Approximately 10% of these patients present severe perioperative bleeding, and 5% require re-exploration<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>. The bleeding may be caused by the surgery itself, or may also be due to uncorrected preoperative coagulopathy or CPB-induced clotting abnormalities. Activation of the fibrinolysis system<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> is one of the causes of postoperative bleeding in on-pump CC. On-pump CC without antifibrinolytics is typically associated with greater blood loss and increased risk of transfusion and reoperation due to postoperative bleeding or cardiac tamponade<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>. Three antifibrinolytics are used in clinical practice: aprotinin, epsilon aminocaproic acid, and tranexamic acid (TXA).</p><p id="par0010" class="elsevierStylePara elsevierViewall">TXA is the most widely used antifibrinolytic in the world. It has a half-life of up to 3 h<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>, and its antifibrinolytic action extends into the immediate postoperative period. TXA has been shown to reduce transfusion requirements and the need for reoperation for bleeding in CC<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>, but there is no consensus on the ideal dose.</p><p id="par0015" class="elsevierStylePara elsevierViewall">In clinical guidelines, the recommended TXA dose in CC ranges from 1 to 20 g administered for 20 min and up to 12 h<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a>. The volume of TXA required to effectively inhibit fibrinolysis <span class="elsevierStyleItalic">in vivo</span> is still unclear, and there is no consensus on the optimal dosing regimen of TXA in CC<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>. Some studies have shown that high-dose TXA obtains better results, while other have shown the opposite<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,8</span></a>; however, high-dose TXA is known to be associated with postoperative seizures<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">We conducted this prospective cohort study simultaneously in 2 tertiary hospitals in Valencia - the General University Hospital (HGU, by its Spanish acronym) and the La Fe University and Polytechnic Hospital (HLF, by its Spanish acronym) - to evaluate the efficacy of 3 different doses of TXA by comparing the standard dose used in the HGU with the 2 doses used in the HLF.</p><p id="par0025" class="elsevierStylePara elsevierViewall">We studied the effect of the TXA doses on fibrinolysis, intra- and postoperative transfusion requirements, bleeding and the appearance of postoperative seizures.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Our working hypothesis is that low doses of TXA in CC with CPB may be as effective as high doses in preventing fibrinolysis, reducing postoperative bleeding and transfusion requirements, and may prevent seizures.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The <span class="elsevierStyleItalic">primary outcome measure</span> was the efficacy of 3 doses of TXA used routinely in 2 hospitals in patients undergoing CC with CPB assessed on the basis of fibrinolysis measured by rotational thromboelastometry (ROTEM®). The effect of the 3 doses of TXA on postoperative bleeding was assessed by measuring chest tube output in the first 12 postoperative hours.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The <span class="elsevierStyleItalic">secondary measures</span> were the use of blood transfusion in the intra and postoperative period and the appearance of seizures.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and method</span><p id="par0045" class="elsevierStylePara elsevierViewall">The study was authorized by the AEMPS (APM-TRA-2018-01) and approved by the clinical research ethics committees of both hospitals. All patients included in the study signed an informed consent form. The inclusion criteria were: age over 18 years, and CC with CPB for valve repair, coronary revascularization, interatrial septal defect closure, cardiac myectomy, ascending aorta repair, and combined surgeries. Exclusion criteria were: emergency surgery, reoperation for bleeding, descending thoracic aorta repair, allergy or hypersensitivity to TXA, inherent hypercoagulability (antithrombin <span class="elsevierStyleSmallCaps">iii</span> deficiency), Factor V Leiden thrombophilia, protein C or S deficiency, dysfibrinogenemia, factor <span class="elsevierStyleSmallCaps">xii</span> deficiency, prothrombin gene mutation, and impossibility of stopping P2Y12 receptor inhibitors and/or anticoagulants within the required safety window due to urgent surgery. If acetylsalicylate acid had already been indicated before surgery, it was maintained throughout the perioperative period, as recommended by leading clinical guidelines<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The TXA dose used in one hospital was compared with the doses used in the other centres in patients who underwent CC with CPB. These are the standard doses included in the protocols implemented in each hospital and were not modified for this study. The HLF protocol is based on Down et al.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>: patients with renal dysfunction (creatinine > 1.3 mg/dl) are given half the dose of TXA administered in patients with normal renal function. In the HGU, the TXA protocol is based on Horrow et al.