In the control X-ray, the catheter was observed to be located on the left side of the mediastinum at the level of the cardiovascular silhouette. Cannulation of the internal mammary artery was suggested, and we decided to approach the internal jugular vein, leaving the first catheter to force the passage to the right. However, after radiological control, it was observed that the catheter was directed cephalad through the external jugular vein. After consultation with a radiologist and reconstruction of a previous CT scan, a persistent left superior vena cava was revealed (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). This anomaly has a prevalence of .3%–.5%.
