Estudio prospectivo, comparativo de no inferioridad" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Laparoscopic cholecystectomy (LC) is the treatment of choice in the management of patients with manifestations secondary to symptomatic cholelithiasis,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">1</span></a> and is increasingly being performed in the outpatient setting both in Spain and other countries. Both the anaesthetic strategy and surgical technique used must be optimised in order to maximise the efficacy of the procedure in this setting. The choice of anaesthesia is pivotal to the success of outpatient laparoscopic cholecystectomy. Inhalational anaesthesia with desflurane and intravenous anaesthesia are some of the techniques most widely used in outpatient surgery.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">2,3</span></a> A recent review evaluating the impact of anaesthetic techniques on LC found no relevant differences between the anaesthetic regimens used. The studies reviewed measured aspects such as recovery times, morbidity, pain, presence of nausea and vomiting, and length of stay in the unit.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">4</span></a> However, the authors point out that other essential parameters, such as patient-perceived quality of recovery, were not shown. The evaluation of postoperative results on the basis of patient-perceived quality is gaining ground in scientific research and among clinical managers.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Advances in anaesthetic techniques and the development of minimally invasive procedures have reduced surgery-induced morbidity, thereby speeding up recovery and allowing patients to resume their daily activities. However, despite growing interest in patient-centred outcome measures, few studies have evaluated the effects of different anaesthetic techniques in LC by analysing the quality of recovery. One of the most widely used quality of recovery evaluation instruments is the QoR-15, a scale that has been shown to be effective in assessing the recovery of patients undergoing outpatient surgery. The QoR-15 evaluates 5 dimensions of health: physical comfort, physical independence, emotional state, psychological support and pain.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">7,8</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of our study was to compare quality of recovery assessed using the QoR-15 scale in patients undergoing outpatient LC in which anaesthesia included desflurane or propofol.</p><p id="par0020" class="elsevierStylePara elsevierViewall">We hypothesised that desflurane is no less effective than propofol in terms of patient-perceived quality of recovery in LC.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">After approval by the Institutional Reveiw Board (secretary Fernando Díaz Otero, Minutes n° 110/16, July 13, 2016) a longitudinal, single centre, prospective cohort study was performed in patients who underwent LC in the outpatient setting. One of the quality targets in our unit is to facilitate outpatient LC procedures, and for this purpose a project to improve the care of patients undergoing LC was introduced in 2015. The project included the implementation of pain management strategies, PONV prophylaxis, and the general evidence-based recommendations for the management of patients undergoing LC published by the PROSPECT working group.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">9</span></a> This optimisation protocol left the choice of maintenance anaesthesia to the discretion of the anaesthesiologist; in our unit, this is usually performed with desflurane or propofol. We designed a comparative study based on an analysis of data collected prospectively.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Patients with American Society of Anaesthesiologist (ASA) status I–III scheduled for outpatient LC were consecutively included in the study. Exclusion criteria were: allergy to any of the study drugs, or previous treatment with analgesics or antiemetics.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Once patients had been transferred to the surgery prep room, a peripheral venous catheter was placed, and they were pre-medicated with 1<span class="elsevierStyleHsp" style=""></span>mg IV midazolam. In the operating room, standard monitoring techniques were used and depth of anaesthesia was monitored using bispectral analysis (BIS) (BIS-VISTA™, Monitoring System, Aspect Medical System Inc. Norwood, USA). Anaestheisa was induced using the standard protocol in place in our unit: 2.5<span class="elsevierStyleHsp" style=""></span>mg/kg propofol, 1.5–2<span class="elsevierStyleHsp" style=""></span>μg/kg fentanyl and 0.6<span class="elsevierStyleHsp" style=""></span>mg/kg rocuronium to facilitate orotrachael intubation. The depth of neuromuscular block (NMB) was monitored with the TOFcuff monitor (RGB Medical Devices, Madrid, Spain) adjusting the administration of rocuronium to obtain deep NMB, with a post-tetanic count of 1–2. In the propofol group, anaesthetic maintenance was computer-controlled to obtain a stable concentration in the effect compartment (“central nervous system”). For this purpose, “target controlled infusion” software based on the Schnider et al.,<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">10</span></a> pharmacokinetic model was used (Alaris<span class="elsevierStyleSup">®</span> PK Cardinal-Health, 1180 Rolle, Switzerland). In the desflourane group, anaesthesia was maintained with desflurane in oxygen and air (40:60) at an end-tidal concentration of 5.5–7.5%, depending on the age of the patient. Desflurane was combined with continuous infusion of 0.05–0.25<span class="elsevierStyleHsp" style=""></span>μg/kg/min, remifentanil to maintain haemodynamic parameters to within 20% of baseline values. In both groups anaesthetic dose was adjusted to maintain a BIS in the range of 45–50.</p><p id="par0040" class="elsevierStylePara elsevierViewall">All patients received 8<span class="elsevierStyleHsp" style=""></span>mg bolusees of dexamethasone; patients with more than 2 risk factors for postoperative nausea and vomiting (PONV) on the Apfel scale (female, non-smoker, history of motion sickness and planned opioid administration in the postoperative period) received droperidol in boluses of 0.625<span class="elsevierStyleHsp" style=""></span>mg.