Adequate control of acute postoperative pain (APP) is an indicator of quality of care, both in terms of efficiency and perceived quality of the system. Analgesic protocols have changed in recent years, but the prevalence of APP has not changed to the same extent.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The current consensus on combining drugs as the most efficient method for effective pain control<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> has led to the development of multimodal strategies. This achieves the synergistic effect of combining drugs and reducing the adverse effects of opioid use.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> For years, multimodal analgesic strategies have incorporated peripheral and fascial space block as an essential component of good pain control.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Single-puncture administration of a local anaesthetic around a nerve, plexus, or in a fascial space is of limited duration. Cessation of its effect may be associated with acute rebound that worsens postoperative recovery.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> If the duration of action needs to be prolonged, increasing the dose of local anaesthetic is not a valid option because it increases the risk of toxicity, neurological damage, etc.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Continuous analgesia via percutaneous catheter in the nerve, plexus, or fascial space has a 20%–50% failure rate due to migration, obstruction, or breakage of the catheter.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a> Successful continuous analgesia techniques require adequate training of professionals to reduce the error rate.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Liposomal bupivacaine or the possibility of adding an adjuvant to the local anaesthetic are attractive options to achieve the goal of prolonged analgesia. Both solutions aim to prolong the duration of action of the local anaesthetic without increasing adverse events. Encapsulating local anaesthetic in a lipid sheath is intended to prolong its release. The main advantage is that it does not require the use of a catheter, although its use is limited by problems of registration (it is not used in Europe) and availability, and meta-analyses do not provide adequate benefit compared to infiltration with normal anaesthetic.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Adding an adjuvant drug to local anaesthetics may accelerate sensory blockade and prolong the analgesic effect. There are multiple drugs that prolong the duration of a peripheral block, but there is debate as to which are the most indicated.</p><p id="par0025" class="elsevierStylePara elsevierViewall">This review examines studies exploring perineural or fascial space administration of the most commonly used adjuvant drugs over the last five years combined with a local anaesthetic. The main objective was to assess whether the addition of an adjuvant to the local anaesthetic is effective in prolonging postoperative analgesia and reducing the use of opioids. The secondary objective was to examine whether this practice is safe by identifying the presence of adverse events associated with the use of the adjuvant.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Material and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">Studies using adjuvants combined with local anaesthetics administered via perineural (nerve or plexus) or fascial spaces in elective surgical procedures were included in the review.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Eligible study types included randomised controlled studies, non-randomised prospective comparative studies, prospective observational studies, case series, and case reports. The level of evidence was graded according to the 2011 Oxford Centre for Evidence-based Medicine levels of evidence document: level 1, evidence from randomised trials or systematic reviews of randomised trials; level 2, evidence from randomised trials or observational studies; level 3, evidence from non-randomised controlled cohorts or follow-up studies; level 4, evidence from case series, case-control studies, or case-control studies.</p><p id="par0040" class="elsevierStylePara elsevierViewall">We used the Cochrane systematic review methodology to conduct the review.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> The search results were reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Peer review guidance was followed to prevent potential search errors.</p><p id="par0045" class="elsevierStylePara elsevierViewall">We searched the databases: MEDLINE, Medline in process/ePubs, Embase, Cochrane Central Register of Controlled Trials, and the Cochrane Database of Systematic Reviews. We also searched the PubMed database (National Library of Medicine) for citations not included in Medline, Scopus, Web of Science and Clinicaltrials.gov (National Institutes of Health), and World Health.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The period assessed was from 1 January 2015 to 31 January 2021. From a preliminary search, full-text literature was extracted for possible keywords and appropriate vocabulary terms. After analysing the full-text literature the keywords used (MeSH) were “adjuvants regional anaesthesia” and “adjuvants peripheral nerve block”. No restrictions on type, status or language of publication were applied. Potentially eligible studies were identified by selecting the title and abstract of all publications identified through the literature search in a population over 18 years of age. The full text of these studies was then obtained for further evaluation. Prolongation time of sensory and motor nerve blockade, onset of action of the blockade, possible adverse effects, and advantages and disadvantages of the adjuvants under review were assessed.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Two authors (MF and SL) independently assessed and selected the studies for inclusion in the review. Disagreements about the eligibility of an article were resolved by a third review author (CA), who analysed the risk of bias of all articles included in the review.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> presents the results of the search strategy.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">The electronic search identified four Cochrane reviews with our search criteria “adjuvant regional anaesthesia”, but only one met the objective of this review<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> and only one, the same, with “adjuvant regional anaesthesia”.