protocols are designed to optimise care processes and facilitate rapid recovery after cardiac surgery&#44; including the pre-&#44; intra- and postoperative periods&#46;<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">1&#44;2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The increase in the number of surgeries performed&#44; together with the need to maximise efficiency in clinical practice&#44; has led to the implementation of safe&#44; fast track strategies in cardiac care&#44; such as anaesthesia with a combination of low-dose&#44; short acting hypnotics and low-dose opioids<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">3</span></a> that permits early postoperative extubation&#46;<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">4&#8211;6</span></a> The latter strategy &#40;EuFT&#41; is now considered to be one of the cornerstones of efficient practice&#44; and different studies have shown that it can reduce hospital stays and time spent in the intensive care unit &#40;ICU&#41;&#44; and has a direct impact on costs and care quality&#46;<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6&#8211;8</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Despite all the scientific evidence&#44; protocols vary greatly between hospitals&#44; and fast track and EuFT criteria and step by step protocols have yet to be standardised on an international level&#46; The aim of this study&#44; therefore&#44; has been to describe and compare the duration of ICU and hospital stay together with postoperative mortality rates in patients undergoing EuFT vs&#46; conventional extubation &#40;CE&#41; after cardiac valve surgery&#44; closure of cardiac defects&#44; coronary bypass and mixed surgery in a cardiovascular reference centre in Colombia&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">This was a retrospective&#44; longitudinal&#44; analytical study in patients aged over 18 years undergoing valvular surgery&#44; closure of cardiac defects&#44; coronary bypass or combined surgeries using both conventional and minimally invasive techniques&#46; All patients treated in a tertiary level hospital in Colombia from April 2009 to December 2015 were included in the study&#46; Patients undergoing reoperation and those that did not survive surgery were excluded&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">The study was evaluated by the Research Ethics Committee of the Universidad Pontificia Bolivariana&#44; and was approved on 23 May 2016&#46; The authors have safeguarded the confidentiality of all patient data&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">No fast-track measures were used in patients who received EuFT&#46; The following standard hospital anaesthetic protocol was followed in all cases&#58; anaesthetic induction with midazolam&#44; fentanyl and vecuronium&#59; intubation and invasive monitoring with arterial line and central venous catheter&#59; anaesthesia maintained with fentanyl infusion and inhalational sevoflurane&#46;</p><p id="par0035" class="elsevierStylePara elsevierViewall">The decision to extubate was not guided by an established protocol&#44; but instead was based on intraoperative haemodynamics and ventilation&#46; The opioid and muscle relaxant strategy did not depend on whether the patient was extubated in the operating room or the ICU&#46; For statistical purposes&#44; patients who were extubated at the end of the surgery&#44; that is&#44; in the operating room before being transferred to the ICU&#44; were assigned to the EuFT group&#46; Patients extubated in the ICU were assigned to the CE control group&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">All the relevant variables were recorded in the database according to the duration of surgery&#46; Preoperative variables included clinical and demographic data&#44; such as age&#44; sex&#44; weight&#44; height&#46; Also included were heart disease-related comorbidities&#44; such as hypertension&#44; diabetes&#44; chronic obstructive pulmonary disease&#44; congestive heart failure&#44; valvular heart disease&#44; congenital heart disease&#44; kidney failure&#44; peripheral vascular disease&#44; previous ejection fraction&#44; coronary involvement&#44; preoperative risk classification according to the American Society of Anesthesiology &#40;ASA&#41;&#44; functional status according to the New York Heart Association &#40;NYHA&#41;&#44; and airway evaluation&#46; Data from the intraoperative period included dose of midazolam&#44; fentanyl and vecuronium&#44; infusion time&#44; aortic clamping time&#44; volume of intraoperative fluids and red blood cells&#44; plasma&#44; cryoprecipitate and platelets administered&#46; The surgical technique was recorded as minimally invasive surgery or off-pump surgery&#44; and Swan-Ganz right heart catheterization&#46; Postoperative variables included length of hospital and ICU stay&#44; cardiac &#40;myocardial ischaemia&#41;&#44; kidney &#40;acute kidney injury&#41;&#44; neurological &#40;stroke&#41; or respiratory &#40;pulmonary vascular congestion shown on postoperative chest X-ray&#41; complications&#44; need for vasopressor or vasodilator drugs&#44; and postoperative mortality&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall">To classify outcomes&#44; myocardial ischaemia was defined as postoperative ST segment depression and T wave inversion&#44; and postoperative inotropic support was defined as the need for norepinephrine at a starting dose of 0&#46;05<span class="elsevierStyleHsp" style=""></span>mcg&#47;kg&#47;min to maintain mean arterial pressures above 70<span class="elsevierStyleHsp" style=""></span>mmHg&#46;</p><p id="par0050" class="elsevierStylePara elsevierViewall">The data were processed and analysed using SPSS&#44; version 20&#46;0&#46; Statistical significance was set at 5&#37; &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>0&#46;05&#41;&#46; The Kolmogorov&#8211;Smirnov and Shapiro&#8211;Wilk tests were used to evaluate the normality of the different continuous variables of both groups as a whole&#44; and of the different subgroups&#44; namely&#44; EuFT and CE&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">Clinical&#44; sociodemographic and surgical variables were described as appropriate&#46; Normally distributed continuous variables were described as mean plus standard deviation&#59; all the remaining variables were described as median plus interquartile range&#46; In the case of nominal variables&#44; the percentage of individuals with and without the characteristic evaluated was calculated&#46; Normally distributed continuous variables were compared between groups &#40;EuFT and CE&#41; using the Student&#39;s <span class="elsevierStyleItalic">t</span> test&#46; Comparisons between non-normally distributed variables were performed using the Mann&#8211;Whitney <span class="elsevierStyleItalic">U</span> non-parametric test&#46; The Chi-square test &#40;<span class="elsevierStyleItalic">&#967;</span><span class="elsevierStyleSup">2</span>&#41; or Fisher&#39;s exact test were used on categorical variables&#44; and the prevalence ratios with their respective 95&#37; confidence intervals &#40;CI&#41; were calculated&#46; Finally&#44; to estimate the effect size measure as an odds ratio &#40;OR&#41; calculated using logistic regression&#44; and to identify the effect of the extubation strategy on each clinical outcome evaluated &#40;cardiac &#91;myocardial ischaemia&#93;&#44; kidney &#91;acute kidney injury&#93;&#44; neurological &#91;stroke&#93; or respiratory &#91;pulmonary vascular congestion shown on postoperative chest X-ray&#93; complications&#44; need for vasopressor or vasodilator drugs&#44; and postoperative mortality&#41;&#44; we performed a stepwise multivariate analysis that included all variables that were shown to be statistically significant in the bivariate analysis of extubation groups and clinical&#44; demographic and intraoperative variables&#46;</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a>&#44; a total of 396 patients undergoing valvular surgery&#44; closure of cardiac defects&#44; coronary bypass or combined surgery were included in the study&#46; Of these&#44; 207 underwent EuFT and 189 CE&#46; The sample selection protocol is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a>&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#44; the groups were not evenly matched&#44; due to statistically significant differences in age&#44; ejection fraction&#44; NYHA classification and history of congenital heart disease&#44; diabetes and hypertension&#46; We found that patients undergoing EuFT were generally younger&#44; with a better functional class&#44; fewer comorbidities&#44; and better ejection fraction&#46; Only 17&#46;4&#37; of patients undergoing EuFT had an ejection fraction of less than 45&#37;&#44; while the ejection fraction of 29&#37; of subject undergoing CE was even lower&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">With respect to intraoperative variables shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#44; statistically significant differences were found between both groups in the percentage of transfusions&#58; only 14&#46;4&#37; of EuFT patients were transfused vs&#46; 28&#46;9&#37; of patients with CE&#46; EuFT was performed in a higher percentage of patients undergoing minimally invasive surgery and heart defect surgery&#44; while CE was performed in a high percentage of patients undergoing combined surgery&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Although no statistically significant differences were found between groups in terms of postoperative mortality&#44; the CE group presented higher cardiovascular morbidity&#44; namely myocardial ischaemia &#40;95&#37; CI&#58; 0&#46;37&#8211;0&#46;86&#59; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;01&#41;&#44; and greater