The surgeon was informed of the finding and a ventriculotomy was performed at the apex extended to the anterior wall, but the thrombus was not found. However, prior to cardiopulmonary bypass a huge and very mobile thrombus was identified in the left ventricular outflow tract with transesophageal echocardiography (TEE) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).