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Of the 92 eligible studies, 13 were literature reviews and were therefore excluded from the analysis.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Seventy-nine articles were included in the review, with a clear prevalence of those related to the use of dexamethasone (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>24) and dexmedetomidine (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>33). Other adjuvants evaluated were magnesium sulphate (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>6), clonidine (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>4), opioids (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3), and vitamin B1 (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1). Of the comparative studies between adjuvants in this time (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>8), evaluating the efficacy of dexamethasone vs. dexmedetomidine in four published studies drew the attention of the scientific society.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Based on these data, we can state that there are currently two adjuvants that clearly stand out from the others: dexamethasone and dexmedetomidine, displacing those that seemed to be the most widely used to date (opioids, bicarbonate).</p><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Dexmedetomidine</span><p id="par0080" class="elsevierStylePara elsevierViewall">Dexmedetomidine works as an alpha-2 agonist with a sympatholytic effect to reduce sympathetic nervous system outflow of noradrenaline. Its high receptor affinity makes it almost a pure agonist. It can cause sedation, anxiolysis, reduced analgesia, reduced salivary secretion, and minimal respiratory depression, with bradycardia and hypotension as side effects. It has a similar perineural mechanism of action to clonidine, but better clinical outcomes.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">Its clinical effect after intravenous administration is due to its effect on peripheral and central receptors. The sedative effect is associated with stimulation of receptors located in the locus coeruleus. However, the analgesic effect is associated with stimulation of receptors located in the medullary posterior horn, inhibiting the release of substance P and glutamine.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Under physiological conditions, expression of alpha-2 receptors in peripheral nerves is absent,<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> but when clonidine or dexmedetomidine is added to the local anaesthetic, the analgesic effect is prolonged by peripheral and central action. The direct effect on peripheral fibres is based on inhibition of action potential generation in unmyelinated C-fibres and cell hyperpolarisation through activation of voltage-dependent channels.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">The most recent scientific evidence from published studies indicates that dexmedetomidine prolongs the duration of intravenous regional anaesthesia, peripheral nerve blocks and intradural anaesthesia. The controversial aspect is possible peripheral neurotoxicity in patients with diabetic neuropathy,<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,16</span></a> or the need to monitor patients intraoperatively for potential, albeit reversible, damage to heart rate (bradycardia).<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">In our analysis of the selected studies (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), we found that perineural dexmedetomidine (50−60<span class="elsevierStyleHsp" style=""></span>μg) in brachial plexus block increases the mean duration of analgesia. We highlight the meta-analysis by Vorobeichik et al.,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> which demonstrates that its administration prolongs sensory and motor block duration (264<span class="elsevierStyleHsp" style=""></span>min vs. 228<span class="elsevierStyleHsp" style=""></span>min and 192<span class="elsevierStyleHsp" style=""></span>min, respectively). This analgesic and quality benefit must be weighed against the risk of prolonged motor block or the abovementioned cardiovascular effects. Helal et al. report similar efficacy and side effects with administration in femoral-sciatic nerve.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">Most studies use doses of 1<span class="elsevierStyleHsp" style=""></span>μg/kg, both at the peripheral nerve level<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> and at the fascial space level,<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> always combined with local anaesthetic. In the latter context, Sun et al. in a meta-analysis including 20 studies with 1212 patients, conclude that the addition of dexmedetomidine reduced postoperative pain scores 8<span class="elsevierStyleHsp" style=""></span>h postoperatively (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.001) and opioid consumption (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001) compared to the control group. It also prolonged the effect of tranversus abdominis blockade, without affecting the incidence of postoperative nausea and vomiting, hypotension, or bradycardia.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Its potential adverse effect profile requires caution in recommending it as an adjuvant in all clinical settings; however, the scientific evidence is inconsistent. In vivo studies evaluating dexmedetomidine in combination with ropivacaine reveal its neuroprotective action by reducing perineural inflammation in rats.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> The effect could be secondary to inhibition of NF-kβ factor activity and thus decrease transcription of inflammatory cytokines and modulation of mast cell degranulation. Associated with lidocaine, it decreases the effective concentration of the drug.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> Therefore, although it is a relatively safe drug, we must not forget its neurotoxic effects,<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> remain cautious and continue researching.