Sedation has to strike a balance between the risk of interruption, which may worsen brain injury by increasing intracranial pressure, and the benefits of faster neurological recovery<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">While orotracheal intubation (OTI) is easier to perform but less comfortable, it has been argued that nasotracheal intubation (NTI) increases patient comfort and tolerance of the tube<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>. Patient well-being affects the choice of analgesia and sedation for pain and agitation management, which have been shown to have an impact on delirium and duration of mechanical ventilation (MV)<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4–6</span></a>. It is therefore reasonable to conjecture that the improved tube tolerance provided by INT may lead to a reduction in sedation and duration of MV. However, to date no study has investigated and demonstrated a clear benefit of NTI over OTI in neurocritically ill patients.</p><p id="par0015" class="elsevierStylePara elsevierViewall">As a primary objective we assessed whether substitution of OTI for NTI performed during the first week was associated with reduced duration of MV compared to OTI, in a neurocritical care setting.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The secondary objective was to compare safety and duration of sedation.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design</span><p id="par0025" class="elsevierStylePara elsevierViewall">This was a retrospective cohort study including all consecutive patients admitted to the institutional neurocritical ICU between 1 January 2011 and 25 June 2020.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Environment</span><p id="par0030" class="elsevierStylePara elsevierViewall">We screened for inclusion all patients admitted to the neuro-ICU of the University Hospital of Padua. This is an eight-bed ICU that admits only patients with acute neurological or neurosurgical disease.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Ethics</span><p id="par0035" class="elsevierStylePara elsevierViewall">This retrospective cohort study (IRB#0042597/2020) was approved by the institutional ethics committee on 25 June 2020. Given the retrospective nature of the study, the institutional ethics committee approved the waiver of consent.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Inclusion/exclusion criteria</span><p id="par0040" class="elsevierStylePara elsevierViewall">All patients older than 18 years with prolonged intubation (defined as more than seven days) were included. Exclusion criteria were: history of lung disease or lung surgery, cervical spine injury greater than C5, rib fractures, pulmonary contusions or pneumonia at ICU admission, Glasgow Coma Scale (GCS) score with motor response less than 6 at ICU discharge (implicitly excluding patients whose outcome was death or severe neurological deterioration).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Nasotracheal Intubation</span><p id="par0045" class="elsevierStylePara elsevierViewall">The decision to substitute OTI for NTI was not standardised and was made at the discretion of the patients' primary care physicians. It was decided by consensus during the daily assessment, all physicians being able to perform the manoeuvre. Reasons for the procedure were lip/tongue pressure injury, patient discomfort or estimation of prolonged ventilation.</p><p id="par0050" class="elsevierStylePara elsevierViewall">However, NTI is standardised in our institution. It is performed during the first and seventh days after ICU admission, using the fibreoptic bronchoscope.</p><p id="par0055" class="elsevierStylePara elsevierViewall">NTI is performed in patients with intracerebral pressure<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>20<span class="elsevierStyleHsp" style=""></span>mmHg and cerebral perfusion pressure<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>60<span class="elsevierStyleHsp" style=""></span>mmHg at the start of the procedure. Intranasal lidocaine is administered into the nostril followed by 2<span class="elsevierStyleHsp" style=""></span>mcg/kg intravenous fentanyl. During the procedure, propofol is administered in titrated boluses to maintain haemodynamic and intracranial pressure stability (mean arterial pressure<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>60<span class="elsevierStyleHsp" style=""></span>mmHg and intracerebral pressure<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>20<span class="elsevierStyleHsp" style=""></span>mmHg), with addition of norepinephrine infusion (0.05–0.2<span class="elsevierStyleHsp" style=""></span>mcg/kg/min) as needed to achieve haemodynamic goals. No antibiotic prophylaxis was administered for the procedure.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">The primary outcome was the mechanical ventilation duration</span><p id="par0060" class="elsevierStylePara elsevierViewall">Secondary outcomes were the comparison of the incidence of complications (accidental extubation, presence of sinus exudate, ventilator-associated pneumonia (VAP), tube decubitus injury), incidence and times of tracheostomy, duration of sedation, hospital length of stay and ICU length of stay (LOS).</p><p id="par0065" class="elsevierStylePara elsevierViewall">In addition, the following data were collected: age (years), sex (M/F), weight (kg), diagnosis on admission, Sequential Organ Failure Assessment (SOFA) scale on admission, GCS on day of intubation and day of tube replacement.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Definitions</span><p id="par0070" class="elsevierStylePara elsevierViewall">Sedation was defined as the administration of at least one of the following drugs, regardless of dose and according to our institutional protocol for sedation: propofol, midazolam and remifentanil. The dosing regimen was midazolam .2–.4<span class="elsevierStyleHsp" style=""></span>mg/kg/h or propofol 3–4<span class="elsevierStyleHsp" style=""></span>mg/kg/h combined with remifentanil .05–.2, initiated during ICU admission and titrated to achieve a bispectral index between 40 and 60, intracerebral pressure<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>20<span class="elsevierStyleHsp" style=""></span>mmHg and cerebral perfusion pressure<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>60<span class="elsevierStyleHsp" style=""></span>mmHg. Norepinephrine infusion .05–.3<span class="elsevierStyleHsp" style=""></span>mcg/kg/min is used to achieve cerebral perfusion pressure as needed. Based on daily assessment considering brain computed tomography, intracerebral pressure and cerebral perfusion pressure, the rate of sedative infusion is gradually reduced, eventually stopping when clinical stability is achieved. Extubation is based on daily clinical assessment and is performed when the patient is conscious, does not require frequent suctioning (greater than every two to three hours), is able to cough, has a PaO2/FiO2 ratio greater than 300 and has successfully passed a spontaneous breathing attempt.