The three-dimensional structures of the mutated proteins of each patient were constructed with Modeller 9.21 using the 5LSA (normal) structure as a template.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Catechol-O-methyltransferase (COMT) is a ubiquitous enzyme that regulates the bioavailability of catecholamines and plays a key role in maintaining basic physiological functions, such as sympathetic tone, mood, and inflammation. These, in turn, influence the development and characteristics of pain. The two principle forms of the COMT enzyme — membrane-bound COMT (MB-COMT) and soluble COMT (S-COMT), differ only by 50 amino acids at the N-terminus.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The COMT gene locus is enriched with single nucleotide polymorphisms (SNP), which have various alleles associated with chronic pain and pain-related phenotypes. There is some evidence that some COMT gene polymorphisms that affect the activity of the COMT enzyme may adversely affect a patient’s ability to cope with pain.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Many studies on the genetic association of COMT rs4680 (Val158Met) have focused on the common single nucleotide polymorphism. The Val158Met A (Val) allele has been associated with increased susceptibility to postoperative pain.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> The rs4680 (Val158Met) polymorphism has been associated with clinically relevant intermediate phenotypes, such as experimental pain,<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> anxiety,<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> depression, and attention.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> The G472A allele causes nonsynonymous substitution of valine by methionine at position 158 (Val158Met), which produces a less thermostable enzyme with reduced activit.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Studies on COMT polymorphism have consistently shown that the Met allele codes for a 25%–40% reduction in both enzyme activity, measured in human red blood cells<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> and tissue,<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> and transfected cell lysates.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> The Val158Met allele, therefore, is a major source of individual variation in COMT activity. <span class="elsevierStyleItalic">In vitro</span> studies have shown that the ACCG haplotype codes for an 80% reduction in enzymatic activity due to formation of a stable mRNA secondary structure that is more resistant to translation.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> This means that SNPs at different loci of the COMT gene can act alone or together to influence enzyme activity and pain. Specifically, the G to A transition at nucleotide 21,881 of the human catechol-O-methyltransferase (COMT) gene represents a functional genetic polymorphism (Val158Met), rendering an enzyme with reduced activity that has been associated with chronic post-sternotomy pain.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In this study, we examine the presence of exon 4 in the catechol-O-methyltransferase gene in sternotomy patients presenting chronic pain.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and method</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients and pain measurement procedure</span><p id="par0020" class="elsevierStylePara elsevierViewall">The study was approved by the Institutional Ethics Committee of the Sri Venkateswara Institute of Medical Sciences (SVIMS), Tirupati, Andhra Pradesh, India (IEC authorization no. 614 dated 04/24/2017) and included in the Clinical Trials Registry — India (ICMR-NIMS) (CTRI) under number CTRI/2018/03/012364. Informed consent was obtained from all study subjects. All study procedures were performed in accordance with good clinical practice standards and the principles of the Declaration of Helsinki.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Patients aged between 18 and 65 years presenting chronic post-surgical pain (CPSP) with a numerical rating scale (NRS) score >4 during the 3-month postoperative period were recruited. Patients rated their pain levels using an NRS, a standard index<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> that measures the perception of pain on a scale of 0–10, where 0 is absence of pain and 10 the worst pain imaginable. We collected demographic data such as age, gender, level of education, history of regular physical activity in adulthood (during a pain-free period) at the start of the study.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Sample collection</span><p id="par0030" class="elsevierStylePara elsevierViewall">On the date of surgery, 5 mL of peripheral blood was drawn from all 100 study patients (n = 100) for genetic testing. Patients underwent cardiac surgery requiring median sternotomy at the SVIMS Department of Cardiothoracic and Vascular Surgery, Tirupati.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Mutation analysis</span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Isolation of genomic DNA using the phenol-chloroform extraction method</span><p id="par0035" class="elsevierStylePara elsevierViewall">Pure genomic DNA was extracted from the patient’s blood using a standardized protocol<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,13</span></a> and then quantified spectrophotometrically and analysed with 1% agarose gel electrophoresis. The extracted DNA was used as a template in the PCR tests.