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>, and the dose is not modified, even in the presence of renal dysfunction.</p><p id="par0055" class="elsevierStylePara elsevierViewall">In the HGU, all patients were given (bolus [mg/kg]/infusion [mg/kg/h]) 10/2 and 50 mg in the CPB circuit (LD group: low-dose TXA).</p><p id="par0060" class="elsevierStylePara elsevierViewall">In the HLF, 2 different regimens (bolus [mg/kg]/infusion [mg/kg/h]) were used:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0065" class="elsevierStylePara elsevierViewall">HD Group (high-dose TXA): 30/10 and 2 mg/kg in the CPB circuit.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0070" class="elsevierStylePara elsevierViewall">ID Group (intermediate-dose TXA): 15/5 with no TXA in the CPB circuit. Patients who presented any of the following characteristics were included in the ID group: over 70 years of age, kidney failure, known neurological disease, ventricular dysfunction, and surgery expected to last more than 6 h.</p></li></ul></p><p id="par0075" class="elsevierStylePara elsevierViewall">The following data were collected: presence of hyperfibrinolysis measured by ROTEM® SIGMA (Instrumentation Laboratory, Werfen Company), administration of blood products in the intra- and postoperative period, chest tube output in the first 12 postoperative hours, and the appearance of seizures.</p><p id="par0080" class="elsevierStylePara elsevierViewall">TXA was started after anaesthesia was induced and was interrupted at the end of the surgery. All patients were given heparin for a target activated clotting time (ACT) > 400 s during CPB. The cell saver was used intraoperatively and blood was reinfused in the immediate postoperative period.</p><p id="par0085" class="elsevierStylePara elsevierViewall">After aortic unclamping, a blood sample was drawn to perform ACT and ROTEM® with the following tests: INTEM, EXTEM, FIBTEM and HEPTEM. Heparin was reversed with protamine sulphate in a ratio of 1 mg per 100 IU of heparin. An extra dose (25 mg) of protamine was administered when post-protamine ACT was higher than baseline. Hyperfibrinolysis was defined as EXTEM maximum lysis (ML) > 15% between 30 and 60 min after start of clotting, or when EXTEM A5 was < 35 mm together with FIBTEM CT > 600 s<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Blood products were administered after reversal of heparin. Transfusion criteria are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">After surgery, the patient was transferred to the Anaesthesia ICU, the prothrombin time, Quick fibrinogen, INR, activated partial thromboplastin time and Clauss fibrinogen were determined, and a new ROTEM® test was performed if previous results had been abnormal or if chest tube output was greater than 100 ml on arrival at the ICU. Red blood cell (RBC) concentrates were transfused to maintain haematocrit above 25%, platelets were administered to achieve around 100,000/mm<span class="elsevierStyleSup">3</span> (in the HLF platelets were administered if the level was below 50,000 mm<span class="elsevierStyleSup">3</span>), fresh frozen plasma (FFP) was administered to obtain INR < 1.4, and fibrinogen was administered for a target of > 150 mg/dl.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Chest tube output in the first 12 postoperative hours and the volume of RBC, FFP and fibrinogen administered intraoperatively and in the first 24 postoperative hours were recorded. If seizures was detected, these were treated according to the protocol in place in each hospital and a brain computed tomography was performed.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0105" class="elsevierStylePara elsevierViewall">Numerical demographic variables were compared using ANOVA if distribution was normal (which was verified by the Shapiro-Wilk test) and the Kruskal-Wallis test when it was not. Categorical variables were compared using Fisher’s test.</p><p id="par0110" class="elsevierStylePara elsevierViewall">If significant differences between variables associated with bleeding were detected, HD and LD, and ID and LD were propensity score-matched.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The criteria used for matching were the EuroSCORE II, preoperative haematocrit, and CPB time, since the literature describes these as independent predictors of postoperative bleeding after CC<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Appropriate statistical tests were performed to compare the LD group with the HD group, and the LD group with the ID group: Fisher's test for categorical variables and the Student's t-test or Mann-Whitney U test for numerical variables. Due to the number of comparisons made, the p-value was corrected using the Benjamini and Hochberg method. Statistical significance, after adjustment, was set at p < 0.05. Binomial logistic regression was performed following a stepwise regression method using the data of 80% of the patients. The dependent variable was transfusion or no transfusion; the independent variables were entered first: EuroSCORE II, preoperative haematocrit, CPB time, and TXA dose (HD, ID, or LD). Single variables were removed until the model with the lowest Akaike information criterion was obtained. In the final model, significance was calculated using the Wald χ<span class="elsevierStyleSup">2</span> test. The goodness of fit was verified using the Hosmer-Lemeshow test. The remaining 20% of the data were used to calculate the accuracy of the model and the relationship between false positives and false negatives using the receiver operating characteristic curve. Statistical analysis was performed on R Version 3.6. 