However, they need to be further developed and perfected before they reach their maximum potential.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">7–10</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">IRS have been introduced in general health care (primary care and hospital care), in specialties (e.g., anaesthesiology) and for particular aspects of care (e.g., medication<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">11</span></a>). They have also been developed on a national, regional or local level by different institutions, scientific societies, associations, centres or other organisations that frequently work together. In Spain, the Ministry of Health, Social Security and Equality has developed a general IRS, the Patient Safety Reporting and Learning System (SiNASP).<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Anaesthesiology has been among the specialties spearheading the implementation of safe practices and in the introduction of IRS in hospitals. Spain and Germany have implemented anonymous, voluntary IRS in the field of anaesthesiology. In other European countries, such as Switzerland, Denmark, Finland or the United Kingdom, anaesthesia-related safety incidents are reported in a general register. In some countries, notification of critical incidents is mandatory.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">13</span></a> In Spain, anaesthesiology-related incidents are reported on the Spanish System of Safety Notification in Anaesthesia and Resuscitation (SENSAR),<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">14</span></a> managed by the Spanish Society of Anaesthesiology, Critical Care and Pain Management (SEDAR), and general incidents are reported on the Patient Safety Observatory (PSO), managed by the Andalusian Agency for Healthcare Quality (ACSA). SENSAR and PSO were launched in 2009.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Surgery is an important aspect of health care where several specialties and professional categories work together to care for a specific type of patient. Despite the considerable number of surgical interventions performed in our hospitals and the high incidence of surgery-related adverse events shown in the ENEAS<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">15</span></a> study (the third most frequent cause of adverse events, after medication and nosocomial infection), in the 2014–2015 SiNASP report, surgery-related incidents accounted for only 11% of all incidents,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">16</span></a> suggesting that many incidents in this area are not notified.</p><p id="par0025" class="elsevierStylePara elsevierViewall">In our hospital, which has been actively involved in promoting the notification of incidents in SENSAR,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a> the management of the Anaesthesiology and Critical Care (ACC) Clinical Management Unit (CMU) and the Surgical Block (SB) Quality Board proposed creating a multidisciplinary group headed by members of the local SENSAR group to analyse the surgery-related incidents that were reported on the PSO. The aim of this study has been primarily to determine the feasibility of the project, and secondly to assess the extent to which the different CMUs and healthcare professionals notify incidents, the circumstances and factors that may have contributed to each incident, and the steps taken as a result of the notification.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and method</span><p id="par0030" class="elsevierStylePara elsevierViewall">In our community, healthcare is organised around CMUs, which organise the work of doctors, nurses, clinical assistants, caretakers and administrative assistants. Our hospital has 27 CMUs, one of the largest of which is the ACC CMU, which includes the surgery suite and the post-anaesthesia recovery unit (PACU). In addition to this unit, the SB includes the General Surgery (GS), Orthopaedic and Trauma Surgery (OTS), Gynaecology and Obstetrics (GYN), Ophthalmology (OPH), Otolaryngology (ENT), Urology (URO), Vascular Surgery (VAS), and Dermatology (DERM) CMUs. In our hospital, a second level centre with around 600 beds, the SB has dedicated Medical and Nursing departments.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Given our hospital's role in SENSAR and the limited number of PSO notifications made by our CMUs, the head of the ACC CMU and the SB Quality Board appointed the 8 professionals they believed best qualified to analyse the adverse incidents arising in our setting. This team was made up of 4 members of the ACC CMU (2 SENSAR analysts and 2 nurses), 1 GS specialist, 1 GYN specialist, 1 OPH specialist and the head of the nursing department. In this way, the group included representatives from the Safety Board (with access to the PSO database), middle management, doctors, a CMU manager and the aforementioned head nurse. The incidents were selected from those notified and defined on the PSO system. We analysed the data in accordance with SENSAR's recommended method based on the Reason<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">18</span></a> model and the contributing factors analysis model developed by Vincent et al.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">19</span></a> The characteristics of SENSAR and PSO are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 1</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">Up to 3 monthly meeting lasting 30–40<span class="elsevierStyleHsp" style=""></span>min were scheduled at 8 am on Thursdays in the surgery suite to enable team members to resume their daily activity without delay. Before forming the study team, the Patient Safety Board invited the hospital's CMUs to attend training sessions on reporting incidents on the PSO. In addition to this, members of SENSAR attending the first 2 study meetings to train the remaining team members in PSO incident analysis techniques.</p><p id="par0045" class="elsevierStylePara elsevierViewall">For the purpose of this cross-sectional descriptive observational study, all the data obtained from the incidents reported and analysed from March 2015 to March 2017 were entered into a dedicated database.