need for postoperative vasopressor support &#40;95&#37; CI&#58; 0&#46;18&#8211;0&#46;49&#59; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;01&#41;&#46; As shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>&#44; after adjusting multivariate statistics for the influence of other variables on clinical outcomes&#44; we found that EuFT maintained its statistical independence and was associated with a lower incidence of cardiac complications&#44; which were higher in the CE group &#40;95&#37; CI&#58; 0&#46;28&#8211;0&#46;73&#59; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;001&#41;&#44; and less need for postoperative vasopressor support &#40;95&#37; CI&#58; 0&#46;23&#8211;0&#46;71&#59; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;002&#41;&#46;</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">In order to estimate effect size&#44; ORs were obtained by binary logistic regression for each of the following dependent variables&#58; postoperative mortality&#44; postoperative respiratory&#44; neurological&#44; kidney&#44; and cardiovascular complications&#44; need for postoperative vasopressor drugs&#44; and need for postoperative vasodilator drugs&#46; All clinical&#44; demographic and surgery-related covariables found to be statistically significant in the bivariate comparison of groups&#44; namely&#44; age&#44; ejection fraction&#44; history of congenital heart disease&#44; diabetes&#44; hypertension&#44; NYHA classification&#44; type of surgery&#44; minimally invasive surgery&#44; and total number of transfusions&#44; were included&#46; <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the clinical outcomes according to extubation group&#44; expressed both as gross OR values and OR values adjusted for the aforementioned post-multivariate analysis variables&#46;</p><p id="par0085" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>&#44; no statistically significant differences were found in respiratory&#44; neurological or kidney complications&#46; Length of hospital stay was similar in both groups&#58; a total of 6 days &#40;IQR 5&#8211;8 for EuFT and IQR 5&#8211;10 for CE&#44; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;16&#41;&#44; as was overall length of ICU stay &#40;24<span class="elsevierStyleHsp" style=""></span>h&#41;&#44; with no statistically significant differences between groups &#40;RIQ 18&#8211;36 for EuFT and RIQ 24&#8211;48 for CE&#44; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;09&#41;&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">Although patients extubated in the operating room &#40;EuFT&#41; presented a lower comorbidity burden and better pre-operative functional status than the CE group&#44; EuFT remained an independent factor for a lower rate of postoperative myocardial ischaemia and less need for postoperative vasopressor drugs&#46; In terms of the 2 latter outcomes&#44; the multivariate analysis specifically showed that&#44; in addition to the type of extubation&#44; only minimally invasive surgery remained a statistically independent beneficial factor for postoperative myocardial ischaemia&#44; with an adjusted OR of 0&#46;56 &#40;95&#37; CI&#58; 0&#46;35&#8211;0&#46;89&#41;&#46; In the same analysis&#44; in addition to the site of extubation&#44; only the type of surgery remained a statistically independent beneficial factor for the need for postoperative vasopressor drugs&#46; Patients undergoing combined surgery presented greater need for vasopressor drugs&#44; with an adjusted OR&#44; taking as a reference point closure of defects&#44; which was the procedure with the least vasopressor requirement &#40;OR 6&#46;6&#44; 95&#37; CI&#58; 1&#46;46&#8211;29&#41; and history of diabetes &#40;adjusted OR 3&#46;6&#44; 95&#37; CI&#58; 1&#46;6&#8211;8&#41;&#46;</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">We believe that EuFT after cardiovascular surgery in patients with no major comorbidities &#40;good ventricular function&#44; ejection fraction &#62;45&#37;&#44; aged under 75 years&#44; and no lung disease<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">5</span></a>&#41; is a valuable anaesthesiology strategy for improving patient safety&#46; Although it had no impact on postoperative mortality&#44; it was associated with fewer cardiac complications&#44; such as myocardial ischaemia&#44; and a less need for postoperative norepinephrine&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">Studies in fast-track surgery and EuFT have shown that early extubation alone has only a marginal benefit on costs&#44; and is not <span class="elsevierStyleItalic">per se</span> a valuable strategy&#46; The best savings are achieved by adopting different recovery strategies&#44; such as complete fast-track and EuFT protocols that include pre-&#44; intra- and postoperative measures&#59; these will have the most important impact in the future&#46; This means that switching to fast-track