0.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0125" class="elsevierStylePara elsevierViewall">A total of 427 patients were selected between January 2019 and January 2020 - 207 from the HGU and 220 from the HLF. The preoperative characteristics, the duration of CPB and the type of surgery are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The presence of hyperfibrinolysis, blood loss in the first 12 postoperative hours, the number of bags of RBC, and postoperative seizures did not differ significantly between the LD group and the other groups: HD (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>) and ID (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall">Difference were observed in the type of blood products administered (<a class="elsevierStyleCrossRefs" href="#tbl0015">Tables 3 and 4</a>). The LD group received significantly less fibrinogen than the HD and ID groups. The LD group received significantly more FFP than the ID group (LD 0.5 [SD 0.9], ID 0.1 [SD 0.5], p = 4 × 10<span class="elsevierStyleSup">−5</span>, adjusted p = 0.0002); this difference was only observed as a trend when comparing the LD and HD groups (LD 0.3 [SD 0.6], HD 0.1 [SD 0.4], p = 0.027, adjusted p = 0.126).</p><p id="par0135" class="elsevierStylePara elsevierViewall">The binomial logistic regression model showed a correlation in the expected direction between the dependent variable (transfusion or no transfusion) and the independent variables (OR of the EuroSCORE II 1.12 [95% CI 1.03-1.22] and of the duration of CPB 1.01 [95% CI 1.00-1.01]). The OR of preoperative haematocrit was 0.91 (95% CI 0.87-0.95). Although all the ORs were close to 1, the model was able to predict whether or not the patient would need a transfusion in 72% of cases, and the area under the receiver operating characteristic curve was 0.7996 (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0140" class="elsevierStylePara elsevierViewall">The search for an approach that would completely inhibit fibrinolysis has given rise to a wide variety of TXA regimens and doses in CC. In studies in which fibrinolysis was measured by tissue plasminogen activator activity and TXA levels in both plasma and human tissue samples, a TXA plasma concentration of 100 mg/l was required to reduce fibrinolysis by 98%-100%, while 80% inhibition of fibrinolysis was achieved with plasma TXA levels of 10 mg/l<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>. In these studies, the use of a low-dose regimen achieved plasma TXA concentrations of 10 mg/l and inhibited fibrinolysis by 80% during CPB, and was therefore effective in reducing postoperative bleeding<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,12,15</span></a>. High-dose regimens, however, (to achieve plasma concentrations of around 125 mg/l and to inhibit fibrinolysis by 100%) were used because it was thought that a high dose would be required to completely inhibit fibrinolysis in patients with a high risk of bleeding<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>.</p><p id="par0145" class="elsevierStylePara elsevierViewall">The practice of inhibiting fibrinolysis in CC on the basis of measuring plasma TXA levels have been called into question. Low-dose regimens similar to those in our LD group have been shown to achieve plasma concentrations of 55 mg/l, and higher doses (such as those used in our HD group) achieve plasma concentrations of up to 209 mg/l - far higher than the level needed to inhibit fibrinolysis<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a>. A recent systematic review of both <span class="elsevierStyleItalic">in vivo</span> and <span class="elsevierStyleItalic">in vitro</span> pharmacokinetic studies on TXA levels needed to reduce fibrinolysis concluded that TXA concentrations of 10-15 mg/l provide almost total inhibition of fibrinolysis, while concentrations between 5-10 mg/l provide greater than 80% inhibition<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>.</p><p id="par0150" class="elsevierStylePara elsevierViewall">TXA can, in theory, have a prothrombotic effect. Very few studies on this effect in CC have been published. In a study in CC patients, a single 50 mg/kg dose of TXA did not increase the incidence of myocardial infarction in the 30 days after surgery when compared with placebo<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>. Although it is unclear whether higher doses could have long-term thrombotic effects, this is unlikely when TXA is administered in the context of hyperfibrinolysis.