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Corticosteroids: dexamethasone</span><p id="par0115" class="elsevierStylePara elsevierViewall">The mechanism responsible for prolonging the duration of action of a block after perineural administration of dexamethasone together with local anaesthetic is very complex.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> It suppresses the activity of unmyelinated C-fibres, inhibits potassium channels, has a local vasoconstrictor effect, promotes hyperpolarisation of peripheral nerve fibres, blocks synaptic transmission, and reduces perineural inflammation. At the systemic level, it has an anti-inflammatory effect and a central effect on alpha-2 receptors.</p><p id="par0120" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> summarises the main studies. Several meta-analyses have evaluated perineural dexamethasone and its use has become common in clinical practice.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,26–28</span></a> In addition to prolonging the analgesic effect of local anaesthetics, perineural administration prolongs the mean duration of motor nerve blockade, reducing pain assessed postoperatively both at rest and on movement at 8−24<span class="elsevierStyleHsp" style=""></span>h and thus cumulative morphine consumption. In axillary block, 4<span class="elsevierStyleHsp" style=""></span>mg dexamethasone appears to be the ceiling with short- and medium-duration local anaesthetics.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a></p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall">There is controversy as to whether the same effect is seen with intravenous dexamethasone administration. Different studies show that both intravenous and perineural administration of dexamethasone prolong the duration of action of local anaesthetics, without providing consensus on whether either administration offers any advantage over the other.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">30</span></a> Studies report that intravenous administration does not provide longer duration of blockade than placebo (saline), although this finding may be because the participants were volunteers and there was no surgical incision. It is possible that the absence of surgical injury did not allow dexamethasone the opportunity to present its anti-inflammatory effect.</p><p id="par0130" class="elsevierStylePara elsevierViewall">However, the review by Heesen et al.,<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> with 10 studies evaluated and 783 patients, found a statistically significant difference in prolongation of analgesia with perineural administration, albeit with a low level of evidence. The meta-analysis by Zorrilla-Vaca<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> reports, after evaluating 13 studies with 937 patients, that perineural administration of dexamethasone produces better analgesic outcomes than intravenous injection, and further supports the hypothesis of local interaction between the corticosteroid and the nerve as one of the mechanisms of pain modulation, because systemic absorption alone could not explain the superior quality of the blockade.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">Dexamethasone may induce increased blood glucose levels. After perineural administration the increase is smaller than after intravenous injection, the maximum increase being found at four hours.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">33</span></a> Concerns about the theory of delayed wound healing or systemic infection were not upheld in a systematic review.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">34</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Regarding neurotoxicity, in vivo studies show that dexamethasone attenuates the cytotoxicity of bupivacaine in mouse neuroblastoma cells<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">33</span></a> or of ropivacaine in the medullary posterior horn, but could crystallise with this same anaesthetic at the nerve level.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">34</span></a> In humans, studies show no association between dexamethasone administration and neurological complications,<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,27</span></a> but crystallisation could occur with ropivacaine.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">35</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">Nevertheless, the lack of evidence for damage is not evidence of its absence. Given the low incidence of nerve damage subsequent to regional anaesthetic techniques, a sample size of 16,000 patients has been estimated to show a complication rate of .4%.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">36</span></a></p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Magnesium sulphate</span><p id="par0150" class="elsevierStylePara elsevierViewall">Magnesium is an N-methyl-D-aspartate (NMDA) receptor antagonist, which has been shown to increase the excitatory threshold, higher in A-β fibres than in C-fibres.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">37</span></a> Its mechanism of action after perineural administration is probably due to the effects of its positive charge on the action potential of neuronal cells and its physiological action as a calcium antagonist.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">38,39</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">The study by Beiranvand et al.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">40</span></a> concludes that the addition of magnesium sulphate to lidocaine prolongs the duration of sensory and motor block in axillary nerve block. Similar results are found by Jebali et al.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">41</span></a> in femoral nerve block. However, Imani et al.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">42</span></a> do not report the same when combining magnesium sulphate with ropivacaine in transverse abdominis plane block; their results show no difference in pain score and time to rescue analgesia between the groups compared. The six studies found are shown in <a class="elsevierStyleCrossRef" href="#sec0060">Appendix B</a> Table 1 of the supplementary material.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Magnesium sulphate could be a promising adjuvant in peripheral nerve block. However, more studies are needed in view of the mixed results.