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Design of primers</span><p id="par0040" class="elsevierStylePara elsevierViewall">The exon 4 primers of membrane-bound COMT were designed on the basis of the sequence obtained from the NCBI database, which were then run through online Primer3 software, and the sequences were synthesised at Sigma-Aldrich.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Polymerase chain reaction (gene amplification) and sequencing</span><p id="par0045" class="elsevierStylePara elsevierViewall">The exon 4 of the COMT gene from the 100 study subjects was amplified using the Mastercycler® gradient thermocycler (Eppendorf, Hamburg, Germany). Forward primer: 5′-GCAAGATCGTGGACGCCGTG-3′, reverse primer: 5′-CTTGTCCTTCACGCCAGCGA-3′. The reaction mixture contained in a final volume of 50 μL consisted of 100 pmoles of each primer, 100 μmol of dNTPS mix, 10 mM Tris–Hcl (pH 8.8), 1.5 mM MgCl<span class="elsevierStyleInf">2</span>, 1 U of Taq DNA polymerase, and 50 μg of genomic DNA. Amplification parameters were 10 min at 65 °C; 10 min at 65 °C; 10 min at 65 °C; 10 min at 72 °C, and 10 min at 72 °C.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,13</span></a> The amplified PCR products were sequenced using the dideoxy chain termination method.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Sequence analysis</span><p id="par0050" class="elsevierStylePara elsevierViewall">The sequenced nucleotides and the corresponding amino acid sequences obtained from the ExPASy translation tool® were used for multi-sequence alignment (MSA) to compare them against the normal wild-type sequence (NM_000754.3) using the Clustal X and Clustal W tools, and generating “.aln” and “.PIR” files. The sequence was saved to a PostScript file.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Structural analysis</span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Structure construction</span><p id="par0055" class="elsevierStylePara elsevierViewall">The crystal structure of human COMT was downloaded from the PDB (ID: 5LSA. Sri Venkateswara Institute of Medical Sciences, Tirupati, Andhrapradesh, India) and analysed using PyMOL® software. This 3D structure was used as a template to build the target structure. The Python® script with the details of the template and the target was created and saved as a. py file. The structures were constructed using Modeller 9.21 and data from patients with an amino acid sequence mutation in COMT. Discrete Optimized Protein Energy (DOPE) scores were obtained from the 20 models generated per patient, and the model with the lowest DOPE score was chosen for the structural analysis.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Structure validation (ProCheck: Ramachandran plot)</span><p id="par0060" class="elsevierStylePara elsevierViewall">The secondary structures of the COMT construction were validated using the Ramachandran plot, calculating the psi (degrees), and phi (degrees) angles. The model with the lowest DOPE score for each patient was analysed with ProCheck, calculating secondary conformational stability by means of the number of residues found in the most favoured regions and in the additionally allowed regions.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Superimposition of normal and mutated COMT structures</span><p id="par0065" class="elsevierStylePara elsevierViewall">Wild-type and mutated COMT structures were superimposed using PyMOL to identify the structural variability of target proteins. The PyMOL alignment program was used to define the linear similarity score as a joint-probability log odds ratio using a method similar to Dayhoff’s amino acid substitution model. The alignment of superimposed structures and similarities was predicted using the root mean square deviation (RMSD) values, which is the measure of the mean distance between the atoms of the superimpose proteins. A higher RMSD value is indicative of greater variations between two superimposed protein molecules.</p></span></span></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall">Twenty-five (25%) of the 100 study patients reported CPSP 3 months after sternotomy. PCR testing was performed on samples from all patients, and 5 showed new mutations. The demographic data of these 5 patients showed that 3 of them had undergone double valve replacement (DVR) and the remaining 2 had undergone coronary artery bypass grafting (CABG) using a graft from the saphenous vein. Pain was measured on a 10-point NRS: 5 patients rated their pain at 5, and 1 at 4. The CPSP was located around the sternal wound, with 4 patients reporting continuous pain and 1 reporting intermittent pain (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">PCR was performed to identify mutations in exon 4 of the COMT gene. Amplification with gene-specific primers showed an amplicon of 220 bp on 1% agarose gel electrophoresis (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The sequence obtained by Sanger sequencing (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>) was analysed and deposited in GenBank® (<a href="http://www.ncbi.nlm.nih.gov">http://www.ncbi.nlm.nih.gov</a>) using the on-line submission tool BankIt® (<a href="https://www.ncbi.nlm.nih.gov/WebSub">https://www.ncbi.nlm.nih.gov/WebSub</a>). Accession numbers for the 5 patients were MK509812, MK509813, MK509814, MK509815, and MK509816, respectively.