</p><p id="par0155" class="elsevierStylePara elsevierViewall">The laboratory diagnosis of hyperfibrinolysis is complex. There are currently no specific high‐throughput fibrinolysis tests, and the lab tests available are far slower than viscoelastic methods (VM) such as ROTEM® and are not standardized, making them difficult to use in emergency situations<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>. In addition, <span class="elsevierStyleItalic">in vitro</span> tests do not always reflect the <span class="elsevierStyleItalic">in vivo</span> situation during CPB<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a>. VMs have poor specificity for fibrinolysis until severe hyperfibrinolysis occurs, but these methods are the most widely used in clinical practice given the absence of a gold standard for measuring fibrinolysis<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a>. In an experimental study in paediatric CC in which fibrinolysis was evaluated using ROTEM®, increasing amounts of tissue plasminogen activator were administered (far higher than the volume usually released during CPB), and TXA was seen to completely abolish fibrinolysis<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a>. A viscoelastometry analyser has recently been developed that provides a test in which fibrinolysis is triggered <span class="elsevierStyleItalic">in vitro</span> using a recombinant tissue plasminogen activator (r-tPA). Fibrinolysis does not appear if plasma TXA levels are sufficient (ClotPro ®; enicor GmbH, Munich, Germany). The test is currently available for research purposes only.</p><p id="par0160" class="elsevierStylePara elsevierViewall">The administration of TXA in CC with CPB, regardless of the dose used, can save up to 300 ml of blood, the equivalent of 1 bag of RBC<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a>. A comparison of a low-dose TXA regimen with a high-dose regimen and placebo in which D-dimer and ML levels were measured by ROTEM® showed that D-dimer increased significantly in the placebo group vs. no increase in the TXA treatment groups. Likewise, there was no evidence of hyperfibrinolysis measured with ROTEM® in any of the groups, and no differences in the incidence of postoperative bleeding or postoperative seizures. This could explain why TXA will not suppress fibrinolysis beyond a certain plasma concentration, but could increase TXA-related side effects<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a>.</p><p id="par0165" class="elsevierStylePara elsevierViewall">The use of biological markers such as D-dimer cannot be considered a gold standard to measure fibrinolysis because these assays vary enormously in sensitivity<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a>. D-dimer and other biological markers such as fibrin monomers and plasmin α2-antiplasmin complex may also be increased in situations such as disseminated intravascular coagulation in which TXA administration could be harmful<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a>. In our study, hyperfibrinolysis was not observed at any TXA dose. One explanation for this is that TXA does not suppress fibrinolysis to a greater degree beyond certain plasma concentrations. Recent meta-analyses have recommended both high-dose<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> and low-dose<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> TXA, showing the lack of consensus on the ideal dosing regimen.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Until 1970, non-surgical bleeding after CPB was thought to be due to an increase in fibrinolytic activity. Later evidence showed that bleeding can be due to a number of factors, among which hyperfibrinolysis has only minor importance since the introduction of frequent ACT monitoring during CPB to prevent under-heparinisation, and with it, over-expression of thrombin<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a>. It is extremely difficult to show a clear association between fibrinolysis and excessive bleeding after CPB, since biological markers vary greatly in the literature<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a>. The EXTEM, FIBTEM and APTEM assays of the ROTEM® test, which contain a reagent that neutralizes the action of heparin, can be used in patients on CPB to determine the status of clotting factors, platelets, fibrinogen, or the presence of hyperfibrinolysis before disconnection from CPB, and to reverse the anticoagulant effects of heparin with protamine<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a>.</p><p id="par0175" class="elsevierStylePara elsevierViewall">In our study, no difference in chest tube output in the first 12 h was observed between the LD and HD groups. The LD group received less fibrinogen than the HD group (p = 0.014) and the ID group (p = 0.040). In contrast, the LD group received significantly more FFP than the ID group, while the difference with the HD group was only detected as a trend. This is probably due to differences in clinical practice between hospitals, because FFP contains between 0.8 and 1 g/300 ml of fibrinogen, and this partly compensates for any possible fibrinogen deficit. In any event, compliance with the transfusion algorithm in place in each hospital will probably reduce these differences, since both comply with current recommendations.</p><p id="par0180" class="elsevierStylePara elsevierViewall">The incidence of seizures in CC increased from between 0.5% and 1% (when antifibrinolytics are not used)<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> to between 6.4% and 7.3% with the administration of high doses of TXA, although age over 70 years, renal failure and the type of surgery (greater in open heart surgery) also contribute to the risk of seizures. Seizures usually present in the immediate postoperative period in the form of generalized tonic-clonic seizures after withdrawal of sedation. They occur when TXA binds to gamma-aminobutyric acid type A receptors and the glycine receptor, which inhibits neurotransmission and increases neuronal excitation<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>. An increase in the incidence of stroke and mortality has also been reported in patients presenting seizures<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,9</span></a>.