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The types of incidents were grouped according to the 15 categories included in the PSO system (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 1</a>). Each incident was analysed to determine the existence of errors or unsafe actions (such as distractions or mistakes), and whether these could have been avoided. The timing of the incident was classified as pre-, intra-, or postoperative, and the setting as inside or outside the operating room. A surgical patient was defined as any patient included in the surgery schedule. The notifier estimated the effect of the incident on the patient (no harm, minor morbidity, intermediate morbidity or major morbidity) and also the risk of each incident using a 2-factor weighting matrix: severity and probability of presentation. Latent factors that contributed to the incident were grouped in relation to the patient, the individual, the procedure, the medical team, the workplace and the organisation, according to the analysis used in SENSAR. Whenever possible, mitigating factors that minimised or limited the incident were also reported. The actions/measures stemming from the analysis of the incident were classified as: meeting/communication, presentation in a meeting, safety alert, creation/review of procedures/protocols/regulations and preparation of reports.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Qualitative data were described as frequencies and percentages, and the number of factors involved in an incident as mean and standard deviation. The correlation between variables was analysed using the Chi-square test or the non-asymptotic Monte Carlo and Fisher tests. The number of latent factors in incidents reported by the ACC CMU vs. other CMUs was compared using the non-parametric Mann–Whitney <span class="elsevierStyleItalic">U</span> test. The average number of factors involved in each type of incident was compared using the nonparametric Kruskal–Wallis test. The statistical analysis was performed using IBM SPSS 23.0 for Windows.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">Over the study period (March 2015–March 2017), 95 surgery-related incidents were reported on the PSO (45 the first year and 50 the second). This accounted for 43% of all incidents reported on the system by the hospital's 27 CMUs over that period. The CMUs reporting the highest percentage of incidents were ACC, with 46 incidents (48.4%) followed by GS, with 21 (22.1%) (differences between the distribution of the incidents by CMU were significant; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.025). The percentage of incidents reported by doctors and nurses also varied according to CMU: compared with other CMUs, in the ACC CMU more incidents were reported by doctors than by nurses (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0005). Only 4 incidents were notified by professionals or members of the public. Three incidents reported by the Gastroenterology CMU, the Haematology (HEM) CMU and an unidentified CMU were included because they affected surgical patients (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 2</a>). Compared with other professionals, doctors from the ACC CMU were more likely to use the full PSO form instead of the abbreviated version to report incidents (80.6% of incidents in their CMU, compared with 33.3% of doctors and 11.5% of nurses from other units).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Ten out of the 15 possible categories were represented in the incidents reported. The incidents most often reported were associated with care processes (30.5%), followed by those related to medication (also 30.5%), documentation, hospital administration, and falls (11.6%) (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 3</a>). While incidents related to care processes and medication occurred mainly in the surgical suite (58.6% and 63.6%, respectively), those related to hospital administration, documentation and falls occurred in other areas.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Overall, most incidents occurred in the surgical suite (48.4%), usually during the preoperative period (42.1%) (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 3</a>). However, more incidents occurring in the intraoperative period were notified by the ACC CMU than all the other CMUs together (47.8% versus 18.4%). With regard to the setting, 76.1% of incidents reported by the ACC CMU occurred in the surgical suite compared to 22.4% reported by other CMUs, where most incidents occurred outside the surgical suite (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 3</a>). Comparing the type of incident reported according to CMU and notifier shows that in the ACC CMU doctors report more incidents involving clinical procedures than nurses (95% vs. 5%), while nurses reported more incidents involving professional conduct and documentation (75% vs. 25% and 100% vs. 0%, respectively). This difference was significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.03). There were no significant differences in this regard in the remaining CMUs.</p><p id="par0075" class="elsevierStylePara elsevierViewall">In terms of harm, 88.4% of reported incidents caused no harm or minor morbidity. None caused major harm involving permanent or long-term injury (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 3</a>). In 2 incidents where the patient was not harmed, the incident led to the surgical procedure being suspended and rescheduled. Examples of intermediate morbidity were intoxication by local anaesthetics that led to the hospitalisation of an outpatient, a fall suffered by a patient after transfer to the ward after surgery, who required reoperation, or the unavailability of specific instruments for the removal of osteosynthesis material, which also led to reoperation.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Notifiers estimated 78.9% of incidents to represent a moderate or high risk for the patient. Similarly, errors were identified in 78.9% of incidents, mainly associated with various types of mistakes (such as unfamiliarity with certain situations and failure to follow safe practice guidelines) that were considered avoidable (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 3</a>). There were no significant differences between the errors identified in incidents reported by doctors and nurses from the ACC CMU.