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Multiple sequence alignment showed homozygous mutations in patient 20, where c.349G>A, c.372G>C, c.373C>G, c.382C>G, c.383G>C gave rise to p.(Ala117Thr), silent mutation p.(Val124Val), p.(Arg125Gly), p.(Arg128Ala). In patient 39, c.382C>G, c.383G>C, c.408C>G gave rise to p.(Arg128Ala), silent mutation p.(Leu136Leu). In patient 52, c.351C>A, c.359G>C, c.370G>A, c.373C>G, c.379G>C, c.382C>G, c.383G>C gave rise to silent mutation p.(Ala117Ala), p.(Gly120Ala), p (Val120Met), p.(Arg125Gly), p.(Ala127Pro), p (Arg128Ala). In patient 72, c.382C>G and c.383G>C, c.408C>G, gave rise to p.(Arg128Ala), silent mutation p.(Leu136Leu), and in patient 94, c.372G>C, c.373C>G, c.382C>G, c.383G>C, c.408C>G, gave rise to silent mutation p.(Val124Val), p.(Arg125Gly), p.(Arg128Ala), silent mutation p.( Leu136Leu). These results showed the presence of new and silent mutations of the COMT gene associated with CPSP (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Of the 20 models generated for each patient on Modeller 9.21, model 4 was selected for patient 20 (DOPE score: -26202.53906), model 7 was selected for patient 39 (DOPE score: -26465.12695); model 12 was selected for patient 52 (DOPE score: -26217.04883); model 7 was selected for patient 72 (DOPE score: -26465.12695), and model 17 was selected for patient 94 (DOPE score: -26253.54297) (Appendix B Supplementary table).</p><p id="par0090" class="elsevierStylePara elsevierViewall">We analysed the COMT structures obtained from these patients to calculate the standard deviation due to the mutations in the sequences. The RMSD values were taken as a measure of the deviation from the normal structure. Each COMT structure was overlaid on 5LSA (wild type) and aligned using PyMOL. The RMSD value for patient 20 was 0.086, the RMSD value for patient 39 was 0.110, the value for patient 52 was 0.090, the value for patient 72 was 0.110, and the value for patient 94 was 0.090 (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">The COMT structures constructed were also analysed using ProCheck to explore the Ramachandran plots, and showed a percentage increase in the of the corresponding residues in the most favoured regions of 94.3%, 93.8%, 92.2%, 93, 8% and 92.7% for patients 20, 39, 52, 72 and 94, respectively, compared to the wild type percentage of 91.5%. However, residues were reduced in the additionally allowed regions in patient 20 (5.2%), patient 39 (5.7%), patient 52 (6.8%), patient 72 (5.7%) and patient 94 (6.8%), compared to the wild type (7.9%) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">Although the rate and severity of chronic pain after cardiac surgery with sternotomy is very high and can persist for more than 3 months and as much a 1 year in some cases, the aetiology of this syndrome is not fully understood. The primary cause is usually presumed to involve damage to the intercostal nerve and the development of neuropathic pain. However, many post-sternotomy patients also report pain at sites unrelated to the surgical incision in the chest wall, such as the neck, shoulder, arms, and back. Therefore, it is reasonable to assume that that cause may go beyond simple tissues and intercostal nerve damage, and could include pathological and genetic factors.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a> In a pilot study conducted in our laboratory, 25% of patients with the COMT (G472A) allele presented CPSP.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> The present study is a continuation of previous studies on the COMT (G472A) allele involving 100 patients undergoing sternotomy.</p><p id="par0105" class="elsevierStylePara elsevierViewall">COMT gene mutations showed a significant association with general pain during the postoperative period. Many SNPs have been correlated with pain sensitivity and the need for analgesics in acute and chronic pain. Of all SNPs, those found in COMT (catechol-O-methyltransferase), a gene that is located on the short arm (p) of chromosome 22, band q11.2, have probably been studied in most detail. Polymorphisms in the gene coding for COMT are linked to individual differences in pain sensitivity and response to opioids.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,16</span></a> Mutations of exon 4 of the COMT gene expressed in the nervous system mediate the perception of pain by regulating catecholamine production.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Six SNPs that display high polymorphism frequency have been reported to date: rs2097903, rs6269, rs4633, rs4818, rs165599 and rs4680.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17–21</span></a> SNP rs4680 generates a valine-to-methionine substitution at codon 158 (Val158Met).<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> On this basis, we evaluated the influence of COMT exon 4 mutations in postoperative cardiac surgery patients. None of our patients required opioids.