In 2020, it accounted for 11.7% of all cancer cases in women, and was the fifth leading cause of cancer-related mortality worldwide.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Although surveillance for recurrence is prioritised in breast cancer survivors, many are affected by complications such as chronic pain, which has a significant impact on their quality of life (QoL).</p><p id="par0010" class="elsevierStylePara elsevierViewall">The International Association for the Study of Pain (IASP) defines Chronic Pain after Breast Surgery (CPBS) as pain that develops or increases in intensity after a surgical procedure in the breast area that persists beyond the healing process, i.e., at least 3 months after the initiating event. Other causes of pain, such as pre-existing pain, infection, or malignancy, have to be excluded in all cases of chronic postsurgical pain.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Initially described as a low incident event, CPBS is now an increasingly recognized problem<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a> with a prevalence ranging from 25% to 60%, and with around 14% of patients presenting moderate to severe chronic pain. CPBS is neuropathic in 25%–31% of cases.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Some recent studies have attempted to identify the perioperative risk factors associated with CPBS; however, the results have been inconsistent.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Younger age, radiation therapy, axillary node dissection, and severity of acute postoperative pain are the most consistent risk factors described in literature.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,5,6</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Health-related quality of life is an important endpoint in cancer-related clinical trials. It measures the degree to which a chronic disease or condition and its treatment can affect the patient’s QoL.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a> In previous studies, breast cancer survivors experienced worse social, cognitive, emotional, physical, and role function up to 10 years after diagnosis compared to the general population.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> QoL evaluation is important for prognosis and can indicate the need for more effective treatment in cancer patients.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The aim of this study was to describe incidence, characteristics and factors associated with CPBS in female patients undergoing breast cancer surgery and its impact on patient QoL.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">We performed an observational prospective, single centre cohort study with 180 days follow up from December 2020 to December 2021 at Centro Hospitalar Universitário de São João, a tertiary teaching hospital at Oporto, Portugal. The study was approved by the local research ethics board (research project no. 240/19).</p><p id="par0030" class="elsevierStylePara elsevierViewall">Adult female patients (≥18 years) scheduled for elective oncological breast surgery were included. Each gave written consent for participation and publication of the results. Exclusion criteria were male gender, bilateral breast cancer, mental illness or inability to communicate, pregnancy or breast feeding, and non-Portuguese speaker.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The anaesthesia protocol was standardized in all patients: midazolam, fentanyl and propofol were used for induction and patients were paralysed with rocuronium. Anaesthesia was maintained with propofol or sevoflurane and either fentanyl or remifentanil and combined with regional analgesia/anaesthesia (paravertebral block with 2−3.75 mg/mL ropivacaine) at the discretion of the attending anaesthesiologist. Acetaminophen, ketorolac, tramadol, and morphine were administered towards the end of the procedure for postoperative analgesia. All intraoperative data were appropriately recorded. Postoperatively, acetaminophen (1 g, 4 times daily), ketorolac (30 mg, 3 times daily), tramadol (100 mg, 3 times daily) and morphine (3−5 mg, as rescue analgesia) were administered. Morphine was tapered by the attending physician and nurses based on patient-reported pain scores following the local protocol. Our acute pain service is on call round the clock and provides acute pain management based on recommended standards.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Outcomes</span><p id="par0040" class="elsevierStylePara elsevierViewall">The primary outcome was incidence, characteristics and factors associated with CPBS in female patients undergoing breast cancer surgery. The secondary outcome was the impact of CPBS on patient QoL.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Data collection and assessment</span><p id="par0045" class="elsevierStylePara elsevierViewall">Demographic characteristics, American Society of Anaesthesiologists physical status, comorbidities, daily medication, and pre-operative chemotherapy, radiotherapy and hormone therapy were recorded. Data were collected using the Portuguese versions of validated questionnaires<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> that were administered preoperatively (T0), 2 days after surgery (T2), and 3 months (T90) and 6 months (T180) after surgery. Prior to surgery (T0), the Pain Catastrophizing Scale (PCS), Brief Pain Inventory - Short Form (BPI-SF), Douleur Neuropathique 4 Questionnaire (DN4) and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire and its Breast Cancer Module (EORTC QLQ-C30 and QLQ-BR23, respectively) were administered. At T2, the BPI-SF was administered. Both T0 and T2 questionnaires were administered in a face-to-face interview. All patients were re-evaluated over the phone at 90 days (T90) and 180 days (T180), during which the BPI-SF, DN4, EORTC QLQ-C30 and QLQ-BR23 were administered. All interviews were standardised to limit interviewer bias, and patients answered the questions without any interference from the interviewer.