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Each incident involved an average of 2.27 contributing factors; most were doctor/surgeon/nurse-related (individually or as a team), while a smaller proportion were patient-related or procedure-related (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 4</a>). More contributing factors were identified in incidents reported by the ACC CMU compared to those reported by the remaining CMUs (2.80 vs. 1.83). The same was true of mitigating factors (identified in 78.3% of incidents reported from the ACC CMU vs. 48.9% from other CMUs; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005) (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 4</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">After analysing the incidents, the group took the measures listed in <a class="elsevierStyleCrossRef" href="#tbl0030">Table 5</a>. When the incident warranted a structured report, these were sent to the managers of the CMUs involved to enable them to implement the required measures (<a class="elsevierStyleCrossRef" href="#sec0030">Appendix A</a>). Feedback from the incidents notified was provided in periodic clinical meetings held in the ACC CMU–weekly in the case of nurses and 6-monthly in the case of doctors. All available information relating to the incident was provided at the start of each meeting. In the other CMUs, patient safety meetings were held at the discretion of the professionals in charge.</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">Incidents were reported on the PSO over the 2-year study period. Most incidents were reported by professionals attached to the ACC CMU, no doubt due to their experience with SENSAR. Both notification systems are used in our hospital. During their patient safety meetings, the local SENSAR group evaluate whether a particular incident warrants notification on the PSO. In many cases, the group decided to report the incident on the PSO, and in doing so significantly increased the overall number of incidents reported. Similarly, some medication-related incidents brought to the attention of the Pharmacy CMU were in turn reported by this unit on the Spanish Reporting and Learning System for Medication Errors (ISMP-Spain), to which the PSO contributes. Interestingly, the type and characteristics of the incidents reported varied according to the professional category of the notifier and to their CMU.</p><p id="par0100" class="elsevierStylePara elsevierViewall">It is not surprising that the CMUs that make up the multidisciplinary analytical group were most active in reporting incidents, disseminating the information received, and encouraging their colleagues to report safety-related incidents. The fact that several units, including the ACC, GS, GYN and ENT CMUs were involved in an ACSA accreditation processes could also have served as an incentive in this regard. We believe that the differences observed among CMUs, such as the preponderance of surgical suite-related incidents reported by members of the ACC CMU compared with other CMUs, is probably due to the fact that members of the ACC CMU spend much of their time in this area. SENSAR is particularly focussed on incidents related to anaesthesia administration in the intraoperative period. Although ACC CMU staff notified the PSO of more incidents relating to the surgical procedure itself, overall (taking all CMUs together) anaesthesia administration is classed as a preoperative event, and thus provides a broader picture of the different situations that can affect surgical patients. Similarly, although medication-related incidents seem to predominate in the reports made by anaesthesiologists,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a> when these are added to the reports made by other SB staff, as in our case, the incidents most frequently reported involved clinical or care procedure. For example, falls, which are rare in the surgical suite, were among the commonest incidents occurring on the ward.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Aggregating the data from all the CMUs increased the notification rate and highlighted aspects that could have been overlooked when analysing the data individually. A trend analysis of the number of incidents reported by UK hospitals on the National Reporting and Learning System, based on data from the National Patient Safety Agency (NPSA) and a questionnaire completed by health professionals, revealed that the higher the reporting rate, the greater the positive effect on safety culture.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">6</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">IRS are known to have certain limitations, and these could have affected our study. Generally speaking, IRS provide incomplete information that may be ambiguous, and in some cases irrelevant.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">21</span></a> This is inevitable, as notifiers report their own personal, limited perspective of a complex clinical and organisational situation. Some authors have even questioned whether notifiers are really aware of which incidents are or are not reportable.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">22</span></a> The voluntary nature of the system together with a reluctance to report incidents with important repercussions for the patient due to a fear of disciplinary and legal consequences,<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">23</span></a> could also limit the use of such systems. IRS are not suitable for monitoring progress made in preventing adverse events.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">24</span></a> This is because, being voluntary, they are used to construct indicators and do not provide the total number of incidents needed for the denominator. Despite all these limitations, hospitals encourage their staff to use IRS, and hospital managers continue to rely on them as a source of information and a means of detecting opportunities for improvement.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">21</span></a> They also complement and are compatible with other strategies to improve patient safety.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">In our hospital, the existence of SENSAR appears to have encouraged more doctors to participate in the system because, anaesthesiologists aside, few incidents were reported by other specialties. In general IRS, most incidents are reported by members of the nursing staff (the largest CMU in our centre).<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">22</span></a> This was particularly true of GS, and the number of incidents reported from this CMU doubled in the second year of our study (24 vs. 12 in the first year). Nurses are often rotated around different departments, so we cannot be sure whether the increase in reporting was related to ACC nurses being transferred to GS. Important differences in the number of staff and the number of interventions performed in the different SB management units must also be taken into account when analysing notification trends.