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">11</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">At the end of surgery, the ports were infiltrated with 0.2% bupivacaine, remifentanil was discontinued and neuromuscular blockade was reversed with 2–4<span class="elsevierStyleHsp" style=""></span>mg/kg sugammadex, depending on the degree of residual blockade, to achieve a TOF ratio >0.9. All patients received 50<span class="elsevierStyleHsp" style=""></span>mg IV dexketoprofen for postoperative pain management.</p><p id="par0050" class="elsevierStylePara elsevierViewall">In the PACU, the nurse, who was blinded to the patient's anaesthesia group, evaluated postoperative pain and the presence of PONV using a verbal 11-point scale (0 no pain and 10 the worst pain imaginable). Paracetamol 1<span class="elsevierStyleHsp" style=""></span>g was administered in the case of mild to moderate pain with a numeric rating scale (NRS) score of ≥2–4, and for severe pain with a score of ≥5, fentanyl 50<span class="elsevierStyleHsp" style=""></span>μg was administered. This dose was repeated at 15<span class="elsevierStyleHsp" style=""></span>min intervals up to a maximum dose of 150<span class="elsevierStyleHsp" style=""></span>μg if severe pain persisted. For vomiting, 4<span class="elsevierStyleHsp" style=""></span>mg IV ondansetron was administered<span class="elsevierStyleMonospace">.</span></p><p id="par0055" class="elsevierStylePara elsevierViewall">The patients remained in the unit until they met the standard protocolised recovery criteria, when an Aldrete score of ≥9 was and a post-anaesthesia discharge score of ≥9 was achieved<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">12</span></a> (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). When these criteria had been met, the unit nurse consulted with the anaesthesiologist and the discharge was signed.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">Quality of recovery was evaluated using the QoR 15 questionnaire (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) administered before the patient entered the operating room and then 24<span class="elsevierStyleHsp" style=""></span>h after surgery by telephone. At discharge, patients were prescribed oral dexketoprofen 50<span class="elsevierStyleHsp" style=""></span>mg every 8<span class="elsevierStyleHsp" style=""></span>h and oral paracetamol 1<span class="elsevierStyleHsp" style=""></span>g every 8<span class="elsevierStyleHsp" style=""></span>h. Oral metamizole was prescribed as rescue analgesia.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0065" class="elsevierStylePara elsevierViewall">Statistical analysis was performed using SPSS 20 (IBM, USA). The Kolmogorov–Smirnov normality test was used to determine normal distribution of numerical variables. Data from both groups were compared using the independent samples <span class="elsevierStyleItalic">t</span>-test for quantitative variables, if the parameters were normally distributed. The Mann–Whitney <span class="elsevierStyleItalic">U</span> test was used in the case of non-normal distribution. Qualitative variables were analysed using the chi-square test or Fisher's exact test. Statistical significance was set at <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Rationale for sample size</span><p id="par0070" class="elsevierStylePara elsevierViewall">The primary endpoint was the overall QoR15 score at 24<span class="elsevierStyleHsp" style=""></span>h after surgery. We evaluated data from a pilot study that showed a difference of 2% between both groups with a standard deviation of 1. We considered, therefore, that for an alpha risk of 0.05 and a beta risk of 90%, and to detect a difference of 2% (3 points) with a standard deviation of 1, which corresponds to a non-inferiority margin of between −1 and 1, 18 patients woulde be needed in each group. More patients were recruited to each group to compensate for possible follow-up losses.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0075" class="elsevierStylePara elsevierViewall">A total of 61 patients were included in the study. Both groups were similar in terms of demographic parameters and associated comorbidity (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). All procedures were performed laparoscopically; open surgery was not required in any case.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">The duration of surgery and anaesthesia was higher in the propofol group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). With regard to intraoperative parameters, patients in the desflurane group received more fentanyl in the operating room than those in the propofol group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05), while patients in the propofol group required more fluids than those in the desflurane group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05) (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Postoperative recovery data are shown in <a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>. No significant differences were observed in pain score in the PACU, PONV, and length of stay in the unit. A higher, though not statistically significant incidence of PONV was observed in the desflurane group, and this group received higher doses of fentanyl than the propofol group.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">There were 5 cases of unforeseen admission, 3 in the desflurane group and 2 in the propofol group, all of them surgery-related due to bleeding and/or intraoperative finding of cholecystitis, requiring postoperative drainage.</p><p id="par0095" class="elsevierStylePara elsevierViewall">The baseline and 24-h QoR-15 scores are shown in <a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>. The most severely affected parameters in both groups were physical comfort (presence of moderate pain, and physical independence) and emotional state (feeling comfortable and in control). No significant differences between groups were observed in any of the parameters evaluated. The global QoR-15 score decreased with respect to baseline parameters by 7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.09% in the desflurane group and by 6.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.07% in the propofol group, with no significant differences between groups.