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Questionnaires</span><p id="par0050" class="elsevierStylePara elsevierViewall">The PCS is a self-reported 13-item scale used to assess catastrophic thinking related to pain.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> The BPI-SF is a 9-item self-administered questionnaire used to evaluate the severity of a patient's pain and its impact on daily functioning.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> The DN4 discriminates neuropathic pain (NP) from nociceptive pain and is composed of 10 items grouped in 4 sections. The first 7 items are patient-reported and relate to the quality of pain and its association with abnormal sensations, and the 3 remaining items are clinician-reported and relate to findings from a neurological examination of the painful area. As we were only interested in collecting self-reported data, the last 3 items of DN4 were eliminated, and patients were classified with NP in accordance with Bouhassira et al. (2008).<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> The EORTC QLQ-C30 and QLQ-BR23 evaluate quality of life: the former is a 30-item questionnaire, and the EORTC QLQ-BR23 is a breast-specific module that comprises 23 questions.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a> Intraoperative surgical procedure and anaesthesia variables were collected prospectively.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">Based on a predicted incidence of pain of 25%, we calculated that 112 participants would ensure a 95% confidence interval for estimating pain incidence with an 8.0% margin of error. All statistical analyses were performed using Statistical Package for the Social Sciences (version 22.0 for Windows; SPSS, Chicago, IL, USA). Continuous variables are expressed as median and interquartile range, normality was assessed with the Kolmogorov-Smirnov test. Statistical differences between groups were compared using the Wilcoxon and U Mann-Whitney test for non-normally distributed variables. Categorical variables are expressed as percentages. Statistical differences between groups were evaluated using the Chi-square test. A p-value of 0.05 was considered significant for all tests.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">A total of 120 patients underwent breast cancer surgery during the study period. Of these, 8 were excluded because they could not be reached for the 3- and 6-month follow-up assessment; therefore, 112 patients (93.9%) completed the study. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows patient demographics, comorbidities, and surgical and anaesthesia procedures in the evaluated patients.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">The incidence of CPBS at 3 months was 34.8% [95% CI: 26.8–42.8%]; 19.6% reported moderate pain and 15.1% reported severe pain. At 6 months, the incidence of CPBS had fallen to 20.5% [95% CI: 12.5–28.5%]; 3.6% reported mild pain, 8.3% reported moderate pain, and 8.0% reported severe pain. Of these, pain assessed by DN4 was possibly neuropathic in 81.7% of cases at T90 and in 16.1% of cases at T180. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the severity and level of interference of CPBS measured by the BPI-SF at T90.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the association between patient demographics, comorbidities, psychological factors, clinical and early post-operative factors, and the incidence of CPBS at 3 months. Diabetes (p = 0.028), acute post-operative pain assessed by BPI-SF at T2 (p < 0.001), and median PCS (p = 0.042) were significantly associated with a higher incidence of CPBS. The incidence of CPBS at 3 months was not affected age (p = 0.436), obesity (p = 0.306), radiation therapy (p = 0.910) and pre-operative opioid or gabapentin use (p = 0.720, p = 0.956 respectively).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">In the QLQ-C30 and QLQ-BR23 questionnaires, patients presenting CPBS at 3 months reported a significant change for the worse in emotional functioning (p < 0.001), fatigue (p = 0.005), nausea and vomiting (p < 0.001), body image (p < 0.001), and therapy-related side effects (p < 0.001), and little or no change in the remaining items. These results are summarized in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">The main findings of our study can be summarized as follows. First, approximately one third of patients developed CPBS; in nearly all cases CPBS was potentially neuropathic. Second, CPBS interfered considerably in daily life and worsened QoL compared with patients with no CPBS. Third, diabetes, catastrophizing, and acute postoperative pain severity were all associated with CPBS.