</p><p id="par0185" class="elsevierStylePara elsevierViewall">In this study, 2 patients in the LD group presented seizures. Neither presented any of the risk factors for seizures previously described (> 70 years, kidney failure, open heart surgery). One possible explanation for this is the history of vascular epilepsy in one of the patients, although he had received his anticonvulsant treatment, and the movements presented by the other at the time of disconnection from CPB. This second case was clinically diagnosed as muscle twitching, but a brain computed tomography was performed due to the suspicion that they could be seizure-induced. The scan, however, showed no pathological findings. As our study was not blinded, there is a risk that seizure-induced movements might have been over-diagnosed. In both cases, follow-up brain computed tomography was normal. The low TXA doses administered to some patients in the ID group could explain why no seizures were observed in the ID and HD groups, despite receiving higher doses of TXA.</p><p id="par0190" class="elsevierStylePara elsevierViewall">Aside from reducing bleeding, the administration of higher doses of TXA than those used in the LD group (single dose of 50 mg/kg) produces a plasmin blockade that modulates both plasma cytokine levels and the number and expression of circulating leukocyte surface markers, thereby significantly reducing infection rates in patients without diabetes<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a>.</p><p id="par0195" class="elsevierStylePara elsevierViewall">One of the limitations of our study is the absence of a gold-standard, readily available method for measuring fibrinolysis in clinical practice. Secondly, the TXA dose required to effectively inhibit fibrinolysis <span class="elsevierStyleItalic">in vivo</span> is still unclear, and there is no consensus on the optimal dosing regimen of TXA in on-pump CC. The use of effective doses of TXA could diminish the role played by fibrinolysis in postoperative bleeding.</p><p id="par0200" class="elsevierStylePara elsevierViewall">The use of clinical outcomes such as chest tube output or blood transfusions can be influenced by many variables, including clinician preferences, standard practice, and the protocols in place in each hospital. In this study, the same transfusion criteria were followed in both hospitals with the exception of platelets, although no differences in administration were observed.</p><p id="par0205" class="elsevierStylePara elsevierViewall">In our study, we used 3 independent predictors of bleeding in the postoperative period of CC: EuroSCORE II, preoperative haematocrit and CPB time<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>; however, these may not be related to the TXA dosing regimen. The scant effect these variables had on the model is not surprising, given the existence of other variables that may or may not affect the transfusion of blood products.</p><p id="par0210" class="elsevierStylePara elsevierViewall">A larger sample could have increased statistical power and revealed smaller differences between groups. It would also have provided more data for the regression model, and therefore increased its accuracy.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusions</span><p id="par0215" class="elsevierStylePara elsevierViewall">Hyperfibrinolysis was not detected in any of our study groups, and there were no significant differences between the 3 therapeutic approaches in terms of bleeding or the number of patients transfused. High doses were not associated with an increased risk of seizures. Further investigation is required to identify the ideal dose of TXA and to develop tools that can measure fibrinolysis in clinical practice. New lines of research in the field of thromboelastography are under way to identify new clotting parameters that can be used to assess fibrinolysis. We recorded the presence of hyperfibrinolysis on rotational thromboelastometry, intra- and postoperative administration of blood products, chest tube output within the first 12 h, and incidence of convulsions. Univariate and multivariate comparisons were performed. Se estudió la presencia de hiperfibrinólisis por tromboelastometría rotacional, la administración de hemoderivados en el intra y postoperatorio, el débito por drenajes en las 12 primeras horas postoperatorias y la aparición de convulsiones. Se realizó un análisis comparativo univariante y multivariante. El análisis univariante de todas las categorías se llevó a cabo tras realizar un emparejamiento por índice de propensión entre DB y DA, y entre DB y DI.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0090" class="elsevierStyleSimplePara elsevierViewall">No hubo diferencias estadísticamente significativas en la hiperfibrinólisis, la administración de concentrados de hematíes, el débito por drenajes en las 12 primeras horas ni la aparición de convulsiones. El grupo DB recibió menos fibrinógeno que el grupo DA (p = 0,014) y DI (p = 0,040).