</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">The main findings of our study have corroborated our hypothesis that there are no significant differences in patient-perceived recovery parameters between desflurane and propofol in out-patient LC. Our study has shown that LC performed with the study anaesthetic technique led to a ≈7% decrease in recovery quality score in both groups. Although various studies have compared both anaesthetic techniques, they have focussed on other postoperative aspects, such as the speed of emergence, extubation times, or the initiation of swallowing reflexes, among others.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">13–17</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">A recent study evaluated the impact of desflurane versus TIVA on the quality of recovery of female patients undergoing thyroid surgery using the QoR-40 scale,<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">18</span></a> a 40-item questionnaire with a maximum score of 200 that has been validated for use in both in-hospital and outpatient surgical procedures. The authors showed that patients in the TIVA group showed better recovery parameters at 24<span class="elsevierStyleHsp" style=""></span>h after surgery than those in the desflurane group. Overall postoperative scores decreased by 12% and 4% with respect to baseline in the desflurane and TIVA group, respectively. The most severely affected parameters were those that evaluate physical comfort, physical independence and psychological support. One of the factors with greatest impact on physical comfort is the presence of PONV, which was highly prevalent in the aforementioned study, presenting in 68% of patients in the desflurane group and in 45% in the propofol group in the first 24<span class="elsevierStyleHsp" style=""></span>h postoperative hours. The authors used ramosetron for PONV prophylaxis, a 5-hydroxytryptamine 3 receptor antagonist with a half-life of between 7 and 9<span class="elsevierStyleHsp" style=""></span>h,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> and, unlike our study, did not perform multimodal prophylaxis based on individual risk factors. We observed a far lower frequency of vomiting episodes, with 13% and 3% in the desflurane and propofol group, respectively reporting nausea in the PACU, with no episodes of vomiting. Interestingly, total fentanyl dose (operating room plus PACU) was higher in the desflurane group in relation to the propofol group, and although a higher percentage of patients in the propofol group required opioids in the PACU, this difference was not significant. We believe that the higher total dose of fentanyl could explain the higher reported rate of nausea in the PACU. However, the incidence of vomiting episodes at 24<span class="elsevierStyleHsp" style=""></span>h was similar between groups and with respect to baseline (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>, item 13).</p><p id="par0110" class="elsevierStylePara elsevierViewall">Other factors may explain the discrepancies between our results and those of the foregoing authors: their patients were all women (female gender is a risk factor for PONV), and although the percentage of women in our series was high (70%), it was still lower than the foregoing series. Furthermore, we adopted a multimodal PONV management strategy based on the presence of Apfel scale risk factors for PONV, and we also used dexamethasone in all our patients. Many studies have shown that dexamethasone, which has a biological half-life of 36–72<span class="elsevierStyleHsp" style=""></span>h, helps reduce vomiting episodes, and this was most probably the case in our series.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">20,21</span></a> Likewise, dexamethasone contributes to pain management and opiate saving. Finally, the aforementioned study focussed on thyroid surgery, while all the patients in our series underwent LC. Nevertheless, although the type of surgery has less impact on incidence of PONV than other factors, incidence of PONV in both thyroid surgery and LC is higher compared to other procedures.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">22</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">To the best of our knowledge, this is the first study to evaluate patient-perceived quality of recovery in outpatient LC using both propofol and desflurane. Today's patients demand a high degree of postoperative comfort due to the impact this will have on their quality of life after surgery, and it is therefore of the utmost importance to study postoperative recovery from their perceptive. The evaluation of the postoperative quality of recovery after surgery is one of the most highly rated results in current medical research.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Patients who undergo day surgery usually present a good state of health compared to other populations. This is further corroborated in our study, where baseline questionnaire scores were on average within 97% of the maximum total score. According to some authors, however, this generally healthy patient population can perceive the impact of surgery as more disabling compared to groups with greater pathology or chronic diseases.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">23</span></a> Taking these considerations into account, the results of our study can be considered very positive.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Limitations</span><p id="par0125" class="elsevierStylePara elsevierViewall">One of the limitations of our study is its non-randomised design. Nevertheless, both groups were homogeneous in terms of characteristics and comorbidity, and we were thus able to compare and test our variables. Another limitation is that the procedures were performed by a highly qualified major outpatient surgery team. This contributed to the good outcome observed in both groups, but limits the extent to which our findings can be extrapolated to teams with less experience in outpatient procedures.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conclusions</span><p id="par0130" class="elsevierStylePara elsevierViewall">The results of our study support the findings of other authors regarding the efficicy and safety of desflurane in patients undergoing outpatient LC. Patient-perceived quality of recovery was good in both the desflurane and propofol groups.
