBI admitted to a 24-bed medical and surgical ICU in a tertiary university hospital between January 2010 and July 2015. The presence of coinfection in patients with CBI-induced VAT or VAP was also analysed.</p><p id="par0015" class="elsevierStylePara elsevierViewall">VAP was defined as the presence of <span class="elsevierStyleItalic">de novo</span> alveolar infiltrate on chest radiograph associated with 2 of the following criteria: fever over 39<span class="elsevierStyleHsp" style=""></span>°C, purulent tracheal secretions, leukopenia or leucocytosis with left shift, or over 15% deterioration in baseline PaO<span class="elsevierStyleInf">2</span>/FiO<span class="elsevierStyleInf">2</span>. The absence of <span class="elsevierStyleItalic">de novo</span> radiographic infiltrate is considered diagnostic of VAT.</p><p id="par0020" class="elsevierStylePara elsevierViewall">CBI copathogens were defined as germs with significant growth in respiratory samples cultured 7 days prior or subsequent to isolation of CBI. Prior antibiotic treatment was defined as that given in the 10 days prior to onset of symptoms suggestive of VAT or VAP.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The average number of admission per year over the study period was 973 (range 897–1016), with a VAP rate of 6.87 pneumonias per 1000 days of mechanical ventilation.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Four cases of CBI-induced VAT and 4 of CBI-induced VAP were diagnosed. The first diagnosis over the study period (2010–2015) was identified in 2013. In 2014, 2 cases of CBI-induced VAP and 1 case of CBI-induced VAT were identified; in 2015, 2 cases of CBI-induced VAP and 2 cases of CBI-induced VAT occurred.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The demographic and clinical characteristics of the 8 patients, together with details of the empirical treatment given and the modification and duration of this treatment are detailed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">Only 1 case showed no coinfection with CBI-induced VAT or VAP. The most common copathogen found was multirresitent <span class="elsevierStyleItalic">Pseudomonas aeruginosa</span>, in 4 of the 8 cases, followed by <span class="elsevierStyleItalic">Stenotrophomonas maltophilia</span> in 2 of the other 4 cases. ESBL-producing <span class="elsevierStyleItalic">Escherichia coli</span> was identified as a copathogen in another case, and <span class="elsevierStyleItalic">Acinetobacter baumannii</span> was a copathogen in one of the cultures positive for <span class="elsevierStyleItalic">P. aeruginosa</span>.</p><p id="par0045" class="elsevierStylePara elsevierViewall">CBI isolation occurred after patients had received mechanical ventilation for 11 days on average (range 7–18). The CBI antibiogram is shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The average duration of mechanical ventilation was 36 days (range 10–140). During the study period, patients diagnosed with VAP in our centre remained on mechanical ventilation for an average of 21 days (range 6–89).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">Mean length of stay in the ICU was 49 days (range 14–180); mean length of stay in the ICU of patients diagnosed with VAT was 24 days (range 8–95).</p><p id="par0055" class="elsevierStylePara elsevierViewall">There were no deaths at 28 days, but intra-hospital mortality occurred in 2 cases (25%), although 1 followed a decision to limit life-sustaining treatment. Despite the absence of evidence of mortality rates associated with this entity in the medical literature, the number of deaths is similar to that observed in other cases previously described (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Discussion</span><p id="par0060" class="elsevierStylePara elsevierViewall">Aside from epidemiological analysis of a sample of 215 patients infected with CBI reported by Chen et al.,<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">2</span></a> of which 91 had nosocomial pneumonia due to either VAT or VAP, the largest series of CBI-induced VAP has so far been 6 patients<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">4</span></a> infected with <span class="elsevierStyleItalic">F. indologenes</span>. Apart from this study, all publications deal with isolated cases of CBI-induced VAP.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">1,3,5–8</span></a> The main characteristics of studies published to date are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><p id="par0065" class="elsevierStylePara elsevierViewall">We believe that the start of specific antibiotic therapy against different ESBL species is a key factor in the increase of different diseases caused by <span class="elsevierStyleItalic">Chryseobacterium</span>, which is inherently resistant to carbapenems due to the production of type A and B lactamase.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">2,4</span></a> The increased incidence of CBI infection has also been linked with prior use of tigecycline and colistin.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">2</span></a> Moreover, the ability of this bacterium to form a biofilm and survive in chlorinated water makes this medium an ideal hospital reservoir that is resistant to conventional decontamination measures. This enables the germ to colonise catheters or orotracheal tubes, which ultimately hampers treatment and eradication of nosocomial infection.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">3,10</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">Finally, effective empirical treatment for CBI infection remains elusive, due to scant data in the literature. Cotrimoxazole, new generation fluoroquinolones, ceftazidime, cefepime and piperacillin–tazobactam have shown reasonable activity against more than 80% of the strains. Although both new generation fluoroquinolones and cotrimoxazole have been put forward as the most appropriate antimicrobial agents, given the recently observed increase in resistance to beta-lactams and quinolones, cotrimoxazole has been suggested as the best treatment.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">11</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">In summary, the isolation of CBI associated with nosocomial respiratory infection in mechanically ventilated patients has increased in recent years. This could be due to broad spectrum antibiotic therapy previously used in these patients. The average length of patients on mechanical ventilation was 36 days (range 10–140). The average ICU stay was 49 days (range 14–180). There was no death at 28 days, but the intra-hospital mortality was 2 cases (25%).
