including intensive care&#46;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The portability of this device now allows it to be used in all hospital areas&#46; The use of ultrasound in intensive care units &#40;ICU&#41; and emergency rooms has hitherto been limited by cost and the lack of training among critical care doctors&#44; but these systems are now available in these settings&#44; and this widespread use has created the need for a consensus document to define a training curriculum and the minimum skills required&#46; The acquisition and accreditation of competencies guarantees the minimum quality and safety standards required in clinical decision-making&#46; Acquiring the skills needed to rapidly and reliably performance ultrasound evaluations and interpret ultrasound images can significantly improve professional medical training&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">The aim of this document is to define the training curriculum and the competencies required to obtain a diploma in transthoracic ultrasound&#44; abdominal ultrasound&#44; and vascular ultrasound in intensive care&#44; anesthesia&#44; critical care and emergency medicine&#46; The minimum competencies required are based on the training standards published by other scientific societies&#46;<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2&#8211;4</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Methodology</span><p id="par0015" class="elsevierStylePara elsevierViewall">This consensus document was developed by a working group made up of 15 experts appointed and approved by SEDAR&#44; SEMI and SEMES &#40;<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#41;&#46; Experts meeting the following criteria defined by SEDAR were invited to form part of the working group&#58;<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">1&#46;</span><p id="par0020" class="elsevierStylePara elsevierViewall">Competence in clinical ultrasound management accredited by a scientific society&#58; European Association of Cardiovascular Imaging&#59; EACVI&#47;EACTA&#44; National Board of Echocardiography &#40;NBE&#41; in the United States&#44; Focused Intensive Care Echo &#40;FICE&#41; in the UK&#44; or Winfocus advanced national or international level&#46;</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">2&#46;</span><p id="par0025" class="elsevierStylePara elsevierViewall">Experience in teaching courses in ultrasound management in intensive&#44; critical and emergency care &#40;preferably endorsed by a national and&#47;or international scientific society&#41;&#46;</p></li></ul></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">The working group was divided into sections according to the area of interest and knowledge&#46; However&#44; the competence levels were agreed by consensus of the whole group&#46;</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">SEDAR&#47;SEMI&#47;SEMES diploma in pulmonary&#44; vascular and abdominal ultrasound in intensive and emergency care</span><p id="par0035" class="elsevierStylePara elsevierViewall">The curriculum in pulmonary&#44; abdominal and vascular ultrasound defines a minimum standard for the performance of ultrasound in intensive and emergency care&#46; The curriculum includes a theoretical module with a final evaluation and a practical training module that consists of compiling a logbook of supervised cases in an approved hospital&#46; The curriculum is structured as follows&#58;<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">1&#46;</span><p id="par0040" class="elsevierStylePara elsevierViewall">Theoretical online training with a multiple choice test at the end of the course&#46;</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">2&#46;</span><p id="par0045" class="elsevierStylePara elsevierViewall">Compilation of a supervised logbook accredited by a tutor in an approved center&#46;</p></li></ul></p><p id="par0050" class="elsevierStylePara elsevierViewall">Supervisors must be qualified in ultrasound management by a national or international society &#40;national or international Winfocus&#41; or by merit based on a Curriculum Vitae endorsed by the evaluation committee&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">The SEDAR&#47;SEMI&#47;SEMES clinical ultrasound working group will create a network of approved centers where supervised logbook can be compiled for the purpose of obtaining the diploma in pulmonary&#44; abdominal and vascular ultrasound in Intensive Care&#44; Anesthesia&#44; Critical Care and Emergency Medicine&#46; The evaluation committee will be made up of members of the SEDAR&#47;SEMI&#47;SEMES ultrasound working group&#46; Re-accreditation will not be required&#46;</p><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Theoretical training</span><p id="par0060" class="elsevierStylePara elsevierViewall">Applicants must complete an online theoretical training course organized by SEDAR&#47;SEMI&#47;SEMES or by any training center previously approved by the evaluation committee&#46; The course must include the required number of hours of training in pulmonary&#44; abdominal and vascular ultrasound&#46; It must also include a final multiple choice exam&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Practical training &#8211; Logbook &#40;to be compiled within 18 months of completion of the theoretical course&#41;</span><p id="par0065" class="elsevierStylePara elsevierViewall">Applicants must complete their logbook within 18 months&#44; and must fulfill the requirements described below and summarized in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Lung ultrasound</span><p id="par0070" class="elsevierStylePara elsevierViewall">Perform 30 scans&#46; Draw up a structured case report describing the findings&#46; All 30 cases must be reviewed by a qualified supervisor&#46; At least 20 cases must include a description of a pathology&#46; In each case&#44; the 4 standard areas of each lung shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a> must be explored&#46; Candidates must