MOF is among the leading causes of prolonged hospitalisation and death in ICUs. The functional status of MOF survivors is considerably diminished,<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">1</span></a> and they suffer from numerous residual physical and neuropsychological clinical symptoms. Recent studies have described a prevalence of major depression among ICU survivors in excess of 30%,<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">2</span></a> driven by somatic symptoms, suggesting that approaches targeting physical rather than cognitive causes could benefit patients after critical care.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Recovery and rehabilitation in these patients is usually a lengthy process, and some can take more than 2 years to regain their previous level of health and functional status.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">1,4</span></a> Studies have shown that 2–7 years after trauma, patients with MOF are 4 times more likely to require assistance in their activities of daily living than patients who did not have MOF.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a> All these consequences usually have a high impact not only on the patient's quality of life, but also on the lives of their families, who often have to adopt significant lifestyle changes in order to attend to the needs of the patient after discharge. A better understanding of how clinical symptomatology after ICU discharge can affect long-term outcomes is essential for the development of high quality long-term follow-up programs for ICU survivors. This would be of great benefit not only for patients and their relatives and caregivers, but also for ICU and community healthcare professionals.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The present study aimed to explore the association between baseline demographic, clinical and functional characteristics of ICU non-polytrauma and non-neurocritical adult patients who suffer MOF during the first 24<span class="elsevierStyleHsp" style=""></span>h of admission, and their clinical symptomatology 6 and 12 months after discharge home.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">A prospective cohort study was carried out in a 26-bed case mix, closed adult ICU in a public hospital in Toledo (Spain).</p><p id="par0025" class="elsevierStylePara elsevierViewall">The cohort comprised 545 consecutively selected patients admitted to the ICU. Inclusion criteria were: age 16 years and over, admitted between January 2011 and September 2012, and diagnosed with MOF during the first 24<span class="elsevierStyleHsp" style=""></span>h following admission. During the ICU stay, each organ system was evaluated at admission, using the most abnormal data, and given a score from 0 (normal function) to 4 (most abnormal) according to the original SOFA definitions. Severe organ failure was defined as a SOFA score ≥3 in any organ system. MOF was defined as severe organ failure in 2 or more organ systems during the ICU stay. Patients admitted for neurocritical disease or polytrauma, defined as the syndrome of multiple injuries with a pathophysiology of polytrauma and an injury severity score of 16 or higher, were excluded for 2 main reasons. First, both the development and the progression of MOF differ considerably from other critically ill medical and surgical patients. Second, in these patients, brain injury is often associated with mobility and cognitive dysfunction, and would require a different approach in the assessment of functional status.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">5,6</span></a> The number of readmissions to the ICU after recruitment was recorded, and the data from the first ICU admission included in this study was used as baseline.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Data collection</span><p id="par0030" class="elsevierStylePara elsevierViewall">A series of 3 structured telephone interviews with each participant was conducted by S.R.V (Sancho Rodríguez-Villar). Another interviewer re-scored 30% of the interviews, showing negligible intra-observer variation.</p><p id="par0035" class="elsevierStylePara elsevierViewall">At admission, the patient's baseline functional status was assessed by the admitting specialist using the Modified Rankin Scale (MRS). Recent evidence points to a decline in functional status well before ICU admission.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">7</span></a> For this reason, baseline functional status was determined by the patient him/herself or by the next of kin when the patient was unable to answer, based on function prior to the start of the pathological process resulting in admission.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a> Severity scores at admission, assessed with the Acute Physiology and Chronic Health Evaluation II (APACHE-II) and the Simplified Acute Physiology Score II (SAPS-II), were also collected. Follow-up structured interviews about clinical symptomatology were conducted over the phone at 6 and over 12 months after hospital discharge.</p><p id="par0040" class="elsevierStylePara elsevierViewall">When the patient was unable to communicate (physical limitation), the interview was carried out with a relative in the presence of the patient.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a> The survey focused on any symptomatology exhibited post-discharge that was not present prior to admission. A second survey was carried out at 12 months post discharge. Depressive symptoms were assessed using the Beck Depression Inventory (BDI-II).