MAS has also helped increase patient satisfaction and reduce surgical waiting times.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">1</span></a> In recent decades, laparoscopic surgery has emerged as the technique of choice in gallbladder surgery and has obtained good results in a variety of benign gynaecological disorders.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">2,3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">A number of risk factors that can lead to unplanned re-admissions and complications in this type of intervention have been described: age over 50 years, American Society of Anesthesiologists (ASA) class higher than III, start of surgery after 13:00 pm, long surgery, or previous abdominal surgery.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">4–7</span></a> Good patient selection increases the success of these interventions and reduces complications. Perioperative management is also important, since up to half of unplanned admissions are caused by inadequate pain control, and around 25% are due to postoperative nausea and vomiting (PONV).<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">1</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Pain and PONV are closely related. Untreated postoperative pain is an important cause of PONV, and the use of opioids for pain management can also cause PONV.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">8</span></a> Pain is one of the most important reasons for delaying discharge both immediately after MAS and at 24 postoperative hours.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">9</span></a> Severe pain caused by a variety of factors<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">11,12</span></a> can present in up to 40% of patients undergoing laparoscopic surgery.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">10</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">PONV contributes to the development of complications, consumes resources, and require longer-lasting care.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">8</span></a> Incidence of PONV in laparoscopy in general is high (20–51%), and even higher (50–80%) in gynaecological laparoscopy.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">8,13–15</span></a> Risk of PONV will increase depending on the characteristics of the patient, factors intrinsic to the intervention and the anaesthesia administered, and pain intensity and pain management strategies in the immediate postoperative period. The aetiology of PONV has not been clearly established, but appears to involve several factors, such as residual pneumoperitoneum after CO<span class="elsevierStyleInf">2</span> insufflation, use of inhalational agents/opioids, and phase of the menstrual cycle.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">13</span></a> The high incidence of PONV after laparoscopy calls for studies to determine the best anaesthetic technique to minimise PONV.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">8</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Although postoperative pain and PONV are minor post-laparoscopy complications, they are time-consuming and have a major impact on the quality of recovery in MAS. The aim of this study has been to determine the perioperative factors and patient characteristics that contribute to the appearance of postoperative pain and PONV in ambulatory laparoscopic surgery.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Materials and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">This was a prospective study in a cohort of adult patients undergoing outpatient laparoscopic surgery at the Universitari i Politècnic La Fe Hospital in Valencia between May 2015 and November 2016. The project was approved by the hospital's Scientific Committee and Research Ethics Committee.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The patients, 297 in total, were selected consecutively using the following inclusion criteria: age over 18 years, and indication for outpatient laparoscopic surgery. Exclusion criteria were: patients with insufficient data for the study, and patients who refused to participate after receiving information on the study. Patients with previous abdominal surgery were not excluded. The sample size corresponds to a confidence level of 95%, a precision of ±5.7%, and an estimated frequency of postoperative symptoms of 50%, in line with results of previous studies.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">8,10</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">The diagnoses, codified according to the International Classification of Diseases (ICD-9), and the surgical indication were provided by the General Surgery and Gynaecology and Obstetrics services. The patients were selected during their pre-anaesthesia consultation, and informed consent was obtained in all cases. The standards of good clinical practice and the ethical principles of the Declaration of Helsinki were followed. Personal data were processed in accordance with the provisions of Organic Law 15/1999 on the Protection of Personal Data.