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The presence of sinus fluid levels on computed tomography (CT) is considered an indicator of rhinosinusitis. VAP was defined as the need for antibiotic therapy following radiological evidence of infiltrate (on chest x-ray, lung ultrasound or CT) and the presence of at least two of the following criteria: 1. fever or hypothermia, 2. leukocytosis or leukopenia, 3. increased volume of respiratory secretions<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>. Tube decubitus lesions are defined as tube-related pressure lesions.</p><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Statistical analysis</span><p id="par0080" class="elsevierStylePara elsevierViewall">To calculate an adequate sample size for the paired analysis, we considered 15.80<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.96 as the duration of MV in neurocritical patients<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a>. Our aim was to reflect a 30% reduction in MV duration in the NTI group compared to the OTI group, which resulted in at least sixty-four matched pairs (128 total matched patients) with a power of 85% and a significance level of .05.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The normal distribution of the data for each continuous variable was analysed using the Shapiro-Wilk test. Results for continuous variables with normal distributions were expressed as mean and standard deviation. Results for non-normal distributions were expressed as median and first interquartile ranges. Analysis of data with normal or non-normal distributions was performed using the two-way Student's <span class="elsevierStyleItalic">t</span>-test and Mann-Whitney <span class="elsevierStyleItalic">U</span>-test, respectively.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The results of the analyses of categorical variables were reported as percentages and compared between groups using Fisher's <span class="elsevierStyleItalic">χ</span><span class="elsevierStyleSup">2</span> or Fisher's exact test, as appropriate.</p><p id="par0095" class="elsevierStylePara elsevierViewall">To neutralise the effect of confounding variables, a propensity score matching analysis was performed. The aim was to balance the probability of receiving or not receiving an NTI procedure. For this reason, we identified the following variables as possibly clinically relevant choices: age, sex, weight, SOFA on admission, GCS at intubation and diagnosis on admission.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Propensity matching was performed using the closest closeness method with a calibre of .1<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Both NTI and OTI cohorts were comparable, as they would have had the same probability. The standardised mean difference was used to test for imbalances after matching, with a value greater than .2 being considered an imbalanced match.</p><p id="par0105" class="elsevierStylePara elsevierViewall">To determine the strength and direction of the association between the two variables, the Bravais-Pearson correlation test was used for normally distributed variables and Spearman's rank correlation test for variables that did not meet the assumptions of normal distribution. <span class="elsevierStyleItalic">P</span>-values ≤.05 were considered statistically significant. All statistical analyses were performed using R version 4.0.2 (2020-06-22).</p></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Results</span><p id="par0110" class="elsevierStylePara elsevierViewall">The digital medical records of the 4,030 patients admitted to our ICU during the period of interest were screened. Three thousand five hundred and forty-five patients were excluded because the duration of MV was less than seven days, and 173 patients had other pre-specified exclusion criteria (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Therefore, 312 patients were finally included. No complications related to tube replacement were reported.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">Propensity score analysis identified 74 matched pairs of patients with adequate balance (standardised difference<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.2 for all variables considered). Baseline demographical and clinical characteristics of included patients are reported in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall">Balancing the pre-determined confounders, we found statistically significant differences in sedation and duration of mechanical ventilation. Duration of sedation and MV were statistically longer in the OTI group compared with the NTI group (9 [6.25–12.75] days vs. 6 [4–9] days, <span class="elsevierStyleItalic">P</span>-value <.001; 17.44<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.42 days vs. 15.06<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.97 days, <span class="elsevierStyleItalic">P</span>-value .031, respectively). No differences were identified in terms of complications, incidence of tracheostomies and times, ICU stay or hospital stay (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). However, self-limited epistaxis as a direct consequence of the manoeuvre was reported in 16 patients (21.33%) in the NTI group.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall">A moderate but significant correlation was found between the day of tube replacement and duration of sedation (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.453, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001).