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The incidence of CPBS in our study is consistent with the literature,<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,16,17</span></a> although our updated incidence is based on the current IASP definition of CPBS. The reasons for the decreased incidence of CPBS at 6 months after surgery are unclear, although it is reasonable to assume that these patients seek help as chronic postsurgical pain progresses and receive appropriate treatment with better follow-up. Alternatively, the decline may be attributable to psychological factors or CPBS pathophysiology.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> Psychological factors are important in pain perception, and resilience and positive affect have been shown to reduce chronic pain. In addition, patients might feel more satisfied with life after cancer surgery, and this may improve their psychological profile. Therefore, improved QoL and greater satisfaction with life over time might reduce the incidence of pain in these patients. The role of contextual and cognitive variables in chronic pain is not only important, but also complex.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Regarding severity, our findings suggest that CPBS is a high intensity pain that interferes considerably with daily activities. Most of our patients classified their CPBS pain as moderate with moderate to severe interference in general activity. CPBS also interfered with the patient’s mood, walking ability, normal work, relations with other people, sleep, and enjoyment of life - a finding echoed in other chronic postsurgical pain studies.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Our results have shown a high proportion of possible NP. Nerve injury–induced NP has been proposed as a major cause of chronic postsurgical pain, and a previous review suggested that the prevalence of NP differs according to the type of surgery performed, and probably depends on the likelihood of surgical iatrogenic nerve injury.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">With regard to other factors associated with pain - younger age, radiation therapy, axillary node dissection, and severity of acute postoperative pain are the most common risk factors described in literature.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,5,20</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">Younger age is a well-known risk factor for CPBS and the only one described as independent.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,6–8</span></a> More aggressive tumours, more aggressive treatment, and greater emotional lability are some possible explanations.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> The median age of our patients was 54, and unlike previous studies we were unable to find an association between age and CPBS – possible due to our small sample size.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Diabetes is known to predispose patients to chronic pain. According to the current literature, chronic pain is very common among diabetic patients and has a significant impact on their self-care routine.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> In the case of CPBS, few studies have examined the impact of diabetes on pain chronification. Andersen et al. (2015), for example, failed to identify an association.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> In our study, however, diabetes was significantly associated with a higher incidence of CPBS at 3 months. These findings could be further investigated in future research and could lead to more effective and personalized treatment plans in these patients.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">Psychological profile (depression/anxiety) and catastrophizing have been the subject of much research in breast cancer patients, albeit with conflicting results.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,20,23</span></a> Although current evidence suggests that psychological factors are important in pain perception, we found no association between anxiety or depression and the development of CPBS. Catastrophizing, however, was associated with CPBS – a finding that has also been reported in previous studies in acute and chronic postsurgical pain.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Another important finding in our study was the lack of association between radiation therapy and the development of chronic pain, even though this has often been described as a risk factor for CPBS.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,20</span></a> This discrepancy could be due to the small number of women who underwent radiation therapy in our study population (only 10%).</p><p id="par0125" class="elsevierStylePara elsevierViewall">Preoperative pain is another well-known and extensively studied risk factor for CPBS. It appears to predispose patients to chronic pain because the transition from acute to chronic pain is facilitated by long-term central and peripheral pain sensitization.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,16,20,24</span></a> In our study, 18.8% of women reported preoperative pain on the BPI-SF, but this was unrelated with CPBS.</p><p id="par0130" class="elsevierStylePara elsevierViewall">We found no association between body mass index (BMI)<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> and CPBS. The literature on this topic is scant and inconsistent: some studies found no association while others identified a positive linear relationship between BMI and CPBS.