Image courtesy of General Electric Group.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Ultrasound is currently used routinely as both a diagnostic and monitoring tool to guide the performance of procedures in different fields of medicine, including intensive care.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The portability of this device now allows it to be used in all hospital areas. The use of ultrasound in intensive care units (ICU) and emergency rooms has hitherto been limited by cost and the lack of training among critical care doctors, but these systems are now available in these settings, and this widespread use has created the need for a consensus document to define a training curriculum and the minimum skills required. The acquisition and accreditation of competencies guarantees the minimum quality and safety standards required in clinical decision-making. Acquiring the skills needed to rapidly and reliably performance ultrasound evaluations and interpret ultrasound images can significantly improve professional medical training.</p><p id="par0010" class="elsevierStylePara elsevierViewall">The aim of this document is to define the training curriculum and the competencies required to obtain a diploma in transthoracic ultrasound, abdominal ultrasound, and vascular ultrasound in intensive care, anesthesia, critical care and emergency medicine. The minimum competencies required are based on the training standards published by other scientific societies.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–4</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Methodology</span><p id="par0015" class="elsevierStylePara elsevierViewall">This consensus document was developed by a working group made up of 15 experts appointed and approved by SEDAR, SEMI and SEMES (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Experts meeting the following criteria defined by SEDAR were invited to form part of the working group:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">1.</span><p id="par0020" class="elsevierStylePara elsevierViewall">Competence in clinical ultrasound management accredited by a scientific society: European Association of Cardiovascular Imaging; EACVI/EACTA, National Board of Echocardiography (NBE) in the United States, Focused Intensive Care Echo (FICE) in the UK, or Winfocus advanced national or international level.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">2.</span><p id="par0025" class="elsevierStylePara elsevierViewall">Experience in teaching courses in ultrasound management in intensive, critical and emergency care (preferably endorsed by a national and/or international scientific society).</p></li></ul></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">The working group was divided into sections according to the area of interest and knowledge. However, the competence levels were agreed by consensus of the whole group.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">SEDAR/SEMI/SEMES diploma in pulmonary, vascular and abdominal ultrasound in intensive and emergency care</span><p id="par0035" class="elsevierStylePara elsevierViewall">The curriculum in pulmonary, abdominal and vascular ultrasound defines a minimum standard for the performance of ultrasound in intensive and emergency care. The curriculum includes a theoretical module with a final evaluation and a practical training module that consists of compiling a logbook of supervised cases in an approved hospital. The curriculum is structured as follows:<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">1.</span><p id="par0040" class="elsevierStylePara elsevierViewall">Theoretical online training with a multiple choice test at the end of the course.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">2.</span><p id="par0045" class="elsevierStylePara elsevierViewall">Compilation of a supervised logbook accredited by a tutor in an approved center.</p></li></ul></p><p id="par0050" class="elsevierStylePara elsevierViewall">Supervisors must be qualified in ultrasound management by a national or international society (national or international Winfocus) or by merit based on a Curriculum Vitae endorsed by the evaluation committee.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The SEDAR/SEMI/SEMES clinical ultrasound working group will create a network of approved centers where supervised logbook can be compiled for the purpose of obtaining the diploma in pulmonary, abdominal and vascular ultrasound in Intensive Care, Anesthesia, Critical Care and Emergency Medicine. The evaluation committee will be made up of members of the SEDAR/SEMI/SEMES ultrasound working group. Re-accreditation will not be required.</p><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Theoretical training</span><p id="par0060" class="elsevierStylePara elsevierViewall">Applicants must complete an online theoretical training course organized by SEDAR/SEMI/SEMES or by any training center previously approved by the evaluation committee. The course must include the required number of hours of training in pulmonary, abdominal and vascular ultrasound. It must also include a final multiple choice exam.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Practical training – Logbook (to be compiled within 18 months of completion of the theoretical course)</span><p id="par0065" class="elsevierStylePara elsevierViewall">Applicants must complete their logbook within 18 months, and must fulfill the requirements described below and summarized in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Lung ultrasound</span><p id="par0070" class="elsevierStylePara elsevierViewall">Perform 30 scans. Draw up a structured case report describing the findings. All 30 cases must be reviewed by a qualified supervisor. At least 20 cases must include a description of a pathology. In each case, the 4 standard areas of each lung shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> must be explored. Candidates must deliver to the evaluation committee 8 clips of the 4 reference points per lung showing the following pathologies: pleural effusion, pneumothorax, consolidation, and interstitial syndrome. <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the basic skills required for competence.