<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Instruments</span><p id="par0045" class="elsevierStylePara elsevierViewall">APACHE II and SAPS-II are severity scoring systems comprising a score and a probability of mortality prediction model calculated with logistic regression. In both cases, the higher the score, the more severe the disease and the higher the risk of death. APACHE II ranges from 0 to 71 points and is based on 12 physiological measurements routinely collected during the first 24<span class="elsevierStyleHsp" style=""></span>h of admission, together with the age and previous health status of the patient. SAPS-II ranges from 0 to 163 points and is based on the worst values of 12 physiological variables measured during the first 24<span class="elsevierStyleHsp" style=""></span>h of admission (0–116 points), age (0–17 points) and type of admission, and 3 disease-related variables (0–30 points).</p><p id="par0050" class="elsevierStylePara elsevierViewall">The MRS is a 7-point scale measuring the level of independence and disability. Scores range from 0 to 6, where 0 is “no symptoms at all”, 1 “no significant disability despite symptoms”, 2 “Slight disability”, 3 “Moderate disability”, 4 “Moderately severe disability”, 5 “Severe disability”, and 6 “dead”.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The GOS-E is an 8-point scale designed to measure the level of functional recovery. Scores range from 1 to 8 as follows: 1 “dead”, 2 “vegetative state. Condition of unawareness with only reflex responses but with periods of spontaneous eye opening”, 3 “lower severe disability. Patient who is dependent for daily support for mental or physical disability, usually a combination of both, and cannot be left alone for more than 8<span class="elsevierStyleHsp" style=""></span>h at home”, 4 “upper severe disability. Same as 3, but can be left alone for more than 8<span class="elsevierStyleHsp" style=""></span>h at home”, 5 “lower moderate disability. Patients have some disability such as aphasia, hemiparesis or epilepsy and/or deficits of memory or personality but are able to look after themselves. They are independent at home but dependent outside. They are not able to return to work even with special arrangements”, 6 “upper moderate disability. Same as 5, but they are able to return to work with special arrangements”, 7 “lower good recovery. Resumption of normal life with the capacity to work even if pre-injury status has not been achieved. Some patients are disabled by minor neurological or psychological deficits”, and 8 “upper good recovery. Same as 7, but their minor neurological or psychological deficits are not disabling”.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">The BDI,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> created by Aaron T. Beck, is a 21-item multiple-choice self-report inventory, and is one of the most widely used instruments for measuring severity of depression. Its development marked a paradigm shift among healthcare professionals, who had until then had viewed depression from a psychodynamic perspective, instead of as a disorder rooted in the patient's own thoughts. The current version of the questionnaire is designed for individuals aged 13 and over, and is composed of items relating to symptoms of depression, such as hopelessness and irritability, cognitive items, such as guilt or feelings of being punished, as well as physical symptoms, such as asthenia, weight loss, and lack of interest in sex. The BDI-II also includes 21 questions, each scored on a scale of 0–3. Higher total scores indicate more severe depressive symptoms. The standardised cut-offs used differ from the original: 0–13: minimal depression; 14–19: mild depression; 20–28: moderate depression; 29–63: severe depression. The test was shown to have a high 1-week test–retest reliability (Pearson <span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.93), suggesting that it was not overly sensitive to daily variations in mood, and high internal consistency (<span class="elsevierStyleItalic">α</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.91).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Variables</span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Functional status variables</span><p id="par0065" class="elsevierStylePara elsevierViewall">The variables of interest describing functional status were the MRS and GOS-E scores after ICU admission. In the inferential analyses, the MRS and GOS-E scores were divided into 2 a priori groups, representing what we considered “good functional status” (MRS 0–2 and GOS-E 6–8) or “poor functional status” (MRS 3–5 and GOS-E 2–5). Scores indicating death (MRS 6 and GOS-E 1) were excluded from these analyses.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Clinical symptomatology variables</span><p id="par0070" class="elsevierStylePara elsevierViewall">The variables of interest describing clinical symptomatology were asthenia, sleep disturbance, presence of at least mild depression (BDI-II<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>10–18 points), myalgia/arthromyalgia, impaired intestinal motility, difficulty concentrating, memory loss, change in appetite, tremor, change in urination, headache/migraine, disturbances in vision and speech. The BDI-II scores were divided into 2 a priori groups, representing what we considered relevant for the aim of the study: “presence of at least mild depression” (>14 points) or “no presence of depression” (<14 points).