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Most studies have shown that Val158Met (G472A) is involved in CPSP. In our study, 20 of the 25 study patients that reported persistent pain had Val158Met (G472A). However, 5 patients reporting persistent pain did not have Val158Met (G472A), but presented new mutations that could be related to pain. In these 5 patients, 100% of c.382C G, c.383G>C mutations gave rise to p(Arg128Ala); 60% of c.373C>G and c.408C>G mutations gave rise to p.(Arg125Gly) and silent mutation p.(Leu136Leu); 40% of c.372G>C mutations gave rise to p.(Val124Val); and 20% of c.349G>A, c.351C>A, c.359G>C, c.370G>A, c.379G>C mutations gave rise to p.(Ala117Thr), silent mutation p.(Ala117Ala), p.(Gly120Ala), p.(Val120Met), p.(Ala127Pro). In this study, we have analysed the correlation between the mutations found (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) in the COMT structure and CPSP.</p><p id="par0120" class="elsevierStylePara elsevierViewall">ProCheck analysis of the mutations identified in the COMT gene showed a stable secondary structure. The shift of amino acid residues to a more favoured region from additionally allowed regions of the Ramachandran plot confirmed the greater stability of the mutated COMT structures, which makes them indispensable for patients. The 3 three-dimensional tertiary structure analyses showed a deviation in the orientation of wild-type COMT structures, indicating that these distortions could reduce COMT activity and give rise to persistent pain (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Interestingly, patients 39 and 72 had similar mutations, although we have observed that changing the nucleotide sequence in some silent mutations does not alter COMT amino acids. This is the first study in which we have observed the impact of new mutations in the exon 4 of COMT in patients with CPSP lasting at least 3 months, with NRS scores ≥4 (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> and <a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). These results are consistent with other studies in which the enzymatic activity and protein expression levels of allelic variations do not correlate with mRNA stability findings. Although mRNA is more stable in COMT transcripts carrying the A allele of SNP rs165774, enzyme activity and protein expression levels are lower. This is an excellent example of how allelic variants can have opposite effects on mRNA stability and protein expression.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> The molecular genetic mechanisms underlying this effect often include the regulation of secondary and tertiary mRNA structures. This has been demonstrated in at least 2 other functional alleles of COMT,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> in which the more stable mRNA structures are protected from degradation at the expense of reducing translation efficiency<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,22</span></a> and/or silence the mechanism involved in regulation.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> We have also observed a stable mutated COMT structure (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>) that was inactive in terms of CPSP in the 25 patients studied. Therefore, the results of our study show that COMT gene mutations decrease the functionality of COMT that contributes to CPSP.</p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Conclusion</span><p id="par0125" class="elsevierStylePara elsevierViewall">Mutations in exon 4 of the COMT gene could be related to persistent CPSP lasting at lease 3 months after sternotomy. In this study, 22.68% of our patients showed the Val158Met (G472A) mutation of COMT. Furthermore, 2.3% of our patients showed new mutations, mainly the c.382C>G, c.383G>C mutations that gave rise to p.(Arg128Ala). The increased stability of these mutated COMT structures observed in <span class="elsevierStyleItalic">in silico</span> studies is related to CPSP. The polymerase chain reaction (PCR) products were sequenced and mutated sequences were deposited in GenBank®. The structural analysis of COMT was performed using ProCheck® and distortions of three-dimensional tertiary structural orientation was evaluated with root-mean-square deviation (RMSD) score.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">Genetic analysis carried out through PCR showed 220 bp amplicons. The 25% of patients with CPSP showed a Numeric Rating Scale (NRS) >4 pain score. 20% of these patients have known Val158Met mutation, 5% of patients showed novel mutations c.382C>G, c.383G>C; p. Se realizó el análisis estructural de COMT utilizando ProCheck®, evaluándose las distorsiones de la orientación estructural terciaria tridimensional con la escala RMSD (raíz de la desviación cuadrática media).</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">El análisis genético realizado con PCR reflejó amplicones de 220 bp. El 25% de los pacientes con CPSP reflejó una puntuación de dolor <4 en la escala NRS. El 20% de estos pacientes tenía mutación Val158Met conocida, el 5% de los pacientes reflejó mutaciones nuevas c.382C>G, c.383G>C; p. (Arg128Ala).