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Abdominal ultrasound</span><p id="par0075" class="elsevierStylePara elsevierViewall">Perform 25 scans. Draw up a structured case report describing the findings. All 25 cases with their corresponding clips must be reviewed by a qualified supervisor. At least 10 scans must demonstrate pathology. In each case, the 6 basic areas included in the e-FAST protocol must be explored. Five of the 25 abdominal ultrasounds must show the abdominal aorta. One of the 5 explorations showing the abdominal aorta must include a clip identifying the abdominal aorta. Candidates must deliver to the evaluation committee 6 clips for each of the 6 planes in 3 pathologies (intra-abdominal free fluid) and 1 clip for 1 plane in a case of abdominal aorta. <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the basic skills required for competence.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Vascular ultrasound</span><p id="par0080" class="elsevierStylePara elsevierViewall">Perform 20 vascular ultrasound explorations including 4 central venous line placements, 4 cases of peripheral vein placement and 4 cases of arterial line placement. Clips of peripheral veins and arteries should be shown in both long and short axes. Eight explorations should involve a study of peripheral thrombi in both the lower and upper limbs. At least 2 explorations must show venous thrombosis. In each case 4 planes should be explored on each side of the 4 thrombosis risk areas (deep femoral vein, popliteal vein, internal jugular vein and veins of the upper extremity up to the subclavian vein). Draw up a structured case report describing the findings. All 20 cases with their corresponding clips must be reviewed by a qualified supervisor. Candidates must deliver to the evaluation committee 8 clips of the 8 planes in the 2 peripheral venous thrombosis explorations. <a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a> shows the basic skills required for competence.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Definition of competencies</span><p id="par0085" class="elsevierStylePara elsevierViewall">Below is a list of the skills to be acquired.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Competence 1: Physics, equipment and artifacts</span><p id="par0090" class="elsevierStylePara elsevierViewall">In order to correctly interpret ultrasound images and artifacts, it is essential to know and understand the physics of ultrasound (US). Images are formed by the interaction of ultrasound waves with tissue, fluid, air, and their interfaces. The US transducers used in clinical practice have a frequency range of between 1 and 20<span class="elsevierStyleHsp" style=""></span>MHz (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Clinicians must be able to choose the most suitable transducer for the structure to be explored. Knowledge of these general principles allows the clinician to anticipate and recognize potential artifacts. Misinterpretation of artifacts can lead to a wrong diagnosis, but a correct interpretation will confirm suspicion and facilitate a correct diagnosis.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">Clinicians must know how to use the equipment correctly to obtain good quality images, and must show that they can control the transducer and understand the anatomy. They must understand how to use the controls of the system in order to optimize the images before recording. They must also be familiar with pre- and post-processing, storage and data transfer functions. These skills will be evaluated by the supervisor as applicants compile their logbooks. <a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a> shows the learning objectives and recommended competencies in physics, equipment and artifacts.</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Competence 2: Lung, pleura and diaphragm ultrasound</span><p id="par0100" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the learning objectives and recommended competencies for ultrasound exploration of the lungs and identification of pathologies.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Competence 3: Abdominal ultrasound</span><p id="par0105" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the competencies to be acquired in abdominal ultrasound.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Competence 4: Vascular ultrasound</span><p id="par0110" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a> shows the competencies to be acquired in vascular ultrasound.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusions</span><p id="par0115" class="elsevierStylePara elsevierViewall">In light of the growing evidence of the benefit of ultrasound in intensive care, the Ultrasound in Intensive Care working group of SEDAR, and the Clinical Ultrasound working group of SEMI and SEMES have drawn up a series of consensus recommendations for training and the acquisition of minimum competencies in the use of ultrasound in Intensive Care, Anesthesia and Emergency Medicine. Candidates completing this training program will be awarded a Diploma in Pulmonary, Abdominal and Vascular Ultrasound in Intensive Care and Emergency Medicine endorsed by SEDAR, SEMI and SEMES. SEDAR, SEMI and SEMES will disseminate this document among their partners to encourage clinicians to obtain the diploma in pulmonary, abdominal and vascular ultrasound in Intensive Care and Emergency Medicine. They will also ensure that all healthcare professionals appointed to act as course tutors maintain a suitable level of competence.