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Variables for severity at admission</span><p id="par0075" class="elsevierStylePara elsevierViewall">APACHE-II and SAPS-II scores, representing the level of severity at admission, were considered independent variables affecting the presence and level of clinical symptomatology after discharge home. Relevant a priori confounders were considered in the analyses, including age, gender, readmissions, primary reason for admission and length of stay in both the ICU and hospital, among others.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Statistical analysis</span><p id="par0080" class="elsevierStylePara elsevierViewall">Descriptive statistics were used to analyse each of the study variables, including demographic and clinical data, baseline severity level and baseline functional status, as well as functional status after hospital discharge. Categorical data were expressed as percentages. Continuous variables were summarised with their mean and standard deviation (SD) or medians or minimum-maximum ranges, depending on their distribution. The chi-squared test and the Fisher-exact test (where appropriate) were used to analyse categorical data. Continuous variables were analysed using the Student's <span class="elsevierStyleItalic">t</span>-test or the non-parametric Mann–Whitney <span class="elsevierStyleItalic">U</span> test (where appropriate).</p><p id="par0085" class="elsevierStylePara elsevierViewall">Logistic regression was used when investigating the factors possibly associated with poor functional status at 6 and 12 months. Variables showing significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05) associations in the bivariate analyses were then used in the multivariate logistic regression analyses. A 2-sided <span class="elsevierStyleItalic">p</span> value of 0.05 or less was considered to indicate statistical significance. All data were processed and analysed using Microsoft Excel 2000 (Microsoft, Redmond, WA) and SPSS 15.0 for Windows (SPSS, Chicago, IL).</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Ethical considerations</span><p id="par0090" class="elsevierStylePara elsevierViewall">The study (which was part of a doctoral thesis) was approved by the regional ethics committee, and oral consent was obtained at the beginning of the telephone interview. The protocol required that informed consent to participate in the study be obtained from patients or their next of kin (when patients were unable to consent) at admission and before conducting the follow-up interviews to confirm participation.</p></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Results</span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Sample</span><p id="par0095" class="elsevierStylePara elsevierViewall">The study flow chart showing the process of preliminary patient selection up to the final sample obtained at 6 and 12 months follow-up is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. Out of 545 eligible patients, 228 (42%) and 215 (39%) survived for 6 and 12 months after hospital discharge, respectively. The baseline characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. In all cases, the first follow-up interview was carried out at 7 months after hospital discharge. The researcher verified whether the patient was alive and, if so, the proceeded with the first structured survey. The second follow-up interview was carried out at 12 months post discharge.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">Sixty-one patients (18.7% of survivors) were lost to follow-up due to inability to contact them post discharge. The proportion of losses in our study was similar to other follow-up studies.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">11</span></a></p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Mortality</span><p id="par0105" class="elsevierStylePara elsevierViewall">Global mortality during the period from ICU admission to the 12-month follow-up interview was 63.4% among MOF patients (45% global hospital mortality and 19% after discharge): 161 (30%) patients died in the ICU, 57 (15%) died on the hospital ward, and 38 (14%) died within the first 6 months following ICU discharge. Global mortality rates after the first 6 months were considerably lower, with 13 patients (5% of the initial sample) dying in the period between the 6-month and 12-month post-discharge interview.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Functional status</span><p id="par0110" class="elsevierStylePara elsevierViewall">According to the MRS, 81% of the sample had “good” (MRS 0–2) functional status prior to admission to the ICU. Six months after hospital discharge, this prevalence fell to 26% of the 228 survivors, of whom 24% were also able to go back to work or engage in most of their pre-admission activities. Twelve months after hospital discharge, the prevalence of “good functional status” increased considerably, to 66% of the 215 survivors. The GOS-E showed similar results, with 30% and 61% of the patients presenting “good” functional status (GOS-E 6–8) at 6 and 12 months after hospital discharge, respectively.</p></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Clinical symptomatology after discharge</span><p id="par0115" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the clinical symptoms and their duration over time (in months). The most common symptoms reported following hospital discharge were asthenia in 173 (76%) subjects, sleep disturbances in 112 (50%), and depression in 109 (48%). At 6 months, 109 (48%) of 228 MOF survivors reported at least mild depression. The most persistent symptoms were arthromyalgia in 10 (4–13) subjects, changes in urination in 9 (6–15), headaches in 9 (3–14), and disturbances in vision and speech in 10 (8–14) and 11 (8–14) subjects, respectively. In our study, all symptoms lasted on average between 9 and 11 months, except for depression and difficulty concentrating, which lasted on average for 3 and 4 months respectively. <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the variables that significantly increased or decreased the presence of certain residual symptoms over time.