deliver to the evaluation committee 8 clips of the 4 reference points per lung showing the following pathologies&#58; pleural effusion&#44; pneumothorax&#44; consolidation&#44; and interstitial syndrome&#46; <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the basic skills required for competence&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Abdominal ultrasound</span><p id="par0075" class="elsevierStylePara elsevierViewall">Perform 25 scans&#46; Draw up a structured case report describing the findings&#46; All 25 cases with their corresponding clips must be reviewed by a qualified supervisor&#46; At least 10 scans must demonstrate pathology&#46; In each case&#44; the 6 basic areas included in the e-FAST protocol must be explored&#46; Five of the 25 abdominal ultrasounds must show the abdominal aorta&#46; One of the 5 explorations showing the abdominal aorta must include a clip identifying the abdominal aorta&#46; Candidates must deliver to the evaluation committee 6 clips for each of the 6 planes in 3 pathologies &#40;intra-abdominal free fluid&#41; and 1 clip for 1 plane in a case of abdominal aorta&#46; <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the basic skills required for competence&#46;</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Vascular ultrasound</span><p id="par0080" class="elsevierStylePara elsevierViewall">Perform 20 vascular ultrasound explorations including 4 central venous line placements&#44; 4 cases of peripheral vein placement and 4 cases of arterial line placement&#46; Clips of peripheral veins and arteries should be shown in both long and short axes&#46; Eight explorations should involve a study of peripheral thrombi in both the lower and upper limbs&#46; At least 2 explorations must show venous thrombosis&#46; In each case 4 planes should be explored on each side of the 4 thrombosis risk areas &#40;deep femoral vein&#44; popliteal vein&#44; internal jugular vein and veins of the upper extremity up to the subclavian vein&#41;&#46; Draw up a structured case report describing the findings&#46; All 20 cases with their corresponding clips must be reviewed by a qualified supervisor&#46; Candidates must deliver to the evaluation committee 8 clips of the 8 planes in the 2 peripheral venous thrombosis explorations&#46; <a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a> shows the basic skills required for competence&#46;</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Definition of competencies</span><p id="par0085" class="elsevierStylePara elsevierViewall">Below is a list of the skills to be acquired&#46;</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Competence 1&#58; Physics&#44; equipment and artifacts</span><p id="par0090" class="elsevierStylePara elsevierViewall">In order to correctly interpret ultrasound images and artifacts&#44; it is essential to know and understand the physics of ultrasound &#40;US&#41;&#46; Images are formed by the interaction of ultrasound waves with tissue&#44; fluid&#44; air&#44; and their interfaces&#46; The US transducers used in clinical practice have a frequency range of between 1 and 20<span class="elsevierStyleHsp" style=""></span>MHz &#40;<a class="elsevierStyleCrossRef" href="#fig0010">Fig&#46; 2</a>&#41;&#46; Clinicians must be able to choose the most suitable transducer for the structure to be explored&#46; Knowledge of these general principles allows the clinician to anticipate and recognize potential artifacts&#46; Misinterpretation of artifacts can lead to a wrong diagnosis&#44; but a correct interpretation will confirm suspicion and facilitate a correct diagnosis&#46;</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">Clinicians must know how to use the equipment correctly to obtain good quality images&#44; and must show that they can control the transducer and understand the anatomy&#46; They must understand how to use the controls of the system in order to optimize the images before recording&#46; They must also be familiar with pre- and post-processing&#44; storage and data transfer functions&#46; These skills will be evaluated by the supervisor as applicants compile their logbooks&#46; <a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a> shows the learning objectives and recommended competencies in physics&#44; equipment and artifacts&#46;</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Competence 2&#58; Lung&#44; pleura and diaphragm ultrasound</span><p id="par0100" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the learning objectives and recommended competencies for ultrasound exploration of the lungs and identification of pathologies&#46;</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Competence 3&#58; Abdominal ultrasound</span><p id="par0105" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the competencies to be acquired in abdominal ultrasound&#46;</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Competence 4&#58; Vascular ultrasound</span><p id="par0110" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a> shows the competencies to be acquired in vascular ultrasound&#46;</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusions</span><p id="par0115" class="elsevierStylePara elsevierViewall">In light of the growing evidence of the benefit of ultrasound in intensive care&#44; the Ultrasound in Intensive Care working group of SEDAR&#44; and the Clinical Ultrasound working group of SEMI and SEMES have drawn up a series of consensus recommendations for training and the acquisition of minimum competencies in the use of ultrasound in Intensive Care&#44; Anesthesia and Emergency Medicine&#46; Candidates completing this training program will be awarded a Diploma in Pulmonary&#44; Abdominal and Vascular Ultrasound in Intensive Care and Emergency Medicine endorsed by SEDAR&#44; SEMI and SEMES&#46; SEDAR&#44; SEMI and SEMES will disseminate this document among their partners to encourage clinicians to obtain the diploma in pulmonary&#44; abdominal and vascular ultrasound in Intensive Care and Emergency Medicine&#46;