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Preoperative evaluation of study patients included anamnesis, physical examination and complementary tests, in accordance with the hospital's preanaesthesia evaluation protocol. Study variables included preoperative factors, such as anthropometric measures, usual medication, comorbidities and ASA classification; intraoperative factors, such as length of surgery and anaesthesia, intraoperative drugs, CO<span class="elsevierStyleInf">2</span> pressure during intraperitoneal insufflation, duration of pneumoperitoneum, infiltration of local anaesthetics at laparoscopic port sites, haemodynamic variables, and intraoperative complications; postoperative factors, such as major and minor complications, recovery times, admissions or visits to the emergency room during the first 10 postoperative days, and quality of recovery at home rated during a follow-up telephone survey. Dependent variables were the postoperative symptoms: nausea, vomiting and postoperative pain.</p><p id="par0050" class="elsevierStylePara elsevierViewall">During the intervention, all patients were monitored with pulse oximetry, heart rate, blood pressure, end-tidal CO<span class="elsevierStyleInf">2</span> (etCO<span class="elsevierStyleInf">2</span>), bispectral index, and acceleromyography. Anaesthesia was induced with propofol (1.5–2.5<span class="elsevierStyleHsp" style=""></span>mg/kg), fentanyl (1–2<span class="elsevierStyleHsp" style=""></span>μg/kg) and rocuronium or cisatracurium for neuromuscular relaxation. Endotracheal intubation or laryngeal mask airway (Ambú, Igel or Supremm models) were used for patients undergoing laparoscopic cholecystectomy (LC), and endotracheal intubation for gynaecological patients. Anaesthesia was maintained at a bispectral index depth of between 40 and 60. Ventilation was controlled mechanically using a mixture of O<span class="elsevierStyleInf">2</span> and air at 35–45% to maintain an end-tidal CO<span class="elsevierStyleInf">2</span> of 35–40<span class="elsevierStyleHsp" style=""></span>mmHg. The surgical plethysmographic index (SPI) was maintained at between 20 and 50 by adjusting doses of remifentanil or fentanyl. Crystalloids were administered for fluid replacement. In LC patients, patient were placed supine in the reverse trendelenburg position with the legs open (Fowler), with lateral rotation to improve access to the gall bladder and bile duct. Gynaecological patients were placed in the lithotomy-Trendelenburg position. Intraoperative analgesia and antiemetic prophylaxis was administered at the discretion of the attending anaesthesiologist, using the Apfel<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">16</span></a> scale to guide PONV prophylaxis. Neuromuscular blockade was reversed with sugammadex (2–4<span class="elsevierStyleHsp" style=""></span>mg/kg) or atropine (0.01<span class="elsevierStyleHsp" style=""></span>mg/kg) plus neostigmine (0.05<span class="elsevierStyleHsp" style=""></span>mg/kg) if necessary.</p><p id="par0055" class="elsevierStylePara elsevierViewall">After surgery, the patients were awakened in the operating room and, depending on their clinical status, transferred to the Same Day Surgery Unit (SDSU), to the Postanaesthesia Care Unit (PACU) or to Critical Care; the presence of postoperative symptoms in patients requiring hospital admission was not recorded. In all patients, heart rate, blood pressure and pulse oximetry were monitored, and PONV and pain severity were rated on a 1–10 visual analogue scale (VAS) administered by a nurse (1: no pain; 10: worst pain imaginable), upon arrival in the corresponding unit or recovery room, and at 2, 4, 6 and 8<span class="elsevierStyleHsp" style=""></span>h. Patients with an Aldrete score of more than 8, who were able to remain in a sitting position, had started oral fluid intake, were haemodynamically stable, conscious, with spontaneous urination, able to walk normally, with controlled pain and reasonable absence of PONV were considered for discharge home.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Twenty four hours after discharge a nurse from the SDSU contacted patients by telephone to evaluate their general condition, degree of pain, oral intake, surgical wound status, presence of fever and adherence to prescribed treatment. Patients were followed up for 10 days after hospital discharge to check for unplanned admissions or visits to the emergency room.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The information was entered into a database, debugged, and statistically analysed using the statistical programme IBM SPSS Statistics v.19. Initially, patient characteristics were described by means of frequency distribution and measures of central tendency and dispersion. Following this, patients with and without postoperative symptoms were compared using comparison of means (Student's <span class="elsevierStyleItalic">t</span> test and Mann–Whitney <span class="elsevierStyleItalic">U</span> test) and proportions (<span class="elsevierStyleItalic">χ</span><span class="elsevierStyleSup">2</span>) in independent groups, with a significance level of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05. The correlation between different perioperative variables and the presence of postoperative symptoms was tested using a logistic regression model. The model was interpreted by verifying the statistical significance of the coefficients using the Wald test and the odds ratio of the explanatory variables.