</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Discussion</span><p id="par0130" class="elsevierStylePara elsevierViewall">To the best of our knowledge, this is the first study to show that replacing the orotracheal tube with a nasotracheal tube in the first week of admission could lead to a reduction in the duration of sedation and MV. Although an increase in patient comfort was claimed in the NTI group, due to reduced difficulty in swallowing secretions, communication by lip movement and oral hygiene<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>, the literature still lacks clear evidence. An older randomised controlled study comparing NTI and OTI in 300 patients<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> found no differences in relation to sedative doses between the NTI and OTI groups. However, bias due to the reported high mortality (39%) acted as an important confounding factor in relation to this outcome.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In our study, patients receiving NTI reflected a two-day reduction in terms of duration of mechanical ventilation. The clinical significance of this difference is debatable, and only future prospective trials will be able to add evidence on this point.</p><p id="par0140" class="elsevierStylePara elsevierViewall">We found only a moderate correlation between day of tube replacement and duration of sedation, which means that NTI causes a reduction in the duration of sedation, and that other factors not identified in the study are associated with the duration of sedation.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Regarding the duration of MV, it is known that the latter is burdened by various complications<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>, the proportions of which increase in relation to the duration of MV. Several factors have been proposed as predictors of MV duration: some are patient-related situations at presentation<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> and others depend on the quality of patient management<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a>. By demonstrating the association between nasal intubation and reduced duration of MV, we introduced a new, valuable and controllable item that could influence the duration of MV.</p><p id="par0150" class="elsevierStylePara elsevierViewall">We found no differences in the complications considered between the two groups, apart from the rate of epistaxis. Our findings are similar to those of previous studies, especially with regard to the incidence of sinusitis, decubitus lesions and VAP<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,16</span></a>.</p><p id="par0155" class="elsevierStylePara elsevierViewall">In our cohort of patients, the incidence of VAP was relatively high (around 30% in both groups). Neurocritical patients represent a cohort of patients with a reported increased risk of VAP (up to 50%<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>) compared to other patients requiring ICU. Furthermore, the definition of VAP we used was arbitrary, so a different definition (e.g., requiring a positive culture) might have yielded different results, probably with a lower incidence of VAP.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Epistaxis is a common complication of NTI and although various measures could be taken to minimise this risk (e.g., application of vasoconstrictor drugs and tube softening), a certain percentage should be considered unavoidable<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>. This complication should be taken into account, especially in patients with coagulation disorders, as massive epistaxis could complicate airway management. In our series, a limited number of patients had self-limited epistaxis, without the need for dedicated treatment. In addition, no patient who developed epistaxis had any other complications related to tube replacement or failure of nasal intubation.</p><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Limitations</span><p id="par0165" class="elsevierStylePara elsevierViewall">Our study had several limitations. Starting with the methods, the main limitation lies in the retrospective and single-centre nature of the study. In particular, this study was conducted in an institution where NTI is routinely performed, which might limit the generalisability of our results to other centres. Secondly, the complication rate might have been affected by recording bias, given the absence in the analysed dataset of specific monitoring and loading tools for endotracheal tube-related complications and associated comfort. In addition, our study has several secondary outcomes. We acknowledge that it may have less statistical power to detect significant differences.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Although our study has clearly revealed the benefits of early replacement with a nasal tube in terms of duration of sedation and MV, the optimal timing of tube replacement remains to be determined. Based on our data it is also legitimate to infer that, when conditions permit, the choice to intubate the patient directly through the nose may be a wise one. It has not been possible to retrieve direct measures of patient comfort. The RASS<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> scale is one of the most widely adopted scales to define the optimal level of patient sedation, although it does not seem to affect the duration of MV<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a>. For this reason, and given that neurological impairment could cause potential bias, we decided not to retrieve it for the purpose of the study. Furthermore, we did not routinely use other dedicated scales to assess global patient comfort, nor are there, to the authors' knowledge, specific scales related to tube comfort.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Third, we recognise that neurocritical patients are a specific population, and that the presence of neurological deficits is a potential bias in assessing patient tolerance to the endotracheal tube.</p></span></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Conclusions</span><p id="par0180" class="elsevierStylePara elsevierViewall">In neurocritical care units using NTI in standard patient management, the nasotracheal route may be associated with shorter mechanical ventilation time.