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">The global mortality rate of MOF patients admitted to the ICU was high, with almost half of all patients dying at some point after their ICU admission. While the majority died in the ICU (161, or 30%) or on the hospital ward (57, or 14.8%), almost 10% died after hospital discharge, and of these, 75% died within the first 6 months post-discharge. These in-hospital mortality rates are similar to those of other studies in patients with MOF, such as 50% to 60% of trauma patients,<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> 50% of patients with pancreatitis, or 30% of patients admitted for burns. The mortality rate after hospital discharge observed in our study is similar to or lower than the rates reported in other studies, which range from 7% to 19%.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">2,13,14</span></a> The strong correlation between severity of organ dysfunction and mortality observed in our study has also been suggested in other studies, where ICU mortality rates among patients with no organ dysfunction at admission were 6%, but as high as 65% in patients with 4 or more organ dysfunctions.</p><p id="par0125" class="elsevierStylePara elsevierViewall">While available studies on the recovery of functionality in critically ill patients in general have described that functional status tends to return to pre-hospitalisation levels within 6–12 months following ICU stay,<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">1,15</span></a> our results suggest a longer recovery period in MOF patients. Longer recovery periods have also been reported in other patient groups, such as those presenting multiple trauma.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">16</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Two new methods have been employed in this study. Firstly, the post-ICU functional status of MOF patients was specifically and directly evaluated by a third investigator, instead of using surrogate quality of life scores, which also include an element of patient satisfaction. Secondly, this is the first study to use GOS-E and MRS scales to assess this type of ICU patient at admission and at 6 and 12 months after discharge. Studies published to date using these measures have all involved neuro-critical patients,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a> which were excluded from this study. The GOS-E score is widely validated for use in structured interviews and is considered to give reliable results, even when compared with quality of life instruments, such as the QD-5.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">17,18</span></a> Additionally, the MRS scale has been shown to be a useful prognostic tool in assessing functional status in stroke patients.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">19</span></a> In our opinion, these prognostic instruments have many advantages, particularly ease of use and interpretation.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Many post-discharge follow-up studies on ICU survivors have shown the significance of long-term psychological and physiological consequences that affect quality of life. To date, residual clinical symptoms in MOF patients have only been described in general terms in the context of health-related quality of life questionnaires, such as the RAND 36-item Short-Form General Health Survey (SF-36),<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a> except for studies focusing specifically on depression,<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">2</span></a> post-traumatic stress disorder, and functional disability in general survivors of critical illness.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">3</span></a> We wished to investigate not only depressive symptoms, but also the specific clinical symptomatology found in MOF survivors, and whether it can be correlated to other factors related to the patient's stay in the ICU and their pre-admission status. Approximately 6% of survivors studied presented physiological and psychological problems: 20% reported severe limitations in functionality at 6 months, with these symptoms persisting at 12 months in 9% of study subjects.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Although asthenia (76%), sleep disturbance (50%), and depression (48%) were the most prevalent residual clinical symptoms, those most frequently reported at 12 months post discharge were myalgia/arthralgia, headache/migraine, vision and speech symptoms. Other symptoms prevalent during the first 6 months after discharge included sleep disorders and changes in bowel habits, in particular, constipation.</p><p id="par0145" class="elsevierStylePara elsevierViewall">At 6 months, 109 (48%) of the 228 MOF survivors reported at least mild depression. This is consistent with other recent studies that have reported depression in 37% of patients at 3 months and 33% at 12 months.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">2,3</span></a> Depressive symptoms were common, even among individuals without a history of depression.