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall">After excluding 7 patients (2 refused to participate, data were incomplete in 5), 297 patients were included in the study and subsequently analysed. The mean age of patients was 44.3 years (SD: 12.4) with a range of 16–78 years. The proportion of men to women was 18.9%/81.1%, and most (76.3%) were Spanish natives. Nearly all (97.0%) patients were ASA class I–II.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Mean preoperative fasting time was 11.3<span class="elsevierStyleHsp" style=""></span>h (SD: 2.2, range 7.0–16.6) In 56.6% of cases, anxiolysis (5–10<span class="elsevierStyleHsp" style=""></span>mg diazepam<span class="elsevierStyleMonospace">)</span>was administered prior to surgery. The mean time spent in the preoperative holding area was 58.7<span class="elsevierStyleHsp" style=""></span>min (SD: 51.0), with a median of 45<span class="elsevierStyleHsp" style=""></span>min (interquartile range: 34, range 9–360).</p><p id="par0080" class="elsevierStylePara elsevierViewall">In 291 cases, the patients underwent surgery in the morning. The remaining 6 cases underwent CL surgery in the afternoon shift and spent the night in the PACU without occupying a hospital bed, which is included in the statistical analysis. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the surgical procedures performed. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> describes the drugs used in the intraoperative period to maintain anaesthesia, analgesia or muscle relaxation, as well as other agents used for pain or PONV prophylaxis. Antiemetic prophylaxis with a combination of ondansetron and dexamethasone was used in most (62.0%) patients.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">In terms of laparoscopy, 4 trocars were used in most patients (95.3%), and Hasson's was the method most frequently used (80.1%). Mean maximum pneumoperitoneum was 14.9<span class="elsevierStyleHsp" style=""></span>mmHg (SD: 1.8, range 12–20), and average pressure was 11.7<span class="elsevierStyleHsp" style=""></span>mmHg (SD: 1.5, range 7–16). Mean duration of pneumoperitoneum was 48.0<span class="elsevierStyleHsp" style=""></span>min (SD: 23.1, range 5–140) Ports were infiltrated with local anaesthetic in 77.0% of cases, using ropivacaine (0.2%-0.6%) in 91.0%. In 3 cases, ultrasound-guided transverse abdominis plane block was performed. Residents were present 86.9% of the time.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Intraoperative anaesthetic complications occurred in 3% of patients. These included severe bronchospasm, haemodynamic alterations, extubation failure, and pharmacological anaphylaxis. Intraoperative surgical complications occurred in 11.1% of cases, and included unforeseen extension of the scope of surgery, severe adhesions, cystic artery lesion, endometrioma rupture, and common bile duct lesion requiring conversion to laparotomy in 2 patients (0.7%). <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the frequency of such complications according to the type of surgery.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">During their stay in the SDSU, 24.9% (95% CI: 19.8–30.0) of patients presented nausea, 2.4% (95% CI: 0.5–4.3) presented vomiting, 49.1% (95% CI: 43.3–55.0) reported a pain score of 4 or higher on a 0–10 VAS, and 8.1% (95% CI: 4.8–11.3) presented other complications (urinary retention, flatulence, or haemodynamic changes). Antiemetic treatment was required in 31.9% of cases, analgesics in 80.1%, and treatment for other complications in 6.7%. <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the distribution of these complications according to the type of surgery. Mean time from arrival at the SDSU to discharge home was 390.9<span class="elsevierStyleHsp" style=""></span>min (SD: 171.2, median 357.5). In the cases of CL, it was 411.3<span class="elsevierStyleHsp" style=""></span>min (SD: 169.4) and in the case of gynaecological surgery, 354.6<span class="elsevierStyleHsp" style=""></span>min (SD: 169.1). A small percentage (8.1%, 24 cases) of patients required hospitalisation due to complications: 70.8% for medical-surgical reasons, 4.2% for social reasons, and 25.0% for logistical reasons. In the case of medical–surgical complications requiring hospitalisation, the 2 main reasons for admission were surgical complications (64.7%) and pain or PONV (23.5%). The mean duration of general anaesthesia was 92.8<span class="elsevierStyleHsp" style=""></span>min (SD: 30.9) and the mean duration of surgery was 67.9<span class="elsevierStyleHsp" style=""></span>min (SD: 28.7).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">Combining PONV and moderate/severe pain (VAS score of 4 or more) into a single variable showed that 58.7% of patients presented one or more of these symptoms (95% CI: 52.8–64.4). These symptoms were significantly more frequent in women than in men (63.8 vs. 35.8%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). Analysing nausea/vomiting and moderate/severe pain as separate variables showed a significantly higher frequency of pain (55.7 vs. 28.3%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and a higher, but not statistically significant, frequency of PONV (26.8 vs. 20.4%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.328) in women than in men. These symptoms were also more frequent in patients with habitual analgesic use than in non-consumers (73.7 vs. 47.9%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.04), and in those who waited more than 45<span class="elsevierStyleHsp" style=""></span>min in the holding area compared to those who started their surgery sooner (75 vs. 40.4%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). In patients presenting symptoms, the average wait before entering the operating room was 64.2<span class="elsevierStyleHsp" style=""></span>min (SD: 57.8) and 51.3<span class="elsevierStyleHsp" style=""></span>min in those not presenting symptoms (SD: 39.4); this difference is statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.03).