anaesthesia must be accompanied by changes in patient recovery models and strategies&#46; This study has shown that EuFT does not reduce the length of hospital stay&#44; ICU stay&#44; or mortality&#44; and therefore has no significant impact on care costs&#46;</p><p id="par0105" class="elsevierStylePara elsevierViewall">The clinical evidence in this regard has shown that fast-track protocols<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">10</span></a> consistently reduce ICU<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6&#44;8&#8211;10</span></a> and hospital stays<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6&#44;8</span></a>&#59; however&#44; none of these studies has managed to find statistically significant differences between patients who received EuFT and CE<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">11</span></a> in terms of outcomes&#44; including risk of postoperative mortality in the ICU<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6&#8211;8</span></a> risk of 30-day mortality&#44;<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">5&#44;9</span></a> risk of myocardial ischaemia&#44;<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6&#44;8</span></a> and risk of reintubation at 24<span class="elsevierStyleHsp" style=""></span>h after surgery&#46;<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6&#44;8</span></a> A study<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">12</span></a> in 100 patients showed a shorter hospital and ICU stay in the EuFT group&#44; with a 53&#37; reduction in intensive care costs&#44; and 25&#37; in surgical costs compared to the CE group&#46; The meta-analysis published by Myles<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">7</span></a> in 1800 patients from 10 studies&#44; found a reduction in ICU stay and a non-significant reduction in hospital stay and mortality&#46; In a recent study&#44; Richey<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">13</span></a> found no reduction in the length of hospital stay between the EuFT and CE groups &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;16&#41;&#44; no reduction of ICU stay &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;09&#41;&#44; and no difference in mortality &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;63&#41;&#46; Furthermore&#44; we believe that the timing of extubation is not just a measure of care quality&#44; but should also be used to evaluate outcomes&#46;<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">14</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">In terms of other parameters&#44; in contrast to studies that found no statistically significant differences in the risk of myocardial ischaemia&#44;<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6&#44;8</span></a> we found a higher risk in the CE group &#40;95&#37; CI&#58; 0&#46;37&#8211;0&#46;86&#59; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;01&#41; and observed minimally invasive surgery and EuFT to be protective factors for postoperative myocardial ischaemia&#46; However&#44; we found a history of diabetes&#44; CE&#44; and combined surgery to be risk factors for a greater need for vasopressive drugs&#46;</p><p id="par0115" class="elsevierStylePara elsevierViewall">Fast-track extubation protocols include extubation in the first 6 postoperative hours&#44;<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">13&#44;14</span></a> which includes extubation in the operating room&#46; In some hospitals&#44; the ICU is located at a distance from the operating room&#44; so all the risks involved in the transfer must also be taken into consideration&#46; In these circumstances&#44; therefore&#44; it is preferable to transfer the patient to the ICU before extubation to avoid airway complications during transfer&#44; such as respiratory depression&#44; residual relaxation&#44; bronchoaspiration&#44; or laryngospasm&#46; The decision to extubate must be taken on an individual basis&#44; taking into account the extent to which hospital infrastructure can optimise and guarantee patient safety&#46;</p><p id="par0120" class="elsevierStylePara elsevierViewall">Finally&#44; despite the limitations of this study and the fact that the full fast-track protocol was not used&#44; fast-track strategies in cardiac surgery reduce the rate of postoperative complications&#44; particularly cardiovascular morbidities&#44; and the need for post-operative vasopressor support&#46; More prospective&#44; randomised studies focussed on exposure to the intervention will contribute higher level evidence that will enable these results to be generalised to the entire population of cardiac surgery patients&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conflicts of interest</span><p id="par0125" class="elsevierStylePara elsevierViewall">The authors have no conflicts of interest to declare&#46;</p></span></span>"