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The presence of certain symptoms appears to be significantly associated with particular variables. For example, the incidence of tremor increased in patients with more advanced age, a history of arterial hypertension, and a high Rankin score at 6 and 12 months, while it was less common in patients with a history of obesity or with better GOS-E scores at 6 and 12 months post discharge. Similarly, the incidence of asthenia increased in patients with a higher severity score at admission (SAPS II) and correlated with the duration of orotracheal intubation. Renal failure, duration of mechanical ventilation and prolonged hospital stay (on the ward the ICU) increased the incidence of sleep disturbance.</p><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0145">Methodological considerations and limitations</span><p id="par0155" class="elsevierStylePara elsevierViewall">Although the MRS has mainly been validated in trauma and stroke critical care patients, which were excluded from this study, we considered it to be an easy-to-use and objective tool for conducting structured telephone interviews. The post-ICU functional status of MOF patients offers a more specific and objective measure of functional status than quality of life scores, which are more focused on subjective measurements of patient satisfaction. In addition, the use of structured interviews to assign grades on the Rankin scale has been found to improve the quality of the scale by reducing inter-rater variability and bias.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a> Although the GOS-E has certain limitations, it has been validated in different populations, including trauma, cardiac arrest, and some medical and cardiothoracic patients,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">18</span></a> and there is evidence of its construct and criterion validity as a measure of recovery.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">19</span></a> Moreover, the GOS-E has also been extensively validated for use in structured interviews<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">21</span></a> and has recently been used in MOF trauma patients.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">We acknowledge the risk of recall bias arising from a retrospective assessment of physical function prior to the development of the condition that led to ICU admission. However, there is evidence that this can be reduced by using specific questionnaire techniques and there is great confidence in the use of retrospective measures, such as those used in this study.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">22,23</span></a> We are equally aware of the limitations of obtaining a large part of the data from surrogates. Unfortunately, the physical status of many patients prevented them from providing self-reported data. Nonetheless, the surrogates interviewed were the patient's next of kin or primary carers involved in the daily life and/or care of the patient, and therefore were the best source of information for the purposes of this study.</p><p id="par0165" class="elsevierStylePara elsevierViewall">Although the optimal time period in which to assess functional recovery after hospital discharge is still unclear, the majority of post-ICU quality-of-life studies are conducted between 6 and 12 months after discharge,<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">24</span></a> and very few have used longer follow-up periods.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">7,17</span></a> The former time frame, in which valuable prognostic information can be obtained with fewer resources, seemed reasonable to us.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Loss to follow-up was relatively high; 61 (18.7%) of the 327 known survivors could not be contacted. However, this proportion was expected, and is similar to other studies.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">11,25</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">In the case of subjects with concomitant physical illness, the BDI's reliance on physical symptoms, such as asthenia, may artificially inflate scores by evaluating symptoms of the illness, rather than of depression.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">26</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">The present study is a single centre study. Differences in ICU admission policies and case-mix may complicate direct comparison with other studies.</p></span></span><span id="sec0090" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0150">Conclusions</span><p id="par0185" class="elsevierStylePara elsevierViewall">Follow-up revealed frequent “residual” clinical symptoms, most notably arthromyalgia and asthenia, persisting for almost 12 months. The presence of symptomatology over time is mainly related to poor functional status at 6 and 12 months, length of hospital stay and severity score at admission. Depressive symptoms during the first 6 months after hospital discharge were common among multiple organ failure survivors.</p><p id="par0190" class="elsevierStylePara elsevierViewall">Clinical symptomatology and associated factors after ICU discharge provide valuable information for improving the quality of long-term follow-up programs for ICU survivors. Follow up in the form of a telephone survey regarding the patients clinical symptoms were conducted at 6 and 12 months after discharge from ICU.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0080" class="elsevierStyleSimplePara elsevierViewall">A total of 266 patients were followed up at both 6 and 12 months post ICU discharge; 62.2% were male; age 60<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>18 years; 67.8% medical patients. La presencia de síntomas depresivos tras los primeros 6 meses del alta poshospitalaria también fue común entre los pacientes supervivientes de FMO.