</p><p id="par0105" class="elsevierStylePara elsevierViewall">No correlation was observed between the appearance of postoperative symptoms and the type of surgery, history of previous surgery, ASA classification, administration of preoperative anxiolytic, fasting time, toxic habits, and other sociodemographic variables.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Regarding intraoperative variables, the symptoms appeared more frequently in patients who did not receive any type of analgesic prophylaxis (paracetamol, morphine, metamizole, etc.) (89.5 vs. 56.6%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005), gastroprotective agents (pantoprazole or ranitidine) (64.0 vs. 52.4%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.04), antiemetics (92.9 vs. 57.1%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.008), or NSAIDs (73.8 vs. 56.1%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.03). In the majority of patients (74.4%), anaesthesia was maintained with propofol; sevoflurane was used in 24.2% of cases, and desflurane in 0.3%. Around one third (33.3%) of patients receiving sevoflurane presented nausea, 2.9% presented vomiting. In patient anaesthetised with propofol, 23.3% and 2.3% presented nausea and vomiting, respectively, although the differences did not reach statistical significance. No correlation was observed between postoperative symptoms and use of muscle relaxants, type of analgesia given, use of other drugs such as midazolam or atropine, type of replacement fluid used, type of hypnotic used, use of muscle relaxants, and type of opioid administered during the intervention.</p><p id="par0115" class="elsevierStylePara elsevierViewall">There was no correlation between the appearance of postoperative symptoms and maximum and mean pneumoperitoneum or duration of pneumoperitoneum. Although symptoms were more frequent in patients not receiving local anaesthetic infiltration of laparoscopy ports, the difference did not reach statistical significance (68.6 vs. 53.6%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05), and no correlation was observed with haemodynamic variables, or with the duration of anaesthesia or surgery.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Logistic regression identified the variables associated with the appearance of symptoms as: female sex (OR: 3.4), waiting time prior to start of surgery >45<span class="elsevierStyleHsp" style=""></span>min (OR: 4.9), and no administration of antiemetic prophylaxis (OR: 12.2) (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall">Within 24<span class="elsevierStyleHsp" style=""></span>h of discharge home, patients were contacted by phone to administer a follow-up survey on their general condition, medication consumption, surgical wound status, fever, etc. <a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a> shows the distribution of patients according to the score obtained from the telephone survey.</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0130" class="elsevierStylePara elsevierViewall">The advantages of MAS clearly outweigh its implicit drawbacks. However, ongoing research is needed to identify and resolve the main problems associated with day surgery procedures in order to maximise patient safety and treatment efficacy in MAS. Our results show that approximately 1 in 4 patients undergoing laparoscopic ambulatory surgery present PONV, and approximately half report moderate to severe pain before discharge home. Despite prophylactic medication, which is administered to the vast majority of MAS patients, PONV and postoperative pain are the complications most frequently encountered in this type of surgery. These complications require symptomatic treatment, and contribute to prolonging hospital stay. Given that pain and PONV are closely related, we found that these symptoms, taken together, affect more than half of patients undergoing surgery, and are more frequent in women and in patients having to wait longer in the preoperative holding area. These symptoms are even more frequent in patients receiving no prophylactic medication.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In our series, only 2 patients presented complications that required treatment in critical care: 1 case of anaphylactic reaction during anaesthetic induction, and 1 of psychomotor agitation during extubation. The other complications, including pain and PONV, were adequately managed both in the SDSU and the PACU. Other complications that required hospital admission were unforeseen extension of the scope of surgery, severe adhesions, cystic artery lesion, endometrioma rupture, and common bile duct injury. In some cases, this involved patients in whom the risk of PONV or postoperative pain could not be determined. The incidence of major postoperative complications after laparoscopy is very low (0.1–0.6%) and is often detected during surgery or in the early postoperative period (4–6<span class="elsevierStyleHsp" style=""></span>h).