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Opioids: buprenorphine</span><p id="par0165" class="elsevierStylePara elsevierViewall">Many studies have been conducted over the years to determine the benefits of perineural administration of opioids as an adjuvant to local anaesthetics. Even now, it is difficult to determine whether this analgesic effect is the result only of its effect on peripheral receptors or whether it is a central effect following redistribution of the drug to the central compartment.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">43</span></a></p><p id="par0170" class="elsevierStylePara elsevierViewall">Within the opioid family, buprenorphine, a partial agonist at the μ opioid receptor and an antagonist at the k-receptor, has been shown to prolong the duration of analgesia after perineural administration with local anaesthetic. Its mechanism of action is probably due to concentration-dependent blockade of sodium-dependent voltage-gated channels, inhibiting the generation of action potentials and interacting with μ receptors in unmyelinated C-fibre axons.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">44</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">In the analysis of different studies on its effectiveness, their ambiguous results do not support recommending its use. On the one hand, the meta-analysis by Schnabel et al.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">45</span></a> evaluated the addition of buprenorphine to local anaesthetic in axillary, femoral and sciatic nerve block. They showed that perineural administration of .1–.3<span class="elsevierStyleHsp" style=""></span>mg together with the local anaesthetic would prolong postoperative analgesia, without differences in the establishment of sensory and motor nerve blockade. This effect was not observed when intravenous buprenorphine was used, clearly indicating a local mechanism of action. On the other hand, van Beek et al.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">46</span></a> report that the addition of buprenorphine in femoral nerve block did not result in postoperative improvement in patients undergoing knee surgery. Both studies report an increased risk of postoperative nausea and vomiting, as well as pruritus.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">46</span></a> Neurotoxicity has been studied in vitro and in vitro in rats<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">47</span></a> and no nerve damage has been found. The three studies found are shown in <a class="elsevierStyleCrossRef" href="#sec0060">Appendix B</a> Table 2 of the supplementary material.</p><p id="par0180" class="elsevierStylePara elsevierViewall">Buprenorphine as an adjuvant to local anaesthetics is a further option due to its effects on prolonging the effect, provided the possibility of manifestation of its adverse effects is assessed.</p><p id="par0185" class="elsevierStylePara elsevierViewall">The characteristics of each adjuvant are important, the ideal complement should always be sought. <a class="elsevierStyleCrossRef" href="#sec0060">Appendix B</a> Fig. 1 of the supplementary material shows the most important characteristics of dexmedetomidine and dexamethasone.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Conclusion</span><p id="par0190" class="elsevierStylePara elsevierViewall">Outpatient surgery programmes and more recently the implementation of ERAS (Enhanced Recovery After Surgery) protocols have promoted anaesthesia and analgesia techniques based on peripheral and fascial space blocks. In both, effective pain control while minimising the use of opioids are key to the success of these programmes.</p><p id="par0195" class="elsevierStylePara elsevierViewall">In this context, the use of adjuvant drugs with local anaesthetics is continuously evolving, adding new agents to improve analgesia. The choice of adjuvant for peripheral regional anaesthesia should consider the duration of analgesia and sensory nerve block vs. the disadvantages of the duration of motor nerve block and its adverse effects (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0200" class="elsevierStylePara elsevierViewall">Multiple meta-analyses evaluate the duration of adjuvants added to local anaesthetics and suggest superior blockade characteristics with dexmedetomidine vs. clonidine and dexamethasone vs. dexmedetomidine when administered perineurally.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,48</span></a></p><p id="par0205" class="elsevierStylePara elsevierViewall">Of the adjuvants used, perineural dexamethasone appears to show the lowest risk of side effects (sedation, hypotension) when compared to dexmedetomidine.</p><p id="par0210" class="elsevierStylePara elsevierViewall">In the absence of perineural administration, the intravenous route at doses of .1–.2<span class="elsevierStyleHsp" style=""></span>mg/kg reduces postoperative pain and opioid consumption, although the effectiveness is superior with local administration. Both routes provide greater patient comfort in the immediate postoperative period, including overnight.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">49</span></a> Used intravenously, dexamethasone has an antiemetic effect that should be taken into consideration. But we must not lose sight of one fact: with the exception of adrenaline, none of the drugs in this review have been approved for use as adjuvants, and therefore all of them are used off-label. It is important to remember that perineural administration is restricted to preservative-free drugs.</p><p id="par0215" class="elsevierStylePara elsevierViewall">Our conclusions are that there is moderate quality evidence suggesting that dexamethasone used as an adjuvant in peripheral nerve blocks prolongs the duration of sensory nerve block and is effective in reducing postoperative pain and opioid consumption. The effect of a local anaesthetic can be prolonged by administering an adjuvant. In this systematic review, we included studies on adjuvants associated with local anaesthetics in peripheral nerve blocks published in the last 5 years in order to evaluate their effectiveness. The results were reported according to the PRISMA guidelines. The 79 studies selected using our criteria showed a clear prevalence of dexamethasone (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>24) and dexmedetomidine (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>33) over other adjuvants. Different meta-analyses comparing adjuvants suggest that dexamethasone administered perineurally achieves superior blockade with fewer side effects than dexmedetomidine.