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,25</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">CPBS is a complex process that involves traumatic nerve injury, inflammation, and peripheral and central sensitization. An extensive body of literature has consistently shown that severe acute postoperative pain is associated with the development of CPBS.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,6,16,20</span></a> In line with these finding, we also found that CPBS was more common among patients with higher acute postoperative pain scores. The reliability of these findings shows the need to promptly and effectively treat acute pain after breast surgery.</p><p id="par0140" class="elsevierStylePara elsevierViewall">QoL assessments are now an important part of breast cancer care. The first study on quality of life in breast cancer was published in 1974 by Moore et al.,<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> and since then considerable progress has been made in the assessment of QoL in breast cancer patients. The earliest literature shows that patients receiving cancer treatment reported worse functional capacity and symptoms compared with healthy individuals. Cognitive function, sexual issues, fatigue, and sleep problems were among the most widely reported long-term problems.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a> More recently, authors such as Yfantis et al. (2020) found that patients demonstrated a fairly good QoL with low/moderate-intensity symptoms after breast cancer treatment.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> This trend in QoL improvement may be explained by the progress made in breast cancer screening tools and treatment options in recent years, and also by the introduction of supportive health care systems with counselling and social support for cancer patients. However, as shown in our study, CPBS worsened QoL - namely, emotional functioning, fatigue, nausea and vomiting, and body image - and increased the incidence of therapy-related side effects. This shows the negative impact of CPBS and the need to identify and treat it at an early stage. The reduced QoL reported by CPBS patients should prompt health professionals and authorities to investigate and implement strategies to prevent this type of pain.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Our study has several strengths. It was an observational prospective study in chronic postsurgical pain after a specific procedure with a low loss-to follow-up rate. We used ICD-11 definitions and performed a comprehensive assessment based on validated questionnaires, a detailed analysis of CPBS incidence and severity, patient-related factors, and the impact of CPBS on QoL.</p><p id="par0150" class="elsevierStylePara elsevierViewall">One of the main limitations of our study concerns the type of anaesthesia administered, as a large proportion of our patients (89.7%) underwent breast cancer surgery without regional anaesthesia. This makes it difficult to detect possible anaesthesia-related differences in CPBS. Although regional anaesthesia techniques are known to protect against pain,<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> the number of patients in our study receiving regional anaesthesia was too small to adequately address its impact on CPBS. Nevertheless, evidence of the role of regional anaesthesia, namely paravertebral block, is often contradictory.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> For example, a Cochrane systematic review showed that regional anaesthesia, namely paravertebral block, reduced the risk of CPBS,<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> while a recent meta-analysis concluded that paravertebral block did not prevent CPBS.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a> Regional anaesthesia is an important technique for reducing the severity of acute postoperative pain,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">30</span></a> which is a known risk factor for CPBS. In our study, patients who presented CPBS reported mild postoperative pain (median severity = 3) on average, suggesting that the multimodal postoperative analgesic protocol used in our patients provided adequate acute pain management, even though few patients received regional anaesthesia.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Our sample was drawn from a single hospital; this could introduce some bias with respect to surgical techniques and acute pain treatment and limits the generalizability of our results.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Conclusions</span><p id="par0160" class="elsevierStylePara elsevierViewall">Although advances in breast cancer therapy have increased survival rates, they also cause several adverse effects, one of the most important being CPBS, which affects the patient’s physical and mental health and daily activities. Data were collected using several questionnaires: Pain Catastrophizing Scale, Brief Pain Inventory-Short Form, Douleur Neuropathique 4 Questionnaire, and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire and its Breast Cancer Module.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">A total of 112 patients completed the study. Approximately, one third (34.8%) developed CPBS, and almost all with potentially neuropathic pain. CPBS interfered with patients’ daily life and reduced their QoL.