<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">2</span></a> In a previous study in LC,<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">1</span></a> 13.1% of patients required hospital admission. the most common reasons being conversion to open surgery, PONV, pain and the use of intraperitoneal drainages.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The success rate of MAS in our patients was greater than 90%. This can be attributed to careful surgical and anaesthetic technique, good postoperative care, and full compliance with selection criteria. The main reasons for delaying discharge home were surgical complications, followed by the appearance of pain and PONV. Good control of pain and PONV, which are the most unpredictable surgical complications, is the cornerstone of successful MAS.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Although most patients received antiemetic medication, particularly the combination of ondansetron and dexamethasone, 1 in every 4 study patients presented PONV, which was mainly treated at the SDSU. The rate of PONV observed in our study is consistent with the published data, and falls within the lower limits of the range already described. Only 2 patients required an overnight stay in the PACU due to PONV refractory to medical treatment. PONV is a common symptom, with an incidence that varies between 20% and 80%, depending on the study consulted.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">8</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">Although PONV only correlated with a longer preoperative waiting time and with the absence of intraoperative prophylaxis in our series, in other studies<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">13–15,17</span></a> it was found to correlate with the duration of anaesthesia, sex, history of PONV, excessive fasting, total volume of CO<span class="elsevierStyleInf">2</span> insufflated, low ASA score, surgeon's experience, correct patient selection, use of low insufflation pressures, type of fluid replacement administered, or anaesthetic technique (inhalation or total intravenous anaesthesia). In our series, although PONV was more frequent in patients maintained with sevoflurane than in those maintained with propofol, the difference did not reach statistical significance. This underlines the need to select and stratify the most influential factors using a scoring system that will help determine the level of risk.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">18</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">Although laparoscopic surgery is considered a procedure of medium to low complexity, it is associated with intense postoperative pain. This is evidenced by our results, in which almost half of our patients presented moderate or severe VAS-rated pain. However, in the telephone survey administered at 24<span class="elsevierStyleHsp" style=""></span>h after discharge, up to 75.6% of the patients undergoing CL and up to 74.5% of those undergoing gynaecological surgery reported either no pain or mild discomfort. This shows that postoperative pain is short-lived and responds well to treatment. Strategies to reduce the number of patients with severe pain must be based on specific treatment protocols.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">10</span></a> Post-CL pain has been associated with the number and size of the incisions, the pressure of pneumoperitoneum, and the presence of bile and blood in the abdominal cavity.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">1</span></a> There are also other risk factors that increase the intensity of pain after ambulatory laparoscopic surgery, such as younger age, female sex, and preoperative pain.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">11</span></a> Pain is known to be the main factor delaying discharge home of patients who undergo outpatient procedures, including laparoscopy.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">11</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">The strategy with the greatest effect in reducing the intensity of postoperative pain is the intraoperative administration of prophylaxis with analgesics and NSAIDs. In this regard, various methods have been recommended to improve such pain in MAS,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">19</span></a> including lung recruitment manoeuvres,<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">20</span></a> intraperitoneal infusion with saline solution or analgesics, low pressure of pneumoperitoneum,<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">21</span></a> and preoperative analgesia.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">10</span></a> Evidence on pneumoperitoneum pressure and postoperative pain, however, is contradictory. Our results show maximum and mean pressures of 14.9 and 11.7<span class="elsevierStyleHsp" style=""></span>mmHg, respectively, but we observed no correlation between these pressures and the appearance and intensity of postoperative pain. The correlation between pneumoperitoneum pressure and postoperative pain after LC<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">21–23</span></a> is not fully established. Low-pressure pneumoperitoneum is not recommended because the improvement in pain scores is minimal and visualisation of the surgical field is affected.