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The repercussion of the incidents reported was consistent with IRS as a whole, with most being innocuous or causing only minor injury.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">24</span></a> It was interesting to note that among the innocuous incidents reported, only 2 required surgery to be suspended: one involved a failure to perform the scheduled pre-surgical bowel preparation, and the other was related to the instillation of the wrong eye drops in eye surgery. These data are interesting, and could be included in surgical IRS. In addition to being more numerous, innocuous incidents are valuable because the lessons learned from these events can also help prevent harmful incidents, as a similar error process is often involved in both.</p><p id="par0125" class="elsevierStylePara elsevierViewall">The analysis of contributing and mitigating factors is also interesting. On the one hand, differences the identification of these factors among units could be due to the fact that many notifications were made by anaesthesiologists. These specialists chose the complete PSO form (more similar to that of SENSAR), as thus provided more detailed information on the incident. On the other hand, individual-related latent factors (where the health professional was directly related to the incident) were more common than procedure-related factors (absence of or poorly designed protocols, procedures or rules). These factors were related to unsafe actions caused by distractions and, above all, to mistakes. We believe that this shows the need to promote training, encourage learning, and disseminate safe practices among the health professionals in our SB. Although very few procedure-related factors were identified, 21 protocols were created or updated as a results of IRS notifications over the study period.</p><p id="par0130" class="elsevierStylePara elsevierViewall">IRS feedback to both the hospitals and the professionals involved is essential. This information will improve performance and promote the use of the system. The system is believed to improve leadership, the credibility and content of information, effective dissemination channels, the capacity for rapid action and the impact of incident reporting at all levels of the organisation.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">25</span></a> The regular feedback provided to ACC CMU professionals has resulted, in the short term, in the release of safety alerts, and in the long term in a series of safety meetings in which doctors were informed of the specific incidents notified and the measures taken once these issues had been analysed. The nursing staff, meanwhile, holds weekly meetings to inform their members of the incidents notified. A recommendation was sent to the other CMUs to hold safety meetings to report and analyse IRS incidents, but we cannot be sure that these have been held, as this variable was not monitored. We believe that feedback may have been scarce in the other units, and this could have affected the notification rate. Despite this, we believe that response to this novel initiative in our setting has been interesting and has given healthcare professionals and managers the chance to engage in an activity that is relatively unknown and underused in most SB CMUs. We also believe that the composition of the analysis group, which included middle management and nursing staff, has helped increase the impact of the system and the measures adopted and proposed after the analysis were taken into account by the management of each unit. It would be impossible to implement a project such as this without the support of the hospital management and the help of the different SB CMUs in nominating the members of the study group. This “institutional” effort undoubtedly facilitated consolidation of the system.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Improving communication and understanding the benefits of teamwork among professionals helps strengthen a safety culture.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">26</span></a> We cannot claim equivocally that this initiative in our hospital has improved the safety culture in the SB, as this variable was not specifically analysed. However, evidence shows that the incident notification rate, far from decreasing, actually increased over the course of the study.</p><p id="par0140" class="elsevierStylePara elsevierViewall">To conclude, we believe that although this experiment in multidisciplinary collaboration in the field of surgery was limited to a single hospital, the results have been positive, and the project can be improved and introduced into other hospitals and patient populations. The aggregate analysis of the incidents has allowed us to detect differences based on the identity of the notifier and the unit to which they are attached. In this way, we can obtain a wider, more cross-sectional perspective of the difference situations that can affect patient safety. The positive attitude towards incident notification among ACC CMU staff has enabled this initiative to be introduced and maintained in the SB, and will allow it to spread gradually to other units now and in the future. Using the experience gained with SENSAR, we evaluated the feasibility and the activity of a multidisciplinary group analyzing incidents in the surgical patient notified to a general community system, that of the Observatory for Patient Safety (OPS).</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and method</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Cross-sectional observational study planned for two years. After training in the analysis, a multidisciplinary group was created in terms of specialties and professional categories, which would analyse the incidents in the surgical patient notified to the OPS. Incidents are classified and their circumstances analysed.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Between March 2015 and 2017, 95 incidents were reported (4 by non-professionals). Doctors reported more than nurses, at 54 (56.84%) vs. 37 (38.94%). The anaesthesia unit reported most at 46 (48.42%) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.025). The types of incidents mainly related to the care procedure (30.52%); to the preoperative period (42.10%); and to the place, the surgical area (48.42%). Significant differences were detected according to the origin of the notifier (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.03). No harm, or minor morbidity, constituted 88% of the incidents. Errors were identified in 79%.