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">22</span></a> However, insufflation up to around 8<span class="elsevierStyleHsp" style=""></span>mmHg during LC reduces postoperative pain compared to pressures of 12–15<span class="elsevierStyleHsp" style=""></span>mmHg.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">3</span></a></p><p id="par0165" class="elsevierStylePara elsevierViewall">Optimal analgesia for moderate or severe postoperative pain after laparoscopic surgery cannot be achieved with a single agent.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">24</span></a> Several studies have shown that paracetamol or other analgesics (metamizole, morphine) and NSAIDs, administered perioperatively, are equally effective in accelerating recovery in patients undergoing LC.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">10,12,24</span></a> This was also observed in our study. NSAIDs have been shown to be effective in the postoperative management of pain in LC patients.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">12</span></a> There is also evidence to show that transverse abdominis plane block after LC reduces pain scores and analgesic requirements compared with placebo.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">25,26</span></a> Although no statistically significant differences were observed when compared with trocar port site infiltration of local anaesthetics,<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">27</span></a> the latter procedure is more cost-effective.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">25,26</span></a></p><p id="par0170" class="elsevierStylePara elsevierViewall">Finally, it is particularly interesting to observe the statistically significant correlation between longer pre-surgical waiting times and the appearance of postoperative symptoms. The literature consulted in this regard is very scarce. A longer waiting time triggers a series of changes in patients, such as anxiety, excessive fasting or worsening of haemodynamic status. These patients are less well prepared for surgical stress, and also appear to be predisposed to postoperative pain and PONV. Pre-surgery waiting time is another factor to include in strategies to improve care quality in MAS.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Our study has several limitations: the information gaps in some variables, which is due to the variety of professionals involved and the use of observer-dependent action protocols. We did not evaluate the presence of postoperative symptoms in patients requiring hospital admission, and this could have biased our results if these symptoms had been more frequent in these patients; however, very few patients required hospitalisation. Any interpretation of our results must take into account the predominance of women in the study sample. This is because symptomatic cholelithiasis is more frequent in women, and gynaecological diseases were included in the study. Other limitations inherent to observational studies should also be mentioned, such as the absence of protocolization of some activities (antiemetic or analgesic medication, nasogastric tube placement, recruitment manoeuvres after pneumoperitoneum, etc.).</p><p id="par0180" class="elsevierStylePara elsevierViewall">In conclusion, approximately 1 in 4 patients undergoing laparoscopic ambulatory surgery present nausea or vomiting, and approximately half report moderate to severe pain before discharge home. These complications often require symptomatic treatment, and are a contributory factor to prolonged hospital stay. Pain and PONV, taken together, affect more than half of patients undergoing surgery, and are more frequent in women and in patients having to wait longer in the preoperative holding area. These symptoms are even more frequent in patients receiving no prophylactic medication.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conflicts of interest</span><p id="par0185" class="elsevierStylePara elsevierViewall">The authors have no conflict of interest to declare. A record was made of preoperative factors (usual medication, anaesthetic risk, etc.), intraoperative (surgical and anaesthetic times, drugs, CO<span class="elsevierStyleInf">2</span> pressure, etc.), and postoperative factors (major and minor complications, recovery times, etc.). As dependent variables, the postoperative symptoms considered were, nausea, vomiting, and/or postoperative pain.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">Considering as a combined variable the occurrence of nausea, vomiting or moderate/severe pain (4 or more points on a visual analogue scale), one or more of these symptoms occurred in 58.7% of the patients (95% CI: 52.8–64.4). Using a logistic regression, the variables associated with the appearance of symptoms were: female gender (OR: 3.4), waiting time over 45<span class="elsevierStyleHsp" style=""></span>min prior to surgery (OR: 4.9) and no anti-emetic prophylaxis (OR: 12.2).</p></span> <span id="abst0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Conclusions</span><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">In patients undergoing ambulatory laparoscopic surgery, one in 4 had postoperative nausea and vomiting, and approximately half of moderate-intensity pain before discharge.
